Colonic ulcerations may predict steroid-refractory course in patients with ipilimumab-mediated enterocolitis

AIM To investigate management of patients who develop ipilimumab-mediated enterocolitis, including association of endoscopic findings with steroid-refractory symptoms and utility of infliximab as second-line *** We retrospectively reviewed all patients at our centerwith metastatic melanoma who were treated with ipilimumab between March 2011 and May 2014. All patients received a standard regimen of intravenous ipilimumab 3 mg/kg every 3 wk for four doses or until therapy was stopped due to toxicity or disease progression. Basic demographic and clinical data were collected on all patients. For patients who developed grade 2 or worse diarrhea(increase of 4 bowel movements per day), additional data were collected regarding details of gastrointestinal symptoms, endoscopic findings and treatment course. Descriptive statistics were *** A total of 114 patients were treated with ipilimumab during the study period and all were included. Sixteen patients(14%) developed ≥ grade 2 diarrhea. All patients were treated with high-dose corticosteroids(1-2 mg/kg prednisone daily or equivalent). Nine of 16 patients(56%) had ongoing diarrhea despite highdose steroids. Steroid-refractory patients received one dose of intravenous infliximab at 5 mg/kg, and all but one had brisk resolution of diarrhea. Fourteen of the patients underwent either colonoscopy or sigmoidoscopy with variable endoscopic findings, ranging from mild erythema to colonic ulcers. Among 8 patients with ulcers demonstrated by sigmoidoscopy or colonoscopy, 7 patients(88%) developed steroidrefractory symptoms requiring infliximab. Bacterial biota in reflux esophagitis and Barrett's esophagus

AIM： To identify the bacterial flora in conditions such as Barrett＇s esophagus and reflux esophagitis to determine if they are similar to normal esophageal flora. METHODS： Using broad-range 16S rDNA PCR, esophageal biopsies were examined from 24 patients [9 with normal esophageal mucosa, 12 with gastroesophageal reflux disease （GERD）, and 3 with Barrett＇s esophagus]. Two separate broad-range PCR reactions were performed for each patient, and the resulting products were cloned. In one patient with Barrett＇s esophagus, 99 PCR clones were analyzed. RESULTS： Two separate clones were recovered from each patient （total = 48）, representing 24 different species, with 14 species homologous to known bacteria, 5 homologous to unidentified bacteria, and 5 were not homologous （〈97% identity） to any known bacterial 16S rDNA sequences. Seventeen species were found in the reflux esophagitis patients, 5 in the Barrett＇s esophagus patients, and 10 in normal esophagus patients. Further analysis concentrating on a single biopsy from an individual with Barrett＇s esophagus revealed the presence of 21 distinct bacterial species. Members of four phyla were represented, including Bacteroidetes, Firmicutes, Proteobacteria, and Actinobacteria. Microscopic examination of each biopsy demonstrated bacteria in intimate association with the distal esophageal epithelium, suggesting that the presence of these bacteria is not transitory. CONCLUSION： These findings provide evidence for a complex, residential bacterial population in esophageal reflux-related disorders. While much of this biota is present in the normal esophagus, more detailed comparisons may help identify potential disease associations.

Characterizing downwind drift deposition of aerially applied glyphosate using RbCl as tracer

Long-term antifibrotic action of interferon-γ treatment in patients with chronic hepatitis B virus infection

BACKGROUND:The first priority in treating fibrosis is to eliminate the causes that result in liver injury,e.g.,hepatitis B and C ***,in many liver diseases the cause is either unknown or *** present study was designed to investigate the long-term antifibrotic effect of interferon-gamma(IFN-γ)treatment in patients chronically infected with hepatitis B virus. METHODS:A total of 42 patients,30 treated with IFN-γand 12 controls,were enrolled from an original clinical trial(Clin Gastroenterol Hepatol 2005;3:819.).Three serial liver biopsies that were obtained at the initiation and end of IFN-γtreatment as well as 4 to 6 years after treatment discontinuation were assessed according to the modified Chevallier scoring system. RESULTS:Twenty-five out of 30 IFN-γ-treated patients were followed up until 4 to 6 years after the treatment was stopped. However,all controls were excluded from follow-up due to death,loss and elevated virus level within 2 ***-five IFN-γ-treated patients had stable serum liver function and liver fibrosis indices without any further antiviral or anti-fibrotic *** inflammatory and fibrotic scores were found after nine months of IFN-γtreatment according to the modified Chevallier scoring system(inflammation:11.8±6.5 at the beginning of IFN-γtreatment vs.9.2±4.1 after 9 months, Pjority of patients demonstrate stable serum biochemical indices and quality of ***,they do not show a long-term benefit according to histological criteria. Methane Emissions from Grazing Holstein-Friesian Heifers at Different Ages Estimated Using the Sulfur Hexafluoride Tracer Technique

