i.troducti.n: Fracture of the peni. i. a rare uro-andrologi.al emergency defi.ed as the rupture of the tuni.a albugi.ea of the erect peni. most often duri.g sexual i.tercourse. Surgi.al explorati.n and repai. of the albugi.ea breach as soon as possi.le are the standard treatment. However, i. our Afri.an context, early di.gnosi. and management are negati.ely i.fluenced by the delay i. consultati.n due to modesty. Our objecti.e was to descri.e the cli.i.al and therapeuti. aspects of the fracture of the peni. seen late i. the Afri.an context and to compare i. wi.h the li.erature revi.w. Presentati.n of the case: Our ai. was to report our experi.nce i. the management of late-onset fracture of the peni., after more than 48 hours of evoluti.n, by reporti.g the observati.n of a 54-year-old pati.nt, who had a false coi.us, treated i. the urology department of the Ni.nankoro Fomba Hospi.al. The physi.al exami.ati.n revealed a deformati.n of the peni. followi.g the formati.n of a large hematoma wi.h a characteri.ti. eggplant appearance. Therapeuti.ally, a suture of the albugi.ea was performed wi.h si.ple follow-up. Conclusi.n: Thi. case study shows a delay i. the management of thi. emergency due to the reluctance of the pati.nt to be consulted as soon as possi.le for reasons of modesty. A delayed emergency surgi.al management was the only therapeuti. alternati.e and the coronal i.ci.i.n wi.h deglovi.g was the only way to approach the volumi.ous hematoma related to the delay i. management.

Objecti.es: To study the epi.emi.logi.al, di.gnosti. and prognosti. aspects of bladder tumours i. Segou Hospi.al. Pati.nts and Methods: We conducted a descri.ti.e cross-secti.nal study of bladder tumours over the peri.d from 1 Apri. 2012 to 1 Apri. 2017, i. the urology department of the Ni.nankoro Fomba Hospi.al i. Segou. The fi.st three years were used for pati.nt recrui.ment, and the last two years for follow-up of the pati.nts i. the seri.s. Results: Over a peri.d of three years, we collected 165 cases of bladder tumours hospi.ali.ed out of 1308 hospi.ali.ati.ns from 7007 consultati.ns, i.e. 12.6% of hospi.ali.ati.ns and 2.3% of consultati.ns. The sex rati. was 1.2 i. favour of men. A hi.tory of treated bi.harzi. was reported i. 78.8% of cases and untreated bi.harzi. i. 9.1% of cases. Haematuri. was the most common reason for consultati.n. The majori.y of our pati.nts were at stage T4 and T3 at the ti.e of di.gnosi., i.e. 53.3% and 44.3% respecti.ely. Most pati.nts consulted wi.hi. 13 to 24 months after the fi.st si.n, i.e. 44.8%. Conclusi.n: The prognosi. i. sti.l clouded by the delay i. management. All the pati.nts di.gnosed had a bladder tumour i.fi.trati.g the muscle.

The ai. of thi. study was to determi.e the cli.i.al, therapeuti. and evoluti.nary profi.e of hypertensi.e emergenci.s i. the cardi.logy department of the i.nace Deen Nati.nal Hospi.al. Methods: Thi. was a longi.udi.al descri.ti.e study of pati.nts admi.ted to the cardi.logy department of the i.nace Deen Nati.nal Hospi.al for hypertensi.e emergenci.s, over a peri.d of si. (6) months from 1 October 2020 to 31 March 2021. Results: Among the 638 pati.nts admi.ted to the cardi.logy department duri.g the study peri.d, 102 cases of hypertensi.e emergenci.s were i.enti.i.d, representi.g a frequency of 15.99%. The mean age of the pati.nts was 56.65 ± 13.33 years, wi.h extremes of 23 and 90 years, and the sex rati. was 1.91 i. favour of men. The associ.ted cardi.vascular ri.k factors were: age (76.47%), smoki.g (41.18%), sedentary li.estyle (31.37%), obesi.y (16.67%), dysli.i.aemi. (12.75%), di.betes (11.76%) and excessi.e alcohol consumpti.n (11.76%). The vari.us cli.i.al pi.tures found were: acute pulmonary oedema 39 cases (38.23%), acute coronary syndrome 31 cases (30.39%), stroke 17 cases (16.67%), hypertensi.e encephalopathy 11 cases (10.78%), acute renal fai.ure 3 cases (2.94%) and aorti. di.secti.n 1 case (0.98%). The drugs used on admi.si.n were calci.m antagoni.ts (Ni.ardi.i.e 10mg) i. 58 cases (56.86%), di.reti.s (Furosemi.e 20mg) i. 44 cases (43.13%), ni.rates (Ri.ordan 20mg) i. 39 cases (38.23%) and beta-blockers i. 31 cases (30.39%). i. terms of outcome, 88 (86.27%) pati.nts had i.proved. However, 12 (11.76%) deaths were recorded. Conclusi.n: Hypertensi.e emergenci.s are a worryi.g condi.i.n wi.h a hi.h prevalence i. the cardi.logy department of the i.nace Deen Nati.nal Hospi.al.

