clinical and cost impact of intravenous proton pump inhibitor use in non-IcU patients

AIM:To assess the appropriateness of the indication and route of administration of proton-pump-inhibitors (PPIs) and their associated cost impact. METHODS:Data collection was performed prospec-tively during a 6-mo period on 340 patients who re-ceived omeprazole intravenously during their hospital stay in non-intensive care floors. Updated guidelines were used to assess the appropriateness of the indication and route of administration. RESULTS:complete data collection was available for 286 patients which were used to assess intravenous (IV) PPIs utilization. Around 88% of patients were receiving PPIs for claimed stress ulcer prophylaxis (SUP) indication; of which,only 17% met the guideline criteria for SUP indication,14% met the criteria for non-steroidal-anti-inflammatory drugs-induced ulcer prophylaxis,while the remaining 69% were identifi ed as having an unjustified indication for PPI use. Theinitiation of IV PPIs was appropriate in 55% of pa-tients. Half of these patients were candidates for switching to the oral dosage form during their hos-pitalization,while only 36.7% of these patients were actually switched. The inappropriate initiation of PPIs via the IV route was more likely to take place on the medical floor than the surgical floor (53% vs 36%,P = 0.003). The cost analysis associated with the appro-priateness of the indication for PPI use as well as the route of administration of PPI revealed a possible saving of up to $17 732.5 and $14 571,respectively. cONcLUSION:This study highlights the over-utili-zation of IV PPIs in non-intensive care unit patients. Restriction of IV PPI use for justified indications and route of administration is recommended.

Influence of proton pump inhibitors on gastritis diagnosis and pathologic gastric changes

AIM: To investigate the influence of proton pump inhibitors(PPIs) exposure on the diagnosis of Helicobacter pylori(H. pylori) gastritis and intestinal ***: chronic PPI use is associated with masking of H. pylori infection. Patients with H. pylori infection are predisposed to gastric and duodenal ulcers, and long-term infection with this organism has been associated with gastric mucosal atrophy and serious long-term complications, such as gastric lymphoma and adenocarcinoma. Three hundred patients diagnosed with gastritis between January 2008 and April 2010 were included in our study. The computerized medical database of these patients was reviewed retrospectively in order to assess whether the type of gastritis diagnosed(H. pylori vs non-H. pylori gastritis) is influenced by PPI exposure. H. pylori density was graded as low, if corresponding to mild density following the Updated Sydney System, or high, if corresponding to moderate or severe densities in the Updated Sydney ***: Patients were equally distributed between males and females with a median age at the time of diagnosis of 50 years old(range: 20-87). The histological types of gastritis were classified as H. pylori gastritis(n = 156, 52%) and non-H. pylori gastritis(n = 144, 48%). All patients with non-H. pylori gastritis had inactive chronic gastritis. Patients with no previous PPI exposure were more likely to be diagnosed with H. pylori gastritis than those with previous PPI exposure(71% vs 34.2%, P cted in the latter patients(1.4% vs 6.5%, P = 0.023). Multivariate analysis has also demonstrated that in the presence of previous PPI exposure(OR = 0.217, 95%cI: 0.123-0.385), GERD(OR = 0.317, 95%cI: 0.132-0.763, P = 0.01), alcohol intake(OR = 0.396, 95%cI: 0.195-0.804, P = 0.01), the detection of H. pylori was less likely. chronic use of PPIs may mask H. pylori infections promoting the diagnosis of non-H. pylori gastritis and le

This study aims to enhance the healthcare services for diabetic patients in the administrative region of Kindia by suggesting dietary interventions to assist diabetics in better managing their condition. conducted over a period of six months, from February 18 to July 18, 2021, this prospective and descriptive cross-sectional study involved 220 diabetic patients. Among these, 48 patients (22%) maintained balanced glucose levels (2 g/l). Positive glycosuria was observed in 54% of the patients, whereas 46% demonstrated normal glycosuria. An analysis of urinary parameters revealed that 15% of the patients had abnormal Ketone Bodies. Normal HbA1c levels (9%) HbA1c levels. Hematological assessments indicated significant variation: 56% of the patients had low hemoglobin levels, 4% suffered from hyper-eosinophilia, and 1% each from hyper-basophilia and hyper-hemoglobinemia. The anemic profile was characterized as mild anemia in 75%, moderate anemia in 20%, and severe anemia in 5%. Furthermore, 25% of patients were affected by Microcytic anemia and 75% by Normocytic anemia. Demographically, women constituted 65% of the study group compared to 35% of men. The most represented age bracket was 41 to 60 years, accounting for 52% of the patients, while those between 61 and 80 years comprised 36%. Every district in Kindia was impacted by diabetes.

Is there any correlation between vitamin D insufficiency and diabetic retinopathy?

