Giant Cell Tumor of Lumbar Spine Treated with RapidArc Intensity Modulated Arc Therapy: Case Report and Review of Literature

Background: Giant cell tumors of the lumbar spine are rare and complete resection without major functional comprise is challenging despite advancements in spine surgery techniques. Radiation therapy has been an option in such cases;however there are high concerns for associated high small bowel toxicity and lack of dose escalation to achieve local control. With advent of intensity modulated radiation therapy (IMRT) it is now possible to give high radiation dose to tumor with minimal toxicity. Herein we present a rare case of giant cell tumor of fourth lumbar (L4) vertebra treated with RapidArc intensity modulated arc therapy (IMAT) with relevant review of literature. Methods: A 30-year-old female had low back pain for 6 months. She underwent surgical decompression at another hospital as having a L4-5 protruded intervertebral disc 2 months previously, but her back pain progressed with weakness of both legs with restricted movement. Radiological and pathological work-up confirmed the diagnosis of giant cell tumor of L4 vertebra. She refused further surgery and was referred to us for radiotherapy. Treatment plans for prescribed radiation dose of 59.4 Gy in 30 fractions were made by 3DCRT and RapidArc IMAT and comparison was made. Student’s unpaired t test was used to determine the significance of the difference between two plans in terms of dose to the tumor and small bowel. A p value of 0.05 was considered statistically significant. Related literature was searched. Results: In RapidArc IMAT and 3DCRT plans mean doses to planning treatment volume (PTV) were 61.24 Gy (55.98 - 66.23) and 60.71 Gy (49.87 - 63.74) respectively (p 0.04) and mean doses to small bowel were found lesser in RapidArc plan [14.78 Gy (range: 0.39 - 53.15)] as compared to 3DCRT plan. Patient was started on RapidArc IMRT and she completed the course without any major sequelae. Conclusion: Lumbar spine giant cell tumors are rare and complete resection is often not possible. Unusual Case of Radiation Induced Balanitis after Chemoradiation for Upper Rectal Adenocarcinoma

Background: Penile toxicity after preoperative concurrent chemoradiation (CCRT) for rectal cancers is extremely rare and only two cases of phimosis and one case of recto-cavernosalfistula have been reported so far in literature. Preoperative CCRT for rectal cancer is given in prone position and with the support of belly board (BBD) to avoid small bowel toxicity. However, positional errors during rectal radiotherapy can lead to unexpected penile toxicity. Case Presentation: A 50-year-old Saudi male with diagnosed case of rectal adenocarcinoma stage cT3N1M0 was given preoperative CCRT 50.4 Gy in 28 fractions with three-dimensional conformal radiation therapy (3DCRT) in prone position using belly board with concurrent oral capecitabine 825 mg/2 twice a day. After the completion of CCRT, he complained of severe soreness, itching over glans penis and dysuria. Examination revealed grade 3 erythema, skin desquamation over glans penis (balanitis). Portal imaging of treatment revealed glans penis to lie within posterior radiation beam. A patient was assured and he recovered fully after local steroids and short course of antibiotics. Conclusion: Penile toxicity after CCRT for rectal cancer is extremely rare manifestation. Radiation oncologists and therapists must be aware of this rare side effect and must assure proper patient education and positioning during CCRT for rectal cancer.

Traumatic brain injury, chronic traumatic encephalopathy, and Alzheimer's disease are three distinct neurological disorders that share common pathophysiological mechanisms involving neuroinflammation. One sequela of neuroinflammation includes the pathologic hyperphosphorylation of tau protein, an endogenous microtubule-associated protein that protects the integrity of neuronal cytoskeletons. Tau hyperphosphorylation results in protein misfolding and subsequent accumulation of tau tangles forming neurotoxic aggregates. These misfolded proteins are characteristic of traumatic brain injury, chronic traumatic encephalopathy, and Alzheimer's disease and can lead to downstream neuroinflammatory processes, including assembly and activation of the inflammasome complex. Inflammasomes refer to a family of multimeric protein units that, upon activation, release a cascade of signaling molecules resulting in caspase-induced cell death and inflammation mediated by the release of interleukin-1β cytokine. One specific inflammasome, the NOD-like receptor protein 3, has been proposed to be a key regulator of tau phosphorylation where it has been shown that prolonged NOD-like receptor protein 3 activation acts as a causal factor in pathological tau accumulation and spreading. This review begins by describing the epidemiology and pathophysiology of traumatic brain injury, chronic traumatic encephalopathy, and Alzheimer's disease. Next, we highlight neuroinflammation as an overriding theme and discuss the role of the NOD-like receptor protein 3 inflammasome in the formation of tau deposits and how such tauopathic entities spread throughout the brain. We then propose a novel framework linking traumatic brain injury, chronic traumatic encephalopathy, and Alzheimer's disease as inflammasomedependent pathologies that exist along a temporal continuum. Finally, we discuss potential therapeutic targets that may intercept this pathway and ultimately minimize long-term neurological decline.

