Background: Skin reactions and musculoskeletal injuries caused by intramuscular injection are an increased risk for skin and soft tissue infections and may culminate in sepsis. Objective: To describe the history of an elderly patient who developed a lesion in her right arm after home administration of intramuscular iron injection in this arm and its outcome. Methods: This is a case report of a patient who participated in a study in an intensive care unit. Results: This study describes a 65-year-old patient with multiple comorbidities who was admitted to the hospital complaining of pain for two months and difficulty in moving her right shoulder associated with skin and soft tissue infections in her right arm after intramuscular iron administration by a relative for the treatment of multifactorial anemia and refractory melena. The patient worsened her general condition and was transferred to the intensive care unit. Despite the therapy instituted, the patient developed sepsis of cutaneous origin, multiple organ dysfunction, and death. Conclusions: The administration of intramuscular medication requires the participation of qualified professionals, such as nurses, mainly within the scope of the Brazilian public health system which commonly comprises patients in a context of vulnerability.

Tuberculosis is one of the 10 leading causes of death in the world. Its symptoms include fever, malaise, weakness, weight loss, chest pain, cough, expectoration, shortness of breath and sepsis, which is its most frequent complication. Due to these symptoms, many patients with tuberculosis require admission to the Intensive Care Unit, where they usually are placed on mechanical ventilation. Tuberculosis is more prevalent in the population in situations of social and economic vulnerability. The main factors that interfere with adherence to treatment and the prognosis of these patients are the patient’s support network and their socioeconomic status. We present the following report of a long-time smoker patient, with chronic kidney disease and previous treatment for tuberculosis, who was brought to the emergency room due to lowering of the sensorium, and was immediately placed on mechanical ventilation. The tests suggested sepsis with a respiratory focus, so a search for Alcohol-Acid-Resistant Bacillus was carried out, with a positive result. The patient was transferred to the Intensive Care Unit due to tachycardia and acute respiratory failure. The patient was a smoker with a high tobacco load, chronic kidney disease, in addition to precarious economic, educational and self-care conditions. This report shows that patients with tuberculosis and its complications can be stabilized through known pharmacological treatment. However, the most effective measure to interrupt the transmission of the disease remains the early diagnosis and the provision of adequate treatment, with guidelines aimed at the cessation of harmful habits, such as smoking.

Multidimensional Pain Inventory(MPI):Adaptation to Orthodontic Patients and Its Psychometric Properties

Background: Pain perception may result from orthodontic treatment and should be considered in clinical management. Its assessment should not be limited to the intensity of perceived pain but should also consider the psychosocial and behavioral aspects involved. Objective: The aims of this study were to adapt and validate the Multidimensional Pain Inventory (MPI) for orthodontic patients, and present a proposal to calculate an overall score of the pain-related aspects and compare these scores with the usual methodology used. Methods: Face validity of the MPI was estimated and orthodontic version (MPI-Orthodontic) was developed. Factorial validity was evaluated by confirmatory analysis. Convergent and discriminant validity and reliability were estimated. The fit of Second-Order Hierarchical Models was estimated. Concurrent validity of MPI-Orthodontic was evaluated against the Visual Analogue Scale. Invariance of the factorial models was evaluated for independent samples and according to sex and age. Overall score was calculated using the matrix of regression weights and compared to simple arithmetic mean. Results: 507 individuals (63.3% women;age: 26.32 (SD = 11.70) years) participated. For the fit of Part I (psychosocial aspects) of the MPI-Orthodontic, it added correlation between two items and excluded one item;for Part II (behavioral aspects) two items were excluded. The models presented adequate fit to the sample. Reliability was adequate. MPI-Orthodontic presented invariance for independent samples and adequate concurrent and divergent validity. Score obtained with the simple arithmetic mean was overestimated for Part I and underestimated for Part II. Conclusion: MPI-Orthodontic was valid, reliable and invariant for the evaluation of the orthodontic pain. It is recommended to calculate overall weighted scores for pain assessment.

<i>In Vitro</i>Priming of Sugarcane Varieties (RB966928 and RB867515)

Sugarcane, as a glycophyte, shows sensitivity to saline soils at various stages of its growth. The aim of this study was to evaluate the in vitro priming response in two sugarcane varieties (RB966928 and RB867515). Micropropagated plants, from meristems, received priming treatments by adding the salts (NaCl and KCl) in different concentrations (0.0;12.5;25.0 and 50.0 mM), in the MS medium. Subsequently, the plants were cultivated in rooting medium without addition of salts, acclimatized and submitted to gradient ex vitro saline stress with 20 → 40 and 60 mM, of each salt, for 30 days. The analyzed variables were dry matter of shoot and root, number of tillers and estimation of chlorophyll content. The experiment was carried out in a 2 × 2 × 4 factorial arrangement, in a completely randomized experimental design. Twenty replicates were used throughout the experiment. Data were submitted to analysis of variance and regression and the means were compared by Tukey’s test, at a 5% probability level. The priming treatments presented a significant effect, with triple interaction, in the chlorophyll index. In the treatment with NaCl, the variety RB966928 showed an increase in the chlorophyll index with the increase of treatment levels, up to an optimal limit of 31.47 mM. On the other hand, the variety RB867515 showed decreasing in chlorophyll index. In contrast, in KCl treatment, the variety RB867515 presented the increase at the chlorophyll index with the maximum point of 25 mM. For the variables, shoot dry matter (SDM) and root dry matter (RDM) there was a significant difference (p 0.05 and p 0.01, respectively) only between the varieties. The variety RB966928 presented higher SDM and RDM in relation to the variety RB867515. Studies are recommended with increasing the duration of the priming treatments and more detailed study of the culture throughout its productive cycle.

