Comparison of contrast sensitivity based on the surgical results for intermittent exotropia

AIM: To compare contrast sensitivity(CS) based on the surgical results for intermittent exotropia(IXT) and to examine the relationship between CS and ***: Medical records of the patients who underwent bilateral lateral rectus muscle recession for IXT between 4 and 12 years old were reviewed retrospectively. They were categorized based on the surgical results;successful correction group(n=36) and overcorrection group(esotropia ≥10 PD at 3 mo postoperatively, n=18). Using CGT-2000 test for CS was performed binocularly, and subjective reports of photophobia was investigated preoperatively and at 3 mo postoperatively. Objective photophobia was defined as a significant decrease in CS in the presence of ***: Preoperatively, there was no difference in CS between the groups. Postoperatively, under mesopic conditions, significant improvement of CS was observed at 6.3°, 4°, and 2.5° in the successful correction group and at 6.3° and 4° in the overcorrection group, regardless of glare. Under photopic conditions, at all visual angles except 0.64°, improvement in CS was noted in both groups while CS worsened significantly at 0.64° in the overcorrection group postoperatively. At all visual angles under photopic conditions postoperatively, regardless of glare, CS in the overcorrected group was significantly worse than that in the successful correction group, and CS was significantly decreased by addition of glare in both groups. All patients except one(96.4%) in the successful correction group and 8 patients(61.5%) in overcorrection group showed improvement of photophobia postoperatively, which correlated with CS under photopic conditions(P=0.001, 0.03).CONCLUSION: After surgery for IXT, CS under photopic conditions improve at all visual angles except 0.64°, while CS is significantly worse in the overcorrection group postoperatively at 0.64°. Objective: To investigate the fungistatic activity and synergistic effects of natural products and their constituents, including traditional oriental medicines(TOMs).Methods: Fungistatic activities of TOMs prepared by hot-water(115 ℃) or ethanol(70%; 40 ℃) extraction were determined by their minimum inhibitory *** assess possible synergistic effects, minimum inhibitory concentrations of various combinations were ***: By evaluating antifungal susceptibility of Trichophyton rubrum, which is a major causative fungus for several types of dermatophytosis, we confirmed that ethanol extracts were more active than hot-water extracts in 25 of the 36 TOMs, suggesting that the constituents with high hydrophobicity tend to contribute significantly to fungistatic *** selected four TOMs with high fungistatic activity, including Aucklandiae radix, Gentianae macrophyllae radix, Scutellariae radix, and Galla rhois, and their synergistic effects were investigated through the combination studies between TOMs or TOM-conventional drug *** combinations between four TOMs, partial synergistic effects were observed in Aucklandiae radix–Galla rhois and Gentianae macrophyllae radix–Galla rhois combinations, as supported by the lowest fractional inhibitory concentration index value of 0.66 for both ***, Galla rhois showed the strongest synergistic effect on growth inhibition of Trichophyton rubrumwith a fractional inhibitory concentration index value of 0.50 in combination with ***: Our findings indicate that the combination of TOMs and TOM-terbinafine may be effective on treatment for chronic and recurrent dermatophytosis by improving fungistatic activity and led to decrease systemic toxicity in clinical practice.

The present study aims to determine the appropriate size of mesh or the number of the element(NoE)for flat-and curved plates,which is suggested to assess its safety subjected to axial compression based on the ultimate limit state(ULS)design and analysis *** unstiffened panel(=plate)and stiff-ened panel,considered primary members of ships and ship-shaped offshore structures,are subjected to repeated axial compression and tension caused by continued vertical bending moments applied to the hull *** are attached with stiffeners by welding,and 6,8 or 10 elements are generally rec-ommended to allocate in flat-plate’s breadth direction in between stiffeners for finite-element(FE)mod-elling,which enables the presentation of the shape of initial deflection applied to the *** the case of the load-shorting curve for curved plate,it is reported that the nonlinear behaviour characteristics,i.e.,snap-through,snap-back,secondary buckling and others,appear in typical flank *** take this into account,we investigated the preferred number of elements(6,8 or 10)generally applied to the flat plate whether it is an appropriate or more fine-sized element(or mesh)that should be considered.A useful guide is documented based on obtained outcomes which may help structural engineers select optimised mesh-size to predict ultimate strength and understand its characteristic of the flat and curved plates.