This article presents a case of a patient with relapsed esthesioneuroblastoma (ENB), an aggressive rare tumor that arises from the specialized sensory epithelial olfactory cells in the skull base area, which was initially treated with endoscopic surgery, followed by adjuvant radiotherapy. After local relapse, new surgical approaches and subsequent lines of platin-based chemotherapy were performed. A PET-CT with 68GALIUM DOTATATOC (PET-DOTATOC) showed intense uptake of disease, compatible with the presence of somatostatin receptors, in the face, nodes, liver, bones, and meningeal area. Treatment with 4 cycles of 177Lutetium-Dotatate was performed, followed by maintenance octreotide, with a major radiological and clinical response that is lasting more than 1 year after treatment. This article describes a rare case of a skull-base tumor, with multiple recurrences, in which disease control was achieved with a targeted Peptide Receptor Radionuclide Therapy (PRRT) with 177Lutetium-Dotatate, and discusses factors that could influence the incorporation of this form of therapy. Previous case reports proved the potential efficacy of this therapy usually given for low-grade neuroendocrine tumors and will be carefully reviewed.

Influence of Colletotrichum truncatum on the Physiological and Chemical Quality in Different Varieties of Soy Seed

The literature highlights that a severe infection by the fungus Colletotrichum truncatum may be capable of inflicting considerable damage to seeds after harvest, potentially affecting their chemical composition and physiological quality. Taking into account that currently there is no categorization in terms of susceptibility and tolerance on this pathogen, the present work is presented with the main objective of “Evaluate the influence of Pathogenicity of C. truncatum on the physiological quality (germination, vigor, viability) and biochemical components in different varieties of soybean seeds (Glycine max)” most planted in the region. The work was carried out in the Agrotec laboratory, located in the Municipality of San Alberto (Alto Paraná), using a completely randomized experimental design, with AxB factorial arrangement, where A indicates ten most planted soybean varieties in the region and B with or without artificial inoculation of Colletotrichum truncatum, with twenty treatments and four repetitions. The variables evaluated were: germination, vigor, viability and chemical composition. The data were subjected to analysis of variance and the Tukey test at 5% error. The results showed a significant statistical difference, accepting the alternative hypothesis proposed “The pathogenicity of Colletotrichum truncatum influences the physiological quality (germination, vigor, viability) and biochemical components (saturated and unsaturated fatty acids) in different varieties of soybean seeds (Glycine max)”.

Although pharmaceuticals have beendetected in the environment only in the range from ng/L to μg/L, it has beendemonstrated that they can adversely affect the health status of aquatic organisms. Lysosomal membrane stability （LMS） has previously been applied as an indicator of cellularwell-being todetermine health status in bivalve mussels. The objective of this study is to evaluate LMS in Ruditapes philippinarum haemolymph using the neutral red retention assay （NRRA）. Clamswere exposed in laboratory conditions to caffeine （0.1, 5, 15, 50 μg/L）, ibuprofen （0.1, 5, 10, 50 μg/L）, carbamazepine and novobiocin （both at 0.1, 1, 10, 50 μg/L） for35days. Results show adose-dependent effect of the pharmaceuticals. The neutral red retention time measured at the end of the bioassaywas significantly reduced by 50% after exposure to environmental concentrations （p 〈 0.05）（caffeine = 15 μg/L; ibuprofen = 10 μg/L; carbamazepine = 1 μg/L and novobiocin = 1 μg/L）, compared to controls. Clams exposed to these pharmaceuticalswere considered to present adiminished health status （retention time 〈45 min）, significantlyworse than controls （96 min）（p 〈 0.05）. The predicted no environmental effect concentration （PNEC） results showed that these pharmaceuticals are very toxic at the environmental concentrations tested. Measurement of the alteration of LMS has been found to be a sensitive technique that enables evaluation of the health status of clams after exposure to pharmaceuticals under laboratory conditions, thus representing a robust Tier-1 screening biomarker.