AIM: To determine a relation between vitamin D level,which is an inhibitor of angiogenesis, and diabetic retinopathy and its risk factorsMETHODS: In a clinic-based cross sectional study two hundred and thirty-five type 2 diabetic patients older than 20 y were selected. Patients were classified according to ophthalmologic examination as following:no diabetic retinopathy(NDR)(n =153), non-proliferative diabetic retinopathy(NPDR)(n =64) and proliferative diabetic retinopathy(PDR)(n =18). Study subjects were tested for fasting blood glucose, glycated hemoglobin A1c(Hb A1c), lipid profile, microalbuminuria, Hs cRP,IGF1, insulin(in patients without history of insulin taking)and 25 hydroxy vitamin D [25(OH) D] levels. Vitamin D insufficiency was defined according to 25(OH) D level less than 30 ng/m L. The relationship between diabetic retinopathy and serum 25(OH) D insufficiency was ***: The prevalence of diabetic retinopathy was34.8% in our patients. Long duration of diabetes,hypertension, poor glycemic control, diabetic nephropathy, hyperinsulinemia and insulin resistance were risk factors for diabetic retinopathy but 25(OH) D level was not significant different between NDR, NPDR and PDR groups. correlation between 25(OH) D level and other known risk factors of diabetic retinopathy was not ***: This study did not find any association between diabetic retinopathy and its severity and vitamin D insufficiency. Vitamin D insufficiency is not related to risk factors of diabetic retinopathy.

Impact of cOVID-19 infection during pregnancy on maternal and fetal outcomes

Objectives:The objective of this study is to assess the impact of coronavirus disease 2019(cOVID-19)infection during pregnancy on maternal and fetal outcome in relation to gestational ***:This retrospective study was conducted between May 2020 and July ***-five women infected with severe acute respiratory syndrome coronavirus 2(SARS-coV-2)and hospitalized at a quarantine hospital were ***-four women received prenatal care at the hospital until delivery,while 31 dropped out from the study due to cOVID-19 recovery and discharge before ***:cOVID-19 was found in 83%of the pregnant women after 20 weeks of *** most common symptoms were cough and *** complications included severe pneumonia and *** maternal mortality rate was 9.2%.Premature rupture of membranes and severe hypertension during labor affected nearly 9%of the pregnant women who completed prenatal care until delivery at the *** labor occurred at a rate of 38.2%.There were seven(20.8%)stillbirths and two cases of infant ***:Hospitalized pregnant women who have coronavirus infection,lymphopenia,and a high c-reactive protein level were at a higher risk of developing severe illness,which can lead to maternal and neonatal complications.

El-Burullus Lake is one of the four Egyptian Ramsar sites that constitute internationally important wetlands as they contain a rich biodiversity and have a large number of water bird *** the valuable resources in El-Burullus Lake and its surrounding area have faced various threats to wetlands over recent *** study was carried out to evaluate the dynamics of land cover change using three change scenes of recent and past satellite data from 1990 to *** study utilized ArcGIS10.7,ERDAS Imagine 14,Landsat TM(1990,1999,and 2010),Landsat OLI-TIRS(2019)to analyze the land-use/land-cover(LULc)of El-Burullus *** Difference Vegetation Index(NDVI)and Normal-ized Difference Water Index(NDWI)were employed to assess the change scenario of the *** indicated that vegetated land has increased significantly with a concomitant shrinkage in the water body and open soil during the study *** 53 km^2(7.0%)of water body and 8.7 km^2(1.3%)of open soil were lost,while vegetation areas expanded to approximately 29.9 km^2(7.4%).Factors that underpin the observed changes in the area are;reclamation projects and fish-farms which are creating the danger of drying up of the southern coastal parts of the lake,as well as spreading of reed beds(mainly:Phragmites australis)that covers about 20%of El-Burullus lake which led to dry up the lake ***,the study suggests the need for urgent attention on conservation of remaining wetland resources for sustainable utilization for the next generation.

In Vitro assessment of the nutritive value of expanded soybean meal for dairy cattle

Little information is available about the nutritive value of expanded soybean meal, which is produced by expansion of soybeans prior to solvent extraction of the oil. During processing, expanded soybean meal is subjected to additional heat, which might increase the concentration of ruminally undegraded protein. Processing of soybeans with heat during oil extraction could affect lysine availability by increasing ruminally undegraded protein or by impairing intestinal digestion. Our objective was to compare solvent and expanded soybeans with regard to chemical composition and nutritive value for dairy cattle. Samples of expanded soybean meal （n = 14） and solventextracted soybean meal （n = 5） were obtained from People＇s Republic of china to study effects of the expansion process on nutritive value for dairy cattle. Solvent-extracted soybean meal （n = 2） and mechanically extracted （heated） soybean meal （n = 2） from the United States served as references for comparison. Samples were analyzed for crude fat, long-chain fatty acids, crude protein, amino acids, chemically available lysine, in situ ruminal protein degradation, and in vitro intestinal digestibility. No differences were found between solvent-extracted soybean meals from china and expanded soybean meals from china for crude fat, crude protein, amino acids, or chemically available lysine. In situ disappearance of nitrogen, ruminally undegraded protein content, and in vitro intestinal digestion of the ruminally undegraded protein were generally similar between solvent-extracted soybean meals made in china and expanded soybean meals made in china; variation among soybean meals was small. Results indicate that the additional heat from the expansion process was not great enough to affect the nutritive value of soybean meal protein for ruminants. Generation of sodium hypochlorite (NaOcl) from sodium chloride solution using c/PbO_2 and Pb/PbO_2 electrodes

Two modified electrodes （Pb/PbO2 and c/PbO2） were prepared by electrodepositing a lead oxide layer on lead and carbon sub- strates. These modified electrodes were used as anodes for the generation of sodium hypochlorite （NaOc1） from sodium chloride solution. Different operating conditions and factors affecting the treatment process of NaOc1 generation, including current density, pH values, con- ductive electrolytes, and electrolysis time, were studied and optimized. By comparison the c/PbO2 electrode shows a higher efficiency than the Pb/PbO2 electrode for the generation of NaOc1.
