Monkeypox(mpox),is a disease from the Poxviridae family that can cause several serious medical issues.This mini-review sought to analyze the existing literature regarding the current mpox outbreak with a focus on the prevalence,diagnostics,and containment measures.Mpox cases have been reported to World Health Organization(WHO)from 85 Member States in all six WHO regions during the period of January 1,2022,through August 3,2022.Standardized or optimized guidelines for the clinical care of patients with mpox are limited,particularly in low-resource settings.In an effort to achieve guidance and meet standards,special attention should be paid to this outbreak in order to eradicate such a rare infectious disease by analyzing prevention and control measures.Patient outcomes may also be poor,and their illnesses may last for a long time.The spectrum of clinical symptoms,including complications and sequelae,as well as aspects of the illness may be indicators of sickness severity and complications;therefore,its clinical presentation must be better understood to improve containment measures.In addition,it is important to create and evaluate a standard of care that takes a variety of parameters into account,including antiviral,immune therapies,and clinical metrics that are particular to mpox.The global emergence of mpox has presented new challenges for public health and has called for further investigation into its epidemiological profile across international contexts.

Infections with Helicobacter pylori and challenges encountered in Africa

Helicobacter pylori(H.pylori)is the causative agent of gastritis,peptic ulcer disease,mucosa associated lymphoid tissue lymphoma and gastric cancer(GC).While this bacterium infects 50%of the world’s population,in Africa its prevalence reach as high as 80%as the infection is acquired during childhood.Risk factors for H.pylori acquisition have been reported to be mainly due to overcrowding,to have infected siblings or parent and to unsafe water sources.Despite this high H.pylori prevalence there still does not exist an African guideline,equivalent to the Maastricht V/Florence Consensus Report of the European Helicobacter and Microbiota Study Group for the management of this infection.In this continent,although there is a paucity of epidemiologic data,a contrast between the high prevalence of H.pylori infection and the low incidence of GC has been reported.This phenomenon is the so-called“African Enigma”and it has been hypothesized that it could be explained by environmental,dietary and genetic factors.A heterogeneity of data both on diagnosis and on therapy have been published.In this context,it is evident that in several African countries the increasing rate of bacterial resistance,mainly to metronidazole and clarithromycin,requires continental guidelines to recommend the appropriate management of H.pylori.The aim of this manuscript is to review current literature on H.pylori infection in Africa,in terms of prevalence,risk factors,impact on human health,treatment and challenges encountered so as to proffer possible solutions to reduce H.pylori transmission in this continent.

Background Anthrax is a non-contagious zoonotic disease caused by the Gram-positive,spore-forming bac-terium Bacillus anthracis.Infection is common in livestock and wild animals such as cattle,goats,sheep,camels,and antelopes.In humans,anthrax may occur after contact with contaminated carcasses or animal products like milk and meat.The best method to prevent anthrax in people is to ensure livestock are vaccinated,which significantly limits the risk of zoonotic spread to humans.However,the rate of vaccination of domesticated animals kept by nomadic pastoralists in West Africa is low.These groups regularly cross over national boundaries with their grazing herds.Nigeria is a country that historically has done comparatively well to contain this public health threat.However,in 2023 several outbreaks of human disease appear linked to the consumption of anthrax-contaminated animal products brought into Nigeria by pastoralists from neighboring countries.Clinical manifestations include skin sores or ulcers,nausea,vomiting,and fever.This article aims to raise awareness of recent outbreaks of anthrax in West Africa and to callfor a renewed focus on measures to combat this neglected public health concern to the region.Main body The imperative to pinpoint pivotal issues relating to the ongoing emergence of anthrax cases in Nigeria cannot be overstated.By delving into the prevalence of anthrax in both livestock and human populations residing along Nigeria's borders,unraveling the genetic diversity and potential sources of B.anthracis strains,and identify-ing the primary animal host(s)responsible for transmission,we stand to enhance our understanding of this critical issue.Furthermore,investigating the multifaceted factors contributing to anthrax transmission,assessing community knowledge and practices,mapping common migratory routes of pastoralists,and formulating targeted interven-tion strategies tailored to the challenges of border communities,are each crucial steps towards effective control andprevention.Conclusion Closing

