Cholera Recurrence within Benin Littoral County: Analysis of Epidemics from 2008, 2016 to 2023

Introduction: Cholera is an acute diarrheal infection caused by the bacillus Vibrio cholerae, which can be fatal in a few hours if left untreated. It rages in endemic-epidemic mode in several countries, including Benin. Despite being the economic capital, the largest city in Benin and the most urbanized municipality in the country, the Littoral County, confounded with the city of Cotonou, is subject to recurrent epidemics. This paper aims to analyze successive cholera epidemics with a view of highlighting the factors contributing to this recurrence. Methods: A secondary analysis was done of the cholera-related databases of the County health office from 2016 to 2023, and its Situation Report No. 79 of December 7th, 2016, to describe the 2016 epidemic. Database data were analyzed in Excel. Results: Similarly to 2008, the 2016 cholera epidemic in Cotonou, which recorded 519 cases and 07 deaths, i.e. a case-fatality rate of 1.35%, spread to the town and six neighboring municipalities. The municipality of So-Ava, home to the index case, had the highest overall attack rate. The two epidemics share the same period of severity. Both epidemic and endemic cases are concentrated in the first seven boroughs of Cotonou, located on the lagoon shore, with low levels of hygiene and sanitation and a poor supply of drinking water. Conclusion: Low levels of hygiene, sanitation and drinking water supply all play a part in the recurrence of cholera epidemics. Sanitation work. Undertaken by the Beninese government is a ray of hope for improving this situation. Meanwhile, public awareness of hygiene measures must continue.

Evaluation of the Functionality of the Networking of Sexual and Reproductive Health Rights Services in the Event of Gender-Based Violence in the Commune of Kpomasse in 2022

Introduction: In developing countries, gender-based violence (GBV) is a real public health problem. In Benin, GBV affects the majority of women and girls (69%). Benin has implemented strategies and set up integrated centers for the management of violence in order to reduce cases of violence and ensure the holistic management of victims. The objective of our study was to assess the functionality of the network of sexual and reproductive health rights (SRHR) in case of GBV in the commune of Kpomasse in 2022. Method: This descriptive and evaluative study was conducted from March 21 to April 11, 2022. The sampling method used was non-probabilistic. Reasoned choice and convenience were the techniques used for the different targets of the study. The functionality of the SRHR service network was assessed first by calculating scores at the structure, process and outcome levels, and then by analysis using the human rights-based approach. Results: Out of the 34 GBV victims identified, only one had received a full response and 54% of the victims had no knowledge of SRHR. The lack of knowledge about health care facilities was 41% for victims and 80% for non-victims in the community who participated in the study. In the case of gender-based violence, the community preferred to settle out of court rather than report it. The functionality of the networking of sexual and reproductive health rights services in the event of the occurrence of gender-based violence in the commune of Kpomassè is insufficient. Lack of knowledge of the roles of rights holders (DD) and duty bearers (DO) explains the insufficient functionality of networking. Conclusion: Training of SRHR service agents and community sensitization are essential to improve the functionality of SRHR service networking in the commune of Kpomasse.

Factors Associated with Using GPS in Road Accidents at Cotonou in 2019

Introduction: Precise spatial location of accidents is relevant for accidentology researches or road safety investigations. Objective: The aim of this study is to investigate a number of factors associated with the geolocation of road accidents in the city of Cotonou in 2019. Methods: It was a cross-sectional, retrospective study with an analytical focus. Results: This study highlighted a prevalence of coordinates of the GPS reported at 41.63% in 2019 relating to the files of accidents on public roads during the period from April 18 to June 12, 2020. The work was carried out from 384 accident files examined in the 6 districts of Cotonou selected randomly. A logistic regression made it possible to sort out associated factors with using GPS in road accidents. The final model retained through the “ascending step by step” modeling was adopted. The average age of the responsible agents in charge of the observation was around 32 years (32.13 ± 3.17). Geolocation identified associated factors were: the level of instruction with odd Ratio 2 and its 95% confidence interval f [1.02 - 2.40], the means of conveyance odd ratio 2.56 and its 95% confidence interval of [1.21 - 5.41], the severity level of the accident with its odd ratio 4.59 and its 95% confidence interval of [2.82 - 8.32], and the type of day on which the accident occurred with odd ratio 0.56 and its 95% confidence interval f [0.437 - 2.553]. As for the quality of the reported GPS coordinates, 85% of them were good quality. Conclusion: The reduction of road accidents, given its serious nature and extent, requires strategies to promote geolocation of accidents to ensure better identification of risk areas and decision-making adapted to the accident phenomenon.

