Bulk rock geochemistry of 169 fine-grained sediment samples of the upper cretaceous to Paleogene Gosau Group （Northern calcareous Alps, Austria and Slovakia） fiom borehole and outcrop localities was performed to separate non-marine and marine deposits. Geochemical characteristics of different Gosau depositional systems, basins and sediment provenance using major-, trace-, and rare earth elements were also investigated. Geochemical proxies such as boron concentrations were tested for seeking the possibilities of paleosalinity indicators. Due to the [act that several pelagic sections are represented by extremely low boron contents, B/AI＊ ratios are recognized as more robust and differentiate reliably between marine （mean： 160 ± 34） and non-marine （mean： 133 ± 33） samples. Using statistical factor analysis, hemipelagic to pelagic samples flom the Gieghi_ibl Syncline and Slovakian equivalents can be differentiated from marginal-marine to non-marine samples from the Grtinbach and Glinzendorf Syncline related to terrigenous （SiO2, AI203, K20, Th, Rb, Zr and others） and pelagic indicative elements （caO, St, TOT/c and B/AI＊）. A clear indication for ophiolitic provenance is traced by high amounts of chromium and nickel. Only non-marine successions of the Glinzendorf Syncline show higher cr and Ni concentrations （up to 250 and 400 ppm, respectively） and enriched cr/V and Y/Ni ratios trending to an ultramafic source.

Background： The importance of non-glucose carbohydrates, especially mannose and inositol, for normal development is increasingly recognized. Whether pregnancies complicated by abnormal glucose transfer to the fetus also affect the regulation of non-glucose carbohydrates is unknown. In pregnant sheep, maternal insulin infusions were used to reduce glucose supply to the fetus for both short （2-wk） and long （8-wk） durations to test the hypothesis that a maternal insulin infusion would suppress fetal mannose and inositol concentrations. We also used direct fetal insulin infusions （1-wk hyperinsulinemic-isoglycemic clamp） to determine the relative importance of fetal glucose and insulin for regulating non-glucose carbohydrates. Results： A maternal insulin infusion resulted in lower maternal （50%, P 〈 0.01） and fetal （35-45%, P 〈 0.01） mannose concentrations, which were highly correlated （r^2 = 0.69, P 〈 0.01）. A fetal insulin infusion resulted in a 50% reduction of fetal mannose （P 〈 0.05）. Neither maternal nor fetal plasma inositol changed with exogenous insulin infusions. Additionally, maternal insulin infusion resulted in lower fetal sorbitol and fructose （P 〈 0.01）. conclusions： chronically decreased glucose supply to the fetus as well as fetal hyperinsulinemia both reduce fetal non-glucose carbohydrates. Given the role of these carbohydrates in protein glycosylation and lipid production, more research on their metabolism in pregnancies complicated by abnormal glucose metabolism is clearly warranted.

Hemoperitoneum is a rare and potentially life-threatening complication of GIST. We reported a 54- year-old man who developed disseminated intra-abdominal recurrence from a previously resected gastrointestinal stromal tumour （GIST） of the small bowel, and the patient presented with hemoperitoneum. Emergent debulking surgery was performed. A high dose imatinib was prescribed. Despite the presence of residual disease, the patient was well clinically 8 months after the operation. Even though, there is no evidence to support the routine use of debulking surgery in the management of GIST. In our patient, disease progression after second line targeted therapy and the absence of alternative treatment options for spontaneous rupture and hemoperitoneum prompted us to treat the patient aggressively. Resection of the ruptured GIST was carried out for control of bleeding and to prevent recurrent bleeding in this patient with good surgical risks. During the treatment decision-making, the patient＇s general condition, the risk of surgery and the extent of dissemination were taken into consideration. In this patient who presented with spontaneous rupture of a small intestinal GIST, the novel use of targeted therapy and aggressive surgical treatment produced reasonably good survival outcome.

