ObJECTIVE Hybrid coronary revascularization(HCR)combines a minimally invasive surgical approach to the left anterior descending(LAD)artery with percutaneous coronary intervention(PCI)for non-LAD diseased coronary arteries.It is associated with shorter hospital lengths of stay and recovery times than conventional coronary artery bypass surgery,but there is little information comparing it to isolated PCI for multivessel disease.Our objective is to compare long-term outcomes of HCR and PCI for patients with multivessel disease.METHODS This cohort study used data from New York’s cardiac surgery and PCI registries in 2010−2016 to examine mortality and repeat revascularization rates for patients with multivessel coronary artery disease who underwent HCR and PCI.Cox proportional hazards methods were used to reduce selection bias.Patients were followed for a median of four years.RESULTS There was a total of 335 HCR patients(1.2%)and 25,557 PCI patients(98.8%)after exclusions.There was no difference in 6-year risk adjusted survival between HCR and PCI patients 83.17%vs.81.65%,adjusted hazard ratio(aHR)=0.90(95%CI:0.67−1.20).However,HCR patients were more likely to be free from repeat revascularization in the LAD artery(91.13%vs.83.59%,aHR=0.51(95%CI:0.34−0.77)).CONCLUSIONS For patients with multi-vessel coronary artery disease,HCR is rarely performed.There are no differences in mortality rates after four years,but HCR is associated with lower repeat revascularization rates in the LAD artery,presumably due to better longevity in left arterial mammary grafts.

Poor bone quality is a major factor in skeletal fragility in elderly individuals.The molecular mechanisms that establish and maintain bone quality,independent of bone mass,are unknown but are thought to be primarily determined by osteocytes.We hypothesize that the age-related decline in bone quality results from the suppression of osteocyte perilacunar/canalicular remodeling(PLR),which maintains bone material properties.We examined bones from young and aged mice with osteocyte-intrinsic repression of TGFβsignaling(TβRII^(ocy−/−))that suppresses PLR.The control aged bone displayed decreased TGFβsignaling and PLR,but aging did not worsen the existing PLR suppression in male TβRII^(ocy−/−)bone.This relationship impacted the behavior of collagen material at the nanoscale and tissue scale in macromechanical tests.The effects of age on bone mass,density,and mineral material behavior were independent of osteocytic TGFβ.We determined that the decline in bone quality with age arises from the loss of osteocyte function and the loss of TGFβ-dependent maintenance of collagen integrity.

When exposed to intense electromagnetic fields, the quantum vacuum is expected to exhibit properties of a polarizable medium akin to a weakly nonlinear dielectric material. Various schemes have been proposed to measure such vacuum polarization effects using a combination of high- power lasers. Motivated by several planned experiments, we provide an overview of experimental signatures that have been suggested to confirm this prediction of quantum electrodynamics of real photon–photon scattering.

Pre-polymerized vinyl trimethoxy silane(PVTMS)@MWCNT nano-aerogel system was constructed via radical polymerization,sol-gel transition and supercritical CO_(2)drying.The fabricated organic-inorganic hybrid PVTMS@MWCNT aerogel structure shows nano-pore size(30-40 nm),high specific surface area(559 m^(2)g^(−1)),high void fraction(91.7%)and enhanced mechanical property:(1)the nano-pore size is beneficial for efficiently blocking thermal conduction and thermal convection via Knudsen effect(beneficial for infrared(IR)stealth);(2)the heterogeneous interface was beneficial for IR reflection(beneficial for IR stealth)and MWCNT polarization loss(beneficial for electromagnetic wave(EMW)attenuation);(3)the high void fraction was beneficial for enhancing thermal insulation(beneficial for IR stealth)and EMW impedance match(beneficial for EMW attenuation).Guided by the above theoretical design strategy,PVTMS@MWCNT nano-aerogel shows superior EMW absorption property(cover all Ku-band)and thermal IR stealth property(ΔT reached 60.7℃).Followed by a facial combination of the above nano-aerogel with graphene film of high electrical conductivity,an extremely high electromagnetic interference shielding material(66.5 db,2.06 mm thickness)with superior absorption performance of an average absorption-to-reflection(A/R)coefficient ratio of 25.4 and a low reflection bandwidth of 4.1 GHz(A/R ratio more than 10)was experimentally obtained in this work.

