BACKGROUND Distant metastasis occasionally occurs in patients who have been diagnosed with colorectal cancer(CRC), but it occurs in a few patients with stage I CRC. The vagina as a metastasis site has also been reported, albeit rarely. Most reported cases of vaginal metastasis(VM) report their origin from advanced CRC. We encountered a patient who was diagnosed with isolated VM originating from stage I colon cancer(T2N0) and herein present the case of this *** SUMMARY A 63-year-old woman visited the outpatient clinic because of a positive result from a stool occult blood test. She underwent laparoscopic anterior resection and was pathologically diagnosed with stage I(T2N0) sigmoid colon cancer. Neither lymphovascular invasion nor perineural invasion was observed. Ten months following the surgery, isolated vaginal metastases were detected on gynecologic examination. The examination was performed due to vaginal spotting. A transvaginal wide excision was performed, and no other adjuvant treatment was provided after discussion with a multidisciplinary team and the ***, a new VM was discovered after 33 mo. An additional transvaginal excision was performed. To date, there has been no evidence of further disease progression. From the time of diagnosis of VM, the patient’s overall survival has been 54 *** VM can occur as a result of early-stage colorectal cancer. Surgeons should consider the possibility of VM following complaints of gynecologic symptoms following surgery.

Nowadays, almost every hospital utilizes picture archiving and communications system (PACS) and electronic medical record (EMR) system to manage patient’s data digitally. However, patient’s pictures taken by nurses or physicians to monitor lesions or various test results are not managed well in general. Although for some disease, these pictures provide meaningful information in diagnosis and treatment. In this paper, we propose a management system for these images to improve the accessibility and storability. To manage the images efficiently, the proposed methods utilized the PACS system in the hospital. After sending the image data of each patient to a local monitoring computer, the patient’s information, such as, name, age, male, and registration number were attached on the image. This file was then converted to a DICOM (digital imaging and communications in medicine) files and transferred to the PACS Server. The proposed system promises easy and convenient access on the images of lesions and various test results of patients to support fast and appropriate treatment.

The noise reduction of the ITE (In-The-Ear) hearing aid (HA) can be achieved by an array of microphones. Each of the right and the left ears is fixed with two ITE HA microphones. This two HA microphones’ array produces particular patterns of directivity which geometrically increases the S/N ratio. The boundary element method (BEM) was used for the three dimensional modeling and simulation of the human head model directivity pattern with the two by two microphone arrays. Three particular structures of the head model were considered;sphere, head Ⅰ, head Ⅱ. All three structures were meshed for boundary surface element grids. Head model Ⅰelement meshing was generated from three dimensional laser scanned points’ data. And Head model Ⅱelement meshing was formed by a CAD program. This numerical analysis was then applied for the calculation of the time delay parameters of the digital hearing aid DSP chip. The separation between two microphones was fixed to 10 mm. Input frequencies are 500, 1000, 1500, 2000, 2500, 3000, 3500, 4000 Hz. Time delays between two microphones were changed to produce the most narrow directivity pattern in the fore front of the head. The variation of the time delay was examined in accordance with input frequencies.

AIM:To determine long-term outcomes of surgical treatments for patients with constipation and features of colonic ***:Consecutive 42 patients who underwent surgery for chronic constipation within the last 13 years were prospectively *** identified a subgroup with colonic pseudo-obstruction(CPO) features,with dilatation of the colon proximal to the narrowed transitional zone,in contrast to typical slowtransit constipation(STC),without any dilated colonic *** outcomes of surgical treatments for chronic constipation with features of CPO were analyzed and compared with outcomes for ***:Of the 42 patients who underwent surgery for constipation,33 patients had CPO with dilatation of the colon proximal to the narrowed transitional *** were 16 males and 17 females with a mean age of 51.2 ± 16.1 *** had symptoms of chronic intestinal obstruction,including abdominal distension,pain,nausea,or vomiting,and the mean duration of symptoms was 67 mo(range:6-252 mo).Preoperative defecation frequency was 1.5 ± 0.6 times/wk(range:1-2 times/wk).Thirty-two patients underwent total colectomy,and one patient underwent diverting transverse *** was no surgery-related *** histologic examination showed hypoganglionosis or agangliosis in 23 patients and hypoganglionosis combined with visceral neuropathy or myopathy in 10 *** contrast,histology of STC group revealed intestinal neuronal dysplasia type B(n = 6) and visceral myopathy(n = 3).Early postoperative complications developed in six patients with CPO;wound infection(n = 3),paralytic ileus(n = 2),and intraabdominal abscess(n = 1).Defecation frequencies 3 mo after surgery improved to 4.2 ± 3.2 times/d(range:1-15 times/d).Long-term follow-up(median:39.7 mo) was available in 32 patients;all patients had improvements in constipation symptoms,but two patients needed intermittent medication for management of *** 32 patients had distinct

