Gastric peritoneal carcinomatosis-a retrospective review

AIM To characterize patients with gastric peritoneal carcinomatosis(PC) and their typical clinical and treatment course with palliative systemic chemotherapy as the current standard of *** We performed a retrospective electronic chart review of all patients with gastric adenocarcinoma with PC diagnosed at initial metastatic presentation between January 2010 and December 2014 in a single tertiary referral *** We studied a total of 271 patients with a median age of 63.8 years and median follow-up duration of 5.1 mo. The majority(n = 217, 80.1%) had the peritoneum as the only site of metastasis at initial presentation. Palliative systemic chemotherapy was eventually planned for 175(64.6%) of our patients at initial presentation, of which 171 were initiated on it. Choice of first-line regime was in accordance with the National Comprehensive Cancer Network Guidelines for Gastric Cancer Treatment. Thesepatients underwent a median of one line of chemotherapy, completing a median of six cycles in total. Chemotherapy disruption due to unplanned hospitalizations occurred in 114(66.7%), while cessation of chemotherapy occurred in 157(91.8%), with 42 cessations primarily attributable to PC-related complications. Patients who had initiation of systemic chemotherapy had a significantly better median overall survival than those who did not(10.9 mo vs 1.6 mo, P ho had initiation of systemic chemotherapy, those who experienced any disruptions to chemotherapy due to unplanned hospitalizations had a significantly worse median overall survival compared to those who did not(8.7 mo vs 14.6 mo, P < 0.001).CONCLUSION Gastric PC carries a grim prognosis with a clinical course fraught with disease-related complications which may attenuate any survival benefit which palliative systemic chemotherapy may have to offer. As such, investigational use of regional therapies is warranted and required validation in patients with isolated PC to maximize their

目的循证医疗有助于提高医疗照护质量,为医疗决策提供依据。为满足医疗卫生人员不断变化的需求和期望,始终需要及时为之提供高质量的证据。本研究旨在评估中国医疗卫生人员对循证实践的知识与培训的需求和期望。方法2020年12月至2021年1月,采用自行编制的调查问卷进行横断面在线调查,共纳入901名调查参与者,包括医疗卫生工作者、政策制定者、研究人员、教育工作者及全日制医学和护理专业研究生。结果研究参与者普遍同意获取与健康相关的研究证据是有益的。只有大约一半的参与者知道或使用过Cochrane资源等循证资源,这与循证医疗资源可获得性和可及性方面的困难有关。他们表示希望获得多种类型和主题的循证资源以及多种专题的循证培训工作坊。结论为满足中国医疗卫生人员对循证实践知识与培训的需求和期望,促进证据的传播和转化、提高证据可及性、提供循证培训及确定重要协作研究领域是目前中国推进循证实践的优先工作领域。

Important role of cardiac spect imaging in patients undergoing percutaneous transluminal coronary angioplasty

Objective To examine the utilization and contribution of cardiac perfusion scans to patients undergoing percutaneous transluminal coronary angioplasty. Methods From July 1996 to January 1998, 34 patients underwent percutaneous transluminal coronary angioplasty and cardiac SPECT imaging was studied. Twenty five patients had pre procedure SPECT scans. The agreement between angioplasty strategies and SPECT findings of ischemia and hypokinesia was *** When radionuclide imaging was performed before revascularization procedures, 84% of the patients had intervention strategies guided by scan findings. Perfusion scan findings including extent and severity of ischemia, degree of hypokinesia directed the selection of target vessel for angioplasty in the patient subgroup with multi vessel disease. Naproxen is one of the most consumed nonsteroidal anti-inflammatory drugs and marketed as Snaproxen as its R-enantiomer is hepatotoxic [1]. Developments of efficient and reliable methods for chiral differentiation and quantification of naproxen are thus essential [2]. In this work [3], we discovered a self-assembled binuclear Cu(II) diastereomeric complex ion in the gas phase, [(Cu IIL-His)2(S/R-naproxen)-3 H]+, that can be applied to effectively determine the absolute configuration of naproxen and quantify the enantiomeric excess of S/R-naproxen mixtures using collision-induced dissociation with a tandem mass spectrometer. The key candidate structure of this gas-phase diastereomeric complex ion has an unusual self-assembled, compact geometry with the two Cu(II) ions bridged closely together by the carboxylate groups of the two histidines, proposed using density functional theory, and supported by ion-mobility spectrometry and collision cross section modeling. The difference in dissociation efficiency of the two diastereomers is attributed to the σ-π interaction between the NH2 group of one histidine and the naphthyl ring of naproxen in the complex. The present report observes and characterizes the diastereomeric complexes of [(Cu IIL-His)2] with aromatic acid, which could contribute to the recognition of chiral aromatic acids, design of catalysts based on binuclear copper-bound complex, as well as the better understanding of metal-ion complexation by His or His-containing ligands.

This study aimed to develop a new junior doctor allocation plan,and evaluate its impact on the door-to-doctor time(DDT)of a specific patient population at the Accident&Emergency(A&E)department of Changi General hospital(CGH)in *** new junior doctor allocation plan was developed by solving an integer linear programming model with the objective of matching available junior doctors with the patient arrival *** to the period prior to the new plan’s implementation at CGH A&E,the average daily median,95th percentile,and standard deviation of DDT of target population were observed to have been reduced by 9.7 minutes(27.3%),24.5 minutes(21.9%),and 8.5 minutes(23.2%),respectively,in the post-implementation *** differences remained statistically significant after adjustment for differences in patient load and other relevant patient characteristics over the pre-and post-implementation ***,if not all,of the previous work on A&E staff allocation studies relied on simulation models to project the impact of new staff allocation plans on DDT *** did not report the extent of DDT improvement in the new plan actualized and experienced by A&E *** paper addresses this gap in A&E overcrowding management *** offers empirical evidence,from an operational A&E,on DDT improvements achieved through implementation of a new junior doctor allocation plan that better matched patient arrival pattern compared to the past.

Background Asthma is a significant chronic health problem worldwide. Management aims at disease control by reducing functional impairment and exacerbations and improving quality of life (QoL). We report a multi-center study to survey asthma control and QoL in four cities in the Pearl River Delta. Methods The conjoint survey involved ten hong Kong pediatric hospitals/units, two Shenzhen hospitals, two Macao hos-pitals, and two Guangzhou hospitals on asthma control (using Asthma Control Test) and QoL (Pediatric Allergic Disease Quality of Life Questionnaire, PADQLQ). Acceptability of a treatment is graded as very good/good/fair/poor. Results Good asthma control was only reported in 80% subjects in hong Kong, but higher in sister cities (85–94%, Phowed better control of asthma was associated with better PADQLQ (B=?0.029, P hodilator (B=?1.488, P = 0.025), negatively with 'smoker in family' (B=?0.83, P = 0.015) and various PADQLQ domains. Conversely, worse PADQLQ was associated with allergic rhinitis severity (B=4.77, P hodilator usage (B=1.05, P home (B=3.9, P < 0.05). Conclusions There are some clinical and cultural differences among the four southern Chinese cities within the Guangdong province. This study identifies potentially modifiable environmental and treatment factors associated with poor asthma control and QoL for health-care interventions. Having a smoker in the family is independently associated with poor asthma control and QoL.
