Introduction: Expert opinions presented in legal proceedings should be scientifically accountable, which is known as evidence-based practice (EBP). Although forensic medical expert opinions are essential in legal proceedings, the methods used to formulate them are not always evidence based or based on standard methods. In forensic medicine, EBP has not been explicitly applied, including in Indonesia. One potential approach to formulate evidence-based expert opinions is called INtegration of Forensic Epidemiology and the Rigorous EvaluatioN of Causation Elements (INFERENCE). In addition, there is also no universal guideline for making forensic pathological reports. One prospective guideline is named the Principles of Evidence-based Reporting in FORensic Medicine-Pathology version (PERFORM-P). Methods: This article describes the validation process of INFERENCE and PERFORM-P in Indonesia. This study uses a mixed method through three interrelated phases, i.e., (1) a cross-sectional survey to determine the characteristics of Indonesian forensic doctors and their current practice, (2) the adaptation and validation process of the two tools through a review by the Indonesian College of Forensic Medicine, and (3) a one-group pre-postintervention study to assess the validity and reliability of forensic medical expert opinions formulated using Indonesian-INFERENCE (i-INFERENCE) and reported using Indonesian-PERFORM-P (i-PERFORM-P). Results and Discussion: In general, both tools received a positive reception and can potentially be used in the Indonesian setting with some additions/clarifications in the user manuals. Participants envision that both tools will be most useful in complex cases. Conclusions: By obtaining the i-INFERENCE and the i-PERFORM-P, it is hoped that Indonesian forensic medical doctors are better equipped in analyzing and reporting complex cases, and the implementation of EBP can be improved.

In the present work,an attempt is made to partially replace high cost silica sand with sustainable eco-friendly *** practical utility of cow dung as an additive material that facilitates binding in sand moulds has been *** Taguchi method is used to plan and conduct nine experiments with three replicates *** analysis of variance study is performed to understand the practical significance of moulding sand variables,that is,percentage of cow dung,clay,water,and degree of ramming on sand mould *** conflicting multiple objective functions(maximize mould hardness,and minimize collapsibility and gas evolution)are optimized by utilizing the Data Envelopment Analysis Ranking(DEAR)*** optimal parameters i.e.6%of clay,4%of water,5%of cow-dung and 4 numbers of ramming strokes are obtained by applying a hybrid Taguchi-DEAR *** parameters show the best moulding properties are:mould hardness 55,gas evolution 5.9 ml·g-1,and collapsibility 470 g·***,a Lovejoy coupling made of aluminium is cast in the sand mould prepared with cow dung and without cow dung,with the optimum set of *** mechanical properties,surface finish,and microstructure of the casting cast with cow-dung are better than that without cowdung.

Engineering of Carbonate Apatite Bone Substitute Based on Composition-Transformation of Gypsum and Calcium hydroxide

Even though a lot of research has been carried out concerning the preparation of carbonate apatite (ChA), they were related to ChA in the form of powder. In the present study, macroporous ChA bone substitutes were prepared through composition-transformation of gypsum and Ca-hydroxide. here, we investigated the effect of added Ca-hydroxide to gypsum, carbonation periods, and hydrothermal temperatures for phosphatization to understand the basic principle of composition-transformation of gypsum added Ca-hydroxide to fabricate ChA bone substitutes. The specimens were characterized in terms of chemical and physical properties, such as extent of transformation of macroporous gypsum added Ca-hydroxide into ChA body, type and content of carbonate, and crystal morphology. It was observed that the transformation was faster with higher hydrothermal temperature. however, higher hydrothermal temperature caused de-carbonation phenomena which resulted in the lack of carbonate ions of the product. Moreover, the higher the percentage of Ca-hydroxide added to gypsum, caused the complete transformation of gypsum into ChA to be slower. These findings have been applied to the standard fabrication procedure of carbonate apatite, which in turn will allow scaling up process, and will be provided for biomedical purposes for the Indonesian community.