＜正＞Background:The cellular functions of DNA repair and programmed cell death are believed to play an important role in controlling the fate of tumor cells in response to chemotherapy or *** has been speculated that single nucleotide polymorphisms （SNPs） in related genes may alter their expression or activity,affect the functions of DNA repair and/or apoptosis,and in turn influence the effects of cancer treatment and *** assess this possibility,we genotyped four single nucleotide polymorphisms （SNPs） in ATM （A60G）,ERccl （Asn118Asn）,APEl （Asn148Glu）,XRccl（Arg194Trp）,XRccl （Arg399Gln） and iASPP （A67T）,and examined their associations with treatment response and overall survival among patients with advanced non-small cell lung cancer （NScLc）. Methods:Included in the study were 230 patients diagnosed with inoperable advanced *** these patients,76 received platinum-based chemotherapy,125 received chemotherapy plus radiation,and 29 received radiotherapy *** SNPs were genotyped using the TaqMan methods. Associations between clinical variables,treatment response and genotype were analyzed using the chi-square *** unconditional logistic regression model was used to analyze the association between genotype and treatment response while adjusting for *** analysis was performed for overall survival using Kaplan-Meier survival curves and cox proportional hazards regression. BAcKGROUND Alcohol-related liver disease(ALD) is a leading cause of liver failure and indication for liver transplantation that arises in the setting of alcohol use disorder(AUD). Previous reviews of transplantation for ALD are limited in scope of outcomes and type of ALD studied. A comprehensive systematic review could improve use of transplantation in ALD and improve future research. We hypothesize that while transplanting ALD may improve mortality and relapse,findings will be limited by pre-specified causes of heterogeneity-assessment and treatment of AUD, definition of ALD, spectrum of ALD studied, assessment and rates of relapse, and study quality and *** To optimize liver transplantation for ALD, understanding existing research to guide future research, we conducted a systematic review with *** We conducted a systematic review, comparing liver transplant to no-transplant in patients with ALD, with a primary outcome of both short-and long-term mortality and relapse. We performed a comprehensive search of MEDLINE,EMBASE, Web of Science, and The cochrane Library databases for peer-reviewed journal articles comparing use of liver transplant in ALD to no-transplant. Two reviewers independently conducted screening, full text review, and data extraction according to the PRISMA guidelines. We report the quality of the evidence according to the GRADE *** We analyzed data from 10 studies. Of 1332 participants, 34.2%(456/1332) had undergone liver transplantation, while 65.8%(876/1332) had not. While random effects meta-analysis suggested transplant in comparison to no-transplant had an association of reduced mortality that did not reach statistical significance, relative risk(RR) = 0.51(0.25-1.05), but not relapse risk, RR = 0.52(0.18-1.53), significant heterogeneity limited these findings. Highly active antiretroviral treatment(HAART) has had a significant impact on survival of individuals with acquired immunodeficiency syndrome(AIDS);however,with the longer life-span of patients with AIDS,there is increasing prevalence of AIDS dementia complex(ADc) and other non-AIDS-defining illness,and cardiovascular diseases(cVD) are also *** influence of these varied disease processes on HIV-1 DNA concentration in brain tissues has not been thoroughly assessed in the post-HAART *** purpose of the current study is to clarify the impacts of ADc and other complications of HIV disease on the viral load in the brains in AIDS patients with *** examined autopsy specimens from the brains of thirteen patients who died from complications of AIDS with quantitative polymerase chain reaction(QPcR).All but one patient had received HAART prior to death since *** patients died with severe cVD,multiple cerebrovascular atherosclerosis(cVA) throughout the brain and five patients died with *** patients had no ADc/cVA.A QPcR was used to measure the presence of HIV-1 DNA in six brain tissues(meninges,frontal grey matter,frontal white matter,temporal subcortex,cerebellum and basal ganglia).In the post-HARRT era,for non-ADc/cVA patients,HIV-1 DNA concentration in brain tissues was statistically higher than that in patients with *** a new finding,two patients who suffered from severe cVD,especially cVA,also had high concentrations of HIV-1 in brain compartments not showing ADc related *** our knowledge,this is the first report of a relationship between the cVA and HIV-1 viral burden in *** current observations suggest that HAART-resistant HIV reservoirs may survive within ADc lesions of the brain as well as the macrophage rich atherosclerosis,which needs to be confirmed by more AIDS cases with cVA.

AIM:To assess nutrition,physical activity and health-ful knowledge in obese children with biopsy-proven nonalcoholic steatohepatitis(NASH or NA)compared to children without liver ***:children with biopsy-proven NASH comprised the NASH ***,sex and ethnicity matched control groups consisted of obese(OB)and lean(cO) children with no liver *** were adminis-tered the School Physical Activity and Nutrition Survey and one blood draw was ***:Fifty-seven patients were enrolled with a mean age of 12.1±2.1 years,and all were *** though the OB and NA had a similar increased body mass index(%),35%of the NA group always read nutrition labels compared to none in the OB(Pchildren felt their diet is"less healthy".NA consumed the least amount of fruits with only 25%having≥1 fruit/d vs 45%in OB and 64.7% in cO(PcO).Only 15%of NA subjects performed light exercise vs 35%and 59%of OB and cO groups,respectively(P=0.02).The mean physical activity score was lowest in the NA group(Pcts with NASH,we found that 100% of patients with grade 2 or 3 fibrosis had a sedentary score>2 compared to only 63.6%of those with grade 1 or no fibrosis(PcONcLUSION:children with NASH had increased sedentary behavior,decreased activity,and fruit *** studies may determine the benefit of changing these behaviors as treatment for NASH.
