Using the win ratio to compare laparoscopic versus open liver resection for colorectal cancer liver metastases

Background:We sought to assess the overall benefit of laparoscopic versus open hepatectomy for treatment of colorectal liver metastases(CRLMs)using the win ratio,a novel methodological ***:CRLM patients undergoing curative-intent resection in 2001-2018 were identified from an international multi-institutional *** were paired and matched based on age,number and size of lesions,lymph node status and receipt of preoperative *** win ratio was calculated based on margin status,severity of postoperative complications,90-day mortality,time to recurrence,and time to ***:Among 962 patients,the maj.rity underwent open hepatectomy(n=832,86.5%),while a minority underwent laparoscopic hepatectomy(n=130,13.5%).Among matched patient-to-patient pairs,the odds of the patient undergoing laparoscopic resection“winning”were 1.77[WR:1.77,95%confidence interval(CI):1.42-2.34].The win ratio favored laparoscopic hepatectomy independent of low(WR:2.94,95%CI:1.20-6.39),medium(WR:1.56,95%CI:1.16-2.10)or high(WR:7.25,95%CI:1.13-32.0)tumor burden,as well as unilobar(WR:1.71,95%CI:1.25-2.31)or bilobar(WR:4.57,95%CI:2.36-8.64)*** odds of“winning”were particularly pronounced relative to short-term outcomes(i.e.,90-day mortality and severity of postoperative complications)(WR:4.06,95%CI:2.33-7.78).Conclusions:Patients undergoing laparoscopic hepatectomy had 77%increased odds of“winning”.Laparoscopic liver resection should be strongly considered as a preferred approach to resection in CRLM patients.

The feral or volunteer cotton(VC)plants when reach the pinhead squaring phase(5–6 leaf stage)can act as hosts for the boll weevil(Anthonomus grandis L.)*** Texas Boll Weevil Eradication Program(TBWEP)employs people to locate and eliminate VC plants growing by the side of roads or fields with rotation crops but the ones growing in the middle of fields remain *** this paper,we demonstrate the application of computer vision(CV)algorithm based on You Only Look Once version 5(YOLOv5)for detecting VC plants growing in the middle of corn fields at three different growth stages(V3,V6 and VT)using unmanned aircraft systems(UAS)remote sensing *** the four variants of YOLOv5(s,m,l,and x)were used and their performances were compared based on classification accuracy,mean average precision(mAP)and *** was found that YOLOv5s could detect VC plants with maximum classification accuracy of 98%and mAP of 96.3%at V6 stage of corn while YOLOv5s and YOLOv5m resulted in the lowest classification accuracy of 85%and YOLOv5m and YOLOv5l had the least mAP of 86.5%at VT stage on images of size 416×416 *** developed CV algorithm has the potential to effectively detect and locate VC plants growing in the middle of corn fields as well as expedite the management aspects of TBWEP.

In the past half century, the mean summertime temperature in China has increased, with nights warm- ing more than days. Using surface station observations, we show that the frequency of extreme heat-stress events in China, caused by extremely hot and humid days as well as by heatwaves lasting for a few days, has increased over the period from 1951 to 1994 When humidity is high, hot weather can cause heat stress in humans. AIM To compare the effectiveness of laparoscopic complete mesocolic excision(CME) with central vascular ligation(L-CME) with its open(O-CME) counterpart. METHODS We conducted an electronic search of the Pub Med/MEDLINE, Excerpta Medica Database, Web of Science Core Collection, Cochrane Center Register of Controlled Trails, Cochrane Database of Systematic Reviews, Sci ELO, and Korean j.urnal databases from their inception until May 2017. We considered randomized controlled trials(RCTs) and controlled clinical trials(CCTs) that included patients with colonic cancer comparing L-CME and O-CME. Primary outcomes included the quality of the resected specimen(lymph nodes retrieved, complete mesocolic plane excision, tumor to arterial high tie, resected mesocolon surface). Secondary outcomes included the three-year and five-year overall and disease-free survival rates, recurrence of the disease, surgical data, and postoperative morbidity and mortality. Two authors of the review screened the methodological quality of the eligible trials and independently extracted data from *** A total of one RCT and eleven CCTs(four from Europe and seven from Asia) met the inclusion criteria for the current meta-analysis. These studies involved 1619 patients in L-CME and 1477 patients in O-CME. The L-CME was associated with the same quality of the resected specimen, with no differences regarding the retrieved lymphnodes(md =-1.06, 95%CI:-3.65 to 1.53, P = 0.42), and tumor to high tie distance(md = 14.26 cm, 95%CI:-4.30 to 32.82, P = 0.13); the surface of the resected mesocolon was higher in the L-CME group(md = 11.75 cm2, 95%CI: 9.50 to 13.99, P < 0.001). The L-CME was associated with a lower rate of blood transfusions(OR = 0.45, 95%CI: 0.27 to 0.75, P = 0.002), faster recovery of gastrointestinal function, and less postoperative overall complication rate. Methionine(Met)is an essential and first limiting amino acid in the poultry diet that plays a significant role in chicken embryonic development and *** present study examined the effect of in ovo inj.ction of DL-Met and L-Met sources and genotypes on chicken embryonic-intestinal development and *** eggs of the two genotypes,TETRA-SL layer hybrid(TSL)—commercial layer hybrid and Hungarian Partridge colored hen breed(HPC)—a native genotype,were randomly distributed into four treatments for each *** treatment groups include the following:1)control non-inj.cted eggs(NoIn);2)saline-inj.cted(SaIn);3)DL-Met inj.cted(DLM);and 4)L-Met inj.cted(LM).The in ovo inj.ction was carried out on 17.5 d of embryonic development;after hatching,eight chicks per group were sacrificed,and the j.j.num was extracted for *** results showed that both DLM and LM groups had enhanced intestinal development as evidenced by increased villus width,villus height,and villus area(Pmd2),occludin(OCLN),superoxide dismutase 1(SOD1),and GST3 genes than the TSL genotype(Pj.ction of Met enhanced intestinal development,and function,with genotypes responding differently under normal *** also influenced the expression of intestinal antioxidants,tight j.nction,and growth-related genes.