Theoretical Assessment of Environmental Factors and Untreated Infectious Individuals in the Transmission Dynamics of Buruli ulcer

Buruli ulcer is the third most common mycobacterial disease worldwide, posing a significant public health burden, especially in impoverished regions of West and Central Africa, such as Benin. The management of Buruli ulcer (BU) in Africa is often hindered by limited resources, delays in treatment, and inadequate medical facilities. Additionally, a portion of the population does not seek hospital care, which facilitates the continued presence of the pathogen in the environment. This paper aims to investigate the role of environmental factors in the transmission of Buruli ulcer. We develop a mathematical model to describe the dynamics of Buruli ulcer transmission, incorporating the presence of the bacterium in the environment. Theoretical results are presented to demonstrate that the model is well-posed. We compute the equilibria, including the disease-free equilibrium and the endemic equilibrium, and study their stability. To achieve this, we derive a threshold parameter called the basic reproduction number ℛ0, which determines whether the disease will persist in a human population. If ℛ0is less than one, the disease will eventually die out;if ℛ0is greater than one, the disease will persist. Sensitivity analysis is performed to understand the impact of various parameters on the dynamics of Buruli ulcer transmission and to identify the parameters that influence the basic reproduction number ℛ0. Finally, numerical simulations are conducted to validate the theoretical results obtained from the mathematical analysis.

Background: The Togolese population, like those around the world, frequently consumes foods high in salt/sodium and low in potassium, thus exposing them to cardiovascular disease (CVD). Nutritional intervention can help reverse this consumption pattern and reduce related CVD morbidity and mortality. The objective of this study was to measure the effects of a nutrition education intervention on the consumption frequencies of foods rich in sodium and potassium. Methods: The study was a quasi-experimental before- and-after study, conducted from 08 January to 16 April 2023. It involved 200 adults aged 25 - 64 years, randomly selected from two areas: an intervention area and a non-intervention area. Data were collected in two phases at 3-month intervals in both groups. The intervention consisted of nutrition education (awareness raising and cooking demonstrations) on reducing salt/sodium intake and increasing potassium-rich food intake. The kobocollect electronic questionnaire was administered to the respondents to collect data on the frequency of consumption of foods rich in sodium and potassium. Results: The median age of the respondents was 33 years old (30;38) and 56% of the participants were women, 44% and 69% respectively in the control and intervention groups. Most participants lived in rural areas (51%), 52.4% and 49.5% in the control and intervention groups respectively. Overall, 4% (p individuals reduced their frequency of adding salt at mealtime from more than 3 times a week to less than 3 times, i.e., 5.6% in the intervention group and 1.7% in the control group. The proportion of individuals who consumed meals with green leafy vegetable sauces was reduced from more than 3 times a week to less than 3 times, i.e., 7.5% (p < 0.022), or 1% in the intervention group and 4% in the control group. Background: Food plays an important role in human health. A healthy diet contributes to the improvement of the health of populations. A diet high in sodium/salt and low in potassium has been shown to contribute to cardiovascular disease. The objective of this study was to assess the knowledge, attitudes and practices (KAP) of the inhabitants of Dapaong and its surroundings on dietary intakes of sodium/salt and potassium. Methods: This was a cross-sectional study conducted from 1 November to 15 December 2022 involving 400 adults aged 25 to 44 years. Sampling was done using the probability method and the multistage random sampling technique with proportional allocation. These two methods were applied to select villages, households, and primary targets in each locality according to whether the area of residence was urban or rural. The electronic kobocollect questionnaire was administered to people in households and the interview guide was to resource persons, administrators and community leaders to collect the data. Results: Overall, 80.25% of the respondents had low knowledge of sodium/salt intakes;88.50% had poor attitudes towards the use of foods that provide more sodium/salt and 92.75% had behaviours that were not conducive to reducing excessive salt consumption. With regard to potassium, the same trend was observed in the order of 91%, 72% and 73.75%. Conclusion: The KAP levels of the people studied were not good. The populations are prone to cardiovascular disease. A multisectoral nutritional intervention would be essential to improve the knowledge of the populations on sodium/salt and potassium intakes and therefore favourable to the fight against CVD/NCD.

Evaluation of the Effectiveness of the 2019 School-Based Awareness Campaign on Road Driving in the Municipalities of Cotonou and Abomey-Calavi in Benin

Being the leading death cause among young people in the world, road accidents are avoidable through the adoption of simple attitudes and behaviors. Handicap International Non-Governmental Organization (NGO) has launched an awareness campaign in secondary schools in the municipalities of Cotonou and Abomey-Calavito bring young Beninese users to adopt good attitudes and safe behavior while driving on the road. This study aims at assessing the effectiveness of this awareness campaign. It was an evaluative cross-sectional here-elsewhere type study, held in fourteen high schools in the cities of Cotonou and Abomey-Calavi, of which seven benefited from the campaign and seven did not. In each school, students were selected on the basis of two-stage random sampling. 656 students, 309 of whom were sensitized and 347 of whom were not, were included in this study. The campaign was quite effective. Students, teachers, and school officials’ perceptions of the campaign were positive. In addition, the sensitized students had a better level of knowledge (p = 0.001) and reported safer behaviors (p = 0.003) while driving on the road than those who were not aware. Although the effects are positive, they are still very small. Efforts must be continued and much remains to be done.

