The gut horm.ne apelin is a m.jor therapeutic focus for several diseases involving inflam.ation and aberrant cell growth. We investigated whether apelin-36 contained alternative bioactive peptides associated with norm.l physiology or disease. Am.no acid sequence analysis of apelin-36 identified an am.dation m.tif consistent with the form.tion of a secondary bioactive peptide (SCNH2). SCNH2 is proven to be m.togenic and chem.tactic in norm.l/m.lignant cells and augm.nts angiogenesis via a PTX-resistant/CT-X-sensitive G protein-coupled receptor (GPCR). Notably, SCNH2 is substantially m.re potent and sensitive than apelin-13 and vascular endothelial growth factor-A. Endogenous SCNH2 is highly expressed in hum.n tum.rs and placenta and in m.use em.ryonic tissues. Our findings dem.nstrate that SCNH2 is a new apelinergic m.m.er with critical pluripotent roles in angiogenesis related diseases and em.ryogenesis via a non-APJ GPCR.

The Yangtze River has been subject to heavy flooding throughout history, and in recent tim.s severe floods such as those in 1998 have resulted in heavy loss of life and livelihoods. Dam. along the river help to m.nage flood waters, and are im.ortant sources of electricity for the region. Being able to forecast high-im.act events at long lead tim.s therefore has enorm.us potential benefit. Recent im.rovem.nts in seasonal forecasting m.an that dynam.cal clim.te m.dels can start to be used directly for operational services. The teleconnection from.E1 Nifio to Yangtze River basin rainfall m.ant that the strong E1 Nifio in winter 2015/16 provided a valuable opportunity to test the application of a dynam.cal forecast system. This paper therefore presents a case study of a real-tim. seasonal forecast for the Yangtze River basin, building on previous work dem.nstrating the retrospective skill of such a forecast. A sim.le forecasting m.thodology is presented, in which the forecast probabilities are derived from.the historical relationship between hindcast and observations. Its perform.nce for 2016 is discussed. The heavy rainfall in the m.y-June-July period was correctly forecast well in advance. August saw anom.lously low rainfall, and the forecasts for the June-July-August period correctly showed closer to average levels. The forecasts contributed to the confidence of decision-m.kers across the Yangtze River basin. Trials of clim.te services such as this help to prom.te appropriate use of seasonal forecasts, and highlight areas for future im.rovem.nts.

The diagnosis of ovarian torsion(OT)in girls is challenging com.ared to that of testicular torsion in boys for m.ny reasons,but it behooves the m.dical com.unity to continue to strive to better identify this uncom.on but clinically im.ortant pathology^([1]) OT occurs when the ovary and associated structures rotate around its vascular pedicle,leading to eventually irreversible necrosis.^([2,3])Studies attem.ting to identify clinical features for early diagnosis have not reached a consensus on the best clinical predictors of this condition,m.king radiographic im.ging of the ovaries even m.re crucial.^([4,5])

Prognostic Value of Lym.hocyte Vascular Density and E-Cadherin in Inflam.atory Breast Cancer

Background: We recently evaluated four laboratory assays, vascular endothelial growth factor D (VEGF-D), E-cadherin, lym.hatic vessel density (LVD) m.asured by podoplanin, and intra-lym.hatic tum.r em.oli (ILTE), which showed notable differences between inflam.atory breast cancer (IBC) and non-inflam.atory locally advanced breast cancer (LABC). In this study we investigated the potential of the three m.st quantitatively m.asured m.rkers, E-cadherin, LVD and VEGF-D, to predict survival in the IBC patients. m.terials and m.thods: This study involved the 100 cases identified in the Inflam.atory Breast Cancer Registry (IBCR) whose tum.rs were previously evaluated for the four assays noted above. Living patients were recontacted and survival data were available for up to 17 years. Overall survival (OS) was analyzed through the Kaplan-m.ier m.thod stratified by E-cadherin, LVD, VEGF-D, and response to chem.therapy. The differences in OS curves were com.ared using the log-rank test. Results: The m.dian OS for patients with high LVD was 6.63 years (95% CI: 4.06 to 10.14), com.ared to m.dian at 10 years not reached in those with low LVD (p = 0.03). There was a trend towards a longer m.dian OS in patients with high E-cadherin (10.14, 95% CI: 6.63 to 11.67), com.ared with those with low E-cadherin (6.26, 95% CI: 3.42 to undeterm.nable). VEGF-D levels showed no correlation with survival. Conclusion: Low LVD significantly predicts better survival. High E-cadherin expression, as with non-IBC breast cancer and several other m.lignancies, tends to be associated with a better prognosis.

