Incidence of kidney stones in kidney transplant recipients:A systematic review and meta-analysis

AIM To evaluate the incidence and characteristics of kidney stones in kidney transplant recipients. METHODS A literature search was performed using MEDLINE, EMbASE, and Cochrane Database of Systematic Reviews from the inception of the databases through March 2016. Studies assessing the incidence of kidney stones in kidney transplant recipients were included. We applied a randomeffects model to estimate the incidence of kidney *** Twenty one studies with 64416 kidney transplant patients were included in the analyses to assess the incidence of kidney stones after kidney transplantation. The estimated incidence of kidney stones was 1.0%(95%CI: 0.6%-1.4%). The mean duration to diagnosis of kidney stones after kidney transplantation was 28 ± 22 mo. The mean age of patients with kidney stones was 42 ± 7 years. Within reported studies, approximately 50% of kidney transplant recipients with kidney stones were males. 67% of kidney stones were calcium-based stones(30% mixed CaO x/CaP, 27%CaO x and 10%CaP), followed by struvite stones(20%) and uric acid stones(13%).CONCLUSION The estimated incidence of kidney stones in patients after kidney transplantation is 1.0%. Although calcium based stones are the most common kidney stones aftertransplantation, struvite stones(also known as "infection stones") are not uncommon in kidney transplant recipients. These findings may impact the prevention and clinical management of kidney stones after kidney transplantation.

AIM： To investigate the intracellular apoptotic signals engaged by resveratrol in three gastric adenocarcinoma cancer cell lines, two of which （AGS and SNU-1） express p53 and one （KATO-M） with deleted p53. METHODS： Nuclear fragmentation was used to quantitate apoptotic cells; caspase activity was determined by photometric detection of cleaved substrates; formation of oxidized cytochrome C was used to measure cytochrome C activity, and Western blot analysis was used to determine protein expression, RESULTS： Gastric cancer cells, irrespective of their p53 status, responded to resveratrol with fragmentation of DNA and cleavage of nuclear lamins A and b and PARP. Resveratrol, however, has no effect on mitochondria-associated apoptotic proteins bcl-2, bcl- xl, bax, bid or Smac/Diablo, and did not promote sub-cellular redistribution of cytochrome C, indicating that resveratrol-induced apoptosis of gastric carcinoma cells does not require breakdown of mitochondrial membrane integrity. Resveratrol up-regulated p53 protein in SNU-1 and AGS cells but there was a difference in response of intracellular apoptotic signals between these cell lines. SNU-1 cells responded to resveratrol treatment with down-regulation of survivin, whereas in AGS and KATO- 111 cells resveratrol stimulated caspase 3 and cytochrome C oxidase activities. CONCLUSION： These findings indicate that even within a specific cancer the intracellular apoptotic signals engaged by resveratrol are cell type dependent and suggest that such differences may be related to differentiation or lack of differentiation of these cells.

The majority of patients with localized pancreatic cancer (PC) who undergo surgery followed by adjuvant therapy will develop metastatic disease, suggesting that surgery alone is not sufficient for cure and micrometastases are present even when are not clinically detected. As such, the delivery of early systemic therapy may be a rational alternative to a surgery-first approach, in an effort to provide oncologic therapies which are commensurate with the disease stage, and improve surgical selection. This review details the rationale for a neoadjuvant approach to localized PC and provides specific recommendations for both pretreatment staging and treatment sequencing for patients with resectable and borderline resectable PC.

In addition to their value as cereal grains, wheat (Triticum aestivum L.) and triticale (× Triticosecale Wittmack) are important cool-season annual forages and cover crops. Yearling steer (bos taurus) performance was compared in the spring following autumn establishment as for age cover crops after soybean [Glycine max (L.) Merr.] grain harvest. Replicated pastures (0.4 ha) were no-till seeded in three consecutive years into soybean stubble in autumn, fertilized, and grazed the following spring near Ithaca, NE, USA. Each pasture (n = 3) was continuously stocked in spring with four yearling steers (380 ± 38 kg) for 17, 32, and 28 d in 2005, 2006, and 2007, respectively. In 2005, average daily gain (ADG) for steers grazing triticale exceeded the ADG for wheat by 0.31 kghd-1d-1. In 2006, wheat ADG exceeded that for triticale by 0.12 kghd-1d-1. In 2007, steers grazing wheat lost weight, while steers grazing triticale gained 0.20 kghd-1d-1. based on the 3-year average animal gains valued at $1.32 kg-1, mean net return ($ ha-1 yr-1) was $62.15 for triticale and $22.55 for wheat. Since these grazed cover crops provide ecosystem services in addition to forage, grazing could be viewed as a mechanism for recovering costs and adds additional value to the system. based on this 3-year grazing trial, triticale was superior to wheat and likely will provide the most stable beef yearling performance across years with variable weather for the western Cornbelt USA.

This paper presents a case study that demonstrates how models can be used to support water management decisions before sufficient data to verify the model are available. In developing areas, and for new reservoirs, extensive data for model calibration and validation are often not available. As a case study we developed a CE-QUAL-W2 model of Aguamilpa Reservoir using minimal data and used the model to create a data acquisition plan to support early planning decisions. We based the model on a two-year period and compared the model results to data recently collected with our acquisition plan. We present how we developed and used the model to design the data acquisition plan which identifies and collects data to update and calibrate the model to support future decisions. We show that a minimally calibrated model based on scarce data can support management decisions and be the first step in a spiral engineering approach to system management. Spiral engineering uses models and data to both support early decisions and to iteratively improve this information to support subsequent decisions and models. This case study can be used as a guide for developing water quality models with minimal data and uses the models to support early decision requirements.

As climatic changes and human uses intensify,resource managers and other decision makers are taking actions to either avoid or respond to ecosystem tipping points,or dramatic shifts in structure and function that are often costly and hard to *** indicates that explicitly addressing tipping points leads to improved management *** on theory and examples from marine systems,we distill a set of seven principles to guide effective management in ecosystems with tipping points,derived from the best available *** principles are based on observations that tipping points(1)are possible everywhere,(2)are associated with intense and/or multifaceted human use,(3)may be preceded by changes in earlywarning indicators,(4)may redistribute benefits among stakeholders,(5)affect the relative costs of action and inaction,(6)suggest biologically informed management targets,and(7)often require an adaptive response to *** suggest that early action to preserve system resilience is likely more practical,affordable,and effective than late action to halt or reverse a tipping *** articulate a conceptual approach to management focused on linking management targets to thresholds,tracking early-warning signals of ecosystem instability,and stepping up investment in monitoring and mitigation as the likelihood of dramatic ecosystem change *** approach can simplify and economize management by allowing decision makers to capitalize on the increasing value of precise information about threshold relationships when a system is closer to tipping or by ensuring that restoration effort is sufficient to tip a system into the desired regime.
