Ophthalmic profile and systemic features of pediatric facial nerve palsy

Background: Facial nerve palsy(FNP) occurs less frequently in children as compared to adults but most cases are secondary to an identii able cause. These children may have a variety of ocular and systemic features associated with the palsy and need detailed ophthalmic and systemic ***: This was a retrospective chart review of all the cases of FNP below the age of 16 years, presenting to a tertiary ophthalmic hospital over the period of 9 years, from January 2000 to December ***: A total of 22 patients were included in the study. The average age at presentation was 6.08 years(range, 4 months to 16 years). Only one patient(4.54%) had bilateral FNP and 21 cases(95.45%) had unilateral FNP. Seventeen patients(77.27%) had congenital palsy and of these, five patients had a syndromic association, three had birth trauma and nine patients had idiopathic palsy. Five patients(22.72%) had an acquired palsy, of these, two had a traumatic cause and one patient each had neoplastic origin of the palsy, iatrogenic palsy after surgery for hemangioma and idiopathic palsy. Three patients had ipsilateral sixth nerve palsy, two children were diagnosed to have Moebius syndrome, one child had an ipsilateral Duane's syndrome with ipsilateral hearing loss. Corneal involvement was seen in eight patients(36.36%). Amblyopia was seen in ten patients(45.45%). Neuroimaging studies showed evidence of trauma, posterior fossa cysts, pontine gliosis and neoplasms such as a chloroma. Systemic associations included hemifacial macrosomia, oculovertebral malformations, Dandy Walker syndrome, Moebius syndrome and cerebral ***: FNP in children can have a number of underlying causes, some of which may be life threatening. It can also result in serious ocular complications including corneal perforation and severe amblyopia. These children require a multifaceted approach to their care.

Microdoses Levels of Butadiene Diepoxide (BDO2) Induced Toxicity in Prostate Cancer Cells

Low-level exposure to environmental pollutants such as BDO2 contributes directly and indirectly to an increase in PCa. The aim of this study was to define the cellular changes associated with micro-doses of Butadiene Diepoxide (BDO2) in prostate cancer cells. We observed that micro-doses of BDO2 resulted in dose- and time-dependent increases in cytotoxicity and increased expression of prostate tumor markers in LNCaP(AR+) and DU145(AR-) cells. There was an increased sensitivity of DU145(AR-) cells to BDO2 toxicity which was reversed by transient transfection of AR into theses cell. Exposure of prostate cells to BDO2 increases cytotoxicity, and apoptosis, which correlates with increases in caspases and Bcl2 protein and mRNA levels. In cell DU145(AR-) cell transient transfected with a functional AR, the levels of cytotoxicity and caspase activity were decreased in the presence of BDO2, but BDO2-induced apoptotic protein expression was unaltered. This study provides evidence that micro-doses of BDO2 modulate prostate cell toxicity by promoting apoptosis and tumor gene expression.

Enhanced Ferroelectric and Converse Piezoelectric Properties of Dense Lead-Free Na0.4K0.1Bi0.5TiO3Ceramics for Actuator Applications

In this work, lead-free Na0.4Bi0.5 TiO3-K0.1Bi0.5TiO3 (NKBT) piezoelectric ceramics were prepared by the solid-state reaction method and their structural, ferroelectric and dielectric properties were systematically studied. NKBT sintered at 1150℃ for 4 h exhibited highly dense (96% - 98% of the theoretical density) and uniform grains of size 1.1 μm. The coexistence of tetragonal (P4bm) and rhombohedral (R3c) phases due to the presence of morphotrophic phase boundary for the chosen composition has been confirmed by the Rietveld refinement studies. Enhanced ferroelectric properties such as remnant polarization (Pr) and coercive field (EC) are found to be 37 μC/cm2 and 30 kV/cm, respectively. The optimized synthesis procedure for NKBT ceramics resulted in enhanced strain (%) and converse piezoelectric coefficient (d33*) of 0.3 % and 554 pm/V which is attributed to smaller grain size and switching of non 180o domains. NKBT ceramics with such excellent piezoelectric properties can be considered as a promising candidate for actuator applications.

The functional interdependence of nerves and blood vessels is a well-established concept during tissue morphogenesis, yet the role of neurovascular coupling in proper and aberrant tissue repair is an emerging field of interest. Here, we sought to define the regulatory relationship of peripheral nerves on vasculature in a severe extremity trauma model in mice, which results in aberrant cell fate and heterotopic ossification(HO). First, a high spatial degree of neurovascular congruency was observed to exist within extremity injury associated heterotopic ossification. Vascular and perivascular cells demonstrate characteristic responses to injury,as assessed by single cell RNA sequencing. This vascular response to injury was blunted in neurectomized mice, including a decrease in endothelial proliferation and type H vessel formation, and a downregulation of key transcriptional networks associated with angiogenesis. Independent mechanisms to chemically or genetically inhibit axonal ingrowth led to similar deficits in HO site angiogenesis, a reduction in type H vessels, and heterotopic bone formation. Finally, a combination of single cell transcriptomic approaches within the dorsal root ganglia identified key neural-derived angiogenic paracrine factors that may mediate neuron-to-vascular signaling in HO. These data provide further understanding of nerve-to-vessel crosstalk in traumatized soft tissues, which may reflect a key determinant of mesenchymal progenitor cell fate after injury.

