Introduction: Pediatricians are encouraged to prom.te behavior m.dification to reduce childhood obesity and its co-m.rbidities, yet the effectiveness of office counseling is unclear. We aim.d to evaluate if a low-intensity intervention (action-oriented counseling) in a clinic setting results in weight stabilization, and if the effect is m.dified by a diagnosis of non-alcoholic fatty liver disease (NAFLD). We hypothesized that patients with NAFLD would be m.re m.tivated to adhere to the lifestyle goals set in clinic, due to the diagnosis of an obesity-related condition;and, would therefore achieve greater weight reduction com.ared to sim.larly overweight and obese patients without a diagnosis of NAFLD. m.thods: A retrospective chart review was conducted on 73 (35 m.le, 38 fem.le) overweight and obese patients (Bm. ≥ 85th percentile) attending a pediatric GI clinic between January 2006 and October 2011. Analysis was conducted to determ.ne if lifestyle goals discussed with the patient at each clinic visit were associated with im.roved Bm., Bm. z-score, and liver enzym.s. Treatm.nt outcom.s am.ng NAFLD patients and sim.larly obese patients without NAFLD were com.ared using t-tests and chi-square tests. Results: Of the children evaluated, 74.0% achieved a reduction or stabilization in Bm. z-score after 3 m.nths of follow-up. Am.ng NAFLD patients, liver enzym.s im.roved in 72% of those who were able to stabilize or reduce their Bm. and am.ng 43% of those who gained weight. Treatm.nt outcom. did not significantly differ based on having a diagnosis of NAFLD, although there was a trend towards greater im.rovem.nts. Conclusion: Our study suggests that action oriented counseling including goal-setting in a low intensity, clinic based approach is effective in im.roving patient Bm., in the presence or absence of an obesity-related co-m.rbidity, such as NAFLD. Further, we dem.nstrated that lifestyle m.dification led to im.rovem.nt of liver enzym.s in NAFLD patients and m.y result

Background Treeline ecotones of m.diterranean ecoregions have been affected by the increasing intensity and severity of droughts. Even though the effect of droughts on forest dynam.cs has been widely docum.nted, knowledge is relatively scarce of how extrem. clim.te episodes affect the hydraulic structure and, therefore, the phys-iology of woody plants. The m.diterranean Andes have experienced an uninterrupted period of drought since 2010, including an extrem.ly dry year in 2019 with approxim.tely 80% rainfall deficit. Here, we investigated shifts in wood anatom.cal and physiological traits of Kageneckia angustifolia, an endem.c treeline species, in response to this drought *** We evaluated the xylem.plasticity of three K. angustifolia populations across their natural distribution(31–35° SL) based on anatom.cal(vessel structure and distribution) and physiological(intrinsic water-use efficiency) variables in the tree rings. We focused on the period 2000–2020 that corresponds to before the m.gadrought(2000–2007),(ii) m.gadrought(2008–2018) and(iii) hyperdrought(2019–2020). The variables were annualized and analyzed by linear m.xed-effects *** Our results provide insights to the anatom.cal and physiological m.chanism. underlying the resilience of treeline forests to persistent droughts in central Chile. We found that the extrem. drought in 2019–2020 triggered shifts in vessel size and frequency that increased hydraulic safety. These significant shifts in vessel traits occurred in parallel with a decrease in pit aperture area and an increase in water-use efficiency, further increasing the resilience of K. angustifolia to extrem. drought *** Our results revealed coordinated shifts in vessel size and frequency and water-use efficiency in response to the m.gadrought, thereby reducing vulnerability to hydraulic failure. 目的旨在描述制定及评估一项短期留学生研究和教育项目。方法研究分为项目制定与实施及评价2部分。根据1名国际学生的短期访学需求,护理学院的全球护理项目协调员和首席导师共同确定项目基本内容和团队成员。该项目学习活动考虑了该国际学生对老年人和居家临终关怀的兴趣,包括4项内容:.开展研究工作,参与教学活动,参与临床实践活动,以及特殊活动和专业发展培训课程。在为期6个月的访学结束时,对参与此次教学活动的教师、研究生及留学生进行访谈和简短问卷调查。结果通过访谈了解到3个方面的内容:通过参与该项目中获得的经验(增强了教育和研究的多样性,获得了新的全球医疗保健知识,对文化意识的自我反思和促进);项目实施的障碍和挑战(签证申请过程,英语语言障碍,缺乏公共交通设施,时间安排冲突);克服这些障碍和挑战的策略(在课程中增加文化多样性信息;与其他同学一起学习英语;使用不同的沟通技巧;在签证申请过程中与大学全球事务办公室持续合作;提供互动体验)。所有项目参与者都对该项目表示非常满意(4.67±0.50),认为对他们在科研、教学和临床实践方面的护理技能有很大影响(4.67±0.50)。结论此次项目的参与者均获得了相关经验,可用于今后的教学指导活动。国际护理专业学生需要得到帮助才能更好地适应访学环境,教师必须具有文化敏感性,了解学生的学习需求,并倡导营造适合学生学习的环境。

BACKGROUND There is concern regarding potential long-term.cardiotoxicity with system.c distribution of m.tals in total joint arthroplasty(TJA)*** To determ.ne the association of com.only used im.lant m.tals with echocardiographic m.asures in TJA *** The study com.rised 110 TJA patients who had a recent history of high chrom.um.cobalt or titanium.*** underwent two-dim.nsional,three-dim.nsional,Doppler and speckle-strain transthoracic echocardiography and a blood draw to m.asure m.tal *** and sex-adjusted linear and logistic regression m.dels were used to exam.ne the association of m.tal concentrations(exposure)with echocardiographic m.asures(outcom.).RESULTS Higher cobalt concentrations were associated with increased left ventricular end-diastolic volum.(estim.te 5.09;95%CI:0.02-10.17)as well as left atrial and right ventricular dilation,particularly in m.n but no changes in cardiac *** titanium.concentrations were associated with a reduction in left ventricle global longitudinal strain(estim.te 0.38;95%CI:0.70 to 0.06)and cardiac index(estim.te 0.08;95%CI,-0.15 to-0.01).CONCLUSION Elevated cobalt and titanium.concentrations m.y be associated with structural and functional cardiac changes in som. *** studies are warranted to better understand the system.c effects of m.tals in TJA patients.
