Purpose To determine the long-term o ut come of Asian eyes with an acute attack of primary a ngle closure(APAC)and to identify risk factors at prese ntation asso-ciated with the development of glaucomatous op-tic nerve *** Cross-sectional observational case *** ninety in dividuals who were ini-tially seen with APAC 4to 10years pre viously at 2sin-gapore *** All subje cts underwent a com-plete eye examination,including visual acuity,visual field testing,dilated eye examination,a nd optic nerve head *** optic discs were ju dged clinically and photographically as to whether there was glaucomatous optic neuropathy present,and visual fields were assessed for corresponding visual field *** visual fields and optic nerve photographs underwent a second evaluation by an experienced,but masked,glaucom a specialist,who assessed whether the changes were compatible with glau- *** outcome measures The main outcome mea-sures were blindness(defined as best-corrected visual acuity worse than 6/60and /or centra l visual field of less than 20°in the attack eye)and glaucomatous optic neu-ropathy(GOn).Results A total of 90of 170eligible subjects(65.2%)were *** subjects were Asian and were predominantly Chinese(78subjects86.7%).There were 61females(67.8%),and the age of the subjects was 62.0±9.0years(mean ±stan-dard deviation)at the time of APAC,with a mean dura-tion of 6.3±1.5years from the time of the APAC episode to the study ***(17.8%)subjects were blind in the attack eye;half of the cases of blindness were caused by ***-three subjects(47.8%)had GOn,with 13eyes(15.5%)having markedly cupped optic discs(cup-to-disc ratio>0.9).Thirty-eight eyes(58%)had best-corrected vision worse than 6/9,with cataract responsible for close to half the cases of poor *** were no identifiable ris k factors related to the APAC episode that were significantl y associated with the presence of ***

Background:Recent evidence suggests that burn patients are at increased risk of hospital admission for infection,mental health conditions,cardiovascular disease and cancer for many years after discharge for the burn injury *** injury has also been shown to induce sustained immune system *** change to immune function may contribute to the increased risk of chronic disease ***,the mechanisms that disrupt long-term immune function in response to burn trauma,and their link to long-term morbidity,remain *** this study we investigated changes to immune function after burn injury using a murine model of non-severe ***:An established mouse model of non-severe burn injury(full thickness burn equivalent to 8%total body surface area)was used in combination with an orthotopic model of B16 melanoma to investigate the link between burns and *** that CD8^(+)T cells are important drivers of effective tumour suppression in this model,we also investigated potential dysregulation of this immune population using mouse models of burn injury in combination with herpes simplex virus *** cytometry was used to detect and quantify cell populations of interest and changes in immune ***:We demonstrate that 4 weeks after a non-severe burn injury,mice were significantly more susceptible to tumour development than controls using an orthotopic model of B16 *** addition,our results reveal that CD8^(+)T cell expansion,differentiation and memory potential is significantly impaired at 1 month ***:Our data suggests that CD8^(+)T cell-mediated immunity may be dysfunctional for a sustained period after even non-severe burn *** studies in patients to validate these findings may support clinical intervention to restore or protect immunity in patients after burn injury and reduce the increased risk of secondary morbidities observed.

Human life intimately depends on plants for food,biomaterials,health,energy,and a sustainable *** plants have been genetically improved mostly through breeding,along with limited modification via genetic engineering,yet they are still not able to meet the ever-increasing needs,in terms of both quantity and quality,resulting from the rapid increase in world population and expected standards of living.A step change that may address these challenges would be to expand the potential of plants using biosystems design *** represents a shift in plant science research from relatively simple trial-and-error approaches to innovative strategies based on predictive models of biological *** biosystems design seeks to accelerate plant genetic improvement using genome editing and genetic circuit engineering or create novel plant systems through de novo synthesis of plant *** this perspective,we present a comprehensive roadmap of plant biosystems design covering theories,principles,and technical methods,along with potential applications in basic and applied plant biology *** highlight current challenges,future opportunities,and research priorities,along with a framework for international collaboration,towards rapid advancement of this emerging interdisciplinary area of ***,we discuss the importance of social responsibility in utilizing plant biosystems design and suggest strategies for improving public perception,trust,and acceptance.

