Neoadjuvant chemoradiotherapy for esophageal cancer:Impact on extracapsular lymph node involvement

AIm: To evaluate the role of CDX2 homeobox protein as a predictor for cancer progression and prognosis as well as its correlation with mUC2 expression. CDX2 represents a transcription factor for various intestinal genes (including mUC2) and thus an important regulator of intestinal differentiation, which could previously be identified in gastric carcinomas and intestinal metaplasia. mETHODS: Formalin-fixed and paraffin-embedded tissues from 190 gastric carcinoma patients were stained with monodonal antibodies recognizing CDX2 and mUC2, respectively. Immunoreactivity was evaluated semiquantitatively and statistical analyses including x2 tests, uni- and multi-variate survival analyses were performed. RESULTS: CDX2 was mostly expressed in a nuclear or supranuclear pattern,whereas mUC2 showed an almost exclusive supranuclear *** antigens were present in >80% of areas exhibiting intestinal metaplasia. An immunoreactivity in >5% of the tumor area was observed in 57% (CDX2) or in 21% (mUC2) of the *** presence of both molecules did not correlate with WHO, Lauren and Goseki classification (with the exception of a significantly stronger mUC2 expression in mucinous tumors). CDX2 correlated with a lower pT and pN stage in the subgroups of intestinal and stage I cancers and was associated with mUC2 positivity.A prognostic impact of CDX2 or mUC2 was not observed. CONCLUSION: CDX2 and mUC2 play an important role in the differentiation of normal, inflamed, and neoplastic gastric tissues. According to our results, loss of CDX2 may represent a marker of tumor progression in early gastric cancer and carcinomas with an intestinal phenotype.

AIm： To analyze the impact of the GNAS1 T393C polymorphism on prognosis and histopathology of gastric cancer. mETHODS： Genomic DNA was extracted from paraffinembedded tissues of 122 patients with primary gastric carcinoma and from the blood of 820 healthy white individuals. Allelic discrimination was performed by quantitative real-time polymerase chain reaction. Genotyping was correlated with histopathologic parameters and with overall survival according to the Kaplan-meier approach and with multivariate analysis by multiple stepwise regression. RESULTS： Thirty-nine （32%） patients displayed a CC genotype, 57 （46.7%） a CT genotype and 26 （21.3%） a TT genotype. The frequency of the C allele （fC） in the patient group was 0.55, which was not significantly different from that of healthy blood donors. The distribution was compatible with the Hardy-Weinberg equilibrium. Analysis of clinicopathological parameters did not show any significant correlation of the T393C genotype with gender （p = 0.50）, differentiation （p = 0.29）, pT-category （p = 0.19）, pN-category （p = 0.30）, pm-category （p = 0.25）, R-category （p = 0.95）, the classifications according to WHO （p = 0.34）, Lauren （p = 0.16）, Goseki （p = 1.00） and ming （p =0.74）. Dichotomization between C＋（CC＋CT） and C-genotypes （FI）, however, revealed significantly more advanced tumor stages （p = 0.023） and lower survival rates （p = 0.043） for C allele carriers. CONCLUSION： The present study provides strong evidence to suggest that the GNAS1 T393C allele carrier status influences tumor progression and survival in gastric cancer with higher tumor stages and a worse outcome for C allele carriers.

A so-called ＂split-bottom ring shear cell＂ leads to wide shear bands under slow, quasi-static deformation. Unlike normal cylindrical Couette shear cells or rheometers, the bottom plate is split such that the outer part of it can move with the outer wall, while the other part （inner disk） is immobile. From discrete element simulations （DEm）, several continuum fields like the density, velocity, deformation gradient and stress are computed and evaluated with the goal to formulate objective constitutive relations for the powder flow behavior. From a single simulation, by applying time- and （local） space-averaging, a non-linear yield surface is obtained with peculiar stress dependence. The anisotropy is always smaller than the macroscopic friction coefficient. However, the lower bound of anisotropy increases with the strain rate, approaching the maximum according to a stretched exponential with a specific rate that is consistent with a shear path of about one particle diameter.

The photorespiratory pathway or photorespiration is an essential process in oxygenic photosynthetic or- ganisms, which can reduce the efficiency of photosynthetic carbon assimilation and is hence frequently considered as a wasteful process. By comparing the response of the wild-type plants and mutants impaired in photorespiration to a shift in ambient C02 concentrations, we demonstrate that photorespiration also plays a beneficial role during short-term acclimation to reduced C02 availability. The wild-type plants re- sponded with few differentially expressed genes, mostly involved in drought stress, which is likely a conse- quence of enhanced opening of stomata and concomitant water loss upon a shift toward low C02. In contrast, mutants with impaired activity of photorespiratory enzymes were highly stressed and not able to adjust stomatal conductance to reduced external C02 availability. The transcriptional response of mutant plants was congruent, indicating a general reprogramming to deal with the consequences of reduced C02 availability, signaled by enhanced oxygenation of ribulose-l,5-bisphosphate and amplified by the artificially impaired photorespiratory metabolism. Central in this reprogramming was the pro- nounced reallocation of resources from growth processes to stress responses. Taken together, our results indicate that unrestricted photorespiratory metabolism is a prerequisite for rapid physiological acclimation to a reduction in C02 availability.

AIm: To investigate the relation of patient characteristics and procedural parameters to the endoscopic detection rate of colonic adenomas. Further to study,which factors may be capable to predict the localization of adenomatous ***: We used the data base of a prospective randomized colonoscopy study(The Colo Cap trial) to identify patients being diagnosed with colon adenoma. Logistic regression analysis was conducted to reveal predictors for adenoma detection in the entire colon and also with respect to the proximal and distal part. Covariates including age, gender, duration of colonoscopy and comorbidities were defined to determine association between predictors and adenoma ***: Equal numbers of adenomas were detected in the proximal and distal side of the splenic flexure [126(57%) vs 94(43%), p = 0.104]. Simultaneous occurrence of adenomas in both sides of the colon was rare. The appearance of both proximal and distal adenoma was associated with increasing age(p = 0.008 and p = 0.024) and increasing duration of colonoscopy(p p = 0.001). male gender was a predictor for adenoma detection in the proximal colon(p = 0.008) but statistical significance was slightly missed with respect to the distal colon(p = 0.089). Alcohol abuse was found to be a predictor for the detection of distal adenoma(p = 0.041). CONCLUSION: Increasing age and longer duration of colonoscopy are factors with a strong impact on adenoma detection both in the proximal and distal colon. Since proximal adenomas occurred in absence of distal adenomas, complete colonoscopy should be performed for screening.
