Non-Ablative Transvaginal Radiofrequency in the Treatment of Stress Urinary Incontinence: Review of the Literature

Background: Urinary incontinence is defined as the involuntary loss of urine, which can affect up to 45% of women after menopause. Radiofrequency is a non-invasive procedure that involves the application of an electromagnetic wave that through heat generation promotes neocollagenesis and neoeslatinogenesis in the vaginal epithelium. This energy-based technology has been studied as a potential alternative for the treatment of genitourinary syndrome of menopause and urinary incontinence. Objective: To review the recent literature (from 2020 to June 2022) on the use of transvaginal radiofrequency in the treatment of stress urinary incontinence, by searching articles at databases of Capes, PubMed Cochrane and Scielo. Methods: The descriptor terms “Urinary Incontinence/therapy” [Majr] AND “Urinary Incontinence, Stress/therapy” [Majr] AND RADIOFREQUENCY-Search Results-PubMed, [“woman” OR “women”] AND [“urinary incontinence” OR “stress urinary incontinence”] AND Radiofrequency were used, with a filter for the period 2020 to 2022. Conclusion: The studies evaluated in this review demonstrated significant results of radiofrequency in the resolution or reduction of complaints of women with urinary incontinence, especially stress urinary incontinence, but most of these studies presented a low methodological quality. There is, therefore, a lack of studies with longer follow-ups, evaluation of cost-effectiveness, randomized clinical trials with objective outcomes and the use of validated questionnaires with international acceptance.

The first group received 600 mg N-acetylcysteine 15 min prior to ERCP, and perrectum administration of 50 mg indomethacin both prior to and after completion of the ERCP. The second group was administered only the 50 mg indomethacin per-rectum both prior to and after the ERCP. The third group was administeredper-rectum 100 mg indomethacin only after the ERCP, representing the control group given the guideline-recommended regimen. The primary end-point was PEP *** Among the total 211 patients evaluated during the study, 186 fulfilled the inclusion criteria and completed the protocol. The percentages of patients who developed PEP in each of the three groups were not significantly different(χ2 =2.793, P = 0.247). Among the acute PEP cases, for all groups, 14 patients developed mild pancreatitis(77.77%) and 4 moderate. No severe cases of PEP occurred, and in all PEP cases the resolution was favorable. AIM To characterize peripheral blood natural killer(NK) cells phenotypes by flow cytometry as potential biomarker of liver fibrosis in human immunodeficiency virus(HIV)/hepatitis C virus(HCV) coinfected *** Peripheral mononuclear cells from 24 HIV/HCV(HBVnegative) coinfected and 5 HIV/HCV/HBV seronegative individuals were evaluated. NK surface cell staining was performed with: AntiCD3(APC/Cy7), anti-CD56(PE/Cy5), anti-CD57(APC), anti-CD25(PE), anti-CD69(FITC), anti-NKp30(PE), antiNKp46(PE/Cy7), anti-NKG2D(APC), anti-DNAM(FITC); anti-CD62L(PE/Cy7), anti-CCR7(PE), anti-TRAIL(PE), anti-Fas L(PE), anti CD94(FITC). Flow cytometry data acquisition was performed on BD FACSCanto, analyzed using Flow Jo software. Frequency of fluorescence was analyzed for all single markers. All patients were on ARV, with undetectable HIV viral load. Liver fibrosis was evaluated by transient elastography in 90% of the patients and with biopsy in 10% of the patients. Mean HCV viral load was(6.18 ± 0.7 log10). Even though, no major significant differences were observed between G1 and G2 regarding NK surface markers, it was found that patients with higher liver fibrosis presented statistically lower percentage of NK cells than individual with low to mild fibrosis and healthy controls(G2: 5.4% ± 2.3%, G1: 12.6% ± 8.2%, P = 0.002 and healthy controls 12.2% ± 2.7%, P = 0.008). Analysis of Biomechanical Behaviour of Anterior Teeth Using Two Different Methods: Finite Element Method and Experimental Tests

