Success of photodynamic therapy in palliating patients with nonresectable cholangiocarcinoma: A systematic review and meta-analysis

To perform a systematic review and meta-analysis on clinical outcomes of photodynamic therapy (PDT) in non-resectable *** studies compared outcomes with photodynamic therapy and biliary stenting (PDT group) vs biliary stenting only (BS group) in palliation of non-resectable cholangiocarcinoma. Articles were searched in MEDLINE, PubMed, and EMBASE. Pooled proportions were calculated using fixed and random effects model. Heterogeneity among studies was assessed using the I2 *** studies (n = 402) that met inclusion criteria were included in this analysis. The P for χ2 heterogeneity for all the pooled accuracy estimates was > 0.10. Pooled odds ratio for successful biliary drainage (decrease in bilirubin level > 50% within 7days after stenting) in PDT vs BS group was 4.39 (95%CI: 2.35-8.19). Survival period in PDT and BS groups were 413.04 d (95%CI: 349.54-476.54) and 183.41 (95%CI: 136.81-230.02) respectively. The change in Karnofsky performance scores after intervention in PDT and BS groups were +6.99 (95%CI: 4.15-9.82) and -3.93 (95%CI: -8.63-0.77) respectively. Odds ratio for post-intervention cholangitis in PDT vs BS group was 0.57 (95%CI: 0.35-0.94). In PDT group, 10.51% (95%CI: 6.94-14.72) had photosensitivity reactions that were self-limiting. Subgroup analysis of prospective studies showed similar results, except the incidence of cholangitis was comparable in both *** palliation of unresectable cholangiocarcinoma, PDT seems to be significantly superior to BS alone. PDT should be used as an adjunct to biliary stenting in these patients.

Progression from low-grade dysplasia to malignancy in patients with Barrett's esophagus diagnosed by two or more pathologists

AIM To evaluate annual incidence of low grade dysplasia(LGD) progression to high grade dysplasia(HGD) and/or esophageal adenocarcinoma(EAC) when diagnosis was made by two or more expert *** Studies evaluating the progression of LGD to HGD or EAC were included. The diagnosis of LGD must be made by consensus of two or more expert gastrointestinal pathologists. Articles were searched in Medline, Pubmed, and Embase. Pooled proportions were calculated using fixed and random effects model. Heterogeneity among studies was assessed using the I2 statistic. RESULTS Initial search identified 721 reference articles, of which 53 were selected and reviewed. Twelve studies(n = 971) that met the inclusion criteria were included in this analysis. Among the total original LGD diagnoses in the included studies, only 37.49% reached the consensus LGD diagnosis after review by two or more expert pathologists. Total follow up period was 1532 patient-years. In the pooled consensus LGD patients, the annual incidence rate(AIR) of progression to HGD and or EAC was 10.35%(95%CI: 7.56-13.13) and progression to EAC was 5.18%(95%CI: 3.43-6.92). Among the patients down staged from original LGD diagnosis to No-dysplasia Barrett's esophagus, the AIR of progression to HGD and EAC was 0.65%(95%CI: 0.49-0.80). Among the patients down staged to Indefinite for dysplasia, the AIR of progression to HGD and EAC was 1.42%(95%CI: 1.19-1.65). In patients with consensus HGD diagnosis, the AIR of progression to EAC was 28.63%(95%CI: 13.98-43.27). CONCLUSION When LGD is diagnosed by consensus agreement of two or more expert pathologists, its progression towards malignancy seems to be at least three times the current estimates, however it could be up to 20 times the current estimates. Biopsies of all Barrett's esophagus patients with LGD should be reviewed by two expert gastroenterology pathologists. Follow-up strict surveillance programs should be in place for these patients.

In-vehicle communication has been optimized day to day to keep updated of the *** area network(CAN)is used as a standard communication method because of its efficient and reliable ***,CAN is prone to several network level attacks because of its lack in security *** methods have been introduced to incorporate this in *** proposed an unsupervised method of intrusion detection for in-vehicle communication networks by combining the optimal feature extracting ability of autoencoders and more precise clustering using fuzzy C-means(FCM).The proposed method is light weight and requires less computation *** performed an extensive experiment and achieved an accuracy of 75.51%with the ML35o in-vehicle intrusion *** experimental result,the proposed method also works better for other intrusion detection problems like wireless intrusion detection datasets such as WNS-DS with accuracy of 84.05%and network intrusion detection datasets such as KDDCup with accuracy 60.63%,UNSW_NB15 with accuracy 73.62%and Information Security Center of Excellence(Iscx)with accuracy 74.83%.Overall,the proposed method outperforms the existing methods and avoids labeled datasets when training an in-vehicle intrusion detection *** results of the experiment of our proposed method performed on various intru-sion detection datasets indicate that the proposed approach is generalized and robust in detecting intrusions and can be effectively deployed in real time to monitor CAN traffic in vehicles and proactively alert during attacks.

