Plasmodium vivax, a major agent of malaria in both temperate and tropical climates, has a limited capacity for infecting humans lacking the Duffy(Fy) blood group antigen as this receptor is critical for erythrocyte invasion. Recent surveys in various endemic regions, however, have reported P. vivax infections in Duffy-negative individuals, suggesting that the parasite may utilize alternative receptor-ligand pairs to complete erythrocyte invasion. Here, we identified and characterized a novel parasite ligand, GPI-anchored micronemal antigen(Pv GAMA), that can bind human erythrocytes regardless of Duffy antigen status. Specific antibodies raised against PvG AMA localize the protein to the parasite microneme, consistent with a role in invasion. These antibodies also inhibited Pv GAMA binding to erythrocytes in a dose-dependent manner. Pv GAMA appears to play a role in a sialic acid-independent, chymotrypsin-sensitive invasion pathway. Thus, Pv GAMA may be an adhesion molecule for invasion of Duffy positive and negative human erythrocytes and is an important P. vivax vaccine candidate.

Risk factors for local recurrence after en bloc endoscopic submucosal dissection for early gastric cancer

AIM: To investigate factors related to recurrence following en bloc resection using endoscopic submucosal dissection(ESD) in patients with early gastric cancer(EGC). METHODS: A total of 1121 patients(1215 lesions) who had undergone ESD for gastric neoplasia between April 2003 and May 2010 were retrospectively reviewed. Data from 401 patients(415 lesions) were analyzed, following the exclusion of those who underwent piecemeal resection, with deep resection margin invasion or lateral margin infiltration, and diagnosed with benign lesions. RESULTS: Local recurrence after en bloc ESD was found in 36 cases(8.7%). Unclear resection margins, long procedure times, and narrow safety margins were identified as risk factors for recurrence. Lesions located in the upper third of the stomach showed a higher rate of recurrence than those located in the lower third of the stomach(OR = 2.9, P = 0.03). The probability of no recurrence for up to 24 mo was 79.9% in those with a safety resection margin ≤ 1 mm and 89.5% in those with a margin > 1 mm(log-rank test, P = 0.03). CONCLUSION: Even in cases in which en bloc ESD is performed for EGC, local recurrence still occurs. To reduce local recurrences, more careful assessment will be needed prior to the implementation of ESD in casesin which the tumor is located in the upper third of the stomach. In addition, clear identification of tumor boundaries as well as the securing of sufficient safety resection margins will be important.

^18F-FDG PET/CT显像能否鉴别诊断特发性肺纤维化患者的孤立性肺结节?

目的特发性肺纤维化(IPF)患者的肺癌发生率高,患者肺功能差,易发生气胸,因此对CT发现的孤立性肺结节(SPN)不宜采用侵入性诊断方法,否则可能导致严重的不良后果。^18F-脱氧葡萄糖(FDG)PET/CT可用于非IPF患者SPN的良恶性鉴别,但在IPF患者中的诊断准确性尚无研究。因此该研究拟调查^18F-FDGPET/CT是否能鉴别诊断IPF患者的SPN。方法回顾性分析来自该院的55例IPF患者[男54例、女1例,年龄(67.8±7.6)岁],患者SPN大小为8~30(18.5±5.7)mm,所有患者在2004年4月至2016年3月间行胸部CT和^18F-FDGPET/CT显像。对SPN的^18F-FDG摄取行视觉分析和半定量分析,图像结果与最终病理诊断(n=52)或影像学随访(n=3)结果进行比较。结果最终诊断结果表明,41个(75%)SPNs为恶性(21个鳞状细胞癌、9个腺癌、5个小细胞癌、4个混合类型癌、1个大细胞神经内分泌癌和1个肉瘤样癌),14个(25%)为良性。视觉分析PET/CT图像诊断SPN为恶性病变的灵敏度、特异性、阳性预测值(PPV)和阴性预测值(NPV)分别为98%、86%、95%和92%。使用最大标准摄取值(SUVmax)为2.0行半定量分析诊断的灵敏度、特异性、PPV和NPV分别为95%、93%、98%和87%。结论^18F-FDGPET/CT可用于IPF患者SPN的良恶性鉴别,如其用于非IPF患者一样。

