to the Editor:Many patients with tumors of the pancreatic head or of the ampulla of Vater require endoscopic manipulation of the duodenal papilla in order to achieve tumor biopsy or for common bile duct *** interventional endoscopy approaches may lead to acute *** iatrogenic acute pancreatitis will influence the surgical strategy in patients scheduled for pancreaticoduodenectomy(PD).Due to the small number of cases,the surgical strategy in patients who require PD in the context of acute pancreatitis has not been standardized.

Background:Although guidelines recommend systemic therapy even in patients with limited extrahepatic metastases from hepatocellular carcinoma(HCC),a few recent studies suggested a potential benefit for resection of extrahepatic ***,the benefit of adrenal resection(AR)for adrenal-only metastases(AOM)from HCC was not proved *** is the first study to compare long-term outcomes of AR to those of sorafenib in patients with AOM from ***:the patients with adrenal metastases(AM)from HCC were identified from the electronic records of the institution between January 2002 and December *** who presented AM and other sites of extrahepatic disease were ***,the patients with AOM who received other therapies than AR or sorafenib were ***:A total of 34 patients with AM from HCC were *** of these,22 patients had AOM,6 receiving other treatment than AR or ***,8 patients with AOM underwent AR(AR group),while 8 patients were treated with sorafenib(SOR group).the baseline characteristics of the two groups were not significantly different in terms of age,sex,number and size of the primary tumor,timing of AM diagnosis,Child-Pugh and ECOG *** a median follow-up of 15.5 months,in the AR group,the 1-,3-,and 5-year overall survival rates(85.7%,42.9%,and 0%,respectively)were significantly higher than those achieved in the SOR group(62.5%,0%and 0%at 1-,3-and 5-year,respectively)(P=0.009).the median progression-free survival after AR(14 months)was significantly longer than that after sorafenib therapy(6 months,P=0.002).Conclusions:In patients with AOM from HCC,AR was associated with significantly higher overall and progression-free survival rates than systemic therapy with *** results could represent a starting-point for future phase II/III clinical trials.

Background:We sought to assess the overall benefit of laparoscopic versus open hepatectomy for treatment of colorectal liver metastases(CRLMs)using the win ratio,a novel methodological ***:CRLM patients undergoing curative-intent resection in 2001-2018 were identified from an international multi-institutional *** were paired and matched based on age,number and size of lesions,lymph node status and receipt of preoperative *** win ratio was calculated based on margin status,severity of postoperative complications,90-day mortality,time to recurrence,and time to ***:Among 962 patients,the majority underwent open hepatectomy(n=832,86.5%),while a minority underwent laparoscopic hepatectomy(n=130,13.5%).Among matched patient-to-patient pairs,the odds of the patient undergoing laparoscopic resection“winning”were 1.77[WR:1.77,95%confidence interval(CI):1.42-2.34].the win ratio favored laparoscopic hepatectomy independent of low(WR:2.94,95%CI:1.20-6.39),medium(WR:1.56,95%CI:1.16-2.10)or high(WR:7.25,95%CI:1.13-32.0)tumor burden,as well as unilobar(WR:1.71,95%CI:1.25-2.31)or bilobar(WR:4.57,95%CI:2.36-8.64)*** odds of“winning”were particularly pronounced relative to short-term outcomes(i.e.,90-day mortality and severity of postoperative complications)(WR:4.06,95%CI:2.33-7.78).Conclusions:Patients undergoing laparoscopic hepatectomy had 77%increased odds of“winning”.Laparoscopic liver resection should be strongly considered as a preferred approach to resection in CRLM patients.

AIM to compare the effectiveness of laparoscopic complete mesocolic excision (CME) with central vascular ligation (L-CME) with its open (O-CME) counterpart. MEtHODS We conducted an electronic search of the PubMed/MEDLINE, Excerpta Medica Database, Web of Science Core Collection, Cochrane Center Register of Controlled trails, Cochrane Database of Systematic Reviews, SciELO, and Korean Journal databases from their inception until May 2017. We considered randomized controlled trials (RCts) and controlled clinical trials (CCts) that included patients with colonic cancer comparing L-CME and O-CME. Primary outcomes included the quality of the resected specimen (lymph nodes retrieved, complete mesocolic plane excision, tumor to arterial high tie, resected mesocolon surface). Secondary outcomes included the three-year and five-year overall and disease-free survival rates, recurrence of the disease, surgical data, and postoperative morbidity and mortality. two authors of the review screened the methodological quality of the eligible trials and independently extracted data from individual studies. RESULtS A total of one RCt and eleven CCts (four from Europe and seven from Asia) met the inclusion criteria for the current meta-analysis. these studies involved 1619 patients in L-CME and 1477 patients in O-CME. the L-CME was associated with the same quality of the resected specimen, with no differences regarding the retrieved lymphnodes (MD = -1.06, 95%CI: -3.65 to 1.53, P = 0.42), and tumor to high tie distance (MD = 14.26 cm, 95%CI: -4.30 to 32.82, P = 0.13); the surface of the resected mesocolon was higher in the L-CME group (MD = 11.75 cm2, 95%CI: 9.50 to 13.99, P the L-CME was associated with a lower rate of blood transfusions (OR = 0.45, 95%CI: 0.27 to 0.75, P = 0.002), faster recovery of gastrointestinal function, and less postoperative overall complication rate. the L-CME approach was associated with a statistical significant better three-year overall

