BACKGROUND Esophageal perforation or postoperative leak after esophageal surgery remain a life-threatening *** optimal management strategy is still *** To determine clinical outcomes and complications of our 15-year experience in the multidisciplinary management of esophageal perforations and anastomotic *** A retrospective single-center observational study was performed on 60 patients admitted at our department for esophageal perforations or treated for an anastomotic leak developed after esophageal surgery from January 2008 to December *** outcomes were analyzed,and complications were evaluated to investigate the efficacy and safety of our multidisciplinary management based on the preservation of the native or reconstructed esophagus,when *** Among the whole series of 60 patients,an urgent surgery was required in 8 cases due to a septic ***-six patients were managed by endoscopic or hybrid treatments,obtaining the resolution of the esophageal leak/perforation without removal of the native or reconstructed *** mean time to resolution was 54.95±52.64 days,with a median of 35.5 *** severe complications were *** patients out of 56(17.9%)developed pneumonia that was treated by specific antibiotic therapy,and in 6 cases(10.7%)an atrial fibrillation was *** patients(12.5%)developed a stricture within 12 months,requiring one or two endoscopic pneumatic dilations to solve the *** was 1.7%.CONCLUSION A proper multidisciplinary approach with the choice of the most appropriate treatment can be the key for success in managing esophageal leaks or perforations and preserving the esophagus.

Time reversal of a discrete system coupled to a continuum based on non-Hermitian flip

Time reversal in quantum or classical systems described by an Hermitian Hamiltonian is a physically allowed process, which requires in principle inverting the sign of the Hamiltonian. Here we consider the problem of time reversal of a subsystem of discrete states coupled to an external environment characterized by a continuum of states, into which they generally decay. It is shown that, by flipping the discrete-continuum coupling from an Hermitian to a non-Hermitian interaction, thus resulting in a non unitary dynamics, time reversal of the subsystem of discrete states can be achieved, while the continuum of states is not reversed. Exact time reversal requires frequency degeneracy of the discrete states,or large frequency mismatch among the discrete states as compared to the strength of indirect coupling mediated by the continuum. Interestingly, periodic and frequent switch of the discrete-continuum coupling results in a frozen dynamics of the subsystem of discrete states.

A flexible multi-body model of a surface miner for analyzing the interaction between rock-cutting forces and chassis vibrations

The discontinuous nature of rock cutting can easily cause unwanted vibrations in the structure of a surface *** these vibrations are not properly addressed,the related stress cycles can gradually damage the chassis resulting in fatigue *** events can seriously undermine the safety of operators and digging operations may be stopped for days,with an obvious economic *** work presents an analysis of the dynamics of a surface miner,focusing on the interaction between cutting machine dynamics and cutting forces,which is a new approach for this type of *** this purpose,the authors developed a numerical model of the cutting process made up of(1)a multi-body model of the cutting machine,which takes into account the chassis's flexibility;(2)a model of the rotating cutting head;and(3)a model of the interaction between the cutting head and rock,based on Shao's *** model was compared with experimental results and then used to investigate the effects of cutting speed and cutting depth on the machine dynamics.

Towards higher impact velocities,ballistic events are increasingly determined by the material *** effects might range from moderate thermal softening to full phase *** particular,it is of great interest to quantify the conditions for incipient or full melting of metals during impact interactions,which result in a transition from still strength-affected to hydrodynamic material *** this work,we investigate to which extent the respective melting thresholds are also dependent on the initial,and generally elevated,temperatures of projectiles and targets before *** is studied through the application of a model developed recently by the authors to characterize the transition regime between high-velocity and hypervelocity impact,for which the melting thresholds of materials were used as the defining *** obtained results are expected to be of general interest for ballistic application cases where projectiles or targets are *** conditions might result,for example,from aerodynamic forces acting onto a projectile during atmospheric flight,explosive shapedcharge-jet formation or armor exposure to environmental *** performed analyses also broaden the scientific understanding of the relevance of temperature in penetration events,generally known since the 1960s,but often not considered thoroughly in impact studies.

The Earth and Space Sciences Informatics division of European Geosciences Union(EGU)and the Open Geospatial Consortium jointly organised a special event entitled:‘Implementation of international geospatial standards for earth and space sciences event’at the EGU General Assembly meeting held in Vienna,April *** event objectives included:(a)to discuss the integration of information systems from different geosciences disciplines;(b)to promote and discuss the present process to scale from specific and monolithic systems towards independent and modular enabling infrastructuresforming an earth system science(ESS)infrastructure;and(c)to show some of the latest advances in implementing open *** manuscript introduces the event motivations and describes the abstract and holistic framework,which can be used to situate the topics and the developments presented by the event *** manuscript introduces important,and relatively new technologies to build a multi-disciplinary geosciences information system:the System of Systems approach and the Model Driven *** achieve that,three important information infrastructure categories are recognised:(a)ESS information infrastructure;(b)geospatial information infrastructure;and(c)distributed information *** Earth should support the discussed framework to accelerate information transfer from theoretical discussions to applications,in all fields related to global climate change,natural disaster prevention and response,new energy-source development,agricultural and food security,and urban planning and management.

Hubbard U through polaronic defect states

Since the preliminary work of Anisimov and co-workers,the Hubbard corrected DFT+U functional has been used for predicting properties of correlated materials by applying on-site effective Coulomb interactions to specific ***,the determination of the Hubbard U parameter has remained under intense discussion despite the multitude of approaches ***,we define a selection criterion based on the use of polaronic defect states for the enforcement of the piecewise linearity of the total energy upon electron occupation.A good agreement with results from piecewise linear hybrid functionals is found for the electronic and structural properties of polarons,including the formation *** values of U determined in this way are found to give a robust description of the polaron energetics upon variation of the considered *** particular,we also address a polaron hopping pathway,finding that the determined value of U leads to accurate energetics without requiring a configurational-dependent *** is emphasized that the selection of U should be based on physical properties directly associated with the orbitals to which U is applied,rather than on more global properties such as band gaps and band *** comparison,we also determine U through a well-established linear-response scheme finding noticeably different values of U and consequently different formation *** origins of these discrepancies are *** case studies,we consider the self-trapped electron in BiVO_(4),the self-trapped hole in MgO,the Li-trapped hole in MgO,and the Al-trapped hole inα-SiO_(2).