bACKGROUND Chronic liver disease(CLD)was associated with adverse clinical outcomes among people with severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)infection.AIM To determine the effects of SARS-CoV-2 infection on the incidence and treatment strategy of hepatocellular carcinoma(HCC)among patients with CLD.METHODS A retrospective,territory-wide cohort of CLD patients was identified from an electronic health database in Hong Kong.Patients with confirmed SARS-CoV-2 infection[coronavirus disease 2019(COVID-19)+CLD]between January 1,2020 and October 25,2022 were identified and matched 1:1 by propensity-score with those without(COVID-19-CLD).Each patient was followed up until death,outcome event,or November 15,2022.Primary outcome was incidence of HCC.Secondary outcomes included all-cause mortality,adverse hepatic outcomes,and different treatment strategies to HCC(curative,non-curative treatment,and palliative care).Analyses were further stratified by acute(within 20 d)and post-acute(21 d or beyond)phases of SARS-CoV-2 infection.Incidence rate ratios(IRRs)were estimated by Poisson regression models.RESULTS Of 193589 CLD patients(>95%non-cirrhotic)in the cohort,55163 patients with COVID-19+CLD and 55163 patients with COVID-19-CLD were included after 1:1 propensity-score matching.Upon 249-d median follow-up,COVID-19+CLD was not associated with increased risk of incident HCC(IRR:1.19,95%CI:0.99-1.42,P=0.06),but higher risks of receiving palliative care for HCC(IRR:1.60,95%CI:1.46-1.75,Pboth acute and post-acute phases of infection,COVID-19+CLD were associated with increased risks of allcause mortality(acute:IRR:7.06,95%CI:5.78-8.63,P<0.001;post-acute:IRR:1.24,95%CI:1.14-1.36,P<0.001)and adverse hepatic outcomes(acute:IRR:1.98,95%CI:1.79-2.18,P<0.001;post-acute:IRR:1.24,95%CI:1.13-1.35,P<0.001),compared to COVID-19-CLD.CONCLUSION Although CLD patients with SARS-CoV-2 infection were not associated with increased risk of HCC,they were more likely to receive palliative treatment

The spirit of now in nowcasting suggests expanding the current to include the near future.Decision theory is then developed by incorporating the consequences of actions into the present.With the future falling into the present discounting it is no longer permitted.Value functions are then observed to be determinate only up to scale and shift that are then locked down by fixing values arbitrarily in two selected states,much like declaring water to freeze and boil at zero and a hundred degrees celsius.The locked down value functions associated policy functions are seen to exist in decision contexts in where the only time is now.Examples are studied in univariate and multivariate dimensions for the decision state space and the dimension of shocks delivering state transitions.The policy functions are expanded from realisitic training sets to the full state space using Gaussian Process Regression.They are implemented on real data with reported performances.

This research focuses on the electric behavior of a mixed ferrielectric sulflower-like nanostructure.The structure includes a core with spin S^(Z)_(i)-1 atoms and a shell with spinσ^(Z)_(j)-5/2 atoms.The blume–Capel model and the Monte Carlo technique(MCt)with the Metropolis algorithm are employed.Diagrams are established for absolute zero,investigating stable spin configurations correlated with various physical parameters.The MCt method explores phase transition behavior and electric hysteresis cycles under different physical parameters.

background Little is known regarding risk factors specific to pedialric upper extremity firearm injuries.The purpose of this study is to evaluate pediatric patients treated for these injuries to identify at-risk populations and recurring mechanisms of injury.Methods A 20-year retrospective review was conducted.Patients 17 years of age and younger,with upper extremity injuries related to a firearm,were included.Analysis involved Fisher's exact and Chi-square tests.Results One hundred and eighty patients were included.The mean age was 12.04±4.3 years.Most included patients were male(85%).Interestingly,females were more frequently victims of assault(P=0.03),and males were more frequently injured due to accidental discharge(Pbe accidental than proximal injuries(P=0.003).Air rifes were the most common firearm type used(56%).Pistols were implicated in 47(26%)cases,rifles in 17(9%),and.shotguns in 10(6%).Ninety-nine(55%)patients had procedures in the operating room.The most frequent procedure was foreign body removal(55%).Conclusions Risk factors such as male sex,W hite-not Hispanic or Latino race/ethnicity,and adolescent age were attributed to increased risk for injury.Male sex was associated with increased risk of injury by accidental discharge and female sex with intentional assault.Air rifles were the most common firearm type overall,although female sex was associated with increased risk for injury by powder weapon.
