Retinal capillary plexus in Parkinson’s disease using optical coherence tomography angiography

AIM:To evaluate the alterations of the retinal microvasculature and foveal avascular zone in patients with Parkinson’s disease(PD)using optical coherence tomography angiography(OCT-A).METHODS:A retrospective study of PD patients examined in the Ophthalmology Department of the General Hospital of Athens,“Georgios Gennimatas”from March 2021 to March 2022 was conducted.Totally 44 patients with PD were included and 18 healthy controls were examined,hence a total of 124 eyes were enrolled in the study.The foveal and parafoveal superficial and deep capillary plexus vascular density(fSCP-VD,fDCP-VD,pSCP-VD,pDCP-CD)and foveal avascular zone(FAZ)were quantified with OCTA.Optical coherence tomography(OCT)was used to measure macular thickness.Our statistical analysis was conducted by using a mixed effect linear regression model.RESULTS:After adjustment for age and gender,the mean parafoveal superficial capillary plexus vascular density(pSCP-VD)and mean parafoveal deep capillary plexus vascular density(pDCP-VD)were significantly decreased in individuals with PD(Pn both)by-2.35(95%CI-3.3,-1.45)and-7.5(95%CI-10.4,-4.6)respectively.fSCP-VD and fDCP-VD didn’t approach statistical significance.The FAZ area and perimeter were significantly decreased(Pn both)by-0.1 mm^(2)(95%CI-0.13,-0.07)and-0.49 mm^(2)(95%CI-0.66,-0.32)respectively.Circularity didn’t approach statistical significance.Central retinal thickness(CRT)was significantly decreased in individuals with PD(Pnd temporal retinal thickness(TRT)was decreased(P=0.025)by-11μm(95%CI-22,-1.5)while nasal retinal thickness(nRT)only approached statistical significance(P=0.066).COnCLUSIOn:The mean pSCP-VD,pDCP-VD,CRT and TRT are significantly decreased and FAZ is altered in individuals with PD.These findings can be potentially used as biomarkers for the diagnosis and evaluation of early PD.

Effective communication and collaboration among healthcare professionals are crucial for delivering high-quality patient care.Interdepartmental miscommunication poses a significant challenge to healthcare systems,potentially undermining the quality of healthcare services provided.In the same manner,communication barriers between referring physicians and radiologists can specifically affect radiology services and patient outcomes.This article attempts to put the spotlight on the ever-present chronic challenges of this issue and prompt readers to recognize the relevant potential pitfalls in their daily clinical practice.Practical solutions are explored and proposed,which should be tailored to the specific needs and issues that each individual institution may face.

Dear editor,Severe acute respiratory syndrome coronavirus-2(SARS-CoV-2),the virus responsible for the coronavirus disease 2019(COVID-19)pandemic,has been associated with hypogonadism in patients with acute infection[1].Angiotensin-converting enzyme 2(ACE2)receptor is regarded as the viral binding site that facilitates entry into host cells[2].Therefore,cells that express ACE2,such as spermatogonia,Leydig,and Sertoli cells,may be susceptible to SARS-CoV-2 infection[2].Testicular damage may possibly be caused directly by viral invasion or secondary to the generalized inflammatory response to COVID-19[3].Studies have demonstrated that viral shedding in the semen can take place during the recovery phase,even though SARS-CoV-2 is not present in the semen of all men with COVID-19[1,4].Herein,we describe the first case report of a male with longitudinal follow-up who experienced reversible cryptozoospermia after COVID-19.

Background This work aims to provide an overview of the Mixed Reality(MR)technology’s use in maritime industry for training purposes.Current training procedures cover a broad range of procedural operations for Life-Saving Appliances(LSA)lifeboats;however,several gaps and limitations have been identified related to the practical training that can be addressed through the use of MR.Augmented,Virtual and Mixed Reality applications are already used in various fields in maritime industry,but their full potential have not been yet exploited.SafePASS project aims to exploit MR advantages in the maritime training by introducing a relevant application focusing on use and maintenance of LSA lifeboats.Methods An MR Training application is proposed supporting the training of crew members in equipment usage and operation,as well as in maintenance activities and procedures.The application consists of the training tool that trains crew members on handling lifeboats,the training evaluation tool that allows trainers to assess the performance of trainees,and the maintenance tool that supports crew members to perform maintenance activities and procedures on lifeboats.For each tool,an indicative session and scenario workflow are implemented,along with the main supported interactions of the trainee with the equipment.Results The application has been tested and validated both in lab environment and using a real LSA lifeboat,resulting to improved experience for the users that provided feedback and recommendations for further development.The application has also been demonstrated onboard a cruise ship,showcasing the supported functionalities to relevant stakeholders that recognized the added value of the application and suggested potential future exploitation areas.Conclusions The MR Training application has been evaluated as very promising in providing a user-friendly training environment that can support crew members in LSA lifeboat operation and maintenance,while it is still subject to improvement and further expansion.

The oxidation of porous ni-yttria-stabilized zirconia(YSZ)and ni-gadolinia-doped ceria(GDC)ceramicmetal(cermet)electrodes in H_(2)O and CO_(2)atmospheres was studied by near-ambient pressure X-ray photoelectron spectroscopy(nAP-XPS).We show that the oxidation of nickel by the two gases is not similar,as is commonly believed,but it depends on the ceramic type.nickel is vulnerable to oxidation in H_(2)O but it resists to CO_(2)in ni-GDC,as compared to the ni-YSZ electrode.Inspired by this observation we conceptualize and fabricate ni-YSZ electrodes modified by ceria nanoparticles,which show significantly higher resistance to CO_(2)oxidation as compared with conventional ni-YSZ electrodes.The preparation of tailormade cermet electrodes with identical bulk/mechanical characteristics but very different surface properties offers a promising fabrication strategy for high-performance and durability solid oxide electrolysis cells for CO_(2)conversion.

