Introduction: Goiter often poses aesth.tic disgrace problem. Th. psych.social impacts of goiter, in particular in woman may be significant. Objective: studying th. psych.logical and social impacts of th. disease in goiter patients with.n th. African context and th.ir consequences on th. treatment. Meth.ds: It was a transversal study th.t took place from May 1to July 31, 2011 in th. Collines and Donga departments situated in th. center and th. north.of Benin. Th. study consisted of a survey conducted th.0}ugh.an individual discussion based on questionnaire submitted to goiter patients wh. accepted to provide th.ir answers. Results: Th.s survey involved 86 patients of wh.ch.83 women and 3 men. Th.ir average age was 43.7 ± 13.41. Goiter was noticeable in th. totality of th. patients. 68.6% of patients declared th.t th.y experienced on daily basis th. sh.me caused by th. disease. 66% th.ugh. th.t it was a natural disease;h.wever, 27.9% believed th.t it was caused by bewitch.ent or sorcery. 46.5% and 37.2% resorted to modern and indigenous medicine respectively;16.3% made no th.rapeutic move. Despite th. psych.social impacts of th. disease, 50% of th. patients rejected surgical intervention possibility. Th. core motives th.y raised were lack of financial means (34.8%), disease recurrence frigh. (23.3%), anesth.sia frigh. (16.3%), and surgical frigh. (14%). Conclusion: Despite th. psych.logical impacts of th. disease, strong resistance exists probably more on cultural aspect th.n th. motives mentioned by th.roidectomy patients.

Introduction Th. distribution of congenital h.art disease (Ch.) in sub-Sah.ran Africa is h.gh.y imprecise and varies from one region to anoth.r due to th. inequality of diagnostic facilities. Th. aim of th.s study was to determine th. in-h.spital prevalence of congenital h.art disease in ch.ldren at th. Moth.r-Ch.ld h.spital of Bingerville (h.E) by specifying th. diagnostic, th.rapeutic and evolutionary aspects. Materials and meth.ds We conducted a retrospective, descriptive, cross-sectional study at h.E of Bingerville from January 2021 to December 2022. All newborns with.congenital h.art disease confirmed by ech.cardiograph. were included in th. study. Results Of 656 admissions to th. neonatology department over th. study period, congenital h.art disease accounted for 14.9% (98/656) of cases. In our series, 76.7% were diagnosed before th. 1st week of life, with.a mean ch.0}nological age of 5.18 days and extremes of 0 and 46 days. Th.re were as many male patients (50%) as female (50%), i.e. a sex ratio of 1. Th.se newborns were premature in 60.2% of cases, with.a mean and median gestational age of 34 weeks’ amenorrh.a. Most were left-righ. sh.nts (90.8%). Persistent ductus arteriosus (PDA) (48.9%) predominated, followed by atrial septal defect (38.7%), ventricular septal defect (13.3%), common trunk artery (CTA) (3.1%) and open septal pulmonary atresia (OspA) (1%) as th. primary cyanogenic h.art disease. Pulmonary arterial h.pertension (PAh. (50%) was primary in 38.8% and secondary (61.2%). Th. mortality rate was 30.6%, and all CTA patients died (100%), with.a significant statistical relationsh.p (p = 0.027). Progression under treatment was marked by clinical stabilization (68/98) in 69.4% of cases. Conclusion: Congenital h.art disease is relatively common at th. Bingerville h.E. Access to ech.cardiograph. sh.uld be facilitated in neonatology departments for rapid diagnosis and optimal management of congenital h.art disease in newborns.

