Factors Affecting Disease Risk Perception and Self-Management Behaviors among Japanese Long-Term Overseas Volunteers

Introduction: Many overseas volunteers have worked in low and middle-income countries. However traveling abroad and staying in an unfamiliar environment with inadequate medical infrastructure exposes volunteers to a wide range of health risks. The aim of this study was to clarify the relationships among disease knowledge and symptoms, disease risk perception, and self-management skills. Methods: This was a questionnaire survey of 189 Japanese who were preparing to volunteer abroad. A factor analysis was conducted on 13 items (six general risk events and seven major diseases) of risk perception. Correlation analysis was performed between the identified risk perception factors and disease knowledge and symptoms. Risk perception scores by factors from the seven disease items were compared between the Self-management Score (SMS) group and disease knowledge and risk of symptoms group, using T-tests. St. Luke’s International University Ethics Committee (19-A092) approved this study. Results: Many participants expected to suffer from digestive diseases (69.3%) and dermatitis (55%) in the field. On the other hand, few expected high risk of malaria (26.5%) or dengue fever (36%). Factor analysis yielded robust factor loadings creating a “Dread” and an “Unknown” factor accounting for 62.2% of total variance. Strong correlation was not found between SMS and disease risk perception. Background: Walking speed is a reliable barometer of adverse health outcomes, particularly among older people. Few studies evaluated factors associated with daily actual walking speed, rather than that measured in a laboratory setting or self-reported data. Methods: In a joint effort with The Pokémon Company, we recruited study participants through a women’s magazine and analyzed data from 63 Pokémon GO players. We measured the true walking speed in daily life among older women using a data-gathering mobile application. Then, using questionnaire survey data collected on these participants in 2019, we estimated the relationship between walking speed and their lifestyle and physiological factors. In the analysis, we employed a bi-directional stepwise linear regression approach, with the Akaike information criterion (AIC) for variable selection. Results: The mean age of the 63 participants was 63.03 years (standard deviation, SD 7.46);the average walking speed was 4.56 km per hour (SD 1.54);and 52 (82.5%) and 55 (87.3%) participants answered that they had excellent/good health physically and mentally, respectively. After adjusted for covariates in a stepwise regression approach, we identified a significant association between −1.33 (95% confidence intervals −2.52 to −0.15) km/hour lower walking speed and the experience of outdoor falls within a year. We also demonstrated that the walking speed was 1.07 (0.33 to 1.81) km/hour faster for those who had played Pokémon GO before the study started. 目的目前湖北省的新型冠状病毒肺炎(COVID-19)确诊和疑似病例的数量仍在增加。国内外多个团队对疫情发展进行了模型预测,但结论并不统一。因此,开展本次疫情的预测模型研究、评估COVID-19的基本再生数(basic reproduction number,R0),对于评估病毒的传播能力以及一系列控制措施的效果具有重要意义。方法收集从湖北省2020年1月17日到2月8日期间每天报告的确诊病例数等数据,分别采用指数增长方法(exponential growth,EG)、极大似然法(maximum likelihood estimation,ML)、序贯贝叶斯方法(sequential Bayesian method,SB)和时间相关基本再生数(time dependent reproduction numbers,TD)估计R0值。结果由观测病例数和4种方法预测的病例数的拟合情况可知,EG方法拟合效果最优。EG方法估计COVID-19湖北省R0的值为3.49(95%CI:3.42~3.58)。采取封城控制手段期间,EG方法估算R0值为2.95(95%CI:2.86~3.03)。结论在传染病流行初期,适合采用EG方法估算R0。同时需要采取及时有效的控制措施,进一步降低COVID-19的传播速率。

An enhanced functional ability questionnaire(faVIQ) to measure the impact of rehabilitation services on the visually impaired

AIM: To develop a short,enhanced functional ability Quality of Vision(faVIQ) instrument based on previous questionnaires employing comprehensive modern statistical techniques to ensure the use of an appropriate response scale,items and scoring of the visual related difficulties experienced by patients with visual impairment. · METHODS: Items in current quality-of-life questionnaires for the visually impaired were refined by a multi-professional group and visually impaired focus groups. The resulting 76 items were completed by 293 visually impaired patients with stable vision on two occasions separated by a month. The faVIQ scores of 75 patients with no ocular pathology were compared to 75 age and gender matched patients with visual impairment. ·RESULTS: Rasch analysis reduced the faVIQ items to 27. Correlation to standard visual metrics was moderate(r =0.32-0.46) and to the NEI-VFQ was 0.48. The faVIQ was able to clearly discriminate between age and gender matched populations with no ocular pathology and visual impairment with an index of 0.983 and 95% sensitivity and 95% specificity using a cut off of 29.·CONCLUSION: The faVIQ allows sensitive assessment of quality-of-life in the visually impaired and should support studies which evaluate the effectiveness of low vision rehabilitation services.