Epigenetic mechanisms,such as DnA methylation and histone modiﬁcations(e.g.,acetylation and deacetylation),are strongly implicated in the carcinogenesis of various malignancies.During transcription,the expression and functionality of coding gene products are altered following the histone acetylation and deacetylation.These processes are regulated by histone acetyltransferases(HATs)and histone deacetylases(HDACs),respectively.HDAC inhibitors(HDACis)have been developed as promising therapeutic agents,to limit exposure to traditional and toxic chemotherapies and offer more alternatives for some speciﬁc malignant diseases with limited options.Mechanistically,these agents affect many intracellular pathways,including cell cycle arrest,apoptosis and differentiation,and their mechanism of action mainly depends on the type of cancer.Currently,ﬁve HDACis have been approved for the treatment of several hematological malignancies(e.g.,T-cell lymphoma subtypes and multiple myeloma);while,many of them are tested for further therapeutic indications in solid tumors(e.g.,colorectal,thyroid,breast,lung and pancreatic cancer).Herein,we review the literature and gather all available evidence,from in vitro and in vivo data to clinical trial results,that recognizes the antitumor activity of HDACis on pheochromocytomas and paragangliomas;and supports their clinical implementation in the treatment of these rare neuroendocrine tumors at metastatic setting.

Tranexamic acid(TXA)has revolutionized modern blood management in orthopaedic surgery,especially in total joint arthroplasty,by significantly reducing blood loss and transfusion rates.It is an antifibrinolytic agent and a synthetic derivative of the amino acid lysine,which can inhibit the activation of plasminogen and the fibrin breakdown process.The administration of TXA can be intravenous(IV),topical,and oral.In patients where the IV administration is contraindicated,topical use is preferred.Topical administration of the drug theoretically increases concentration at the operative site with reduced systemic exposure,reduces cost,and gives the surgeon the control of the administration.According to recent studies,topical administration of TXA is not inferior compared to IV administration,in terms of safety and efficacy.However,there are concerns regarding the possible toxicity in the cartilage tissue with the topical use of TXA mainly in hemiarthroplasty operations of the hip,unilateral knee arthroplasties,total knee arthroplasties where the patella is not resurfaced,and other intraarticular procedures,like anterior cruciate ligament reconstruction.The purpose of the present review is to present all the recent updates on the use of TXA focusing on the toxicity on chondrocytes and the articular cartilage that may or may not be provoked by the topical use of TXA.

In 75%of women with polycystic ovary syndrome(PCOS),insulin action is impaired.In obesity,visceral adipose tissue becomes dysfunctional:Chronic inflammation is favored over storage,contributing to the development of metabolic complications.PCOS,metabolic syndrome(MetSy)and non-alcoholic fatty liver disease(nAFLD)apparently share common pathogenic factors;these include abdominal adiposity,excess body weight and insulin resistance.Alterations in the gut microbiome have been noted in women with PCOS compared to controls;these may lead to deterioration of the intestinal barrier,increased gut mucosal permeability and immune system activation,hyperinsulinemia and glucose intolerance,which hamper normal ovarian function and follicular development(all being hallmarks of PCOS).It has been proposed that PCOS may entail higher susceptibility to coronavirus disease 2019(COVID-19)via its associated comorbidities(nAFLD,obesity,MetSy and alterations in the gut microbiome).Studies have found an association between acute respiratory distress syndrome(seen in severe cases of COVID-19)and the intestinal microbiome.Furthermore,apparently,severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)can gain entry to the gastrointestinal tract via locally-expressed angiotensin converting enzyme type 2 receptors.Excess body weight is associated with more severe COVID-19 and increased mortality.Although robust links between SARS-CoV-2 infection and PCOS/nAFLD/gut microbiome/metabolic consequences are yet to be confirmed,it seems that strategies for adapting the intestinal microbiome could help reduce the severity of COVID-19 in women with PCOS with or without nAFLD,MetSy or obesity.

One of the inspirations behind Peter Lax＇s interest in dispersive integrable systems, as the small dispersion parameter goes to zero, comes from systems of ODEs discretizing 1-dimensional compressible gas dynamics [17]. For example, an understanding of the asymptotic behavior of the Toda lattice in different regimes has been able to shed light on some of von neumann＇s conjectures concerning the validity of the approximation of PDEs by dispersive systems of ODEs. Back in the 1990s several authors have worked on the long time asymptotics of the Toda lattice [2, 7, 8, 19]. Initially the method used was the method of Lax and Levermore [16], reducing the asymptotic problem to the solution of a minimization problem with constraints （an ＂equilibrium measure＂ problem）. Later, it was found that the asyraptotic method of Deift and Zhou （analysis of the associated Riemann-Hilbert factorization problem in the complex plane） could apply to previously intractable problems and also produce more detailed information. Recently, together with Gerald Teschl, we have revisited the Toda lattice; instead of solu- tions in a constant or steplike constant background that were considered in the 1990s we have been able to study solutions in a periodic background. Two features are worth noting here. First, the associated Riemann-Hilbert factorization problem naturally lies in a hyperelliptic Riemann surface. We thus generalize the Deift- Zhou ＂nonlinear stationary phase method＂ to surfaces of nonzero genus. Second, we illustrate the important fact that very often even when applying the powerful Riemann-Hilbert method, a Lax-Levermore problem is still underlying and understanding it is crucial in the analysis and the proofs of the Deift-Zhou method!