We investigated ^(50,52-54)Cr-induced fusion reactions for the synthesis of the superheavy element in the 104≤Z≤122 *** cross sections produced in this investigation using ^(54)Cr projectiles were compared with those obtained in prior *** estimated cross sections from this analysis are consistent with the findings of prior *** the current study,the predicted cross section was found to be 42fb at 236 MeV for ^(53)Cr+^(243)Am,23.2 fb at 236 MeV for ^(54)Cr+^(247)Cm,95.6 fb at 240 MeV for ^(53)Cr+248Bk,and 1.33 fb at 242 MeV for ^(53)Cr+***,these projected cross sections with excitation energy and beam energy will be useful in future Cr-induced fusion reaction investigations.

A new empirical formula for the astrophysical S-factor has been suggested as a function of the Coulomb interaction parameter,center of mass energy,and barrier *** 22 fusion reactions with ^(40,48)Ca as projectiles were considered for different targets,leading to compound nuclei with atomic and mass numbers varying between 40≤Z≤≤Z≤112 and 88≤A≤27888≤A≤278,*** fusion cross-sections have been estimated using the geometric factor,the Gamow-Sommerfield factor,and the empirical formula for the *** study's findings showed better agreement with those of available experiments when compared to Wong's *** present work leads to a smaller standard deviation value than Wong's formula when used to correlate the experimental data of calcium-induced fusion ***'s formula provides a good approximation of fusion cross-sections when the center of mass energy is below the fusion barrier when compared to above the fusion barrier.

A novel diurnal and seasonal variation analysis of BTEX in Bengaluru urban area

In the urban atmosphere of Bengaluru, various volatile organic compounds(VOCs), particularly Benzene,Toluene, Ethylbenzene, and Xylene(BTEX), have shown an increasing trend in concentration. The present research was conducted during summer and monsoon seasons, focusing on Kadubeesanahalli, a high-traffic area within the Bengaluru Metropolitan City. Hourly sample data was collected using a BTEX analyzer(Model GC955-600) and subsequently transformed into daily, monthly, and seasonal values. The study revealed distinct patterns in benzene concentrations. Benzene levels were lowest during the early morning hours, specifically from 1:00 *** 7:00 a.m.. Concentrations then increased from 7:00 a.m. to 9:00 a.m. and again from 4:00 p.m. to 11:00 p.m.,corresponding to the morning and evening peak traffic hours. However, between 10:00 a.m. and 4:00 p.m., the concentration decreased due to reduced traffic levels. These diurnal variations in benzene concentration are influenced by meteorological parameters. Comparing the two seasons, higher concentrations of Benzene, EthylBenzene, and MP-xylene were observed during the summer season. This increase is attributed to the elevated temperatures during summer, which promote the vaporization of BTEX compounds. Conversely, lower BTEX concentrations were recorded during the monsoon season due to the wet deposition process. The observed positive correlation(r > 0.5) among BTEX parameters strongly suggests a common source, most likely originating from vehicular emissions.

