Extended-spectrum β-lactamase (ESBL) appeared some years after the introduction in hospital environment of unhydrolysable or extended-spectrum cephalosporins. Several studies have been reported on the blaTEM, blaCTX-M and blaSHV genes in ESBL producing Enterobacteria, however, very few studies reported in the literature were related to blaCTX-M subgroup blaTOHO. TOHO enzymes were responsible for healthcare-associated infections in hospitals and in the community. In Burkina Faso, data related to these types of enzymes were scarce. The purpose of this study was to detect TOHO enzymes in Escherichia coli and Klebsiella pneumoniae in order to know the prevalence of infections related to bacterial resistance due to TOHO enzymes at Saint Camille Hospital of Ouagadougou (Burkina Faso). The study was conducted firstly by microbiological identification of ESBLs-producing by Escherichia coli and Klebsiella pneumoniae using API 20 E gallery;secondly the antibiogram was performed by the diffusion method and finally the molecular characterization was made by conventional PCR to search for the blaTOHO gene. The visualization of the specific bands was made using the ultraviolet lamp (Gene Flash) for the photography of the gels. Data were entered and analyzed using Excel 2013 and EPI Info version 6.0 software. A p-value strains constituted by 21 (53.8%) Escherichia coli and 18 (46.2%) Klebsiella pneumoniae. Molecular characterization showed the presence of the blaTOHO gene in 25 bacterial strains (64.1%). It was therefore established in this study the existence of blaTOHO gene at Saint Camille Hospital in Ouagadougou in Burkina Faso. Our study made it possible to know the distribution of the blaTOHO gene in Escherichia coli and Klebsiella pneumoniae.

Heavy Metal Levels and Potential Ecological Risks Assessed at an Agroecosystem Site in Tropical Region

The concentrations and distribution of thirteen metals and metalloids were investigated in soils, sediments, and two biological matrices (the fish Clarias gariepisnis and the earthworm Pontoscolex corethrurus) from the CECOMAF agroecosystem, in Kinshasa, Democratic Republic of the Congo, in order to assess the impact of anthropogenic activities. The results revealed high concentrations of heavy metals, such as Cu, Zn, As, Cd, Pb, and Hg all above values recommended by sediment quality guidelines and their probable effect levels on biota. According to the calculated Enrichment Factor, soil and sediments ranked from moderately to heavily polluted by Cu, Zn, Cd, Pb and Hg. The Contamination Degree and other ecological risk indices indicated very high contamination and very high ecological risks posed by Cd and Hg, respectively. The Geoaccumulation Index indicated that current metal concentrations in the agroecosystem originated from anthropogenic activities, while the Spearman correlation matrix values indicated that Hg could originate from different sources and pathways than the other metals. It was concluded that metals from unchecked anthropogenic activities have negatively impacted agricultural activities and fish production at the CECOMAF agroecosystem. Action to reduce the contamination level and the ecological risks by remediating and preventing metal pollution in the CECOMAF agroecosystem site is recommended.

Evaluation of Thromboprophylaxis Practice in Hospitalized Patients in the Medical Department of Saint Jean de Dieu Hospital of Thies, Senegal

Introduction: Venous thromboembolism disease (VTE) is a common complication of hospitalization. Objective: To assess the practice of thromboprophylaxis in hospitalized patients in the department of medicine. Patients and Methods: It is an observational cross-sectional study conducted from 01 February to 31 March 2019 in the medical departments of the Saint Jean de Dieu Hospital of Thies. All the patients hospitalized with at least 18 years of age were included. The thromboembolic risk was assessed with the Padua score and thromboprophylaxis?assessed according to the ACCP 2012 recommendations. Results: One hundred and forty four patients were included. Seventy-one (49.3%) were in the high thromboembolic risk category. The most frequently reported risk factors were mobility impairment (n = 72;50%), age of 70 years or above (n = 35;24.3%), heart failure (n = 23;16%), acute infections (n = 14;9.7%) and cancers (n = 13;9.02%). The prevention of venous thromboembolism was practiced in 25 of the 71 patients with high thromboembolic risk or a rate of 35.2%. Thromboprophylaxis was exclusively drug-based with Enoxaparin. Conclusion: The practice of prophylaxis for VTE was low in the medical departments. Efforts still need to be made to reduce the high rate of thromboembolic complications associated with hospitalization.

