Endoscopic transpapillary brush cytology and forceps biopsy during endoscopic retrograde cholangiopancreatology are generally used to obtain pathological evidence of biliary strictures. Recently, the new endoscopic scraper Trefle~? has been reported and demonstrated high cancer detectability in malignant biliary strictures. This device is used to scrape the stricture over the guidewire, and, in the original method, the tissue and/or cell samples obtained are subjected to histological and/or cytological analysis separately. However, discrimination of chunks of tissue is hampered by the opacity of the surrounding fluid. We have developed a cell block technique for the Trefle~? device without dividing obtained specimens into tissue and cellular components, which is the simplest method and enables immunohistochemical analysis. We present a case of obstructive jaundice diagnosed immunohistochemically as pancreatic metastasis from colon cancer using cell block sections obtained with the Trefle~? device, which procedure is as easy as conventional brush cytology.

Laparoscopic appendectomy for acute appendicitis: How to discourage surgeons using inadequate therapy

Acute appendicitis(AA) develops in a progressive and irreversible manner, even if the clinical course of AA can be temporarily modified by intentional medications. Reliable and real-time diagnosis of AA can be made based on findings of the white blood cell count and enhanced computed tomography. Emergent laparoscopic appendectomy(LA) is considered as the first therapeutic choice for AA. Interval/delayed appendectomy at 6-12 wk after disease onset is considered as unsafe with a high recurrent rate during the waiting time. However, this technique may have some advantages for avoiding unnecessary extended resection in patients with an appendiceal mass. Nonoperative management of AA may be tolerated only in children. Postoperative complications increase according to the patient's factors, and temporal avoidance of emergent general anesthesia may be beneficial for high-risk patients. The surgeon's skill and cooperation of the hospital are important for successful LA. Delaying appendectomy for less than 24 h from diagnosis is safe. Additionally, a semi-elective manner(i.e., LA within 24 h after onset of symptoms) may be paradoxically acceptable, according to the factors of the patient, physician, and institution. Prompt LA is mandatory for AA. Fortunately, the Japanese government uses a universal health insurance system, which covers LA.

Translation and Validation of the Japanese Version of the Family Sense of Coherence Scale-Short Form in Nurses

Purpose: The purpose of this study was to translate the Family Sense of Coherence Scale-short form (FSOC-S) into a Japanese version (J-FSOC-S) for Japanese nurses and to evaluate its reliability and validity. Methods: First, we obtained permission to translate the FSOC-S from the original author and translated it into Japanese using the translation/back translation technique. Second, the J-FSOC-S was reviewed by a panel of experts in nursing research and practice, then, a validation survey was conducted. The internal consistency of the J-FSOC-S was assessed using Cronbach’s α coefficient. Test-retest reliability was examined through intra-class correlation coefficients (ICC). Construct validity was investigated by measuring the correlation coefficients between the J-FSOC-S and Sense of Coherence (SOC) Scale (concurrent validity) and the Family Adaptability Partnership, Growth, Affection, and Resolve Index (Family APGAR) (convergent validity). Results: A total of 374 nurses (valid response rate = 58.6%) were recruited from seven hospitals. Thirty-four participants completed the questionnaire twice at an interval of 2 - 4 weeks to test the reliability. The mean age of the participants was 35.0 years (SD = 7.6, range = 25 - 59). The mean years of experience as a registered nurse were 11.0 years (SD = 7.1, range = 5 - 36). Of the total number of participants, 53.7% had children. The J-FSOC-S showed good internal consistency (Cronbach’s α = 0.85) and test-retest reliability (ICC = 0.77). The J-FSOC-S correlated positively with the SOC Scale (r = 0.41, p Conclusions: The J-FSOC-S is a reliable and valid instrument for measuring family sense of coherence in Japan.

Reinduction Chemotherapy with Gemtuzumab Ozogamicin and Intermediate/High-Dose Cytarabine: A Single-Center Experience

We performed a retrospective analysis of 9 patients with acute myeloid leukemia (AML) treated with gemtuzumab ozogamicin (GO) plus cytarabine as a salvage regimen (GO reinduction) for patients who did not achieve complete remission (CR) after the first cycle of induction chemotherapy or at first relapse. Cases of AML secondary to myelodysplastic syndrome or myeloproliferative disorder were included. CR was achieved in 6 of 9 patients, and 2 of 6 responders became long-term survivors. No non-responders survived longer than 6 months. Toxicity was mild, and the median duration of myelosuppression was less than 30 days. Stomatitis, nausea and sepsis occurred as non-hematological adverse events. Although our sample size was too small to permit definitive conclusions, GO reinduction should be considered for patients who relapse or do not achieve CR after the first cycle of induction chemotherapy. Some AML subtypes may respond more robustly than others, and further investigation is warranted.

The Effect of Active Knee Extension in Sitting on Lumbopelvic Curvature in Individuals with Clinically Tight Hamstring Muscles: A Cross-Sectional Reliability Study

Relative flexibility between the hamstring and lumbar extensor muscles, which can be evaluated using lumbopelvic curvature during active knee extension in sitting, can sometimes be assessed in physical therapy. However, reliability for its quantitative measure has not been established yet and its establishment was the aim of the current study. Twenty-seven individuals with clinically tight hamstring muscles were recruited. On two separate sessions, the lumbopelvic curvature was evaluated in sitting when the right knee was moved from 90° flexion to 10° flexion on 15 occasions using a flexible ruler by two examiners on Day 1 and one on Day 2. Lines drawn tangential to the lumbopelvic curvature were traced at T12 and S2 vertebral levels and the angle between the two vertical lines was calculated. Using Day 1 data, the minimum number of repetitions and inter-examiner reliability were assessed. Inter-session reliability was also examined. As a result, there was no statistical difference (P?> 0.05) in the mean absolute difference between the mean value of N-1 and N repetitions (6 ≤ N ≤ 15) in the lumbopelvic curvature angle, indicating that five was considered the minimum number of repetitions. Intraclass correlation coefficient (ICC)(1, 5)?for the inter-session reliability and ICC(2, 5)?for the inter-examiner reliability was 0.97 and 0.93, respectively, indicating excellent reliability. The measure for the lumbopelvic curvature during active knee extension in sitting, which was established in the current study, will be a foundation for further research regarding the relative flexibility of the lumbar and adjunct regions.