The main objective of this study was to compare the results obtained with both virtual and experimental research methods, when the biomechanical behavior of teeth restored with esthetic posts was investigated. The finite element method was used to develop models of healthy maxillary canines and maxillary canines restored with definitive crowns and glass-fiber posts, quartzfiber posts, and titanium posts. Stress distribution was observed when external loads were applied. Load was applied in-vitro to analyse the fracture resistance of 48 maxillary canines restored in the same way as it was considered in the virtual method. The analysis of results using the finite element method led to the conclusion that restored teeth, in which the elastic modulus of the post was similar to that of the dentine and the material of the core had the best biomechanical performance. The experimental study validated the virtual analysis.

Dissatisfaction with the Health Service and Non-Adherence to Antihypertensive Medication Treatment in Brazil

Purpose: Possibly the dissatisfaction with health services influences the non-adherence to medication treatment process. However, such association needs further investigation to extrapolate its results to different groups, for instance, those using only public health services. The aim of the study was to investigate the association between dissatisfaction with the public health service and non-adherence to antihypertensive pharmacotherapy. Methods: Cross-sectional descriptive study. 392 patients with hypertension participated;these were undergoing outpatient treatment at Primary Health Care, in a city of Brazil. Data collection occurred between December 2011 and March 2012 through home visits with the application of semi-structured questionnaire. Results: The majority of the interviewed were satisfied with the care received. However, it was found that there was association between non-adherence to pharmacotherapy and dissatisfaction with the reception service, scheduling appointment, care received from the health team, solvability of health problems, group activities, and physician professional. Conclusion: When health professionals do not aim for a service of quality that promotes user’s satisfaction with the health service, it cannot reach good levels of adherence to therapy.

Superoxide Anion Release by Human Blood Phagocytes Can Increase the Microbicidal Activity Induced by a New Microemulsioned System Containing Cotton Oil

The study aimed to develop and characterize a microemulsified system based on cotton oil and verify its effect on superoxide release anion and microbicidal activity by human peripheral blood cells. Microemulsions were formulated using distilled water, degummed cotton oil, Span 80 (SP), Tween 80 (TW), and 1-butanol (BT). The pseudo-ternary diagram delimited ME regions, and the points were pre-selected. The physical-chemical and rheological characterization of the microemulsions was carried out. The ME activity on the interactions between leukocytes and bacteria was analyzed by superoxide release, phagocytosis, and microbicidal activity. The developed formulation was classified as Oil/Water, with an average pH of 5.76, and the viscosity showed resistance to temperature changes. The rheological model of the Power Law classified the microemulsion as a non-Newtonian fluid with pseudoplastic characteristics. The cell viability of cotton oil microemulsion was greater than 90%. There was an increase in the superoxide release by MN phagocytes when treated with cotton oil microemulsion. The cotton oil microemulsion increased phagocytosis and microbicidal activity. The present study suggests that cotton oil is an alternative biomaterial for therapeutic applications, especially in treating infections.

Maternal Understanding Regarding Women Nutrition during Breastfeeding

Introduction: Breastfeeding mothers’ nutritional status affects milk quality and children’s nutrition in a key period for life. Objectives: Identifying and describing breastfeeding mothers’ social representations during this period related with nutrition as self-care;connotations referred to food products and supplements;post-partum weight retention and information sources consulted on this matter. Materials and Methods: Ten in depth interviews were performed in healthy adult breastfeeding mothers. Interviews were analyzed with a qualitative methodology considering Health Beliefs, PRECEDE and Health Promotion models, employing the Grounded Theory. Results: Women consider nutrition as a health conditioning factor and refer dietary changes implementation during pregnancy, which are not always kept during breastfeeding. Disorganization was mentioned regarding feeding and commensality, prioritizing their child’s health over their own. Vegetables and liquids are attributed to positive connotations, referring increased consumption. Certain foods are limited as they are considered less healthy (“junk food”, sweets, sodas, cookies, spices, coffee and mate) while others, like milk, have been associated with negative effects. Insufficient weight gain during pregnancy is valorized as an achievement due to pairs approval and appearance. Association between breastfeeding and previous weight recovery is weak and doubted. Lack of information on feeding and breastfeeding even among health professionals is recognized. In case of doubts, mothers follow the closest belief affectively. Conclusions: Identifying and characterizing beliefs and barriers for maternal eating practices allow including socio-cultural aspects both in the design of public health policies and in individual nutritional counseling.

