Obj.ctive:there are few studies evaluating the impact of Aspirin-exacerbated respiratory disease(AERD)treatment on otologic *** aim of this study is to evaluate the effects of endoscopic sinus surgery(ESS)and aspirin desensitization(AD)on otologic symptoms in subj.cts with ***:Retrospective chart review of adult patients diagnosed with AERD at our tertiary Care Academic Medical Center-Otorhinolaryngology *** of adult patients diagnosed with AERD who underwent ESS and ASA desensitization at our institution’s AERD Center from 2016 to 2019 were ***-Nasal Outcomes test 22-item survey(SNOt-22)scores were evaluated for patients at various time points including:pre-surgery,post-surgery/pre-aspirin desensitization,and various times post-desensitization up to>12 *** the SNOt-22,otologic-specific subdomain scores were evaluated at similar time *** on immunomodulatory medications other than corticosteroids were excluded from ***:SNOt-22 scores were analyzed for 121 *** was a significant improvement in overall SNOt scores from pre-surgery(44.62)to post surgery/pre-desensitization(23.34)(Pt-22 otologic-specific scores also improved after surgery prior to desensitization(3.19-2.04)(P=0.005).Following AD,the improvement in the overall SNOt-22 continued to improve for up to 12 months(Pthe otologic-specific SNOt-22 scores remained stable after surgery and ASA ***:ESS and AD reduce otologic-specific SNOt-22 scores and parallel trends in overall SNOt-22 *** effect of treatment is durable over the course of 12 *** work should aim to correlate otologic SNOt-22 scores with obj.ctive otologic data.

为研究微空心阴极放电类辉光现象,在气压100 torr、空心半径100μm和阴极表面施加电压-600 V条件下,建立了二维轴对称流体计算模型,包括控制方程、边界条件及化学反应,并进行了数值模拟。结果表明,在类辉光放电模式下,放电电压约为-95V,在空心内阴极附近出现一个清晰的环形鞘层结构,鞘层厚度沿阴极外表面随着半径的增加而增加;带电粒子数密度约为10^(19) m^(-3),阴极鞘层和等离子体区域内电子温度近似约为10.2 eV和1.5 eV;基态电离和彭宁电离反应速率峰值约分别为1.2×10^(3)、2.7×10^(3) mol/(m^(3)·s);电子的整体净体积产生率约为2.4×10^(27) L/(m^(3)·s),主要产生在空心阴极区,同时在阴极侧孔外和平坦的外阴极区域鞘层边缘也有少量电子产生。

Evaluation of Different Levels of Addition of Souari Nut Peel (Caryocar brasiliense) on the Quality of the Mombasa Grass Silage

the obj.ctive of this study was to evaluate the effects of the addition of souari nut peel on the fermentative characteristics and chemical composition of the Mombasa grass silage.A completely randomized design with five levels of addition(0%,5%,10%,15%and 20%in natural matter basis)of souari nut peel with three replicates was applied in the *** silos were opened at 55 d after starting the ensiling *** data were analyzed by means of linear/quadratic regression analysis,according to the best fit of the data and the probability values to 5%,using the Statistical Analysis System(SAS,2002).there were a linear decreasing effect on the neutral detergent fiber(NDF),ash and increasing values for the effluent in the *** effect was observed for crude protein(CP),lignin and *** the other factors dry matter(DM),acid detergent fiber(ADF)and gases there was no influence on the addition of souari nut *** 15%addition level of souari nut peel increased CP content,decreased NDF and pH,which was recommended to improve the fermentation profile of Mombasa grass silage.

Descriptive Study:Congenital Malformations in Pediatric Patients of a Public Maternity Hospital of the City of Manaus,Amazonas

the Pan American Health Organization(PAHO)defines congenital malformation as any functional or structural anomaly in the development of the fetus,due to factors originating before birth,whether genetic,environmental or *** aim of the present study was to identify the frequency of malformations,the type of congenital malformations and to correlate this with risk factors in live-born infants,using SLB *** data were collected through consultation of the medical records of live-born infants of the Balbina Mestrinho Maternity Hospital,15,621 live births were reported,of which 248(1.58%)presented congenital *** was a higher prevalence of malformations among live-born males(49.7%),with Apgar≥7 at the first and fifth minute,gestational age ranging from 37-41 weeks,with 46%being born with appropriate weight between 3,000-4,000 *** association of two or more defects was observed in 38.7%of the total cases and isolated anomalies in 67.3%,with predominance of alterations of the digestive system(26.3%),followed by malformations of the musculoskeletal system(21.2%),nervous system(20.2%)and cleft lip/cleft palate(9.1%).the results presented here may guide strategic actions to improve care for families of people with congenital malformations.