Production of morphologically and physiologically variable seeds is an important strategy that helps plants to survive in unpredictable natural ***,the model plant Arabidopsis thaliana and most agronomically essential crops produce visually homogenous *** automated phenotype analysis,we observed that small seeds in Arabidopsis tend to have higher primary and secondary dormancy levels than large *** analysis revealed distinct gene expression profiles between large and small *** seeds have higher expression of translation-related genes implicated in germination *** contrast,small seeds have elevated expression of many positive regulators of dormancy,including a key regulator of this process,the DOG1 *** in DOG1 expression are associated with differential production of its alternative cleavage and polyadenylation isoforms;in small seeds,the proximal poly(A)site is selected,resulting in a short mRNA ***,single-seed RNA sequencing analysis demonstrated that large seeds resemble DOG1 knockout mutant ***,on the single-seed level,expression of genes affected by seed size is correlated with expression of genes that position seeds on the path toward *** results demonstrate an unexpected link between seed size and dormancy phenotypes in a species that produces highly homogenous seed pools,suggesting that the correlation between seed morphology and physiology is more widespread than initially assumed.

Functional Recovery of Axonal Injury Induced by Gelatin-hydrogel Film and PRP: An Initial Study in Rats

In today’s society, axonal regeneration is considered very important because of high incidence of nerve injuries. The injury may cause vast damage of extracellular matrix leading to the condition wherein cell lost their environment to live. In this situation, new strategies by the introduction of scaffold to function as synthetic extracellular matrix are needed. In the field of tissue engineering, a large variety of carriers and matrices have been utilized as a scaffold to direct axonal growth. In this study, a new strategy by the combined application of gelatin hydrogel film and Platelet Rich Plasma (PRP) to induce functional recovery of sciatic nerve crushed injury was investigated in Wistar rat model. Assessment of motorist and sensory functions were done to evaluate the recovery processes. It was found that the application of gelatin hydrogel film with controlled release manner combined with PRP results faster axonal regeneration. Further research by different injury model and other investigation factors are needed to confirm the effectiveness of the strategy.

目的:通过对周围型肺癌及肺结核瘤MSCT征象的分析,探讨MSCT在周围型肺癌与肺结核瘤鉴别诊断的应用价值。材料与方法:对190例周围型肺癌及结核瘤患者的CT征象进行回顾性分析,其中100例为经病理学证实的周围型肺癌,90例为肺结核瘤(38例经病理学证实,52例为临床实验室资料并随访证实)。对所有结节的大小、位置、内部特征、边缘特征、周围特征进行分析、统计。结节的内部特征包括是否出现空洞或空泡征、钙化、支气管气像、支气管征等。结节的边缘特征可分为光滑、不规则、非常不规则三类。对于边缘不规则的结节,我们进一步评价是否出现分叶以及毛刺。病灶的周围征象包括:胸膜凹陷征、卫星灶、叶间裂膨隆、血管集束征、晕征等。全部数据经SPSS(11.5)统计分析软件处理,两组间不同征象的出现率采用卡方检验,P<0.05为显著性意义。结果:1. 190例结节中,100例为周围型肺癌(91例腺癌,9例细支气管肺泡癌),90例为肺结核瘤。所有结节直径为0.55mm-38.4mm(平均直径22mm)。2. 100例周围型肺癌的CT征象及统计学结果如下:周围型肺癌较结核瘤常见的CT征象为边缘不规则(96%,P<0.05)、细短毛刺(61%, P<0.05)、空洞或空泡征(47%, P<0.05)、支气管征(57%, P<0.05),较结核瘤少见的CT征象为卫星灶(8%, P<0.05)。与结核瘤比较无统计学意义的征象为粗长毛刺(9%, P=0.381)、支气管气像(79%, P=0.056)。3. 90例肺结核瘤的CT征象及统计学结果如下:结核瘤较周围型肺癌常见的CT征象为边缘规则(57.8%,P<0.05)、钙化(75.5%, P<0.05)、卫星灶(54.4%, P<0.05) ,较周围型肺癌少见的CT征象为支气管征( 37.8 % ,P<0.05)、空洞或空泡征(19.1%,P<0.05)。与周围型肺癌比较无统计学意义的征象为粗长毛刺(42.2%,P=0.381)。结论:1. MSCT对于周围型肺癌与肺结核瘤的鉴别诊断具有意义。2.对于鉴别周围型肺癌和结核瘤有帮助的CT征象包括:边缘光滑/分叶、细短/粗长毛刺、钙化、支气管征、卫星灶、空洞或空泡征,其中支气管征是周围型肺癌的特异性征象。