Permo-Triassic high-pressure metamorphism in the central western Korean Peninsula, and its link to Paleo-Tethyan Ocean closure: Key issues revisited

Permo-Triassic high-pressure(HP) mafic granulites, together with the Bibong retrogressed eclogite,preserved along the central western Korean Peninsula provide important insights into the Late Permian to Triassic collisional orogeny in northeast Asia. The metamorphic pressureetemperatureetime(P-T-t)paths of these rocks, however, remain poorly constrained and even overestimated, owing to outdated geothermobarometers and inaccurate isopleth techniques. Here we evaluate the metamorphic Pe T conditions of Triassic HP mafic granulites including those in Baekdong, Sinri and Daepan and the Bibong Triassic retrogressed eclogite in the Hongseong area, and the Permo-Triassic Samgot mafic granulite in the Imjingang Belt of the central western Korean Peninsula through the application of modern phase equilibria techniques. The Baekdong and Samgot mafic granulites and the Bibong retrogressed eclogite yield a range of 12.0 -16.0 kbar and 800 -900℃, representing HP granulite facies conditions. The Sinri and Daepan granulites from the Hongseong area show relatively lower grade metamorphic conditions between HP granulite and normal granulite facies, and are characterized by sub-isothermal decompression during exhumation. The similarities in the metamorphic ages and the post-collisional igneous activity from the central western Korean Peninsula indicate that the Triassic ages represent the retrograde stage of the metamorphic Pe T paths. In contrast, the Late Permian metamorphic ages, which are older than protolith ages of the post-collisional igneous rocks, correspond to the possible prograde stage of metamorphism. The P-T-t paths presented in this paper, together with the metamorphic ages and post-orogenic igneous events reported from these areas suggest trace of the subduction, accretion and exhumation history, and indicate a tectonic linkage among the northeast Asian continents during the Paleo-Tethyan Ocean closure.

A Mathematical Model for the Deficiency-Excess Mechanism of Yin-Yang in Five Viscera

To raise traditional medicine to a higher level of scientific research, a mathematical model has been proposed using symbolic notations and operators to describe several disease symptoms generally recognized in traditional medicine. Even though this model to a certain degree offers a mathematical approach to identify the relationships between yin-yang and the five viscera, it is not an efficient means of explaining the pathology in traditional medicine due to its use of superfluous notations and definitions. In this paper, we introduce two concise operators, a self-development operator and an action operator： the former describes the effect of a viscus in the unbalanced state on other viscera： the latter explains the engendering and restraining relationships between the two viscera. These tools are useful to elucidate the interactions among the states of the five viscera based on yin-yang and the five elements theory. Our mathematical model with these two operators facilitates description for the scheme of deficiency-excess of yin-yang in the five viscera. Accordingly, we have mathematically refined the existing results and shown clinical applications as well.

AIM:To evaluate and compare the clinical outcomes of prophylactic submucosal saline-epinephrine injection and saline injection alone for large colon polyps by conventional polypectomy. METHODS:A prospective study was conducted from July 2003 to July 2004 at 11 tertiary endoscopic centers. Large colon polyps (> 10 mm in diameter) wererandomized to undergo endoscopic polypectomy with submucosal saline-epinephrine injection (epinephrine group) or normal saline injection (saline group). Endoscopic polypectomy was performed by the conventional snare method,and early ( 2 cm) and neoplastic polyps were independently and significantly associated with the presence of PPB. CONCLUSION:The prophylactic submucosal injection of diluted epinephrine does not appear to provide an additional advantage over the saline injection alone for the prevention of PPB.

Colonoscopy is a widely used method for diagnosing and treating colonic disease. The number of colonoscopies is increasing worldwide, and concerns about associated adverse events are growing. Large-scale studies using big data for post-colonoscopy complications have been reported. A colon perforation is a severe complication with a relatively high mortality rate. The perforation rate, as reported in large studies(≥ 50,000 colonoscopies) published since 2000, ranges from 0.005-0.085%. The trend in the overall perforation rate in the past 15 years has not changed significantly. Bleeding is a more common adverse event than perforation. Recent large studies(≥ 50,000 colonoscopies) have reported postcolonoscopy bleeding occurring in 0.001-0.687% of cases. Most studies about adverse events related to colonoscopy were performed in the West, and relatively few studies have been conducted in the East. The incidence of post-colonoscopy complications increases in elderly patients or patients with inflammatory bowel diseases. It is important to use a unified definition and refined data to overcome the limitations of previous studies. In addition, a structured training program for endoscopists and a systematic national management program are needed to reduce post-colonoscopy complications. In this review, we discuss the current trends in colonoscopy related to adverse events, as well as the challenges to be addressed through future research.