AIM： to generate and characcerize the synthetic transoiptional control units for transcriptional targeting of the liver, thereby compensating for the lack of specificity of currently available gene therapeutic vector systems. MEtHODS： Synthetic transcriptional control unit constructs were generated and analyzed for transcriptional activities in different cell types by FACS quantification, semi-quantitative Rt-PCR, and Western blotting. RESULtS： A new bifunctionally-enhanced green fluorescent protein （EGFP）/neor fusion gene cassette was generated, and could flexibly be used both for transcript quantification and for selection of stable cell clones. then, numerous synthetic transcriptional control units consisting of a minimal promoter linked to “naturally” derived composite enhancer elements from liver-specific expressed genes or binding sites of liver-specific transcription factors were inserted upstream of this reporter cassette. Following liposome-mediated transfection, EGFP reporter protein quantification by FACS analysis identified constructs encoding multimerized composite elements of the apolipoprotein B100 （APOB） promoter or the ornithin transcarbamoylase （OtC） enhancer to exhibit maximum transcriptional activities in liver originating cell lines, but only background levels in non-liver originating cell lines. In contrast, constructs encoding only singular binding sites of liver-specific transcription factors, namely hepatocyte nuclear factor （HNF）, HNF3, HNF4, HNF5, or CAAt/enhancer binding protein （C/EBP） only achieved background levels of EGFP expression. Finally, both semi-quantitative Rt-PCR and Western blotting analysis of Hep3B cells demonstrated maximum transcriptional activities for a multimeric 4xAPOB cassette construct, which fully complied with the dataobtained by initial FACS analysis CONCLUSION： Synthetic transcriptional control unit constructs not only exhibit a superb degree of structural compactness, but also provide new means for liver

Some geographic regions induce beneficial effects on health status. the world health organization is interested in analyzing this type of climate area in order to establish a network health status. Understanding the parameters of this type of climate creates the prerequisites for further research to define their influence on humans, as an external determining factor of the exposome. Supported by historic and medical epidemioiogy data, the Cacica area, Roman has beneficial effects on human health. this climatic natural are, a will be characterized by monitoring their parameters. thus, they will be rigorously defined and considered of major influence in the exposome. Air quality measurements were made and the local meteorological parameters were monitored. these were correlated with LIDAR data. the atmospheric layers have been evidenced and a possible scenario for the elimination of the pollutant ~erosols has been created, in case, the area suffers contamination during a certain season. the analyzed area shows as climatic characteristics both the absence of pollutant factors and the natural structure of the geo-system. the characterization of this area associated with the special terrain （the through）, involves the introduction of new local development directions： education, ecology and economy.

the paper presents a study on increasing energy independence of a 200 m2 horticultural hothouse, by means of heating it with thermal energy from a tLUD （top-lit-up-draft） gasification procedure of local residual agricultural biomass, chopped at 10-50 mm and dried at 10%-15% RH （relative humidity）. It produces an average of 14% higt quality biochar. Hot-air heating system and forced circulation are equipped with two GAZMER 40/150G energetic modules, which are rechargeable, simple, safe, efficient and environmentally friendly. they can gasify chopped or pelletised biomass. to study the microclimate evolution, it was used a complex numerical model for a 200 m2 hothouse for growing vegetables. Simulated experiments were carried out for frosty days and, estimated, for the whole warming period. Each year 13.44 t of biomass are consumed, resulting 1.78 t ofbiochar which, when introduced in soil, produces a -6.2 t/year negative balance of CO2.

We report the case of a 54-year-old African-American male with IgG multiple myeloma (MM) with disease resistant to multiple chemotherapy regimens and immunomodulatory treatment with thalidomide. In spite of achieving a partial remission of short duration, his disease accelerated to peripheral plasmacytosis and subsequent development of cutaneous plasmacytomas. the malignant plasma cells derived from the dermal lesions were CD45+, CD38+, CD138+and matched the immunophenotype of the plasmacytes during the leukaemic phase. Occurrence of extramedullary lesions in the setting of MM treated with thalidomide is of concern, although currently there are very few reports describing this association. We discuss the possible relationship between the patient’s unusual disease course and the administered chemo-and immunotherapy. the significance of the changes in adhesion molecules, especially CD 138 and CD 56, relevant to the development of cutaneous plasmacytomas is discussed.