h.sto-Epidemiological Profile of h.ad and Neck Cancers in Benin

Background: Th. collection of data on cancers is essential to assess th.ir importance in a population and plan control strategies. In Benin as in many sub-Sah.ran Africa countries, cancer data are often not well informed. Purpose: To study th. epidemiological and anatomopath.logical profiles of h.ad and neck cancers. Meth.ds: Th.s was a descriptive cross-sectional study of h.ad and neck cancers in Benin from January 2009 to 31 December 2014. Th.se tumours were collected in th. registers of th. 5 anatomy-path.logical laboratories in Benin as well as in th.t of th. only h.spital in th. country th.t sends its requests for h.stological examination abroad. Results: During th. period, 611 cases of h.ad and neck tumour were reported, including 298 malignant cases, or 48.8%. Th. average age was 45.3 ± 18.7 years (extremes: 1 year and 91 years) with.a maximum of cases between 50 and 60 years (19.1%). Th. sex ratio was 1.2. Th. cancers occurred mainly in th. ph.rynx (27.9%), th. oral cavity (19.8%), th. nose, sinuses and jaws (18.1%), th. th.roid (12.8%), th. salivary glands (8.7%) and th. larynx (8.4%). Th.y were of epith.lial origin in 79.5% of cases, mainly squamous cell carcinomas (50.3%) and lymph.mas in 12.8% of cases. Conclusion: h.ad and neck cancer is preventable. A special place sh.uld be given to th. figh. against cancer in Benin. Strategies to prevent and manage th.se cancers cannot be put in place with.ut accurate data collection.

Risk Factors for Acute Otitis Media in Ch.ldren Aged 0 to 5 Years in Parakou

Introduction: Acute otitis media (AOM) is one of th. most common ENT ch.ldh.od diseases. Objective: To identify risk factors for AOM in ch.ldren aged 0 to 5 years. Meth.ds: Th.s research.work was a cross-sectional, descriptive and analytical study. It focused on 2040 ch.ldren aged 0 to 5 years from both.sexes, wh. were h.alth. or sick, and living in th. Local government of Parakou for at least one year. Th.se ch.ldren were randomly selected in all th.ee districts of Parakou. Results: Frequency of AOM was 2.8% and th.ir prevalence was estimated at 16.3%. Th. identified risk factors were persistent or ch.0}nic cases of rh.nitis, exposure to ch.rcoal and wood smoke, low socioeconomic status, personal h.story of AOM, AOM h.story among th. siblings, and ch.ldren’s poor nutritional status. No relationsh.p could be establish.d between AOM occurrence and factors like sex, passive smoking, attendance of a day-care centre or stay in nursery, prematurity, exclusive breastfeeding and large number of siblings. Conclusion: Th. identification of th.se risk factors will h.lp put in place appropriate measures to reduce AOM prevalence in Parakou.

Systemic ch.moth.rapy for h.patocellular carcinoma in non-cirrh.tic liver:A retrospective study

AIM:To investigate th. efficacy and toxicity of systemic ch.moth.rapy in a retrospective study of patients with.h.patocellular carcinoma(h.C)occurring in normal or fibrotic liver with.ut cirrh.sis. METh.DS:Twenty-four patients with.metastatic or locally advanced h.C in a normal or a fibrotic liver were given systemic ch.moth.rapy(epirubicin,cis- platin and 5-fluorouracil or epirubicin,cisplatin and capecitabine regimens).Tumor response,time to pro- gression,survival,and toxicity were evaluated. RESULTS:Th.re were 7 women and 17 men,mean age 54±10 years;18 patients h.d a normal liver and 6 h.d a fibrotic liver(F1/F2 on biopsy).Mean tumor size was 14 cm,5 patients h.d portal vein th.0}mbosis and 7 h.d *** received a median of 4 ch.moth.rapy *** tolerance was good. Th.re were 5 partial responses(objective response rate =22%),and tumor control rate was 52%.Second line surgical resection was possible in two *** survival was 11 mo,and 1-and 2-year overall survival rates were 50%±10%and 32%±11%,respectively. CONCLUSION:In patients with.h.C in a non-cirrh.tic liver,ch.moth.rapy was well tolerated and associated with.an objective response rate of 22%,including two patients wh. underwent secondary surgical resection.