This study was conducted to understand the effect of the livestock liquid fertilizer treatment at field-scale up to catchment-scale on the water quality properties. Cultivated paddy rice and upland plots located in Gyeonggi province, Korea were treated with two different liquid fertilizers, SP (Liquid fertilizer with storage process) and SCB (Liquid fertilizer with slurry composting and bio-filtration process). Plots with no fertilizer (control A) and chemical fertilizer (control B) were also prepared for comparison. Water quantity and quality were monitored at the catchment outlet for assessing the effect on water quality of stream water. As a result, the losses of N and P and the values of EC (Electronic Conductivity) in the surface drainage water from paddy rice plots treated with SP and SCB were higher than those from the control plots (A and B). In addition, the losses of N and P by the runoff water from upland plots with SP and SCB treatment were higher than those from control plots (A and B). The nutrient outflow from paddy rice fields and uplands with application of liquid pig manure was higher than those from the control plots (A and B). Particularly, the outflow from uplands may directly affect the water quality in neighboring streams. This caused the major eutrophication problem in stream water. In conclusion, it is necessary to establish the proper management practices to prevent the nutrient losses from agricultural fields and the pollutants against water environments.

AIM： To identify the clinical features and outcomes of infrequently reported leptomeningeal carcinomatosis （LMC） of gastric ***： We analyzed 54 cases of cytologically confirmed gastric LMC at four institutions from 1994 to ***： The male-to-female ratio was 32：22, and the patients ranged in age from 28 to 78 years （median,48.5 years）. The majority of patients had advanced disease at initial diagnosis of gastric cancer. The clini-cal or pathologic tumor, node and metastasis stage ofthe primary gastric cancer wasin 38 patients （70%）.The median interval from diagnosis of the primarymalignancy to the diagnosis of LMC was 6.3 mo, rang-ing between 0 and 73.1 mo. Of the initial endoscopic f indings for the 45 available patients, 23 （51%） of the patients were Bormann typeand 15 （33%） patientswere Bormann type. Pathologically, 94% of cases proved to be poorly differentiated adenocarcinomas. Signet ring cell component was also observed in 40% of patients. Headache （85%） and nausea/vomiting （58%） were the most common presenting symptoms of LMC. A gadolinium-enhanced magnetic resonance imaging was conducted in 51 patients. Leptomeningeal enhancement was noted in 45 cases （82%）. Intrathecal （IT） chemotherapy was administered to 36 patients-primarily methotrexate alone （61%）, but also in combi-nation with hydrocortisone/± Ara-C （39%）. The median number of IT treatments was 7 （range, 1-18）. Concomitant radiotherapy was administered to 18 patients, and concomitant chemotherapy to seven patients. Sev-enteen patients （46%） achieved cytological negative conversion. Median overall survival duration from the diagnosis of LMC was 6.7 wk （95% CI： 4.3-9.1 wk）. In the univariate analysis of survival duration, hemoglobin, IT chemotherapy, and cytological negative conversion showed superior survival duration （P = 0.038, P = 0.010, and P = 0.002, respectively）. AIM： To examine whether trans-10,cis-12 CLA （t10C12） or cis-9,trans-11 CLA （c9t11） inhibits heregulin （HRG）-β- stimulated cell growth and HRG-β-ErbB3 signaling in HT-29 cells. METHODS： We cultured HT-29 cells in the absence or presence of the CLA isomers and/or the ErbB3 ligand HRG-β. MTT assay, [^3H]thymidine incorporation, Annexin V staining, RT-PCR, Western blotting, immunoprecipitation, and in vitro kinase assay were performed. RESULTS： HRG-β increased cell growth, but did not prevent DNA t10c12-induced growth inhibition. T10C12 inhibited DNA synthesis and induced apoptosis of HT-29 cells, whereas c9t11 had no effect. T10c12 decreased the levels of ErbB1, ErbB2, and ErbB3 proteins and transcripts in a dose-dependent manner, whereas c9t11 had no effect. Immunoprecipitation/ Western blot studies revealed that t10c12 inhibited HRG- β-stimulated phosphorylation of ErbB3, recruitment of the p85 subunit of phosphoinositide 3-kinase （PI3K） to ErbB3, ErbB3-associated PI3K activities, and phosphorylation of Akt. However, c9t11 had no effect on phospho Aid： levels. Neither t10c12 nor c9t11 had any effect on HRG-β-induced phosphorylation of ERK-1/2. CONCLUSION： These results indicate that the inhibition of HT-29 cell growth by t10c12 may be induced via its modulation of ErbB3 signaling leading to inhibition of Akt activation.

A nonlinear dynamic friction control is dealt with using dynamic friction observer and intelligent cantrol. The adaptive dynamic friction obsrver based on the LuGre friction is proposed to estimate the friction parameters and a directly friction state variable The dynamic structured Fuzzy Neural Network （RFNN） is designed to give additional robustness to the cantrol system under the presence of the friction model uncertainty. A proposed composite cantrol scheme is applied to the position tracking control of the servo systen. The performances of the proposed friction observer and the friction controller are demonstrated by simulation.
