Osteoporosis is the most common metabolic bone disorder globally and is characterized by skeletal fragility and microarchitectural *** pleiotropy occurs when a single genetic element is associated with more than one *** aimed to identify pleiotropic loci associated with bone mineral density(BMD)and nonbone phenotypes in genome-wide association *** the discovery stage,the nHGRI-EBI Catalog was searched for genome-wide significant associations(P valueng bone-related *** was used to identify proxies of the significantly associated single nucleotide polymorphisms(SnPs)(r2=1).We then assessed putative genetic associations of this set of SnPs with femoral neck(Fn)and lumbar spine(LS)BMD data from the GEFOS *** variants were claimed at a false discovery raten-BMD andnetic markers was performed among more than 400000 UK Biobank participants of European ancestry with available genetic and heel bone ultrasound *** the discovery stage,72 BMD-related pleiotropic SnPs were identified,and 12 SnPs located in 11 loci on 8 chromosomes were replicated in the UK *** SnPs were associated,in addition to BMD,with 14 different *** pleiotropic associations were exhibited by rs479844(AP5B1,O V O L 1 genes),which was associated with dermatological and allergic diseases,and rs4072037(M U C 1 gene),which was associated with magnesium levels and gastroenterological *** conclusion,12 BMD-related genome-wide significant SnPs showed pleiotropy with nonbone *** associations can deepen the genetic understanding of bone-related diseases by identifying shared biological mechanisms with other diseases or traits.

The effect of high-flow oxygen via tracheostomy on respiratory pattern and diaphragmatic function in patients with prolonged mechanical ventilation:A randomized,physiological,crossover study

Laparoscopic cholecystectomy: A report from a single center

AIM： To review and evaluate our experience in laparoscopic cholecystectomy. METHODS： A retrospective analysis was performed on data collected during a 13-year period （1992-2005） from 1220 patients who underwent laparoscopic cholecystectomy. RESULTS： Mortality rate was 0%. The overall morbidity rate was 5.08% （n = 62）, with the most serious complications arising from injuries to the biliary tree and the cystic artery. In 23 （1.88%） cases, cholecystectomy could not be completed laparoscopically and the operation was converted to an open procedure. Though the patients were scheduled as day-surgery cases, the average duration of hospital stay was 2.29 d, as the complicated cases with prolonged hospital stay were included in the calculation. COnCLUSIOn： Laparoscopic cholecystectomy is a safe, minimally invasive technique with favorable results for the patient.

Hemobilia as the initial manifestation of cholangiocarcinoma in a hemophilia B patient

Hemobilia is a rare manifestation of hemophilia and is usually iatrogenic following liver biopsy. There are only few reports of spontaneous hemobilia in hemophilia patients. Cholangiocarcinoma is a well-established cause of hemobilia. We describe a case of a 70-year-old male, with known haemophilia B and a past history of papillotomy, who presented with classical symptoms of hemobilia. The initial diagnostic work-up failed to demonstrate a potential cause of bleeding other than the coagulopathy. Three months later, he was readmitted to our hospital with a second episode of hemobilia. During the second work-up, a cholangiocarcinoma was diagnosed both by imaging studies and by a significant elevation of cancer antigen 19-9. Although hemobilia could be attributed to hemophilia, especially in a patient with previous papillotomy, an underlying malignancy of the biliary tree should be suspected.

Bacterial community in response to packaging conditions in farmed gilthead seabream

The objective of this study was to evaluate the effect of modified atmosphere packaging(MAP)on fish skin,gills and intestines bacterial *** gilthead seabream was packed aerobically or under modified atmospheres(60%CO_(2),30%n_(2),10%O_(2))and stored isothermally at 0℃.next Generation Sequencing(nGS)analysis was applied for the characterization of fish microbiome on fish skin,gills,and intestines initially(time of packaging,1 day after harvesting)and after 10 days of isothermal storage at 0℃.nGS results indicated statistically significant differences in families’richness and diversity in the tested fish tissues between aerobic and MAP packaging during storage at 0℃.The most persistent bacteria were Proteobacteria for both packaging *** fish skin microbiota,the initially prevailing families were Comamonadaceae,Enterobacteriaceae and Moraxellaceae while in the intestines Comamonadaceae,Anaplasmataceae,Bacillaceae and Enterobacteriaceae were the dominant *** the end of storage period,the fish microbiota was dominated by psychotropic and psychrophilic families(Pseudoalteromonadaceae,Psychromonadaceae,and Shewanellaceae),while families such as Comamonadaceae were persistent under MAP *** 8 days of fish storage at 0℃,МАРsamples exhibited higher sensory scorings than the respective aerobically stored fish,indicating better retention of fish freshness and fish quality attributes under *** on the results of the study,MAP modified significantly the microbiological status and extended the shelf life of *** was a powerful tool that provided a more complete assessment compared to a culture-based analysis.