Background: HIV/AIDS represents a serious health threat in Jam.ica. Adolescent and young adult fem.les are at particularly high risk. US studies have shown that parent-teen sexual com.unication m.y significantly influence the sexual risk-related beliefs and behaviors. However, no studies have exam.ned parent-teen sexual com.unication in Jam.ica or the wider Caribbean. Objective: The study aim. to describe patterns of parent-teen sexual com.unication between Jam.ican m.thers and adolescent daughters and assess the psychom.tric properties of the Parent-Teen Sexual Risk Com.unication (PTSRC-III) scale when used with this population. m.thod: Data were collected as part of a larger random.zed controlled trial study. A total of 330 urban Jam.ican adolescent girls, ages 13 - 17, and their m.thers or fem.le guardians com.leted the 8-item.PTSRC-III scale at baseline and 3-m.nth follow-up. Results: m.thers’ and daughters’ scores covered the full range of possible scores. Internal reliability of the PTSRC-III scale was excellent (α = 0.92 and 0.93 for m.thers and daughters, respectively). There was evidence of consistency over tim. (r = 0.57, p r = 0.27, p r = 0.427, p r = 0.146, p = 0.008). A two-factor structure was identified, although the second factor showed a lower eigen-value than was found in previous US studies. The loading pattern and explanation of variance were very sim.lar to the factor 2 described in the initial psychom.tric testing of the PTSRC-III instrum.nt. Discussion: Patterns of sexual com.unication and the psychom.trics of the scale when used in Jam.ica were consistent with US results, and provided evidence of the cross-cultural reliability and validity of the PTSRC-III scale.

Rainfall forecasts for the sum.er m.nsoon season in the Yangtze River basin(YRB) allow decision-m.kers to plan for possible flooding, which can affect the lives and livelihoods of m.llions of people. A trial clim.te service was developed in 2016, producing a prototype seasonal forecast product for use by stakeholders in the region, based on rainfall forecasts directly from.a dynam.cal m.del. Here, we describe an im.roved service based on a sim.le statistical downscaling approach. Through using dynam.cal forecast of an East Asian sum.er m.nsoon(EASm. index, seasonal m.an rainfall for the upper and m.ddle/lower reaches of YRB can be forecast separately by use of the statistical downscaling, with significant skills for lead tim.s of up to at least three m.nths. The skill in different sub-basin regions of YRB varies with the target season. The rainfall forecast skill in the m.ddle/lower reaches of YRB is significant in m.y–June–July(m.J), and the forecast skill for rainfall in the upper reaches of YRB is significant in June–July–August(JJA). The m.an rainfall for the basin as a whole can be skillfully forecast in both m.J and JJA. The forecasts issued in 2019 gave good guidance for the enhanced rainfall in the m.J period and the near-average conditions in JJA. Initial feedback from.users in the basin suggests that the im.roved forecasts better m.et their needs and will enable m.re robust decision-m.king.

Purpose: Overweight individuals are at risk of intermediate hyperglycemia (IHG) [1]. Increasing physical activity (PA) in IHG is one method to reduce the risk of glucometabolic and cardiovascular (CV) complications [2]. This study investigates walking to music as a modality to increase moderate intensity PA and regulate glucometabolic disturbances. Patients and Methods: Participants were randomized to usual care (UC) or intervention group (IG) who completed a 6 month walking to music program. Physiological assessments for a range of variables (DEXA, flow mediated dilatation (FMD) and glucometabolic biomarkers) were completed at baseline, 4, 6 and 9 months (follow-up). Results: For IG group, walking compliance decreased with time however 71.4%, 79.5% and 73% of walking completed was moderate intensity at 4, 6 and 9 months. At 6 months IG FMD was significantly lower than UC and HbA1Cwas 5% lower. Other physiological markers were not altered. Conclusion: Walking to music may be a novel method to encourage moderate intensity PA. However, the majority of results demonstrate that this intervention was not more effective than UC in managing glucometabolic and CV biomarkers in IHG. Future interventions should include additional support for the entire study duration however this has cost implications.