Survival rates according to barcelona clinic liver cancer sub-staging system after transarterial embolization for intermediate hepatocellular carcinoma

AIM: To investigate the survival rates after transarterial embolization(TAE).METHODS: One hundred third six hepatocellular carcinoma(HCC) patients [90 barcelona clinic liver cancer(BCLC) B] were submitted to TAE between August 2008 and December 2013 in a single center were retrospectively studied. TAE was performed via superselective catheterization followed by embolization with polyvinyl alcohol or microspheres. The date of the first embolization until death or the last follow-up date was used for the assessment of survival. The survival rates were calculated using the Kaplan-Meier method, and the groups were compared using the log-rank ***: The overall mean survival was 35.8 mo(95%CI: 25.1-52.0). The survival rates of the BCLC A patients(33.7%) were 98.9%, 79.0% and 58.0% at 12, 24 and 36 mo, respectively, and the mean survival was 38.1 mo(95%CI: 27.5-52.0). The survival rates of the BCLC B patients(66.2%) were 89.0%, 69.0% and 49.5% at 12, 24 and 36 mo, respectively, and the mean survival was 29.0 mo(95%CI: 17.2-34). The survival rates according to the BCLC B sub-staging showed significant differences between the groups, with mean survival rates in the B1, B2, B3 and B4 groups of 33.5 mo(95%CI: 32.8-34.3), 28.6 mo(95%CI: 27.5-29.8), 19.0 mo(95%CI: 17.2-20.9) and 13 mo, respectively(P = 0.013).CONCLUSION : The BCLC sub-stagingsystem could add additional prognosis information for postembolization survival rates in HCC patients.

Protective Role of Golden Flaxseed (Linum usitatissimum L.) Against Oxidative Damage in Lipids and Proteins of Healthy Volunteers

Flaxseed (Linum usitatissimum L.) is composed mainly of bioactive components such as polyphenols, polyunsaturated fatty acids, fiber and lignans. Flaxseed can be found in different presentation forms (grain or flour) and varieties (brown or golden);however, questions have arisen as to whether the presentation form and/or variety may influence the health effects. The objective of this study was to evaluate the effects on blood pressure, anthropometric and oxidative parameters in healthy human volunteers. All subjects received 40 gram aliquots of flaxseed and were instructed to consume them in their entirety mixed with water in the morning for a period of 14 days. Oxidative parameters showed significant reductions (p < 0.05) in oxidative damage to lipids and proteins via dietary intervention with golden flaxseed grains. There were no significant differences in anthropometric parameters, blood pressure, DNA damage and micronuclei frequency after 14-day supplementation. This research indicates that golden flaxseed grains can be a valuable adjunct for disease prevention and protecting the organism against oxidative damage.

Photodynamic Therapy for Medication-Related Osteonecrosis of the Jaws: A Case Report

Medication-related osteonecrosis of the jaws (MRONJ) is a relatively new disease. MARX reported first cases in 2003. MRONJ relates to oral and parenteral bisphosphonates as well as to the so-called target cancer therapies but the list of medications only grows. Although MRONJ is a relatively rare condition, it can be associated to significant morbidity with feeding limitations and intense pain. More severe cases can lead to potentially life-threatening infections. Every patient initiating bisphosphonate and/or target cancer therapy must visit a dentist before starting medication because preventive measures for MRONJ are much more effective compared to surgical management of the lesions. Surgical resolution can be especially difficult to obtain in the coexistence of certain complication factors like wider bone exposures, history of nitrogen containing bisphosphonates use (mainly zolendronate) and immunodeficiency. Recently, researchers have given attention to laser therapy associated to photosensitive agents as a possible option to management of some MRONJ lesions. Our case report demonstrates the use of photodynamic therapy in a denosumab related lesion in the mandible. It seems that denosumab related lesions are more amenable to treatment and total resolution because of the marked differences between its chemical and metabolic characteristics when compared to bisphosphonates.

SPECIFICITY OF MANUAL ACUPUNCTURE NEEDLING ON PAIN CONTROL:WHAT IS THE DIFFERENCE BETWEEN BL60, SP6 AND A NON-ACUPOINT?

Background: In the past three decades researchers have focused on studying the analgesia induced by various forms of electroacupuncture ***, physiological mechanisms of manual acupuncture remain *** this study, the mouse model of acetic acid-induced writhing was used to investigate the possible specificity of manual acupuncture needling in modulating different endogenous pain control ***: Swiss mice were treated with manual acupuncture before administration of intraperitoneal injection of acetic *** needling was performed unilaterally in one acupoint of the mice lower limb(BL60-sural nerve or SP6-tibial nerve or non-acupoint-inferior gluteal nerve).To study the possible endogenous pain control systems involved in the antinociceptive effect induced by acupoint stimulation, animals were pre-treated with antagonists of opioid, serotonergic, adrenergic and dopaminergic ***: We found that the non-acupoint point did not reduce the number of writhes of the mice compared to the control group(not treated).In other hand, acupuncture needling of BL60 and SP6 reduced the number of writhes in 70%±7% and 59%±7%, *** induced by BL60 needling was abolished by pre-treating the animals with naloxone(nonselective opioid antagonist) and WAY100635(5HT1A serotonergic antagonist), but was not altered by ketanserin(5HT2A serotonergic antagonist), ondansetron(5HT3 serotonergic antagonist), yohimbine(α2-adrenergic antagonist) and sulpiride(D2-dopaminergic antagonist).Antinociception induced by SP6 needling was abolished by pre-treating the animals with sulpiride but was not altered by naloxone, yoimbine, WAY100635, ketanserin and ***: These results indicate that the stimulus to acupoints analogous to BL60 and SP6 at the lower limb can reduce the pain-associated behavior in mice, while the non-acupoint point ***, our findings suggest that there is point specificity not only in re