Clinical Management of Mid-Root Fracture in Maxillary Central Incisors:Case Reports

Management of mid-root fractures presents a formidable challenge for clinicians because of the difficulty of achieving a stable reunion of fracture fragments. This article presents two varied treatment options for mid-root fractures. A 15-year-old female reported an impact injury to the maxillary anterior teeth 2 days after its occurrence. Clinically,the maxillary left central incisor was palatally-extruded with a negative vitality response and radiographic evidence of an oblique fracture at the middle third of the root. An endodontic implant was employed which utilized an open technique and has been on follow-up for ten months. A 32-year-old male reported an injury,which resulted in a mobile maxillary right central incisor,three months after its occurrence. Through clinical and radiographic means,a discolored,extruded,and non-vital maxillary right central incisor with an oblique root fracture at the alveolar-crest level was observed. Exploratory surgery was performed; an apical barrier was created with a mineral trioxide aggregate and obturated with gutta percha. The fragments were stabilized with a fiber post and patient has been on follow-up for five months. Short-term follow-up for both of the cases showed promising results both clinically and radiographically.

Stem cells are of global excitement for various diseases including heart diseases. It is worth to understand the mechanism or role of stem cells in the treatment of heart failure. Bone marrow derived stem cells are commonly practiced with an aim to improve the function of the heart. The majority of studies have been conducted with acute myocardial infarction and a few has been investigated with the use of stem cells for treating chronic or dilated cardiomyopathy. heterogeneity in the treated group using stem cells has greatly emerged. Ever increasing demand for any alternative made is of at most priority for cardiomyopathy. Stem cells are of top priority with the current impact that has generated among physicians. however,meticulous selection of proper source is required since redundancy is clearly evident with the present survey. This review focuses on the methods adopted using stem cells for heart diseases and outcomes that are generated so far with an idea to determine the best therapeutic possibility in order to fulfill the present demand.

以往应用基因芯片对骨髓增生异常综合征(MDS)患者细胞的基因表达谱的研究发现,RAS蛋白超家族成员RAP1GAP转录水平被上调,并在MDS患者大量病例中应用定量RT-PCR得到证实。本研究探讨MDS细胞中RAP1GAP的表达及其与临床的相关性。应用流式细胞术检测19例MDS患者骨髓细胞内RAP1GAP的表达,并与非恶性血液病以及急性白血病患者骨髓细胞中RAP1GAP的表达进行比较。对RAP1GAP表达水平与MDS患者的血红蛋白水平,白细胞计数,血小板计数,骨髓细胞中原始细胞的百分数以及IPSS危险积分等临床指标之间的相关性进行了分析。结果发现,RAP1GAP的表达在MDS患者骨髓细胞中显著高于非恶性血液病或急性白血病的细胞内的表达(8.420±8.365%比2.974±4.750%或2.256±4.239%)。在MDS患者中,MDS-RA的RAP1GAP表达显著高于MDS-RAEB内的表达(11.637±9.067%比4.368±4.646%)。然而,在检测的MDS患者中RAP1GAP的表达水平与上述临床指标无确切的相关性。结论:RAP1GAP在MDS中的表达明显增高,RAP1GAP表达水平与临床实验室血液学参数及IPSS积分之间无相关性。RAP1GAP表达在MDS进展至AML中的作用及其临床意义值得进一步探讨。
