Choriocapillaris in non-neovascular age-related macular degeneration as evaluated by optical coherence tomography angiography

Dramatic advances in retinal imaging technology over the last two decades have significantly improved our understanding of the natural history and pathophysiology of non-neovascular age-related macular degeneration(AMD).Currently,aside from micronutrient supplements,there are no proven treatments for non-neovascular or dry ***,a number of pharmacological agents have been evaluated or are under evaluation for treatment of patients with end-stage dry AMD manifesting as geographic atrophy(GA).It may preferable,however,to intervene earlier in the disease before the development of irreversible loss of visual *** intervention would require a more precise understanding of biomarkers which may increase the risk of progression from early and intermediate stages to the late stage of the *** development of optical coherence tomography angiography(OCTA)has allowed the layers of the retinal microcirculation and choriocapillaris(CC)to be visualized and *** deficits in the CC have been observed to increase with age,particularly centrally,and these flow deficits appear to worsen with development and progression of *** such,OCTA-based CC assessment appears to be a valuable new biomarker in our assessment and risk-stratification of *** in the CC may also provide new insights into the pathophysiology of the *** of choriocapillaris function may also prove to be a target of future therapeutic strategies or as a biomarker to monitor the effectiveness of *** such,CC imaging may be anticipated to be an integral tool in the management of dry AMD.

Fractal analysis of retinal vasculature in normal subjects on ultra-wide field fluorescein angiography

AIM: To evaluate the fractal feature of the retinal vasculature of normal eyes on a stereographic projected and montaged ultra-wide field(UWF) fluorescein angiography(FA).METHODS: Prospective, observational, cross-sectional study. Totally 59 eyes of 31 normal subjects were imaged using the Optos 200 Tx. Images obtained at different gaze angles stereographically projected and montaged. The early-phase UWF FA frames were processed to segment the retinal vasculature and the results were exported as binary masks. The fractal dimension(FD) was calculated using the box-counting ***: The global FD for the entire retina was 1.6±0.04, with no difference between males and females(1.59±0.04 vs 1.61±0.04, P=0.084) or between right and left eyes(1.6±0.04 vs 1.6±0.05, P=0.61). FD was nonuniformly distributed among four quadrants(Preased as the distance from the fovea increased(Prved between FD and age(r=-0.37, P=0.006), and this relationship was observed in the posterior and mid-peripheral retina(Pr-periphery(P>0.05).CONCLUSION: Fractal geometry is non-uniformly distributed across the retina in normal eyes and decreases from the fovea to the far-periphery. Subjects with an older age tend to have a smaller FD, however, the FD in the farperiphery does not appear to be influenced by age.

Nonfibrillar amyloid-βoligomers(AβOs)are a major component of drusen,the sub-retinal pigmented epithelium(rPE)extracellular deposits characteristic of age-related macular degeneration(AMD),a common cause of global *** report that AβOs induce rPE degeneration,a clinical hallmark of geographic atrophy(GA),a vision-threatening late stage of AMD that is currently *** demonstrate that AβOs induce activation of the NLrP3 inflammasome in the mouse rPE in vivo and that rPE expression of the purinergic ATP receptor P2rX7,an upstream mediator of NLrP3 inflammasome activation,is required for AβO-induced rPE *** classes of small molecule inflammasome inhibitors—nucleoside reverse transcriptase inhibitors(NrTIs)and their antiretrovirally inert modified analog Kamuvudines—both inhibit AβOs-induced rPE *** findings crystallize the importance of P2rX7 and NLrP3 in a disease-relevant model of AMD and identify inflammasome inhibitors as potential treatments for GA.

Long term outcome after anti-VEGF treatment-what do we know about the impact of morphological parameters?

Despite the initial positive effect of anti-VEGF treatment in eyes with neovascular AMD(nAMD),long-term results still remain un-satisfying(1-3).Five-year outcomes from the CATT study showed a mean decline of 11 letters from the 2-year follow-up visit,and 3 letters compared to the original baseline(3).This is similar to the long-term results from an observational study reported by Gillies et *** a 2.6 letter loss of visual acuity(VA)at 7 years compared to baseline(2),as well as the results from the SEVEN-UP study which showed an even more dramatic 8.6 letter loss relative to baseline 7 years after the initiation of anti-VEGF therapy(1).Several explanations for these suboptimal lower outcomes may be considered.

To the editor： Endogenous bacterial endophthalmitis accounts for a small minority （2%-6%） of cases of endophthalmitis, but the visual outcome is almost always poor.＇ Group B streptococcus （GBS） accounts for only 5.2% among 267 endogenous endophthalmitis cases,1 and only about 30 cases have been reported in literature. We describe a rare case of bilateral GBS endogenous endophthalmitis and suggest that GBS should be a leading suspect in bilateral endogenous endophthalmitis.

Selective Laser Trabeculoplasty Complicated by Cystoid Macular Edema:report of Two Cases

Purpose: Selective laser trabeculoplasty,a relatively novel treatment for open angle glaucoma,is frequently associated with mild post-operative intraocular inflammation. Methods: We report two uncommon cases of cystoid macular edema within a few weeks of routine selective laser trabeculoplasty. results:Visual acuities and macular thicknesses of the two cases returned to baseline after medical treatment,but in one case, the cystoid macular edema persisted for months. Conclusion:Cystoid macular edema after selective laser trabeculoplasty is fortunately a rare complication, but it might be more common in patients with predisposing factors, and it can be resistant to treatment.
