Isolated vaginal metastasis from stage Ⅰ colon cancer: A case report

BACKGROUND Distant metastasis occasionally occurs in patients who have been diagnosed with colorectal cancer(CRC), but it occurs in a few patients with stage I CRC. The vagina as a metastasis site has also been reported, albeit rarely. Most reported cases of vaginal metastasis(VM) report their origin from advanced CRC. We encountered a patient who was diagnosed with isolated VM originating from stage I colon cancer(T2N0) and herein present the case of this *** SUMMARY A 63-year-old woman visited the outpatient clinic because of a positive result from a stool occult blood test. She underwent laparoscopic anterior resection and was pathologically diagnosed with stage I(T2N0) sigmoid colon cancer. Neither lymphovascular invasion nor perineural invasion was observed. Ten months following the surgery, isolated vaginal metastases were detected on gynecologic examination. The examination was performed due to vaginal spotting. A transvaginal wide excision was performed, and no other adjuvant treatment was provided after discussion with a multidisciplinary team and the ***, a new VM was discovered after 33 mo. An additional transvaginal excision was performed. To date, there has been no evidence of further disease progression. From the time of diagnosis of VM, the patient’s overall survival has been 54 *** VM can occur as a result of early-stage colorectal cancer. Surgeons should consider the possibility of VM following complaints of gynecologic symptoms following surgery.

Radiotherapy for 65 patients with advanced unresectable hepatocellular carcinoma

AIM: To evaluate the efficacy of radiotherapy (RT) in patients with advanced unresectable hepatocellular carcinoma (HCC). METHODS: A total of 65 patients were treated with RT in the Korea University Medical Center. The median age of the patients was 60 years, and 86.2% were men. 18.5% and 81.5% of the patients were diagnosed as TNM stage Ⅲ and Ⅳ-A, respectively. Treatment response was assessed 4 mo after initiation of RT. Tumor regression rate 1 mo after initiation of RT (TRR1m) was also assessed. Duration of survival was calculated from the initiation of RT. RESULTS: The objective treatment response was 56.9%. The 12 mo survival rate was 34.7%. Predictive factors for survival were Child-Pugh grade, α-fetoprotein level and treatment response. An objective response was achieved more frequently in patients with TRR1m ≥ 20% than in those with TRR1m < 20% (P < 0.001). CONCLUSION: RT is effective in treating advanced HCC with a tumor response rate of 56.9%.

Mechanical Ventilation Strategy for Subway Cabins Using Numerical Simulations

Seoul metropolitan subway network is known to be the one of the most heavily used transportation means in the world. The subway cabins are naturally ventilated when the cabin doors are opened and through the gaps caused by the incomplete air-tightening of cabin bodies. Although, subway trains are equipped with a mechanical exhaust fan, the apparatus is rarely operated due to the problem of heating/cooling efficiency especially in the summer and winter seasons. In this study, we analyzed the transient CO2 （carbon dioxide） concentration level during the journey of a subway cabin when a heavy load of passengers of up to 200% of designed capacity using a CFD （computational fluid dynamics） analysis. With an increased journey time, the CO2 concentration increased by up to 5,000 ppm depending on passenger load. Through the operation of a mechanical exhaust fan, the high concentration of CO2 decreased down to 1,500 ppm. The effect of the exhaust fan operating on the dilution of indoor air was estimated by comparison with a closed cabin. In addition, the energy consumption for cooling in summer time was assessed for exhaust fan operations.

Impacts of Pig Manure-Based Liquid Fertilizer Agricultural Application on the Water Quality of Agricultural Catchment