AIM:To evaluate the incidence and risk factors of Korean tuberculosis(TB) infection in patients with inflammatory bowel disease(IBD) undergoing anti-TNF ***:The data of IBD patients treated with anti-TNFs in 13 tertiary referral hospitals located in the southeastern region of Korea were collected *** failed to show response or were intolerant to conventional treatments,including steroids or *** measures for latent TB infection(LTBI)and the incidence and risk factors ofactive TB infection after treatment with anti-TNFs were ***:Overall,376 IBD patients treated with antiTNF agents were recruited(male 255,mean age of anti-TNF therapy 32.5±13.0 years);277 had Crohn’s disease,99 had ulcerative colitis,294 used infliximab,and 82 used *** anti-TNF treatment,screening tests for LTBI including an interferon gamma release assay or a tuberculin skin test were performed in 82.2%of *** patients(8%)had *** cases of active TB infection including one TB-related mortality occurred during 801 personyears(PY)follow-up(1997.4 cases per 100000 PY)after anti-TNF ***(OR=5.76,95%CI:1.57-21.20,P=0.008)and WBC count<5000 mm3(OR=4.5,95%CI:1.51-13.44,P=0.007)during follow-up were identified as independently associated risk ***:Anti-TNFs significantly increase the risk of TB infection in Korean patients with *** considerable burden of TB and marked immunosuppression might be attributed to this risk.

研究了核桃楸树皮的化学成分及其抗氧化活性。采用Sephadex LH-20柱色谱及薄层色谱等方法进行分离,从其70%丙酮提取物乙酸乙酯及水溶性部分中分到11种化合物,经波谱分析及理化性质鉴定化合物分别为:短叶松素(1)、花旗松素(2)、蛇葡萄素(3)、山奈酚(4)、槲皮素(5)、杨梅素(6)、阿福豆苷(7)、紫云英苷(8)、槲皮苷(9)、异槲皮苷(10)、杨梅苷(11)。化合物1、3、5、6、8、10为首次从该植物中分得。经DPPH试验,测定了正己烷溶性、二氯甲烷溶性、乙酸乙酯溶性和水溶性部分以及粗提物和分得化合物的抗氧化活性。其中乙酸乙酯和水溶性部分及化合物1～6与对照组相比具有很强的抗氧化活性。

接枝聚合法制备PANI/CeO_2-APTMS复合材料及其电化学性能

对氧化铈(CeO_2)进行3-氨丙基三甲氧基硅烷(APTMS)修饰得到改性氧化铈(CeO_2-APTMS),并与苯胺(An)发生接枝聚合,制备出CeO_2-APTMS为载体,负载聚苯胺(PANI)的复合材料PANI/CeO_2-APTMS。采用红外光谱、X射线衍射、热重分析及旋转圆盘电化学测试技术,将PANI/CeO_2-APTMS复合材料同未经改性PANI/CeO_2复合材料、纯PANI电化学性能进行比较。结果表明,PANI/CeO_2-APTMS中PANI接枝效果最好,而且CeO_2-APTMS表面接枝PANI与纯PANI结构相同;经过APTMS修饰CeO_2与PANI之间通过价键接枝成PANI/CeO_2-APTMS的起始分解温度在242℃左右,且在200~600℃内的分解速率最慢,PANI/CeO_2-APTMS表现出很好的热稳定性;PANI/CeO_2-APTMS复合材料修饰电极在65g/L Zn^(2+),150 g/L H_2SO_4的硫酸锌体系中,通过循环伏安测试其具有良好峰对称性以及最高阳极峰电流,极化曲线测试对比3类修饰电极时发现在不同转速下PANI/CeO_2-APTMS/GC修饰电极的析氧电位值均偏低,体现出良好的稳定性及析氧电催化活性。

自2019年12月新型冠状病毒肺炎(Coronavirus Disease 2019, COVID-19)暴发以来,已有大量相关临床研究完成注册或已启动。然而,不同研究间临床结局指标存在显著的异质性,同时一些不关键的指标会造成研究资源的浪费。本研究旨在构建COVID-19临床试验核心结局指标集(core outcome set, COS),为相关临床研究方案设计、开展和证据转化提供参考。本研究的实施参照《有效性试验核心结局指标(COMET)手册》(1.0版),研究组成员包括呼吸系统和危重医学、中医学、循证医学、临床药理学、统计学专家以及医学杂志编辑等。通过检索临床试验注册网站(***和***)中COVID-19相关临床研究方案,提取方案中的结局指标,形成指标池。78个COVID-19临床试验方案纳入研究,收集结局指标259个,合并规范后确定132个结局指标,涉及7个指标域。经遴选,形成了包括58个结局指标在内的初始指标清单。经过两轮德尔菲调查和一轮共识会议,最终确立针对COVID-19不同临床分型的核心指标集(COS-COVID)。COS-COVID包括1个轻型指标(病毒核酸检测转阴时间)、4个普通型指标(住院时间、复合事件发生率、临床症状积分和病毒核酸检测转阴时间)、5个重型指标(复合事件发生率、住院时间、PaO2/FiO2、机械通气时间和病毒核酸检测转阴时间)、1个危重型指标(全因死亡率)及1个康复期指标(肺功能)。COS-COVID作为目前临床评价COVID-19干预效果最具临床价值及操作性的指标集合,对证据评估和决策具有重要意义。随着对疾病认知的不断深入和COS-COVID应用的反馈,课题组将对本核心指标集进行进一步完善并更新。