Self-perception of body image is one factor to be considered when planning and performing preventive initiatives and interventions in childhood and adolescence. The aim of this study was to analyse self-perception of body image in relation to body-mass index, physical inactivity, not eating breakfast, dieting, smoking and alcohol consumption or toxic habits. Materials and Methods: using data from the “Quito municipal schools” cohort study 6964 students aged from 9 to 17 years were described using a multivariate log linear model of the multidimensional table generated by the variables. A logistic regression model was then fitted to assess associations via OR. Results: 8.2% perceived themselves as overweight. Of those subjects overweight according to their BMI, 21.8% perceived themselves as such, while among those not overweight according to BMI, 96.8% considered themselves as slim or of normal weight. Among students who were dieting, 15.1% perceived themselves as overweight. Among those pupils who perceived themselves has having excess weight, the most common reasons for dieting were: lose weight (56.8%), be healthier (22.6%), and maintain current weight (8.4%). Self-perception of excess weight interacts with excess weight (ORadjusted 8.42;CI95% 6.92-10.25), no breakfast (ORadjusted 2.83;CI95% 2.13-3.77), diet (ORadjusted 2.38;CI95% 1.95-2.89), and with all the variables except toxic habits (ORadjusted 1.01;CI95% 0.78 a 1.29). Conclusion: Interventions to prevent obesity in childhood and adolescence ought to take account of specific determinants within the personal, behavioural and socio-environmental factors, such as the promotion of a positive body image.

Childhood stressful events, HPA axis and anxiety disorders

Anxiety disorders are among the most common of all mental disorders and their pathogenesis is a major topic in psychiatry, both for prevention and treatment. Early stressful life events and alterations of hypothalamic pituitary adrenal(HPA) axis function seem to have a significant role in the onset of anxiety. Existing data appear to support the mediating effect of the HPA axis between childhood traumata and posttraumatic stress disorder. Findings on the HPA axis activity at baseline and after stimuli in panic disordered patients are inconclusive, even if stressful life events may have a triggering function in the development of this disorder. Data on the relationship between stress, HPA axis functioning and obsessive-compulsive disorder(OCD) are scarce and discordant, but an increased activity of the HPA axis is reported in OCD patients. Moreover, normal basal cortisol levels and hyperresponsiveness of the adrenal cortex during a psychosocial stressor are observed in social phobics. Finally,abnormal HPA axis activity has also been observed in generalized anxiety disordered patients. While several hypothesis have attempted to explain these findings over time, currently the most widely accepted theory is that early stressful life events may provoke alterations of the stress response and thus of the HPA axis, that can endure during adulthood, predisposing individuals to develop psychopathology. All theories are reviewed and the authors conclude that childhood life events and HPA abnormalities may be specifically and transnosographically related to all anxiety disorders, as well as, more broadly, to all psychiatric disorders.

Conditioned Taste Aversion Diminishes Sugar Intake

Animals learn to avoid particular food when some of its sensory properties are associated with gastrointestinal discomfort. Twenty rats were exposed to free access to food and a sugar solution for ten days in first phase. During second phase, experimental group received a dose of LiCl. Control group received an injection without LiCl. Both groups had free access to a sugar solution and food restriction for three days. In the final phase, both groups returned to the conditions of first phase. Results showed a significant decrease in sugar intake after aversive conditioning regarding the intake registered in the initial phase in experimental group. Control group did not show any differences in its sugar intake before and after the experimental manipulation. The procedure carried out is discussed as a means to decrease sugar intake.