This study was conducted to understand the effect of the livestock liquid fertilizer treatment at field-scale up to catchment-scale on the water quality properties. Cultivated paddy rice and upland plots located in Gyeonggi province, Korea were treated with two different liquid fertilizers, SP (Liquid fertilizer with storage process) and SCB (Liquid fertilizer with slurry composting and bio-filtration process). Plots with no fertilizer (control A) and chemical fertilizer (control B) were also prepared for comparison. Water quantity and quality were monitored at the catchment outlet for assessing the effect on water quality of stream water. As a result, the losses of N and P and the values of EC (Electronic Conductivity) in the surface drainage water from paddy rice plots treated with SP and SCB were higher than those from the control plots (A and B). In addition, the losses of N and P by the runoff water from upland plots with SP and SCB treatment were higher than those from control plots (A and B). The nutrient outflow from paddy rice fields and uplands with application of liquid pig manure was higher than those from the control plots (A and B). Particularly, the outflow from uplands may directly affect the water quality in neighboring streams. This caused the major eutrophication problem in stream water. In conclusion, it is necessary to establish the proper management practices to prevent the nutrient losses from agricultural fields and the pollutants against water environments.

Gastric leptomeningeal carcinomatosis: Multi-center retrospective analysis of 54 cases

AIM： To identify the clinical features and outcomes of infrequently reported leptomeningeal carcinomatosis （LMC） of gastric ***： We analyzed 54 cases of cytologically confirmed gastric LMC at four institutions from 1994 to ***： The male-to-female ratio was 32：22, and the patients ranged in age from 28 to 78 years （median,48.5 years）. The majority of patients had advanced disease at initial diagnosis of gastric cancer. The clini-cal or pathologic tumor, node and metastasis stage ofthe primary gastric cancer wasin 38 patients （70%）.The median interval from diagnosis of the primarymalignancy to the diagnosis of LMC was 6.3 mo, rang-ing between 0 and 73.1 mo. Of the initial endoscopic f indings for the 45 available patients, 23 （51%） of the patients were Bormann typeand 15 （33%） patientswere Bormann type. Pathologically, 94% of cases proved to be poorly differentiated adenocarcinomas. Signet ring cell component was also observed in 40% of patients. Headache （85%） and nausea/vomiting （58%） were the most common presenting symptoms of LMC. A gadolinium-enhanced magnetic resonance imaging was conducted in 51 patients. Leptomeningeal enhancement was noted in 45 cases （82%）. Intrathecal （IT） chemotherapy was administered to 36 patients-primarily methotrexate alone （61%）, but also in combi-nation with hydrocortisone/± Ara-C （39%）. The median number of IT treatments was 7 （range, 1-18）. Concomitant radiotherapy was administered to 18 patients, and concomitant chemotherapy to seven patients. Sev-enteen patients （46%） achieved cytological negative conversion. Median overall survival duration from the diagnosis of LMC was 6.7 wk （95% CI： 4.3-9.1 wk）. In the univariate analysis of survival duration, hemoglobin, IT chemotherapy, and cytological negative conversion showed superior survival duration （P = 0.038, P = 0.010, and P = 0.002, respectively）. Dynamic Friction Control Using Dynamic Structured RFNN and Friction Parameter Estimator

A nonlinear dynamic friction control is dealt with using dynamic friction observer and intelligent cantrol. The adaptive dynamic friction obsrver based on the LuGre friction is proposed to estimate the friction parameters and a directly friction state variable The dynamic structured Fuzzy Neural Network （RFNN） is designed to give additional robustness to the cantrol system under the presence of the friction model uncertainty. A proposed composite cantrol scheme is applied to the position tracking control of the servo systen. The performances of the proposed friction observer and the friction controller are demonstrated by simulation.

Primary adenocarcinoma of the appendix is a rare malignancy that constitutes 〈 0.5% of all gastrointestinal neoplasms. Moreover, primary signet ring cell carc noma of the appendix is an exceedingly rare entity. We have encountered 15 cases of primary appendiceal cancer among 3389 patients who underwent appen- dectomy over the past 18 years. In the present report, we describe a rare case of primary signet ring cell carcinoma of the appendix with ovarian metastases and unresectable peritoneal dissemination occurring in a 67-year-old female patient. She underwent ap- pendectomy and bilateral salpingo-oophorectomy with a laparoscopy procedure. She then received palliative systemic chemotherapy with 12 cycles of oxaliplatin, 5-flurorouracil, and leucovorin （FOIFOX-4）. The patient currently is well without progression of disease 12 mo after beginning chemotherapy.
