Periprocedural complications and one-year outcomes after catheter ablation for treatment of atrial fibrillation in elderly patients:a nationwide Danish cohort study

OBJECTIVES To investigate complications within 30-days following first-time ablation for atrial fibrillation(AF),including a composite of cardiac tamponade,hematoma requiring intervention,stroke or death,in patients≥75 years of age,compared to pa-tients aged 65−74 *** addition,one-year all-cause mortality and AF relapse were ***&RESULTS All patients receiving their first catheter ablation for AF between 2012 and 2016 were identified us-ing Danish nationwide *** aged 65−74 years served as the reference group for patients≥75 *** of AF within one year was defined as cardioversion following a three-month blanking period,re-ablation or confirmed relapse within *** composite complication outcome did not differ between the two age groups,with 39/1554(2.8%)in patients 65−74 years of age,versus 5/199(2.5%)in older patients(adjusted HR=0.94),95%CI:0.37−2.39,P=0.896).Patients≥75 years or older had no increased hazard of death within 30 days after the procedure,with an incidence of 3/1554(0.2%)in younger pa-tients and 2/199(1.0%)in patients≥75 years of age(adjusted HR=4.71,95%CI:0.78−28.40,P=0.091).There was no difference in relapse of AF after one year between age groups(≥75 years adjusted HR=1.00,95%CI:0.78-1.26,P=0.969).CONCLUSION In patients≥75 years of age selected for catheter ablation for AF,the incidence of periprocedural complica-tions,as well as one-year freedom from AF showed no statistical difference,when compared to patients 65−74 years of age.

Experimental Study of An Aquaculture Net Cage in Waves and Current

Model experiments of a floating fish cage subjected to waves and current have been performed. The objective was to study the dynamic behaviour of the fish cage model in waves and current. The fish cage model was composed of a model net, a flexible floating collar of the circular plastic type and a weight system. It was found that there are many wave periods in which cancellation of the wave-induced forces on the model occur. These cancellation wave periods are within the range of dimensioning wave periods commonly used for testing of fish farm structures and hence are important to be aware of. Large deformations of the net under realistic wave and current conditions were observed, where contact between the net and other parts of the structure were identified. This may cause damages to the net due to abrasion.

Drag on Hydroid-Fouled Nets – An Experimental Approach

The present study investigated the drag increase on aquaculture nets due to biofouling of the colonial hydroid Ectopleura larynx. It had two main parts： firstly the growth characteristics of E. larynx were investigated by use of field tests at a Norwegian aquaculture site; secondly the hydrodynamic drag on the fouled twines was studied in a towing tank by using fabricated models of net twines with artificial hydroid fouling. In the field tests, the growth of the hydroids was first measured after three weeks of immersion and then again after six weeks. During this interval, the density of hydroids and the thickness of the hydroid stem were almost constant（1.4 hydroids/mm and 0.29 mm, respectively）, while the average length of the hydroids increased from 6.4 to 11.2 mm. The hydroid length followed a Rayleigh distribution, while the thickness was normal（Gaussian） distributed. Replicas of twines with three different levels of hydroid growth were made（1.5 hydroids/mm twine, hydroid length 9 mm, 16 mm and 20 mm）, and the drag on these twines was measured at different towing velocities（0.1 to 1.4 m/s） and with different twine configurations. For the twine with the shortest hydroids（9 mm）, the drag was from 1.5 times（Re=4000） to 2.2 times（Re=1000） the drag on a clean twine. For the longest hydroids（21 mm）, the drag was 2 times and 3.8 times, respectively.