Obj.ctive:Malleostapedotomy allows to completely by-pass the incus in otosclerosis *** its use has been rivaled by hydroxyapatite cement for cases of mild and moderate necrosis of the ***,it remains gold standard for cases of extensive necrosis,incus dislocation,or epitympanic *** heat-crimping pistons make surgery easier and *** study focuses on our experience with this ***:Retrospective analysis of patient’s files and pre-and post-operative audiograms,for cases of surgically treated otosclerosis with ***:twelve patients underwent malleostapedotomy for otosclerosis between 2011 and *** them there were 10 revision surgeries and 2 primary cases.75%had incus long-process necrosis,17%had epitympanic fixation and one had a history of incus *** patients(75%)had closure of air-bone gap(ABG)ofthreshold of 20 dB(ptive ABG was 31 dB(15 dBe55 dB),and mean post-operative ABG was 7 dB(0 dBe21 dB;pthere was no sensorineural hearing loss nor any other post-operative ***:Malleostapedotomy is a safe and reliable technique,allowing an ABG closure comparable to conventional incus to vestibule *** remains the preferred technique whenever the incus cannot be used.

the incidence of Mycobacterium avium complex pulmonary disease (MACPD) has been globally increasing. the present study aims to provide a comprehensive analysis of clinical characteristics and treatment response among patients in the Amazon region of Brazil and conduct a retrospective cohort study at a prominent referral hospital in the State of Par, Brazil, from 2012 to 2020. the M. avium group represented 58.9% of cases, followed by M. intracellulare (35.7%), M. colombiense (3.6%), and M. chimaera (1.8%). the maj.rity of patients (73.2%) were female and with an average age of 63 years. Primary clinical manifestations included persistent cough and sputum production. the Charlson comorbidity index (CCI) averaged 2.47, with 36.3% of patients having a CCI score equal to higher than 3. Notably, over 70% of patients had received treatment for pulmonary tuberculosis before initiating treatment for MAC infection. the fibrocavitary radiologic form was the most prevalent (55.4%), frequently exhibiting a bilateral distribution (53.6%). Antimicrobial susceptibility revealed a significant prevalence of MAC resistance to drugs in the therapeutic regimen. Despite this, most of the patients experienced clinical improvement (50%). In conclusion, this study highlights a higher prevalence of MAC infections among middle-aged women, with a history of pulmonary tuberculosis treatment and presenting the fibrocavitary radiological form was predominant.

Background: About 20%of patients with familial hemiplegic migraine (FHM) deve lop progressive cerebellar signs. Genetic studies have established an associatio n with mutations in the CACNA1A gene. However, the mechanisms underlying cerebel lar involvement are largely unknown. Obj.ctive: to use proton MR spectroscopy (1 H-MRS) to investigate metabolic alterations in the cerebellum as well as cortic al regions known to be involved in the propagation of migraine aura. Methods:Fif teen CACNA1A mutation carriers from three FHM families and 17 healthy control su bj.cts were studied. Eleven patients had clinical signs of cerebellar involvemen t. LCModel fits were used to estimate absolute concentrations of N-acetylaspart ate (NAA), myo-inositol (ml), glutamate (Glu), choline containing compounds, to tal creatine, and lactate in the superior cerebellar vermis (SCV), parietal cort ex, and occipital *** control for atrophy effects, automated image segment ation was performed using SPM99. the brain parenchyma fraction(BPF) was determin ed for all three regions. Results: Compared with controls, the brain parenchyma fraction (BPF), NAA,and Glu were significantly reduced and mI was significantly elevated in the SCV of patients with FHM. In contrast, no metabolite alterations were found in supratentorial regions. BPF and NAA in the SCV significantly corr elated with cerebellar scores, in particular, gait ataxia. Conclusions: the find ings suggest that there is a regionally distinct neuronal impairment in the superio r cerebellar vermis that exceeds macroscopic tissue loss. correlations with clin ical scores emphasize the functional relevance of localized atrophy (brain paren chyma fraction) and N-acetyl aspartate levels. these measures may be useful to monitor disease progression. the observed reductionin glutamate may in part refl ect impaired glutamatergic neurotransmission.