Impact of an HIV Prevention Intervention on HIV Risk Behavior and Sexually Transmitted Infection among Female Sex Workers in Tamil Nadu, India

Background: In 2003 a large-scale HIV prevention program was launched for key populations in six high HIV prevalence states of India. This paper assesses the effect of exposure to the intervention on condom use with commercial clients and experiences of sexually transmitted infection (STI) among female sex workers (FSWs) in Tamilnadu, a southern Indian state, between 2006 and 2009. Methods: Data were drawn from two rounds of cross-sectional surveys conducted in 2006 (N = 2010) and 2009 (N = 2500) among FSWs in Tamilnadu, recruited through probability-based sampling. A series of multivariate logistic regression models were constructed to examine the association between exposure to the intervention and change over time with condom use and self-reported STI. All the analyses were performed using STATA 11.1. Results: Overall, 48% of FSWs in 2006 and 90% in 2009 reported exposure to the intervention. Compared to 2006, there was a considerable increase in the proportion of FSWs reporting consistent condom use with regular and occasional clients at follow-up (2009). Further, the increase in consistent condom use over time with occasional (adjusted OR = 3.53, 95% CI: 2.47 - 5.05) and regular clients (adjusted OR = 4.97, 95% CI: 3.43 - 7.16) was significantly higher among FSWs exposed to the intervention than those not exposed. Additionally, a significant decline was observed in self-reported STI overtime among FSWs exposed to the intervention compared to their counterparts (adjusted OR = 0.39, 95% CI: 0.26 - 0.59). Conclusion: The HIV prevention program in Tamilnadu resulted in increased consistent condom use and a decrease in self-reported STI among FSWs exposed to intervention. These findings suggest that HIV prevention programs should aim to saturate coverage among key populations to sustain the gains achieved.

Toll like receptors are the primary component of innate immune response. The comparison of expression of innate immune genes TLR 1 - 10 was carried out by real time PCR by quantifying mRNA in bovine PBMCs. The bovine PBMCs in vitro stimulated with FMD infectious virus lead to up regulation of TLR 2, 3, 4 and down regulation of TLR 9. But stimulation with DNA vaccine carryingVP1 genes of FMDV leads to up regulation of TLR 2 only. Difference in the TLR gene expression by the virus and DNA vaccine, may be related to the outcome of the vaccine and carrier or persistent state of the animal which is seen in FMDV.

Evaluation of Depression and Quality of Life in Patients With Psoriasis

Objective:To measure the prevalence of depression in patients with psoriasis and to evaluate the relationship between the severity of psoriasis and depression and its effect on patients’quality of ***:A total of 154 patients with a confirmed diagnosis of psoriasis were assessed to determine the severity of psoriasis based on the psoriasis area and severity index score,presence,and severity of depression using the patient health questionnaire 9,and quality of life using the dermatology life quality index *** correlation coefficient was used to demonstrate the relationship between continuous variables with 95%confidence intervals(CIs);P<0.00001 was taken to indicate statistical ***:The severity of psoriasis was mild in 36.36%of patients,moderate in 25.97%,severe in 32.47%,and very severe in 5.20%.Of the 154 patients,139(90.3%)had depression;the severity of depression was mild in most affected patients(46.7%)and severe in 2.6%of *** had a moderate effect on the quality of life in 37.01%of patients and a very large effect in 33.77%of *** severity of psoriasis was positively correlated with depression(Pearson correlation coefficient,r=0.42,P<0.00001,95%CI:0.28–0.54)and quality of life(r=0.43,P<0.00001,95%CI:0.29–0.55).Conclusion:Depression is a common comorbidity in patients with *** severity of psoriasis is positively correlated with the severity of depression and is associated with poor quality of life.

Spectrum of cardiac manifestations and its relationship to outcomes in patients admitted with scrub typhus infection

AIM To study the spectrum of cardiac manifestations in scrub typhus infection and assess its relationship to *** Demographic data, electrocardiographic(ECG) changes, left ventricular (LV) systolic and diastolic function, myocardial injury (defined as troponin T > 14 pg/mL), and pericardial effusion were documented. Myocarditis was diagnosed when myocardial injury was associated with global LV systolic dysfunction. The relationship between myocarditis and outcomes was assessed using logistic regression analysis and expressed as odds ratio(OR) with 95%*** The cohort(n = 81; 35 males) aged 49.4 ± 16.1 years(mean, SD) presented 8.1 ± 3.1 d after symptom onset. The APACHE-Ⅱ score was 15.7 ± 7.0. Fortyeight (59%) patients were ventilated, and 46 (56%) required vasoactive agents. Mortality was 9.9%. ECG changes were non-specific; sinus tachycardia was the most common finding. Myocardial injury was evident in 61.7% of patients and LV systolic dysfunction in 30.9%. A diagnosis of myocarditis was made in 12.3%. In addition, seven patients with regional wall motion abnormalities had LV systolic dysfunction and elevated cardiac enzymes. Mild diastolic dysfunction was observed in 18 (22%) patients. Mild to moderate pericardial effusion was seen in 51%. On multivariate logistic regression analysis, patients with myocarditis tended to be older (OR = 1.04, 95%CI: 0.99-1.09), had shorter symptom duration (OR = 0.69, 95%CI: 0.49-0.98), and tended to stay longer in hospital(OR = 1.17, 95%CI: 0.98-1.40). Myocarditis was not associated with increased *** In scrub typhus infection, cardiac manifestations are frequent and associated with increased morbidity but not mortality.