Polo-like kinase 1 expression is a prognostic factor in human colon cancer

AIM：To clarify the expression patterns and prognostic implications of the mitotic regulator Polo-like kinase 1 （PLK1） in colon cancer. METHODS： Expression of PLK1 was investigated by immunohistochemistry （158 cases） and immunoblotting in tissue of colon adenomas and adenocarcinomas. PLK1 expression patterns were correlated with clinicopathological parameters and patient prognosis. In addition, expression of PLK1 was evaluated by immunoblot and PCR in colon carcinoma cell lines, and coexpression of PLK1 with the proliferation marker Ki-67 was investigated. RESULTS： Weak PLK1 expression was observed in normal colon mucosa and adenomas. In contrast, 66.7% of carcinomas showed strong expression of PLK1. Overexpression of PLK1 correlated positively with Dukes stage （P〈0.001）, tumor stage （P = 0.001） and nodal status （P〈0.05）. Additionally, PLK1 expression was a prognostic marker in univariate survival analysis （P〈0.01） and had independent prognostic significance （RR = 3.3, P = 0.02） in patients with Iocoregional disease. Expression of PLK1 mRNA and protein was detected in all cell lines investigated. Coexpression of PLK1 and Ki-67 was observed in the majority of colon cancer cells, but a considerable proportion of cells showed PLK1 positivity without Ki-67 expression. CONCLUSION： PLK1 is a new prognostic marker for colon carcinoma patients and may be involved in tumorigenesis and progression of colon cancer. Strategies focusing on PLK1 inhibitionin vivo might therefore represent a promising new therapeutic approach for this tumor entity. 2005 The WIG Press and Elsevier Inc. All rights reserved

Wanting a Life in Decency!—A Qualitative Study from Experienced Electric Wheelchairs Users’ perspective

Background: The functionality and the safety of the electric wheelchairs were essential for users’ everyday life. Some evidence indicated that the wheelchair Per Se highly influenced users’ occupational life, their personal identity and social life;further, the wheelchair became an extension of the body and more than a technical device. Besides, there was still both environmental and self-efficacy or/and mental health factors obstacles for full social participation. Even so, there was to some extent stigma related to being a wheelchair user. There was a need to reflect users’ perspective on being depended on electric wheelchair. The aim, accordingly, was to describe and to get a deeper insight into electric wheelchairs users’ perspective and experiences of utilizing this device;a qualitative design with an inductive approach was used. Method: Qualitative latent and interpretative content analysis [1] [2] was used after repeated face-to-face semi-structured interviews with three experienced Swedish electric wheelchair users during the autumn 2017. Findings: The findings showed a high degree of dependability of the assistants that supported the users, and of the quality of that working relationship. The findings were formulated, abstracted and interpreted in several steps. It showed one theme of meaning: “Living in a space shifting between potential violation of or respect for human dignity”. Conclusion: The study showed that electric wheelchair users were relatively content with their lives as well as with their devices in turns of mobility and accessibility, but the meaning of their narrations showed a life at constant risk of having the respect of human rights and human respect violated. Besides, the importance of having access to good and high quality devices, good staffing, and environmental support, all in concordance with human rights, the clinical and practical implications of this study narrows down to a question of encountering the other person as a whole an

The built environment correlates of objectively measured physical activity in Norwegian adults:A cross-sectional study

Background: Built environments that are designed to provide accessible, attractive, and convenient locales promote regular physical activity(PA).Norway has great variability in its geographic, natural, and built environment features. Urban areas have well-developed built environment features, whereas the rural areas are less walkable and this may influence the mode of transport. In general, active transport is more common in urban centers. Further, public transportation is more developed in urban areas, whereas motorized transport may be more widespread in the rural areas. Despite this, in Sogn & Fjordane, a rural county in western Norway, high PA levels are frequently observed. Thus, the aims of this study were to(1) explore perceived built environment features and characterize their associations with objectively measured PA levels in Norwegian adults and(2) explore the differences in these correlates between Sogn & Fjordane and the rest of ***: In this cross-sectional study, participants used questionnaires to rate perceptions of their built environments, and their PA was objectively measured for 7 consecutive days using the Acti Graph GT1 M accelerometer. There were 972 Norwegian adults who were included in the study. The average age was 46.9 ± 6.5 years and 43.8% of participants were men. Data were analyzed using multiple linear ***: Total PA and moderate-to-vigorous physical activity(MVPA) were both associated with perceived walkability, the community perception score, and active transport for commuting(all p ≤ 0.004). We also observed geographic-area-specific associations: the community perception score was negatively associated with total PA and MVPA in the rest of Norway(p ≤ 0.012) but not in Sogn & Fjordane. Experiences on Participation in Literary Activities: Intellectual Stimuli Empower People with Mental Health Problems