Although the effect of animal and diet factors on enteric methane （CH4） emissions from confined cattle has been extensively examined, less data is available regarding CH4 emissions from grazing young cattle. A study was undertaken to evaluate the effect of the physiological state of Holstein-Friesian heifers on their enteric CH4 emissions while grazing a perennial ryegrass sward. Two experiments were conducted： Experiment 1 ran from May 2011 for 11 weeks and Experiment 2 ran from August 2011 for 10 weeks. In each experiment, Holstein-Friesian heifers were divided into three treatment groups （12 animals/group） consisting of calves, yearling heifers, and in-calf heifers （average ages： 8.5, 14.5, and 20.5 months, respectively）. Methane emissions were estimated for each animal in the final week of each experiment using the sulfur hexafluoride tracer technique. Dry matter （DM） intake was estimated using the calculated metabolizable energy （ME） requirement divided by the ME concentration in the grazed grass. As expected, live weight increased with increasing animal age （P 〈 0.001）; however, there was no difference in live weight gain among the three groups in Experiment 1, although in Experiment 2, this variable decreased with increasing animal age （P 〈 0.001 ）. In Experiment 1, yearling heifers had the highest CH4 emissions （g·d^-1） and in-calf heifers produced more than calves （P 〈 0.001 ）. When expressed as CH4 emissions per unit of live weight, DM intake, and gross energy （GE） intake, yearling heifers had higher emission rates than calves and in-calf heifers （P 〈 0.001）. However, the effects on CH4 emissions were different in Experiment 2, in which CH, emissions （g·d^-1） increased linearly with increasing animal age （P 〈 0.001）, although the difference between yearling and in-calf heifers was not significant. Global mitigation efforts cannot neglect emerging emitters

International efforts to avoid dangerous climate change have historically focused on reducing energy-related CO2emissions from countries with either the largest economies(*** EU and the USA)and/or the largest populations(*** and India).However,in recent years,emissions have surged among a different and much less-examined group of countries,raising concerns that a next generation of high-emitting economies will obviate current mitigation ***,we analyse the trends and drivers of emissions in each of the 59 countries where emissions in 2010-2018 grew faster than the global average(excluding China and India),project their emissions under a range of longer-term energy scenarios and estimate the costs of decarbonization *** emissions from these ’emerging emitters’ reach as much as 7.5 GtCO2/year in the baseline 2.5° scenari—substantially greater than the emissions from these regions in previously published scenarios that would limit warming to 1.5℃ or even 2℃.Such unanticipated emissions would in turn require non-emitting energy deployment from all sectors within these emerging emitters,and faster and deeper reductions in emissions from other countries to meet international climate ***,the annual costs of keeping emissions at the low level are in many cases 0.2%-4.1% of countries’ gross domestic production,pointing to potential trade-offs with poverty-reduction goals and/or the need for economic support and low-carbon technology transfer from historically high-emitting *** results thus highlight the critical importance of ramping up mitigation efforts in countries that to this point have been largely ignored.

Effect of Parent Internal Excitation on Product State Distribution in Methyl Radical Photodissociation at 212.5nm

Photodissociation dynamics of the CH3 radical at 212.5 nm excitation has been studied experimentally using the H atom Rydberg tagging time-of-flight method. CH3 radicals are produded by photodissociation of CH3I at 266 nm. Translational energy distribution and angular distribution for the CH2 product from CH3 photodissociation at different vibrational levels via the 3s Rydberg state have been measured. From these distributions, product j state distributions are obtained for photodissociation of different vibrationally excited CH3 radicals. The effect of parent vibrational as well as rotational excitation on the dissociation dynamics of CH3 is also investigated in detail. Experimental results in this work show that parent vibrational excitation in the umbrella mode has a significant effect on both rotational excitation and angular distribution of the CH2 product, while parent rotational excitation has obvious effect only on the angular distribution of CH2 product.