ENT Path.logies Screening in Woodworkers in Parakou,Benin

Wood dust may induce h.alth.risks on exposed timber or wood workers, one of wh.ch.is ENT disorders. Th.s article aimed to detect ENT path.logies found among woodworkers in Parakou. It was a cross-sectional descriptive study carried out from 1st March.to 31st May, 2012 in Parakou, North.Benin. It involved 703 carpenters and sawyers operating in timber worksh.ps in Parakou, regardless of age and sex. Th. mean age of th. wood-workers was 26.14 ± 7.77 years. Th.ir seniority in th. timber profession was on average 4.9 ± 2.64 years. All of th.m were males. It h.d been noticed th.t 81.6% of th.m did not comply with.any safety measure for th.ir protection. ENT path.logy h.d been diagnosed in 60.3% of th. timber workers. Rh.nitises came first and affected 43.1% of th. workers, followed by ph.ryngitises (14.1%). Th. h.stological nature of th. only case of tumor observed in Parakou could not h.ve been specified, as th. patient refused to undergo anatomopath.logical examination. Measures sh.uld be taken to get Parakou timber workers to protect th.mselves.

Th. cumulative effects of periodic redox cycling on th. mobility of As,Fe,and S from alluvial sediment to groundwater were investigated in bioreactor *** particular sediments from th. alluvial floodplain of th. Mekong Delta River were investigated: Matrix A (14 m deep) h.d a h.gh.r pyrite concentration th.n matrix B (7 m deep) *** present in matrix B but absent in matrix *** th. reactors,th. sediment suspensions were supplemented with.As(III) and SO4^2-,and were subjected to th.ee full-redox cycles entailing ph.ses of nitrogen/CO2,compressed air sparging,and cellobiose *** differences in As concentration and speciation were observed upon redox *** support th. fact th.t initial sediment composition is th. main factor controlling arsenic release and its speciation during th. redox ***,a h.gh.pyrite content associated with.a low SO4^2- content resulted in an increase in dissolved As concentrations,mainly in th. form of As(III),after anoxic h.lf-cycles;wh.reas a decrease in As concentrations mainly in th. form of As(V),was instead observed after oxic *** addition,oxic conditions were found to be responsible for pyrite and arsenian pyrite oxidation,increasing th. As pool available for *** same processes seem to occur in sediment with.th. presence of gypsum,but,in th.s case,dissolved As were sequestered by biotic or abiotic redox reactions occurring in th. FeeS system,and by specific ph.sico-ch.mical condition (***).Th. contrasting results obtained for two sediments sampled from th. same core sh.w th.t many complexes and entangled factors are at work,and furth.r refinement is needed to explain th. spatial and temporal variability of As release to groundwater of th. Disease activity and cancer risk in inflammatory bowel disease associated with.primary sclerosing ch.langitis

AIM: To investigate th. ph.notype of inflammatory bowel disease associated with.primary sclerosing ch.langitis (PSC-IBD). METh.DS: Data from 75 PSC-IBD patients evaluated in our tertiary center between 1963 and 2006 were collected and compared to 150 IBD patients with.ut PSC, match.d for sex, birth.date, IBD diagnosis date and initial disease location regarding ileal, different colonic segments, and rectum, respectively. RESULTS: Wh.le PSC-IBD patients received more 5-aminosalicylates (8.7 years/patient vs 2.9 years/ patient, P h.y required less immuno-suppressors (24% vs 46% at 10 years; P h. 25-year cumulative rate of colectomy was 25.1% in PSC-IBD and 37.3% in controls (P = 0.004). Th. 25-year cumulative rate of colorectal cancer was 23.4% in PSC-IBD vs 0% in controls (P = 0.002). PSC was th. only independent risk factor for th. development of colorectal cancer (OR = 10.8; 95% CI, 3.7-31.3). Overall survival rate with.ut liver transplantation was reduced in PSC-IBD patients (67% vs 91% in controls at 25 years, P = 0.001).CONCLUSION: Th.s study confirms th.t patients with.PSC-IBD h.ve a particular disease ph.notype independent of th. initial disease location. Alth.ugh.th.ir disease is less active and th.y use more 5-aminosalicylates, th.y present a h.gh.r risk of colorectal cancer.
