Biologic Augmented Microdrilling Surgery for Multiple and Large Full-thickness Cartilage Lesions in the Knee: Early Clinical and Radiological Results

Background: Various biologic treatments are available for articular cartilage lesions in the knee, but no one exists that is applicable to the full range of chondral disease and that is compliant with United States Food and Drug Administration regulations. the aim of this study was to evaluate outcomes following microdrilling surgery augmented with postoperative injections of bone marrow aspirate concentrate (BMAC), platelet rich plasma (PRP) and hyaluronic acid (HA). Methods: Eighteen patients with at least one symptomatic, full-thickness chondral lesion underwent arthroscopic microdrilling surgery. Immediately following surgery, patients received an intra-articular injection of fresh BMAC, PRP, and HA. this injection was repeated once per week for 5 consecutive weeks. At 4 and 12 months postoperatively, patients received 3 additional weekly injections for a total of 12 injections. Outcome Measures: IKDC (International Knee Documentation Committee) scores, KS (Knee Society) scores and plain radiographs. Results: the mean treated area was 6.2 ± 4.5 (range, 0.6 - 14.7) cm2. the mean preoperative IKDC and KS scores (±SE) were 43.0 ± 3.2 and 68.3 ± 3.6 respectively. At 24 months postoperatively, IKDC and KS scores improved to 85.3 ± 4.2 and 94.7 ± 4.4, respectively;both changes from baseline were significant (p p = 0.05). this change was greater in patients with preoperative JS p for the difference between groups = 0.1). Conclusions: Microdrilling of cartilage lesions augmented with BMAC, PRP, and HA may be a viable treatment for a range of chondral disease with good early clinical and radiological results.

Impacts of Forest Harvest on Active Carbon and Microbial Properties of a Volcanic Ash Cap Soil in Northern Idaho

Soil quality assessments are essential for determining impacts on belowground microbial community structure and function. We evaluated the suitability of active carbon (C), a rapid field test, as an indicator of soil biological quality in five paired forest stands (clear cut harvested 40 years prior and unharvested) growing on volcanic ash-cap soils in northern Idaho. Active C was compared with several traditional measures of soil microbial properties (microbial biomass, respiration, fungal hyphal biomass, bacterial number and biomass and PLFA community structure). Despite the significant differences in forest vegetation between paired stands, no differences in active C and only a few significant differences in microbial properties were detected. total bacteria (microscope counts) and PLFA signatures (gram positive bacteria, gram negative bacteria, actinomycetes) were significantly higher in the managed stands. Our results indicate that either mineral soil biological properties in managed stands were relatively unaffected at the time of harvest or some biological recovery occurred 40 years later. Additionally, volcanic ash-cap soils in moist ecosystems could be highly resilient to the impacts of harvest operations and therefore few significant biological changes could be detected.

Seasonal variation and living alone are related to pulmonary rehabilitation non-completion

AIM: to identify baseline characteristics that independently predict pulmonary rehabilitation non-completion and compare these findings against the participant's reasons for non-completion. MEtHODS: Participants with chronic obstructive pulmonary disease(COPD) who attended a standardised twice weekly, eight week pulmonary rehabilitation program(located in the sub-tropics, latitude 27°29' South) between 2010 and 2012 were recruited. thebaseline characteristics of program completers and non-completers were compared in a case-controlled design. Participants who attended ter. Non-completers(those who missed > 4 session of the program) were asked by one independent investigator to participate in a survey about their pulmonary rehabilitation experience. Baseline characteristics were assessed for differences between program completers and non-completers. the baseline characteristics included disease severity, exercise capacity, smoking history, participant's social support and the season when each participant commenced rehabilitation. Non-completers that agreed to participate in the survey were asked to indicate what personal factors or external factors contributed to their program non-completion. Comparisons of completers and non-completers baseline characteristics were performed using cross-tabulations and t-tests, with significant measures analysed in a multivariate binary logistic regression model. Non-completers survey responses were compared to the identified independent predictors using ***: twenty-six participants(23.4%) of the 111 participants with COPD [(mean ± SD) age was 67.4 ± 9.2 years and FEV1 54.6% ± 22.3%)], were classified as non-completers. Methods:Included in the study were 230 patients diagnosed with inoperable advanced *** these patients,76 received platinum-based chemotherapy,125 received chemotherapy plus radiation,and 29 received radiotherapy *** SNPs were genotyped using the taqMan methods. the Swiss Agency for Development and Cooperation (SDC) has funded the Rural Water and Sanitation Support Programme (RWSSP) that has increased the access to public water supply throughout Europe’s youngest state—Kosovo—in the past ten years. the Programme, implemented by Dorsch International Consultants GmbH and Community Development Initiatives has, among other activities, implemented groundwater protection methods. Nevertheless, groundwater protection remains a challenge in Kosovo. the water law describes that water source protection is similar to German rules, yet modelling-based planning of water source protection zones remains challenging. In the present study, the development of the hydrogeological and the mathematical groundwater model for the technical delineation of the well head protection area for the Ferizaj well fields under limited data availability is described in detail. the study shows that even when not all data are available, it is possible and necessary to use mathematical groundwater models to delineate well head protection areas.

Salmonella enterica Flagellin Is Recognized via FLS2 and Activates PAMP-triggered Immunity in Arabidopsis thaliana

Infections with Salmonella enterica belong to the most prominent causes of food poisoning and infected fruits and vegetables represent important vectors for salmonellosis. Recent evidence indicates that plants recognize S. enterica and raise defense responses. Nonetheless, the molecular mechanisms controlling the interaction of S. enterica with plants are still largely unclear. Here, we show that flagellin from S. enterica represents a prominent pathogenassociated molecular pattern （PAMP） in Arabidopsis thaliana, which induces PAMP-triggered immunity （PtI） via the recognition of the fig22 domain by the receptor kinase FLS2. the Arabidopsis fls2 mutant shows reduced though not abolished PtI activation, indicating that plants rely also on recognition of other S. enterica PAMPs. Interestingly, the S. enterica type III secretion system （t3SS） mutant prgH- induced stronger defense gene expression than wild-type bacteria in Arabidopsis, suggesting that t3SS effectors are involved in defense suppression. Furthermore, we observe that S. enterica strains show variation in the fig22 epitope, which results in proteins with reduced PtI-inducing activity. Altogether, these results show that S. enterica activates PtI in Arabidopsis and suggest that, in order to accomplish plant colonization, S. enterica evolved strategies to avoid or suppress PtI.
