Management of obstructed colorectal carcinoma in an emergency setting:An update

Colorectal carcinoma is common,particularly on the left *** 20%of patients,obstruction and ileus may be the first clinical manifestations of a carcinoma that has advanced(stage II,III or even IV).Diagnosis is based on clinical presentation,plain abdominal radiogram,computed tomography(Ct),Ct colonography and positron emission tomography/*** best management strategy in terms of short-term operative or interventional and long-term oncological outcomes re-mains *** the most common left-sided obstruction,the first choice should be either emergency surgery or endoscopic decompression by self-expen-dable metal stents or *** operative plan should be either one-stage or two-stage ***-stage resection with on-table bowel decompression and irrigation can be accompanied or not accompanied by proximal defunctioning stoma(colostomy or ileostomy).Primary anastomosis is more convenient but has increased risks of anastomotic leakage and ***-stage resection(Hart-mann’s procedure)is safer and the most widely used despite temporally affecting quality of *** control surgery in high-risk frail patients is less frequently performed since it can be successfully substituted with endoscopic stenting or *** the less common right-sided obstruction,one-stage surgical resection is more beneficial than endoscopic *** role of minimally invasive surgery(laparoscopic or robotic)is a subject of *** laparoscopic-assisted management is advantageous to some extent but requires much expertise due to inherent difficulties in dissecting the distended colon and the risk of rup-ture and subsequent septic *** decompressing stent as a bridge to elective surgery more substantially decreases the risks of morbidity and mortality than emergency surgery for decompression and has equivalent medium-term overall survival and disease-free survival *** combination with neoadjuvant chemotherapy or radiation may have a p

Hot spot engineering in plasmonic nanostructures plays a significant role in surface-enhanced Raman scattering(SeRS)for bioanalysis and cell ***,creating stable,reproducible,and strong SeRS signals remains challenging due to the potential interference from surrounding chemicals and locating SeRS-active analytes into hot-spot ***,we developed a straightforward approach to synthesize intra-gap nanoparticles encapsulating 4-nitrobenzenethiol(4-NBt)as a reporter molecule within these gaps to avoid outside *** made three kinds of intra-gap nanoparticles using nanorods,bipyramids,and nanospheres as cores,in which the nanorods based intra-gap nanoparticles exhibit the highest SeRS *** advantage of our method is the ease of preparation of high-yield and stable intra-gap nanoparticles characterized by a short incubation time(10 min)with 4-NBt and quick synthesis without requiring an additional step to centrifuge for the purification of core *** intense localized field in the synthesized hot spots of these plasmonic gap nanostructures holds great promise as a SeRS substrate for a broad range of quantitative optical applications.

Polycyclic aromatic hydrocarbons(PAHs)are ubiquitous environmental contaminants of growing concern due to their potential ecological and human health *** study presents a comprehensive assessment of PAHs contamination in the surface sediments of Burullus Lake,a vital and second largest delta lake in *** aim was to evaluate the eco-toxicity and potential health risks associated with the presence of these *** seven sediment samples were collected from various drains in the southern part of Burullus *** extraction method was employed to extract PAHs(16PAHs)from the sediment ***,target compounds were located using *** results showed that samples contained PAHs levels ranging from 0.038×10^(-6)to 0.459×10^(-6),which is considered heavily polluted by the european standard for PAHs ***,there was no apparent source of PAHs in the elKhashah drain or the Brinbal Canal,as HPLC found none of the *** most prevalent compound in sediment samples along the study area was *** diagnostic indices in the present study indicated that the hydrocarbons in the region originated from pyrolytic and man-made sources along the drains of Burullus *** principal component analysis(PCA)and diagnostic ratios revealed that coal combustion and pyrolytic sources were responsible for the PAHs contamination in the surface *** non-carcinogenic risk(HI),which is the product of the HQs for the adult and child populations,respectively,was *** values under 1,therefore,demonstrated that they had no carcinogenic effects on human *** and MeQs in the sediments of Burullus Lake do not have a cancer-causing impact on *** the safety of nearby wildlife,aquatic life,and people,all activities that raise petroleum hydrocarbon levels in Burullus Lake must be adequately regulated and *** to the ecological risk assessment,there is little chance that PAHs will be found

基于美国页岩油气勘探开发历程的回顾,对主要页岩盆地的地质演化、主要页岩层系的油气地质特征及勘探开发进展进行系统梳理和总结。美国重要页岩盆地的富烃页岩主要发育于6个地质时期:中奥陶世、中—晚泥盆世、早石炭世(中—晚密西西比世)、早二叠世、晚侏罗世和晚白垩世(塞诺曼期—土伦期),构造沉积环境主要包括克拉通内盆地、前陆盆地和被动大陆边缘盆地。古生界富烃页岩主要分布于以下6个盆地:阿巴拉契亚盆地(尤蒂卡页岩和马塞勒斯页岩)、阿纳达科盆地(伍德福德页岩)、威利斯顿盆地(巴肯页岩)、阿卡马盆地(费耶特维尔页岩)、沃思堡盆地(巴内特页岩)和二叠盆地(狼营组和斯帕瑞组/骨泉组页岩)。中生界富烃页岩主要发育在西墨西哥湾盆地(海恩斯维尔组和鹰滩组)和落基山盆地群(以丹佛和粉河盆地的尼尔布拉勒组页岩为主)。通过对各页岩区带的详细分析发现,页岩的岩相和矿物成分存在差异,“页岩储层”并非只是页岩,很多情况下页岩油气还产自粉砂岩和碳酸盐岩等多种岩石类型。美国的页岩油气资源丰富,页岩油地质资源量超过0.246×1012 t,页岩气地质资源量超过290×1012 m3。在开启“页岩革命”的水平井水力压裂技术出现之前,美国经过了长达20年的勘探开发实践和理论技术积累。自2007年至2023年,美国页岩油气产量从约11.2×104t/d油当量增加到超过300.0×104t/d油当量,2017年页岩油气产量超过了常规油气,2023年在油气总产量中的占比增加到60%以上。页岩油气的开发主要得益于钻完井技术的改进,“立体开发”的攻关,以及重复压裂、提高原油采收率和“U”形井等技术的应用。基于美国丰富的页岩资源基础和技术的不断进步,页岩油气的产量将继续为全美的油气生产做出更大贡献。
