Boswellia resin extract and propolis derived polyphenols in patients with type III chronic prostatitis/chronic pelvic pain syndrome:An Italian prospective multicenter study

Objective:To assess the efficacy and safety of a treatment regimen based on rectal administration of Boswellia resin extract and propolis derived polyphenols in patients with type IIIa and type IIIb chronic prostatitis and chronic pelvic pain syndrome(CP/CPPS).Methods:Patients with type IIIa and type IIIb CP/CPPS received one rectal suppository a day for 15 days per month for 3 consecutive months.Participants were evaluated with National Institutes of Health-Chronic Prostatitis Symptom Index(NIH-CPSI),the International Prostate Symptom Scores(IPSS),International Index of Erectile Function(IIEF),four-glass test,uroflowmetry,and prostate-specific antigen assessments at baseline and at Week 4,and Week 12.Primary endpoints were improvement in pain domain of NIH-CPSI and improvement of NIH-CPSI total score.Secondary outcomes included improvement of micturition and quality of life(QoL)domains of NIH-CPSI questionnaire.Results:A total of 61 males were enrolled.No adverse events were reported.Significant improvements from baseline to Day 30 were reported for NIH-CPSI total score(mean difference:-9.2;p<0.01),NIH-CPSI pain domain(mean difference:-5.5;p<0.01),NIH-CPSI micturition domain,NIH-CPSI QoL domain,and IPSS total score(mean difference:-5.6;p<0.01).No significant changes from baseline in terms of IIEF score or maximum flow rate were observed.At final follow-up(Day 90),further significant improvements in terms of NIH-CPSI total score(mean difference:-12.2;p<0.01),NIH-CPSI pain domain(mean difference:-6.6;p<0.01),NIH-CPSI micturition domain,NIH-CPSI QoL domain,and IPSS total score were reported.Conclusion:Rectal administration of Boswellia resin extract and propolis derived polyphenols is well tolerated and delivers a significant symptomatic improvement in most patients with type IIIa and type IIIb CP/CPPS.

Red wine and green tea reduce H pylori- or VacA-induced gastritis in a mouse model

AIM: To investigate whether red wine and green tea could exert anti-H pylori or anti-VacA activity in vivo in a mouse model of experimental infection.METHODS: Ethanol-free red wine and green tea concentrates were administered orally as a mixture of the two beverages to H pylori infected mice, or separately to VacA-treated mice. Gastric colonization and gastric in? ammation were quantifi ed by microbiological, histopathological, and immunohistochemical analyses. RESULTS: In H pylori -infected mice, the red wine and green tea mixture significantly prevented gastritis and limited the localization of bacteria and VacA to the surface of the gastric epithelium. Similarly, both beverages significantly prevented gastric epithelium damage in VacA-treated mice; green tea, but not red wine, also altered the VacA localization in the gastric epithelium. CONCLUSION: Red wine and green tea are able to prevent H pylori -induced gastric epithelium damage, possibly involving VacA inhibition. This observation supports the possible relevance of diet on the pathological outcome of H pylori infection.

Improvement of Somatic Cervical Pain and Disability after the Application of a Portable TENS Device

Background: Cervical facet joints and neck muscles are common nociceptive pain generator, with neck and shoulder muscles pain, and limited retroflexion. Objective: To test the hypothesis that the portable TENS device would relieve cervical somatic pain. Methods: Forty-four patients with chronic cervical pain patients with somatic pain, but without radicular symptoms were evaluated in a double-blind, prospective, randomized fashion, divided into sham and active groups. The active TENS (TANYXò) or sham device was placed over C7-T1 spinous process, perpendicular to the spine, for 20 min at 12-hour interval during 3 days. The two groups were: placebo group (PG), with a sham device and the active TENS group (TG), which produced a mixed (85 Hz) frequency of stimulation, conventional, and burst. Diclofenac up to three times daily was available. Efficacy measures were pain relief, rescue analgesics and neck disability. Results: The active TENS device induced pain relief after its first application, which persisted during the 3-day treatment. By the end of the TENS application, the capability of rotation, lateral extension and retroflexion was improved (p < 0.05). The pain score and rescue analgesics consumption reduced in the TG (p < 0.01, p < 0.05, respectively), and the mean pain score dropped from 8 to 3 points (p < 0.01). There were no adverse events. Conclusions: Somatic cervical pain and disability improved after active TENS application during the three consecutive days, which persisted upon the 1-month reevaluation.