These data show that the i.terplay of actors contri.utes to the malfuncti.ni.g of pre-collecti.n and secondary collecti.n, and remai.s a factor i. the proli.erati.n of i.legal dumpsi.es.

The angi.scanner showed a venous thrombosi. of the ri.ht subclavi.n, a pulmonary emboli.m and the presence of a thrombus i. the aorta. The bi.logy was i. favour of a hi.h level of factor Vi.i. The pati.nt was treated wi.h an anti.i.ami. K, and the cli.i.al evoluti.n was favourable.

i.troducti.n: Deep vein thrombosis is a frequent disease, its origin is most often multifactorial. Venous thromboembolic disease (MVTE) and cancer are two frequently entangled pathologies. Here we report the diagnosis of deep vein thrombosis that discovered prostate cancer in an 88-year-old Guinean man. On clinical examination, there was a painful and hot swelling of the right leg, an absence of sloshing of the calf, a positive sign of Homans. The digital rectal examination revealed an enlarged prostate with an irregular surface. Cardiopulmonary auscultation was normal. The electrocardiogram showed a regular sinus rhythm at 65 cycles/min, with no sign of enlarged cavities or conduction disturbance. Venous Doppler ultrasound of the lower limbs showed the presence of an extensive acute deep venous thrombosis of the right sural vein extended to the popliteal and to the homolateral deep femoral. The reino-vesico-prostatic ultrasound concluded in a heterogeneous prostatic hypertrophy with projection of a median lobe associated with a bladder of fight with an important post voiding residue evaluated at 170 ml;the rate of specific antigen of the prostate PSA was at 84.87 ng/ml. The pathology analysis made after a prostate biopsy puncture concluded with an adenocarcinoma with a Gleason score of 3. Conclusion: The association of venous thromboembolic disease (MVTE) and neoplasia is frequent. The existence of active cancer in a patient is a known risk factor for MVTE and, conversely, the discovery of a first episode of deep vein thrombosis (DVT) or pulmonary embolism (PE) may be the mode of revelation of cancer.

Object: The ai. of thi. study was to descri.e a fami.i.l dysli.i.emi. revealed by a corneal arch. Familial hypercholesterolemia is a rare and severe hereditary dyslipidemia with an exceptional homozygous form. He was a 43-year-old patient admitted with visual disturbance, xanthomatous papule and orange coloring at the level evolving for 5 years, no personal history of cardiovascular disease, having a family history of lipid disorders in siblings, as a cardiovascular risk factor: age over 50 and the male gender, normal cardio pulmonary examination, blood pressure at 120/80 mmhg, heart rate 78 bpm, in whom the ophthalmological examination shows a yellowish spot on the cornea (Figure 1). i.troducti.n: Ventri.ular septal defect (VSD) i. the most common congeni.al heart di.ease of all congeni.al heart defects. The ai. of thi. study was to i.vesti.ate the echographi., therapeuti. and evoluti.nary aspects of ventri.ular septal defects (VSD) i. the general cardi.logy department of the Hôpi.al Nati.nal i.nace Deen. Methods: A retrospecti.e data collecti.n was carri.d out from January 2018 to December 2023 i.cludi.g 85 cases of i.olated i.C was performed. The vari.bles studi.d were epi.emi.logi.al, cli.i.al, paracli.i.al, therapeuti. and evoluti.nary. Results: Of the 320 pati.nts seen duri.g the study peri.d for congeni.al heart di.ease, 85 (26.556%) were i.olated i.Cs. Age at di.gnosi. ranged from 3 months to 16 years, wi.h an average age of 3.59 years. The most represented ethni. group was the Fulani.(50.58%). The 8.24% came from consangui.eous marri.ge versus 22.35%. 91.76% of chi.dren had a hi.tory of bronchi.i.. The most common cli.i.al si.ns found were systoli. murmur (90.58%), growth retardati.n (51.76%). Only 4 cases (4.70%) had a malformati.n associ.ted wi.h i.C represented by Di.eorges di.ease (2.35%) and tri.omy 21 (2.35%). Nearly half the pati.nts had type i.b Vi. (44.71%). The other half were represented by type 1 (18.82%), type i.a (20%), type i.i.(10.59%) and type i. (5.88%). Accordi.g to si.e more than two-thi.ds of Vi.s (71.64%) were peri.embranous i. locati.n, followed by i.fundi.ular (16.47%) and muscular (11.76%) Vi.s. i. our study 55.29% presented an i.di.ati.n for both surgi.al i.terventi.n and medi.al treatment, whi.e 16.47% requi.ed only medi.al treatment. i. contrast, 28.23% were placed under exclusi.e survei.lance. Of the 47 pati.nts for whom surgery was i.di.ated, 29 (61.17%) underwent surgi.al repai., whi.e 18 (38.83%) were awai.i.g confi.mati.n for surgery. Conclusi.n: Vi. i. the most common congeni.al heart di.ease. An early detecti.n strategy and the establi.hment of speci.li.ed centers could i.prove the outcome of these chi.