Omicron,the severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)variant that is now spreading across the world,is the most altered version to emerge so far,with mutations comparable to changes reported in earlier variants of concern linked with increased transmissibility and partial resistance to vaccineinduced immunity.This article provides an overview of the SARS-CoV-2 variant Omicron(B.1.1.529)by reviewing the literature from major scientific databases.Although clear immunological and clinical data are not yet available,we extrapolated from what is known about mutations present in the Omicron variant of SARS-CoV-2 and offer preliminary indications on transmissibility,severity,and immune escape through existing research and databases.

Rabeprazole, clarithromycin, and amoxicillin Helicobacter pylori eradication therapy: Report of an efficacy study

AIM:To investigate the efficacy of a standard triple therapy(comprising rabeprazole,clarithromycin,and amoxicillin)for Helicobacter pylori(H.pylori)eradication,noting factors that influence the outcome and documenting any adverse events.METHODS:Following institutional ethical approval,fifty consecutive and consenting symptomatic patients with evidence of H.pylori infection by either a positive urea breath test(UBT)and/or a campylobacter-like organism test who presented to the Gastroenterology clinic of Lagos State University Teaching Hospital between 2012 and 2013 were recruited into the study.Patients were openly randomized to either a 7-d or a 10-d regimen of amoxicillin 1 g,clarithromycin 500mg and rabeprazole 20 mg twice daily.The extent of symptom resolution was noted following the treatment,and at the end of one month after the completion oftreatment,a repeat UBT was performed in each patient to document the eradication of the infection.All data(demographics,symptoms,and eradication rates)were collated and analyzed with SPSS version 18.RESULTS:Forty-seven patients completed the study(three were excluded from the analysis for breaching the study protocol).The patients included 18 males and 29 females within the age range of 13-80 years(mean 43.7,SD 16.8).The clinical features of the study subjects were dyspepsia,reflux symptoms and features of gastrointestinal bleeding.The average eradication rate was 87.2%.Eighteen subjects were enrolled in the 7-d arm,while 29 were in the 10-d arm.There was no statistically significant difference in the age or sex distributions of the two arms.There was no significant advantage of the 10-d treatment duration over the 7-d duration(P=0.78),and the eradication outcomes were not influenced by the gender or age of the subjects.No adverse effects were reported in either arm.CONCLUSION:The triple therapy regime,employing a combination of amoxicillin,clarithromycin and rabeprazole,showed great efficacy and safety in the eradication of H.pylori,and this outcome was not influenced b

Survey of food handlers in bukas (a type of local restaurant) in Lagos, Nigeria about typhoid fever

Purpose: The study was conducted to survey the knowledge and behavioural practices of food handlers in bukas (a type of local restaurant) in Nigeria with the aim of assessing the hygiene practices of food handlers and whether they were knowledgeable about typhoid fever and its mode of transmission. Methods: One hundred and seventy four (174) Respondents were ad- ministered questionnaires on their sociode- mographic characteristics, behavioural practices and knowledge of typhoid fever. Results: Ma- jority of the food handlers drank pure water (32.1%), borehole water (32.6%) and public tap water (31.1%) at the about the same frequecy. More than half (62.2%) washed their hands with water only before eating while 27.7% did not wash their hands always before preparing food. After using toilets, 71.9% washed their hands with soap and water while 28.1% washed their hands with only water. When asked if they had heard about typhoid fever 90% said they had heard, out of which15.6% did not know how it was contracted while the others had partial knowledge. Conclusion: Food handlers play a prominent role in the transmission of typhoid fever and so it is important that the food handlers are well informed about their hygiene status and the causes of typhoid fever trans- mission and ways by which typhoid fever spread is prevented. This will go a long way to help reduce the incidence of typhoid fever in the country.