Some research acknowledges engagement in creative and literary activities as constructively impacting rehabilitation and recovery. Nevertheless, there is a deficit of literature describing experiences of participation in literary activities. This study examined how members of a non-profit association for mental health, with experiences of mental problems or illnesses, experienced group-based literary activities facilitated of a bi-disciplinary research team during one semester. This study held a constructive/naturalistic design and used a qualitative methodology with a latent analysis to describe how participants experienced literary activities (creative writing, reading and conversations about published and own written poetry and prose texts). Both individual and focus-group interviews were performed early 2014. Despite previous negative experiences, the findings showed that the participants perceived the literary activities empowering. Four categories were created: Presumptions of personal literacy, leadership role, participants’ role, and literacy development. These were abstracted and interpreted into three themes: Identifying a reading and writing self-image, challenging and redefining one’s reading and writing identity, and strengthening one’s intellectual and humanistic capacity. Literary activities seemed to positively change the participants’ attitudes towards writing and reading, and also enhanced their literacy skills. We argue that literacy skills are interconnected with health literacy. Involvement of literary activities into rehabilitation in mental health practice may be beneficial for some groups, and merits further studies.

Thai Psychiatric Nurses’ Experiences and Perceptions of the Professional Role When Caring for Older People Displaying Depressive Symptoms

Purpose:?The aim of this study was to describe Thai psychiatric nurses’ experiences and perceptions of their professional role when caring for older people whom displayed symptoms of clinical depression.?Methods:?A qualitative descriptive research design was used. Thirteen psychiatric nurses working in the field of mental health and psychiatric nursing at a rural district hospital were purposively included. The data were collected through in-depth,?face to face interviews and analyzed using qualitative latent content analysis.?Results:?All participants were female, and the mean age was 43 years, and the mean experience of caring for mentally ill patients was 8.3 years. The psychiatric nurses’ experiences and perceptions of their professional role were mirrored in the following themes:?1) managing a central role in the care of the patients;?2) conflicting interests between the professional needs of caregiving and other requests;and?3) being compassionate beyond the profession.?Conclusion: The psychiatric nurses were challenged by the complexity of the caregiving situations and the partners involved in the process of caring for older people with depression. Professionally, the psychiatric nurses played an important role in preventive care and managerial work. They underwent stress under the influence of stakeholders, but they also contributed to the holistic patient care.?Health service authorities may use these results to develop plans for psychiatric nurses in managing a central role and in advocating for holistic care to mitigate the influence of stakeholders to enhance quality of caring for older?individuals with depression.

The Positive Effects of Proprietary Undenatured β-Glucans on Stressed Marine Life

The safety of beta-glucans, including undenatured beta-glucan (UDBG), has been exhaustively studied over the recent past and has shown to be safe and efficacious in rodents as well as in humans with less than optimal immune systems. As such, we sought to find the effect of UDBG on certainly stressed crustaceans and fish. The survival of shrimp larvae fed the UDBG and other diets was measured on day 16. The diet containing 11.7% UDBG reduced larvae mortality by 18%;which is almost twice as much as the mortality reduced by the MacroGard diet, and up to 19.5% less than a standard Commercial Diet. In an effort to understand how UDBG protects against stressed marine life, UDBG was given to LPS-challenged trout. The UDBG positively influenced the level of transcription of several genes relevant to the immune system and overall health of the trout. The experimental findings indicated that UDBG: 1) modifies inflammatory responses, 2) makes responses more flexible and versatile, 3) protects from cellular stress, and 4) enhances “effector” mechanisms. Importantly, there was no evidence of immune system over-activity.

Safety and Efficacy of a Proprietary Undenatured Beta-Glucan on Different Human Populations

After proving safety in animal and human models, a series of clinical trials were conducted to examine the administration of 1 mg/mL bid of an undenatured beta-glucan on the immune system;and on self-perceived effects on subjects’ quality of life (QoL) using a Visual Analogue Scale (VAS). In one study, a subject population that was self-perceived to be fatigued was administered an undetaured beta-glucan, to determine if this supplementation affected their quality-of-life as a result of an increased immune system response. 44 subjects were administered 1 mg/mL bid of the undenatured beta-glucan (Lentinex®) for four weeks. Prior to the beginning of the administration, and following the four-week administration period, the subjects were asked to fill out a quality-of-life VAS questionnaire. The changes in the two questionnaires revealed differences that occurred as a result of the beta-glucan administration. More than 75% of the subjects scored “better” (more than 1 cm on the VAS) with regard to feeling in a good mood, and feeling rested. More than 60% increased their energy and reduced their tiredness and exhaustion, and 70% felt less stressed. In a second study, subjects aged 40 years and older, believed to be in good general health, seeking therapy for tiredness/exhaustion/fatigue or similar conditions (neurasthenia) which were administered 1 mg/mL bid Lentinex®.
