a.though islet tra.spla.ta.ion for individua.s with type I dia.etes ha. been shown to yield superior blood glucose control, it rema.ns ina.equa.e for long-term control. This is pa.tly due to islet injuries a.d stresses tha. ca. lea. to beta.cell loss. Inhibition of excess IL-1β a.tivity might minimize islet injuries, thus preserving function. The IL-1 receptor a.ta.onist （IL-1Ra., a. endogenous inhibitor of IL-1β, protects islets from cytokine-induced necrosis a.d a.optosis. Therefore, a. imba.a.ce between IL-1β a.d IL-1Ra.might influence the courses of a.logeneic a.d a.toimmune responses to islets. Our group previously demonstra.ed tha. the circula.ing serine-protea.e inhibitor huma. a.pha.1-a.titrypsin （ha.T）, the levels of which increa.e in circula.ion during a.ute-pha.e immune responses, exhibits a.ti-infla.ma.ory a.d islet-protective properties, a. well a. immunomodula.ory a.tivity. In the present study, we sought to determine whether the pa.crea.ic islet a.logra.t-protective a.tivity of ha.T wa. media.ed by IL- 1Ra.induction. Our results demonstra.ed tha. ha.T led to a.2.04-fold increa.e in IL-1Ra.expression in stimula.ed ma.ropha.es a.d tha. ha.T-pre-trea.ed islet gra.ts exhibited a.4.851-fold increa.e in IL-1Ra.tra.script levels, which were a.socia.ed with a.modera.e infla.ma.ory profile. Unexpectedly, islets tha. were isola.ed from IL-1Ra.knockout mice a.d pre-trea.ed with ha.T before gra.ting into wild-type mice yielded a. increa.e in intra.ra.t IL-1Ra.expression tha. wa. presuma.ly derived from infiltra.ing host cells, a.beit in the a.sence of ha.T trea.ment of the host. Indeed, ha.T-pre-trea.ed islets genera.ed ha.T-free conditioned medium tha. could induce IL-1Ra.production in cultured ma.ropha.es. Fina.ly, we demonstra.ed tha. ha.T promoted a.distinct phosphoryla.ion a.d nuclea. tra.sloca.ion pa.tern for p65, a.key tra.scription fa.tor required for IL- 1Ra.expression.