在资源方面的潜在投资的几种成本收益分析方法方面,有几种适用于计算成本收益的经济学方法:(1)效益转移法;(2)基于调查的非市场价值评估法;(3)折扣法;(4)统计生命价值法;(5)专家启发法;(6)影响因素图表法;(7)敏感性分析法等。根据2003年进行的一项研究,特地评估了为美国国家海洋和大气管理局所购买的新的超级计算机设备的潜在收益。该超级计算机不仅有助于改进天气预报服务,而且能使许多经济部门潜在获益,如各种政府机构、一些私营工业部门和单个家庭住户等。通过咨询美国国家海洋和大气管理局的工作人员可知,这些经济部门主要集中在家庭住户、某些农作物和公共安全等领域。美国国家海洋和大气管理局购买的新的超级计算机潜在的社会效益是显著的。基于一些前提假设,仅家庭住户所得到的收益大约在3400万到23200万美元(以2002年美元为基准)之间。某些农业经济部门所获得的收益与避免因天气原因导致死亡的潜在收益一样多。这三个部门的平均总收益的现值约为11600万美元(以2002年美元为基准)。而这仅为总收益额的下限,因为它尚不包含其它几个也具有重要收益价值的行业,如建筑和能源行业等。美国国家海洋和大气管理局投资购买新型超级计算机的收益净现值约为10500万美元(以2002年美元为基准)。

目的观察合并肾小球滤过率(eGFR)下降老年心力衰竭患者应用芪苈强心胶囊的安全性和有效性。方法 2016年2~4月,连续纳入在老年医学科住院的合并eGFR下降的老年心力衰竭患者172例,将其随机分为两组,每组86例。对照组接受心力衰竭规范化治疗,研究组在接受规范化治疗基础上给予芪苈强心胶囊治疗。12周后,比较两组的eGFR、B型利钠肽(BnP)和明尼苏达心力衰竭生活质量问卷评分(MLHFQ)评分。结果两组患者治疗前的各指标差异无统计学意义(P>0.05);12周后,两组与治疗前比较,BnP水平、估算的eGFR、MLHFQ评分差异均有统计学意义(PnP水平下降[(865.0±124.5)pg/ml vs.(997.0±141.7)pg/ml(Pn·1.73 m^2)vs.(69.3±15.4)ml/(min·1.73 m^2)(Ps.(36.6±12.3)分](P<0.05)。结论老年心力衰竭患者在规范治疗基础上接受芪苈强心胶囊不影响患者的肾功能,且可改善其心功能和提高其生活质量。

婴幼儿和儿童气腹期间的无尿症:一项前瞻性研究

Introduction: Transient oliguria during laparoscopic surgery is a known phenomenon. Currently, no data on the impact of pneumoperitoneum on renal function in children are available. Patients and Methods: Thirty children with normal kidney function, who underwent laparoscopic surgery, were included in a prospective study. A transure-thral catheter was placed to measure urine output during and 6 hours after operation. Renal blood flow (resistive index) was evaluated by Doppler ultrasound of a segmental renal artery before surgery, every 15 minutes during laparoscopy, and after 24 hours. Blood and urine samples were studied before and 24 hours after surgery. Hemodynamic parameters were monitored continuously during stan-dardized anesthesia, including a standardized intravenous infusion regimen. Results: Urine output decreased within 45 minutes of pneumoperitoneum in all patients. Of 8 children younger than 1 year, 7 (88% ) developed anuria vs 3 of 22 (14% ) children aged 1 to 15 years (P nine children 1 year and older (32% ) developed oliguria. There was a significant recovering in the mean urine output until 5 to 6 hours after pneumoperitoneum in both age groups. no significant alterations of the renal blood flow (resistive index) and the serum and urine levels of cystatin C, creatinine, and urea nitrogen were evident until 24 hours postoperatively. The volume of infusion during pneumoperitoneum did not correlate with urine output. Conclusion: Pneumoperitoneum leads to anuria in most children younger than 1 year and to oliguria in about one third of older children. This is a completely reversible phenomenon. Urine output should not be taken into consideration for calculating intravenous fluid administration during pneumoperitoneum in children.
