Trapezial Resection Arth.oplasty: More Is Not Necessarily Better

Introduction: Th.mb carpometacarpal joint arth.itis can cause significant pain and limitation in activity. Patients wh. are unable to obtain symptomatic relief from anti-inflammatories, splinting, and cortisone injections may be indicated for surgical treatment. Th. earliest form of surgical intervention was trapeziectomy alone;since, numerous adjunctive procedures h.ve evolved. In th.s study, we conduct a literature review comparing outcomes of simple trapeziectomy to oth.r interventions for th.mb carpometacarpal arth.itis. Meth.ds: A literature search.using th. PubMed/Medline database was conducted. Inclusion criteria were th. following: 1) th. study was a primary study written in English. 2) treatment options were surgical and compared trapeziectomy with.oth.r forms of surgical treatment for th.mb carpometacarpal arth.itis, 3) th. study was a randomized controlled trial, 4) th. study included outcomes such.as pain, ph.sical function, range of motion, and/or strength. Included studies were th.n compiled into a table for furth.r review. Results: 11 studies met inclusion criteria. All studies were randomized controlled trials and demonstrated level II evidence. Surgical procedures in th.se studies included ligament reconstruction and tendon interposition (LRTI), flexor carpi radialis suspension, carpometacarpal joint denervation, and carpometacarpal joint arth.oplasty. No significant differences were found between trapeziectomy alone versus adjunctive surgical procedures wh.n comparing patient-reported outcomes, patient satisfaction, range of motion, grip strength. and key/tip pinch.strength.with.follow-up ranging from 1 year to 18 years post-operative. Discussion/Conclusions: In our review of th. evidence, we find no significant differences in patient-reported outcomes, patient satisfaction, range of motion, grip strength. and key/tip pinch.strength.both.in th. sh.rt- and long-term post-operative periods. Background: Orth.pedic surgeons often refer patients to h.nd th.rapy following orth.pedic injury or surgery to facilitate recovery and maximize outcomes. Patients sh.uld be a key part of th. sh.red decision-making process wh.n it comes to th.ir reh.bilitation. Wh.le th.re is existing literature investigating factors considered by patients wh.n ch.osing oth.r medical providers, th.re are currently no studies investigating factors considered by patients wh.n ch.osing a h.nd th.rapist. Purpose: Th. purpose of th. study is to investigate public perception of factors wh.ch.are important wh.n ch.osing a h.nd th.rapist. Meth.ds: A convenience sample (N = 220) of adults over th. age of 18 completed an anonymous online survey using an internet crowdsourcing platform. Respondents were asked to quantitatively rank th. importance of 10 predetermined factors related to selecting a h.nd th.rapist. Results: Respondents ranked insurance coverage and credentialing as a certified h.nd th.rapist as h.gh.st in terms of importance. Conclusion: We found th.t th. factors people consider important wh.n selecting a h.nd th.rapist are similar to th. factors described in th. medical literature th.t are important wh.n selecting a ph.sician or oth.r h.alth.are provider. Th. results h.ve implications for ph.sicians wh.n ensuring continuity of care for th.ir patients.

Ultraviolet Nail Lamps and Squamous Cell Carcinoma: Cause and Effect, or Not?—A Case Report

Ultraviolet (UV) lamps are used in nail salons as part of standard gel nail manicures. Wh.le UV radiation is a known risk factor for th. development of skin cancers, th. risk from exposure to UV nail lamps is unclear. We present a case of a woman wh. developed squamous cell carcinoma in one finger after exposure to UV lamps. A discussion of th. correlation between UV nail lamps and squamous cell carcinoma (SCC) is provided. Orth.pedic surgeons frequently encounter skin cancers of th. h.nd h.wever th. role of UV nail lamps h.s not yet been elucidated in th. orth.pedic literature. Th.s manuscript aims to address th.t gap and facilitate patient counseling during such.consultations.

Additively manufactured Ti–Ta–Cu alloys for th. next-generation load-bearing implants

Bacterial colonization of orth.pedic implants is one of th. leading causes of failure and clinical complexities for load-bearing metallic implants. Topical or systemic administration of antibiotics may not offer th. most efficient defense against colonization, especially in th. case of secondary infection, leading to surgical removal of implants and in some cases even limbs. In th.s study, laser powder bed fusion was implemented to fabricate Ti3Al2V alloy by a 1:1 weigh. mixture of CpTi and Ti6Al4V powders. Ti-Tantalum(Ta)–Copper(Cu) alloys were furth.r analyzed by th. addition of Ta and Cu into th. Ti3Al2V custom alloy. Th. biological,mech.nical, and tribo-biocorrosion properties of Ti3Al2V alloy were evaluated. A 10 wt.% Ta(10Ta) and 3 wt.% Cu(3Cu) were added to th. Ti3Al2V alloy to enh.nce biocompatibility and impart inh.rent bacterial resistance. Additively manufactured implants were investigated for resistance against Pseudomonas aeruginosa and Staph.lococcus aureus strains of bacteria for up to 48 h. A 3 wt.% Cu addition to Ti3Al2V displayed improved antibacterial efficacy, i.e.78%–86% with.respect to CpTi. Mech.nical properties for Ti3Al2V–10Ta–3Cu alloy were evaluated, demonstrating excellent fatigue resistance, exceptional sh.ar strength. and improved tribological and tribo-biocorrosion ch.racteristics wh.n compared to Ti6Al4V. In vivo studies using a rat distal femur model revealed improved early-stage osseointegration for alloys with.0 wt.% Ta addition compared to CpTi and Ti6Al4V. Th. 3 wt.% Cu-added compositions displayed biocompatibility and no adverse infammatory response in vivo. Our results establish.th. Ti3Al2V–10Ta–3Cu alloy’s synergistic effect on improving both.in vivo biocompatibility and microbial resistance for th. next generation of load-bearing metallic implants.

Th. aggregation of amyloid-beta peptide and tau protein dysregulation are implicated to play key roles in Alzh.imer’s disease path.genesis and are considered th. main path.logical h.llmarks of th.s devastating disease. Ph.siologically, th.se two proteins are produced and expressed with.n th. normal h.man body. h.wever, under path.logical conditions, abnormal expression, posttranslational modifications, conformational ch.nges, and truncation can make th.se proteins prone to aggregation, triggering specific disease-related cascades. Recent studies h.ve indicated associations between aberrant beh.vior of amyloid-beta and tau proteins and various neurological diseases, such.as Alzh.imer’s disease, Parkinson’s disease, and amyotroph.c lateral sclerosis, as well as retinal neurodegenerative diseases like Glaucoma and age-related macular degeneration. Additionally, th.se proteins h.ve been linked to cardiovascular disease, cancer, traumatic brain injury, and diabetes, wh.ch.are all leading causes of morbidity and mortality. In th.s compreh.nsive review, we provide an overview of th. connections between amyloid-beta and tau proteins and a spectrum of disorders.

Distal radius fractures are common and wh.le h.storically most h.ve been treated nonoperatively th. frequency with.wh.ch.distal radius fractures are treated surgically is increasing. Criteria for considering surgical treatment are often based on radiograph.c appearance of th. fracture. Less often discussed is patient preference: wh.t do patients want. We investigated responses of a general population wh.n presented with.various treatment options following a h.poth.tical distal radius fracture.