While hypoxic signaling has been shown to play a role in many cellular processes,its role in metabolism-linked extracellular matrix(ECM)organization and downstream processes of cell fate after musculoskeletal injury remains to be ***(HO)is a debilitating condition where abnormal bone formation occurs within extra-skeletal *** andhypoxia-inducible factor 1α(HIF-1α)activation have been shown to promote ***,the underlying molecular mechanisms bywhich the HIF-1αpathway in mesenchymal progenitor cells(MPCs)contributes to pathologic bone formation remain to ***,we used a proven mouse injury-induced HO model to investigate the role of HIF-1αon aberrant cell ***-cell RNA sequencing(scRNA-seq)and spatial transcriptomics analyses of the HO site,we found that collagen ECM organizationis the most highly up-regulated biological process in *** mesenchymal cell-specific deletion of Hif1α(Hoxa11-CreER^(T2);Hif1a^(fl/fl))significantly mitigated HO in ***-seq analysis of these Hoxa11-CreER^(T2);Hif1a^(fl/fl)mice identified the PLOD2/LOXpathway for collagen cross-linking as downstream of the HIF-1αregulation of ***,our scRNA-seq data and mechanisticstudies further uncovered that glucose metabolism in MPCs is most highly impacted by HIF-1α*** a translational aspect,a pan-LOX inhibitor significantly decreased HO.A newly screened compound revealed that the inhibition of PLOD2 activity in MPCssignificantly decreased osteogenic differentiation and glycolytic *** suggests that the HIF-1α/PLOD2/LOX axis linked tometabolism regulates HO-forming MPC *** results suggest that the HIF-1α/PLOD2/LOX pathway represents a promisingstrategy to mitigate HO formation.

Plasticized Polymer Electrolyte Membranes Based on PEO/PVdF-HFP for Use as an Effective Electrolyte in Lithium-ion Batteries

Plasticized polymer electrolytes were prepared using poly（ethylene oxide）(PEO)/poly（vinylidene fluoridehexafluoro propylene）(PVd F-HFP) with lithium perchlorate（Li Cl O4） and different plasticizers. XRD and FTIR spectroscopic techniques were used to characterize the structure and the complexation of plasticizer with the host polymer matrix. The role of interaction between polymer hosts and plasticizer on conductivity is discussed using the results of alternating current（a.c.） impedance studies. TG-DTA and SEM were used for thermal and physical characterizations. Maximum ionic conductivity(3.26 × 10;S·cm;) has been observed for ethylene carbonate（EC）-based polymer electrolytes. Electrochemical performance of the plasticized polymer electrolyte is evaluated in LiFePO;/plasticized polymer electrolytes（PPEs）/Li coin cell. Good performance with low capacity fading on charge discharge cycling is demonstrated.

Culture-expanded mesenchymal stromal cell therapy: does it work in knee osteoarthritis? A pathway to clinical success

Osteoarthritis(OA)is a degenerative multifactorial disease with concomitant structural,inflammatory,and metabolic changes that fluctuate in a temporal and patient-specific *** complexity has contributed to refractory responses to various *** have shown promise as multimodal therapeutics in mitigating OA symptoms and disease ***,we evaluated 15 randomized controlled clinical trials(RCTs)and 11 nonrandomized RCTs using culture-expanded MSCs in the treatment of knee OA,and we found net positive effects of MSCs on mitigating pain and symptoms(improving function in 12/15 RCTs relative to baseline and in 11/15 RCTs relative to control groups at study endpoints)and on cartilage protection and/or repair(18/21 clinical studies).We examined MsC dose,tissue of origin,and autologous *** origins as well as patient clinical phenotype,endotype,age,sex and level of OA severity as key parameters in parsing MSC clinical *** relatively small sample size of 610 patients limited the drawing of definitive ***,we noted trends toward moderate to higher doses of MsCs in select OA patient clinical phenotypes mitigating pain and leading to structural improvements or cartilage *** from preclinical studies is supportive of MsC anti-inflammatory and immunomodulatory effects,but additional investigations on immunomodulatory,chondroprotective and other clinical mechanisms of action are *** hypothesize that MsC basal immunomodulatory"fitness"correlates with OA treatment efficacy,but this hypothesis needs to be validated in future *** conclude with a roadmap articulating the need to match an OA patient subset defined by molecular endotype and clinical phenotype with basally immunomodulatory"ft"or engineered-to-be-fit-for-OA MsCs in well-designed,data-intensive clinical trials to advance the field.

Effects of stretching/shrinking on the thermal performance of a fully wetted convective-radiative longitudinal fin of exponential profile

The present investigation focuses on the thermal performance of a fully wet stretching/shrinking longitudinal fin of exponential profile coated with a mechanism like a conveyer *** modeled equation is non-dimensionalized and solved by applying the Runge-Kutta-Fehlberg(RKF)*** effects of parameters such as the wet parameter,the fin shape parameter,and the stretching/shrinking parameter on the heat transfer and thermal characteristics of the fin are graphically analyzed and *** is inferred that the negative effects of motion and internal heat generation on the fin heat transfer rate can be lessened by setting a shrinking mechanism on the fin *** current examination is inclined towards practical applications and is beneficial to the design of fins.
