Prostate cancer is one of the most often diagnosed malignancies in males and its prevalence is rising in both developed and developing *** deprivation therapy has been used as a standard treatment approach for advanced prostate cancer for more than 80 *** primary aim of androgen deprivation therapy is to decrease circulatory androgen and block androgen *** a partly remediation is accomplished at the beginning of treatment,some cell populations become refractory to androgen deprivation therapy and continue to *** evidences suggest that androgen deprivation therapy may cause cadherin switching,from E-cadherin to N-cadherin,which is the hallmark of epithelial-mesenchymal *** direct and indirect mechanisms are involved in this switching and consequently,the cadherin pool changes from E-cadherin to N-cadherin in the epithelial *** E-cadherin represses invasive and migrative behaviors of the tumor cells,the loss of E-cadherin disrupts epithelial tissue structure leading to the release of tumor cells into surrounding tissues and *** this study,we review the androgen deprivation therapy-dependent cadherin switching in advanced prostate cancer with emphasis on its molecular basis especially the transcriptional factors regulated through TFG-βpathway.

AIM： To evaluate the omeprazole maintenance therapy in patients with recurrent ulcer bleeding after surgery for duodenal ulcer. METhODS： We studied 15 consecutive patients with recurrent ulcer bleeding after surgery for duodenal ulcer. Omeprazole （20 mg/d） maintenance therapy was given after ulcer healing. In addition to clinical follow-up, ambulatory 24-h gastric ph assay was performed before and during omeprazole therapy in those patients and controls with previous duodenal ulcer surgery but no ulcer recurrence. RESULTS： All the 15 ulcers were healed after being treated with omeprazole （40 mg/d） for 2 too. Eleven patients with two （1-9） episodes of recurrent ulcer bleeding completed the follow-up （43, 12-72 too）. None of them had a bleeding episode while on omeprazole. One patient discontinued the therapy and had recurrent bleeding. The median 24-h fraction time of gastric ph 〈4 in patients was 80, 46-95%, and was reduced to 32, 13-70% by omeprazole （P= 0.002）. CONCLUSION： Long-term maintenance therapy with omeprazole （20 rag/day） is effective in preventing recurrent ulcer bleeding.

AIM: During endoscopic retrograde cholangiopancreatography (ERCP), selective cannulation of the common bile duct (CBD) is required in most of the ***: From June 2001 till December 2002, all patients referred to our unit for ERCP were considered for entry into the study. Selective CBD cannulation was first attempted with a standard catheter with or without the use of a guidewire. In cases, where CBD cannulation was considered unsuccessful, patients were crossed over to a double-lumen sphincterotome and a guidewire. All patients were hospitalized for 24 h after the procedure in order to assess the incidence of post-ERCP ***: The study sample consisted of 158 *** CBD cannulation using a standard ERCP catheter with or without the assistance of a guidewire, was accomplished in 129 patients (success rate: 81.65%).From the 29 patients who were crossed over to a sphincterotome and a guidewire, selective CBD cannulation was achieved in 24; the overall success rate rising to 96.8%. Meanwhile, the use of this technique did not increase the incidence of post-ERCP ***: The use of a sphincterotome and a guidewire increases the success rate of selective bile duct cannulation in cases that this has not been accomplished with a standard catheter.

The production of thermostable laccases from a native strain of the white-rot fungus Pycnoporus sanguineus isolated in Mexico was enhanced by testing different media and a combination of inducers including copper sulfate（CuSO4）.The best conditions obtained from screening experiments in shaken flasks using tomato juice,CuSO4,and soybean oil were integrated in an experimental *** levels of tomato juice as the medium,CuSO4and soybean oil as inducers（36.8%（v/v）,3 mmol/L,and 1%（v/v）,respectively） were determined for 10 L stirred tank bioreactor *** combination resulted in laccase titer of 143000 IU/L（2,2＇-azino-bis（3-ethylbenzthiazoline-6- sulfonic acid）,ph 3.0）,which represents the highest activity so far reported for *** in a 10-L *** interesting media resulting from the screening included glucose-bactopeptone which increased laccase activity up to 20000 IU/L,whereas the inducers Acid Blue 62 and Reactive Blue 19 enhanced enzyme production in this medium 10 *** on a partial characterization,the laccases of this strain are especially promising in terms of thermostability（half-life of 6.1 h at 60 °C） and activity titers.

人工膝关节翻修术常采用钽Cone修复骨缺损,作者分析了72例人工膝关节感染一期翻修使用钽Cone病例的资料,使用旋转铰链膝或者纯铰链膝与钽Cone组合,随访(49.9±18.8)个月。15例进行了再翻修,其中8例为感染,7例为无菌性松动;12例更换了钽Cone。钽Cone生存率为83%±3.8%,排除感染后钽Cone生存率为89%±4.2%。作者认为,人工膝关节感染一期翻修术采用钽Cone修复骨缺损中期疗效满意。

AIM: To evaluate the role of drains in clamp-and-tie total thyroidectomy(c TT) for large goiters. METhODS: A hundred patients were randomized into group D(drains maintained for 24 h) and ND(no drains). We recorded epidemiological characteristics, thyroid pathology, hemostatic material, intraoperative events, operative time and difficulty, blood loss, biochemical and hematological data, postoperative vocal alteration and pain, discomfort, complications, blood in drains, and ***: The groups had comparable preoperative characteristics, pathology, intraoperative and postoperative data. hemostatic material was used in all patients of group ND. Forty patients in group D and 9 in ND felt discomfort(P he surgeon. The only "major disadvantage" is the discomfort for the patient. Inversely, drains probably influence surgeons' serenity,especially when c TT is performed in nonspecialized departments.