Alcohol, Tobacco and Tramadol Daily Consumption and Road Traffic Crashes among Motorcycle Taxi Drivers in Cotonou (Benin)

Background: In Benin, motorcycle taxi drivers make up a significant portion of road transport. The use of psychoactive substances, a behavior recognized as being at risk of road crashes, is frequent in this group. This study aimed to analyze the association between the consumption of psychoactive substances and the occurrence of road crashes among motorcycle taxi drivers. Methods: This was a cross-sectional study involving 430 motorcycle taxi drivers selected from motorcycle taxi stands in Cotonou (Benin) using a two-stage cluster sampling technique. An interview with a structured questionnaire was used to collect data, which took place in March and April 2019. Alcohol, tramadol, and tobacco were the psychoactive substances considered in this study and their daily use was investigated. A binary logistic regression was used for the analysis. Results: Among the motorcycle taxi drivers surveyed, 27.79% (95% CI = 23.71 - 32.25) were involved in a road crash. The prevalence of psychoactive substances daily use was: 30.41% (95% CI = 26.17 - 35.00) for alcohol, 28.50% (95% CI = 24.37 - 33.02) for tramadol and 4.03% (95% CI = 2.51 - 6.42) for tobacco. Alcohol daily consumption (AOR = 2.09, 95% CI = 1.27 - 3.45, p = 0.004) was associated with a significant risk of road crashes. Conclusion: Interventions aiming to reduce psychoactive substances use should, therefore, be included in road safety policies, especially for this group.

Scale and Associated Factors of Using Pyrethroid-Impregnated Mosquito Nets as Fishing Tools on Nokoué Lake within Sô-Ava Municipality in Benin, 2020

Background: In lacustrine communities, whether in Benin or elsewhere, populations use impregnated mosquito nets (IMNs) as fishing nets. This depletes the lake of its fishery resources, which in turn are contaminated by the pyrethroids impregnated in the nets. This study aims to determine the scale and factors associated with the use of pyrethroid-impregnated mosquito nets as fishing tools in the municipality of Sô-Ava in Benin. Methods: This is a cross-sectional and analytical study with two components. First, a quantitative component was made up of 280 volunteers who were interviewed in the seven districts of the city. Data collection was done in two phases: the first from September to October 2020, and the second in August 2022. Data were analyzed with Stata and logistic regression was used. Another qualitative component was made up of forty participants chosen by reasoned choice, of which 32 were split into four focus groups of eight members each, and the rest participated in semi-structured interviews. Triangulation of the different sources was used to analyze the data. Results: Around 67% of the population reported using impregnated mosquito nets as fishing tools and 33% exclusively for malaria. Seasonal fishermen (ORa = 2.03, CI = 1.35 - 4.97, P = 0.004) and years of professional experience (ORa = 1.53, CI = 1.00 - 2.05, P = 0.021) increase the risk of using these nets as fishing tools. The use of insecticide-treated mosquito nets against mosquitoes causes skin scratching and impairs breathing because of the impregnation products, as reported by respondents. Conclusion: A high prevalence of inappropriate use of impregnated mosquito nets in fishing practices is highlighted in this study. Interviews in the field revealed that nets are diverted for fishing purposes to increase the volume of catches. Consequently, it will be appropriate to assess the pyrethroid content in water and fishery products.

Microbiological Quality Assessment of Drinking Water in Lalo Commune, Benin (West Africa)

Although drinking water is readily available in Benin, its quality presents a public health concern. The district of Ahomadégbé in the commune of Lalo is characterized by several artesian wells. Unfortunately, anthropogenic factors negatively affect the drinking water quality in this area. The aim of this research was to study the microbiological quality of drinking water in the Ahomadégbé district, and to review household water treatment methods employed by the local population. Thirty-five water samples were taken—at water collection points, at selected points along the water transportation system and from water storage facilities, and microbiological parameters were measured. Qualitative data were collected via in-depth interviews with key informants regarding local household water treatment methods. Results reveal that there is a significant degree of microbiological pollution of drinking water in this district, particularly during the water transportation and storage stages where microbiological pollution exceeds levels approved by the World Health Organization. Local residents are familiar with several household water treatment methods. However, these methods are inconsistently applied, which limits their effectiveness. In addition to improving the quality of the drinking water resource itself, it is important to set up interventions relating to water treatment methods in local households.
