Recent advancements in noninvasive brain modulation for individuals with autism spectrum disorder

Autism spectrum disorder is classified as a spectrum of neurodevelopmental disorders with an unknown definitive etiology.Individuals with autism spectrum disorder show deficits in a variety of areas including cognition,memory,attention,emotion recognition,and social skills.With no definitive treatment or cure,the main interventions for individuals with autism spectrum disorder are based on behavioral modulations.Recently,noninvasive brain modulation techniques including repetitive transcranial magnetic stimulation,intermittent theta burst stimulation,continuous theta burst stimulation,and transcranial direct current stimulation have been studied for their therapeutic properties of modifying neuroplasticity,particularly in individuals with autism spectrum disorder.Preliminary evidence from small cohort studies,pilot studies,and clinical trials suggests that the various noninvasive brain stimulation techniques have therapeutic benefits for treating both behavioral and cognitive manifestations of autism spectrum disorder.However,little data is available for quantifying the clinical significance of these findings as well as the long-term outcomes of individuals with autism spectrum disorder who underwent transcranial stimulation.The objective of this review is to highlight the most recent advancements in the application of noninvasive brain modulation technology in individuals with autism spectrum disorder.

Comparison of five video-assisted intubation devices by novice and expert laryngoscopists for use in the aeromedical evacuation environment

Background: The critically ill or injured patient undergoing military medical evacuation may require emergent intubation. Intubation may be life-saving, but it carries risks.The novice or infrequent laryngoscopist has a distinct disadvantage because experience is critical for the rapid and safe establishment of a secured airway. This challenge is compounded by the austere environment of the back of an aircraft under blackout conditions. This study determined which of five different video-assisted intubation devices(VAIDs) was best suited for in-flight use by U.S. Air Force Critical Care Air Transport Teams by comparing time to successful intubation between novice and expert laryngoscopists under three conditions, Normal Airway Lights on(NAL), Difficult Airway Lights on(DAL) and Difficult Airway Blackout(DAB), using manikins on a standard military transport stanchion and the floor with a minimal amount of setup time and extraneous light emission.Methods: A convenience sample size of 40 participants(24 novices and 16 experts) attempted intubation with each of the 5 different video laryngoscopic devices on high-fidelity airway manikins. Time to tracheal intubation and number of optimization maneuvers used were recorded. Kruskal-Wallis testing determined significant differences between the VAIDs in time to intubation for each particular scenario. Devices with significant differences underwent pair-wise comparison testing using rank-sum analysis to further clarify the difference. Device assembly times, startup times and the amount of light emitted were recorded. Perceived ease of use was surveyed. Results: Novices were fastest with the Pentax AWS in all difficult airway scenarios. Experts recorded the shortest median times consistently using 3 of the 5 devices. The AWS was superior overall in 4 of the 6 scenarios tested. Experts and novices subjectively judged the Glide Scope Ranger as easiest to use. Color-coded perfluorocarbon nanodroplets for multiplexed ultrasound and photoacoustic imaging

Laser-activated perfluorocarb on n anodroplets are an emerging class of phase-cha nge, dual-c ontrast age nts that can be utilized in ultraso und and photoacoustic imaging. Through the ability to differe ntiate subpopulations of nano droplets via laser activatio n at differe nt wavelengths of n ear-infrared light, optically-triggered color-coded perfluorocarb on nano droplets prese nt themselves as an attractive tool for multiplexed ultrasound and photoacoustic imaging. In particular, laser-activated droplets can be used to provide quantitative spatiotemporal information regarding distinct biological targets, allowing for their potential use in a wide range of diagnostic and therapeutic applications. In the work prese nted, laser-activated color-coded perfluorocarb on nan odroplets are syn thesized to selectively resp ond to laser irradiati on at corresp on ding wavele ngths. The dyn amic ultraso und and photoacoustic signals produced by laser-activated perfluorocarbon nano droplets are evaluated in situ prior to implementation in a murine model. In vivo, these particles are used to distinguish unique particle trafficking mechanisms and are show n to provide ultraso und and photoacoustic contrast for up to 72 hours within lymphatics. Overall, the con ducted studies show that laser-activated color-coded perfluorocarbo n nano droplets are a promising agent for multiplexed ultraso und and photoacoustic imaging.

BACKGROUND Pancreatic acinar cell carcinoma(PACC)is a rare tumor.Up to 45%of PACCs have alterations in the DNA damage repair pathway and 23%harbor rearrangements in the BRAF or RAF1 genes.We present a PACC case with a germline BRCA2 likely pathogenic variant(LPV)to highlight the impact of genomic testing on treatment decisions and patient outcomes.In our larger case series,we provide clinic-based information on additional 10 PACC patients treated in our center.CASE SUMMARY A 70-year-old male was diagnosed with advanced PACC.At presentation,he was cachectic with severe arthralgia despite prednisolone and a skin rash that was later confirmed to be panniculitis.He was treated with modified FOLFIRINOX(mFFX)with the knowledge of the germline BRCA2 LPV.Following 11 cycles of mFFX,a computed tomography(CT)scan demonstrated significant tumor response in the pancreatic primary and hepatic metastases,totaling 70%from baseline as per Response Evaluation Criteria in Solid Tumors.Resolution of the skin panniculitis was also noted.We identified two additional PACCs with druggable targets in our case series.Our data contribute to practical evidence for the value of germline and somatic profiling in the management of rare diseases like PACC.CONCLUSION This patient and others in our larger case series highlight the importance of genomic testing in PACC with potential utility in personalized treatment.

目的在脂肪抽吸物中活性脂肪来源干细胞（ADSC）的提取对自体脂肪移植的长期效果起决定性的作用。然而，提取方法是否会改变脂肪提取物的性质尚不得知。我们对3种提取技术进行测评，以明确其是否会对脂肪细胞的活性和基质血管细胞群（SVF）的细胞特性产生影响。方法我们对采用3种不同提取脂肪方法的隆乳患者数据进行前瞻性分析。收集每例患者脂肪提取物的样本，并用细胞核计数（NucleoCounter）对细胞的数量和活性进行分析。用流式细胞仪测定SVF细胞表面标记物的表达，并分成4种不同的成分。根据结果对3种脂肪提取方法进行比较。结果本组共2l例患者。对7例患者采用手动操作注射器吸脂，7例采用水动力辅助吸脂，7例采用传统设备吸脂。每种方法的细胞活性、CFU数量和每克脂肪组织提取物的细胞数量均无差异。各组间根据细胞荧光的特征明确细胞成分的数量基本相同。3种提取方法中，SVF中CD34＋／CD31-／CD45-的比例基本无差异（27．3％，27．5％，27．5％）。结论ADSC细胞的数量、活性、形成单元细胞团（CFU）、或SVF成分的比例不受传统脂肪提取方法的影响。需要进一步研究洗涤和净化脂肪提取物是否会对试验和临床结果产生影响。