The Antinociceptive Effect of Genicular Nerves Block Compared to Intra-Articular Dexamethasone in Grade III or IV Knee Osteoarthritis

Introduction: Knee pain secondary to osteoarthritis is a common pathology reflecting on the capacity of undertaking daily activities. Among non-surgical procedures, intra-articular corticosteroids represent an alternative complementary therapy. However, blocking the genicular nerves is a recently described technique, easily applied for clinical management. Objectives: To compare efficacy of both intra-articular corticosteroid and genicular block for chronic knee pain. Methods: 20 patients with osteoarthritis were evaluated for: 1) the intensity of pain;2) the quality of sleep;3) the capacity to undertake daily activities. Two groups were formed randomly, the first submitted to intra-articular blocking and the second to blocking the genicular nerves. A solution of plane 90 mg lidocaine 1%, and dexamethasone 10 mg was standard and used for intervention in both procedures to a final 10-ml volume. After a weekly appraisal for 12 consecutive weeks, the patients were recalled and submitted to another proposed procedure and in this way, all the patients acted as their own control (a “crossover” study design). In the following weeks, as also in the pretest, the intensity of pain, the quality of sleep and the capacity of undertaking daily activities were evaluated. Results: Both the intra-articular block and the genicular nerves block resulted equally in important reduction of pain during 11 weeks, with similar improvement in the quality of night sleep, and in the capacity of daily activities (p < 0.05). Conclusions: The blocking of the genicular nerves was a safe alternative, minimally invasive and highly efficient, similar to the intra-articular corticosteroid.

Comparison of Botox^(█) or Prosigne^(█) and Facial Nerve Blockade as Adjuvant in Chronic Migraine

Background: The treatments suggested for chronic migraine (CM) include: 1) intramuscular (im) botulinum toxin (BTX) every 12 weeks, and 2) blockade of peripheral nerves of the head. The present study evaluated the efficacy of facial nerve blockade in combination with a single administration of different low BTX. Methods: Forty patients with CM submitted to unilateral facial nerve blockade (supraorbital, supratrochlear and auriculotemporal) were divided into 4 randomized groups in a double-blind manner in order to receiveim, after 7 days: 25 IU Botox®(Botox group), 25 IU Prosigne®(25-Pro group), 33.3 IU Prosigne®(33-Pro group) or saline (control group), with the dose divided for application to 10 sites in the frontal and bilateral temporal regions. Analgesia and adverse effects were evaluated: 1) before blockade of the facial nerves and 2) 4 weeks, 3) 8 weeks and 4) 12 weeks after BTX-A or saline application (HC clinical trial no. 12465). Results: Botox®(25 IU) or Prosigne®(33.3 IU) resulted in at least seven-day intervals between headache attacks associated with 70% reduction in frequency and intensity of crises over 12 weeks (P ®resulted in 8 weeks of analgesia. The conversion factor between Botox®and Prosigne®was 1:1.3. Conclusions: Im application of Botox®(25 IU) or Prosigne®(33.3 IU) one week after nerve blockade on the painful side was equally effective for 12 weeks in patients with chronic daily headache, with the conversion factor between Botox®and Prosigne®being 1:1.3.

Spinal Cord Stimulation for Refractary Neuropathic Pain and Its Action on the Neurovegetative Systems

BACKGROUND: Spinal Cord Stimulation (SCS) has been used for chronic refractory pain, however its effectivity on neurovegetative systems has not been evaluated to data. OBJECTIVES: The study was conducted to evaluate the effect of SCS in patients suffering from refractory neuropathic pain, related to analgesia, quality of life and neurovegetative actions on the digestive and urinary tracts. Fifteen patients acted as their own control related to all evaluations. P < 0.05 was considered significantly. RESULTS: 13 patients completed the study. The incidence of indication was 6% in our Center for Pain Treatment in the Teaching Hospital. The SCS improved quality of life, bowel transit and urinary function, as improved overall analgesia and decreased daily rescue analgesic (p < 0.001). CONCLUSIONS: In accordance with literature, the incidence of complications was 20%, and SCS improved analgesia and quality of life. In addition, patients also referred to improved bowel activity and urinary function.

INTRAOCULAR PRESSURE DECREASE IN SURGICAL TREATMENT FOR CONGENI-TAL GLAUCOMA

Purpose: To evaluate results of intraocular pressure decrease in surgical procedures for congenital glaucoma We reviewed our experience with goniotomy, trabeculotomy, trabeculectomy and the combined procedure during the last 23 years. Methods: Pre-operative and postoperative intraocular pressures concerning 104 eyes (64 patients) that underwent goniotomy (45 eyes), trabeeu-lotoray (25 eyes), trabeculectomy alone (15 eyes) or a combined trabeculotomy-trabeculectomy (19 eyes) as a primary surgical procedure for congenital glaucoma were recorded. Follow up ranged from 1 month to 77 months (mean: 6.4 months). Intraocular pressure was measured by Schiotz or hand-held applanation tonometry with the patient under anesthesia Results: Mean pre-treatment intraocular pressure in 104 eyes was 27.3 (SD 5.3) mmHg, decreasing to 19.9 (SD 7.5) mmHg following treatment with a single surgical procedure. Combined trabeculotomy-trabeculectomy and trabeculectomy alone were the most successful as the initial operation, decreasing mean intraocular pressure to 15.2 (SD 6.4) and 16.7 (SD 9.2)mmHg, respectively. Surgical results with goniotomy and trabeculotomy, decreased mean intraocular pressure to 21.0 (SD 6.2) and 23.6 (SD 7.1) mmHg, respectively. Conclusion: Our results indicate that trabeculectomy (either alone or combined with trabeculotomy) is an efficient surgical procedure to control of intraocular pressure in primary congenital glaucoma. With goniotomy or trabeculotomy we had disappointing surgical results.