AIM To investigate the impact of inflammatory bowel disease (IBD) on α2-Heremans-Schmid Glycoprotein(AHSG/fetuin A) and potential associations with disease and patient *** AHSG serum levels were determined in treatment-na?ve newly-diagnosed patients, 96 with ulcerative colitis(UC), 84 with Crohn's disease (CD), 62 with diarrheapredominant or mixed irritable bowel syndrome (IBS, D- and M- types) and 180 healthy controls (HC), by an enzyme linked immunosorbent assay (ELISA).All patients were followed for a minimum period of 3 years at the Gastroenterology Department of the University Hospital of Larissa, Greece. C-reactive protein(CRP), anti-glycan antibodies, anti-Saccharomyces cerevisiae mannan antibodies Ig G, anti-mannobioside carbohydrate antibodies Ig G, anti-laminariobioside carbohydrate antibodies Ig G and anti-chitobioside carbohydrate antibodies Ig A were also determined via immunonephelometry and ELISA, *** The mean ± SE of serum AHSG, following adjustment for confounders, was 0.32 ± 0.02 g/L in IBD, 0.32 ± 0.03 g/L in CD and 0.34 ± 0.03 g/L in UC patients, significantly lower than in IBS patients(0.7 ± 0.018 g/L) and HC (0.71 ± 0.02 g/L) (P n all cases). AHSG levels were comparable between the CD and UC groups. Based on AHSG levels IBD patients could be distinguished from HC with about 90% sensitivity and specificity. Further adjusted analysis verified the inverse association between AHSG and penetrating, as well as stricturing CD (partial correlation coefficient: -0.45 and -0.33, respectively) (P ng for confounding factors, inverse correlations between AHSG and CRP and the need for anti-TnFα therapy or surgery, were found (partial correlation coefficients:-0.31,-0.33,-0.41, respectively, P n all cases). Ethnicity might be associated with treatment outcomes in advanced prostate *** study aimed to evaluate the efficacy and safety of androgen deprivation therapy(ADT)combined with apalutamide in East Asians with metastatic castration-sensitive prostate cancer(mCSPC).The original phase 3 Targeted Investigational Treatment Analysis of novel Anti-androgen(TITAn)trial was conducted at 260 sites in 23 *** subgroup analysis included patients enrolled in 62 participating centers in China,Japan,and *** progression-free survival(PFS),time to prostate-specific antigen(PSA)progression,and PSA changes from baseline were compared between groups in the East Asian *** intent-to-treat East Asian population included 111 and 110 participants in the apalutamide and placebo groups,*** 24-month radiographic PFS rates were 76.1%and 52.3%in the apalutamide and placebo groups,respectively(apalutamide vs placebo:hazard ratio[HR]=0.506;95%confidence interval[CI],0.302–0.849;P=0.009).Median time to PSA progression was more favorable with apalutamide than placebo(HR=0.210;95%CI,0.124–0.357;Pn maximum percentages of PSA decline from baseline were 99.0%and 73.9%in the apalutamide and placebo groups,*** most common adverse event(AE)was rash in the apalutamide group,with a higher rate than that in the placebo group(37.3%vs 9.1%).The most common grade 3 or 4 AEs were rash(12[10.9%])and hypertension(12[10.9%])for *** efficacy and safety of apalutamide in the East Asian subgroup of the TITAn trial are consistent with the global results.

AIM： To investigate the efficacy of combination treatment of IFn-α and lamivudine compared to lamivudine monotherapy, after 24 mo of administration in HBeAgnegative hepatitis B patients. METHODS： Fifty consecutive patients were randomly assigned to receive IFn-α-2b （5 MU thrice per week, n = 24） plus lamivudine （100 mg daily） or lamivudine only （n = 26） for 24 mo. Patients were followed up for further 6 mo. The primary outcome was the proportion with sustained virological response （undetectable serum HBV DnA concentrations） and or sustained biochemical response （transaminase levels within normal range） at 30 mo （6 mo after the end of therapy）. Secondary end-points were timed from initial virological （biochemical） response to VBR （BBR, respectively） and the emergence of YMDD mutants across the two arms. RESULTS： Five of twenty-four （21%） patients in the combination arm vs 3/26 （12%） in the lamivudine arm had sustained response （i.e., normal serum transaminase levels and undetectable HBV DnA by PCR assay） 6 mo after treatment discontinuation. A reduction in the emergence of YMDD mutants and in the development of virological breakthroughs was observed in patients receMng combination treatment （10% vs46% , P= 0.01 and 14% vs46% , P= 0.03, respectively）. Time from initial virologic response to virologic breakthrough （VBR） was greater among initial responders receiving combination treatment compared to those receiving lamivudine （22.9 mo vs 15.9 mo, respectively; P = 0.005）.COnCLUSIOn： Our results demonstrate that IFn-α plus lamivudine combination therapy does not increase the sustained response, compared to lamivudine. However, combination therapy reduces the likelihood of VBR due to YMDD mutants and prolongs the time period until the breakthrough development.