Introduction: Acute rheumatic fever (AAR) is a non-suppurative complication of late infection by group A. Infections due to streptococci remain a public health problem in the Central African Republic. The present study aims to determine the incidence rate of AAR cases and its complications. Methodology: This was a retrospective and analytical study over a period of 4 years (from January 2015 to December 2018) at the National Laboratory of Clinical Biology and Public Health (LNBCSP) in Bangui. It focused on samples concerning the diagnosis of AAR and patient files seen in consultation in the capital’s reference health establishments. Laboratory registers and patient consultation files were used to collect data. They were entered into Excel 2010 to be analyzed with Epi Info 7. A univariate analysis by logistic regression, Ficher’s exact test, and chi2 at the 5% threshold (p Results: We analyzed 94 cases meeting Jones’s criteria. The ages of the patients ranged from 18 to 85 years (mean age 52 years and mode 45 years). The incidence rate of AAR for the female sex was higher than that of the male sex during the study period (p > 0.05). It went from 166.6 in 2015 to 200 in 2016 and 2018 cases of AAR per 1000 people per year. The average incidence rate was 296.18 cases of AAR per 1000 people per year. The average incidence rate was 223.5 cases of AAR per 1000 people per year for joint damage. Joint damage represented 80.85% (RR = 0.62;Chi2 = 4.88;95% CI [0.39-0.97];p Conclusion: The study allowed data on the impact rate and complications of the AAR in Bangui. Special attention to data management will help produce a complete result on the problem. Joint complications predominated. Sustaining an effective surveillance system and preventing infection would help reduce the risk of AAR occurrence.

Multidrug-Resistant of Escherichia coli and Salmonella spp. strains in Chicken Feces Intended for Consumption in Open Spaces of Ouagadougou, Burkina Faso

Resistant bacteria can be transmitted to humans through feces or contaminated meat from local chickens. Bacterial strains were isolated from the intestinal contents of 400 local chicken samples from various sales sites. These strains were then characterized using bacteriological and biochemical methods to identify resistant strains. In a study conducted in Ouagadougou, we systematically collected chicken fecal samples from 20 locations across the city, followed by isolation and identification of Salmonella spp. using specific enrichment and culture methods, as well as Escherichia coli. Bacterial strains were characterized using antibiotic resistance profiles were determined through agar diffusion tests, revealing sensitivity or resistance to a range of antibiotics based on established scientific criteria. The results showed that out of the 400 samples collected, 81.25% and 63.5% were contaminated by Escherichia coli and Salmonella spp., respectively. Among these, 86.15% of identified Escherichia coli and 50.78% of Salmonella spp. displayed resistance to at least one tested antibiotic. Among 280 Escherichia coli isolates identified resistant to at least one antibiotic, 31.07% were resistant to cefotaxime (CTX), 20.35% to ceftazidime (CAZ), 21.07% to ceftriaxone (CTR), 75% to amoxicillin clavulanic acid (AMC), 23.57% aztreoname (ATM) and 27.14% were resistant to imipenem (IMP). In the case of the 129 Salmonella spp. isolates resistant to at least one tested antibiotic, 34.88% were resistant to CTX;41.08% to CAZ;35.65% to CTR, 92% to AMC, 39.53% to ATM and finally 47.28% were resistant to IMP. Our study revealed high prevalence of resistance in bacterial strains isolated from local chickens sold outdoors in Ouagadougou. These findings raise significant public health concerns, due to the possible transmission of these resistant strains to humans through the consumption of contaminated meat, thus complicating the treatment of bacterial infections.

Background: AntiRetroViral Treatment (ART) remains an important tool for reducing morbidity and mortality and improving the quality of life of people living with the Human Immunodeficiency Virus (PLHIV). Under the pressure of ART, several parameters of PLHIV change and are the subject of different evaluations. Objective: The objective of this work was to study the clinical and anthropometric evolution of PLHIV after 6 months of ART based on Dolutegravir (DTG). Methods: The present study was a non-interventional prospective cohort to assess the clinical and anthropometric evolution of PLHIV after 6 months of ART in the Ambulatory Treatment Center (CTA) for HIV in Kinshasa. The patients included were followed for 6 months in compliance with the schedule promulgated by the National Program. The parameters of interest were: Age, Sex, Marital status, Weight, Height, Body Mass Index and Clinical status. Results: On inclusion, 119 patients (56.3% women;43.7% men) were selected. During the consultation appointments, 42 patients (66.7% women;33.3% men) were present at M1, 37 patients (70.3% women;29.7% men) at M3, and 67 patients (61.3% women and 38.7% men) in M6. On inclusion, 41.5% of patients were at stage 3 and 47.0% had a normal clinical condition. In M1, 48.7% of patients were at stage 1 and 65.8% had a normal clinical condition. At M3, 41.7% of patients were at stage 3 and 67.6% had a normal clinical condition. At M6, 61.8% of patients were at stage 3 and 67.9% had a normal clinical condition. On D0, 45.5% of patients were in the 45 to 55 kg range and 56.5% had a normal Body Mass Index (BMI). At M1, 45.7% were in the 45 to 55 kg range and 50.0% had a normal BMI. At M3, 34.4% were in the 45 to 55 kg range and 64.3% had a normal BMI. At M6, 31.8% were in the range of 45 to 55 kg and, respectively, 37.5% of patients were lean and had a normal BMI.
