BACKGROUND Porocarcinoma is a rare type of skin cancer that originates from sweat gland *** is an aggressive malignant skin cancer that is difficult to diagnose clinically owing to its rarity and similarity to squamous cell carcinoma(SCC).CASE SUMMARY This case involved a 92-year-old woman,a farmer by profession,presented with an exophytic and verrucous mass on her left palm that had formed 2 years prior and caused chronic pain and frequent ***,the patient was diagnosed with SCC using a punch biopsy;however,a repeat biopsy with addi-tional immunohistochemical tests was performed for ***,the patient was diagnosed with porocarcinoma and reconstruction was planned using a full-thickness skin *** treatment,the range of motion of the palm was preserved,and the aesthetic outcome was *** 6 mo of follow-up,the patient was satisfied with the *** Porocarcinoma is commonly misdiagnosed as SCC;therefore,clinicians should consider porocarcinomas when evaluating mass-like lesions on the hands.

AIM:To investigate the attenuation patterns and detectability of common bile duct(CBD) stones by multidetector computed tomography(MDCT).METHODS:Between March 2010 and February 2012,191 patients with suspicion of CBD stones undergoing both MDCT and endoscopic retrograde cholangiopancreatography(ERCP) were enrolled and reviewed *** attenuation patterns of CBD stones on MDCT were classified as heavily calcified,radiopaque,less radiopaque,or *** association between the attenuation patterns of CBD stones on MDCT and stone type consisting of pure cholesterol,mixed cholesterol,brown pigment,and black pigment and the factors related to the detectability of CBD stones by MDCT were ***:MDCT showed CBD stones in 111 of 130 patients in whom the CBD stones were demonstrated by ERCP with 85.4% *** attenuation patterns of CBD stones on MDCT were heavily calcified 34(26%),radiopaque 31(24%),less radiopaque 46(35%),and undetectable 19(15%).The radiopacity of CBD stones differed significantly according to stone type(P < 0.001).From the receiver operating characteristic curve,stone size was useful for the determination of CBD stone by MDCT(area under curve 0.779,P < 0.001) and appropriate cut-off stone size on MDCT was 5 *** factors related to detectability of CBD stones on MDCT were age,stone type,and stone size on multivariate analysis(P < 0.05).CONCLUSION:The radiopacity of CBD stones on MDCT differed according to stone *** type and stone size were related to the detectability by MDCT,and appropriate cut-off stone size was 5 mm.

BACKGROUND Musculoskeletal nontuberculous Mycobacterium(NTM)infections are rare,partic-ularly post-acupuncture therapy,and present diagnostic challenges due to their infrequency and potential *** recognition and appropriate manage-ment are crucial for optimal ***-infected wounds involving the joints are difficult to treat,and only a few cases have been *** SUMMARY We present a case of a chronic intractable NTM-infected wound on the elbow joint that completely healed with conservative wound care and antibiotic *** 81-year-old woman presented with a chronic,ulcerative wound on the right elbow joint where she had undergone repeated acupuncture therapy for chronic intolerable *** resonance imaging revealed synovial thickening,effusion,and subcutaneous cystic *** orthopedic surgeon performed open synovectomy and serial ***,1 month postoperatively,the wound had not healed and became chronic.A wound culture revealed NTM(Mycobacterium abscessus),and the patient was referred to the Department of Plastic and Reconstructive *** of surgical intervention,conservative wound care with intravenous antibiotics was provided,considering the wound status and the patient’s poor general *** wound healing was achieved in 12 months,with no impact on the range of motion of the elbow *** With clinical awareness,musculoskeletal NTM infection can be treated with conservative wound care and appropriate antimicrobial agents.

The design and optimization of a self-complementary two-arm Archimedean spiral antenna backed by an absorptive cavity were presented. Parametric studies on the proposed antenna structure were carried out by using CST MWS. Simulation results show that the proper choice of spiral turns and cavity depth can miniaturize the dimensions of the cavity-backed spiral antenna presented here. According to simulation results, prototype antennas operating in the 6 12 GHz band are fabricated and the dimension of the proposed cavity-backed spiral antenna is 22 mm (diameter)×15 mm (height). The performance of the proposed antenna was measured and compared with the simulation results. It is shown that the experimental results are consistent with the theoretical predictions and the suggested antenna is good enough to adapt for various wideband applications.

AIM:To evaluate the clinical efficacy of dietary supplement of high dose DHA omega-3 in dry eye with meibomian gland dysfunction(MGD).METHODS:Prospective randomized double-masked,placebo-controlled clinical trial was conducted in mild to moderate dry eye patients with *** have no history of taking any dietary omega-3 supplements before 3 *** were divided into two groups:24 patients in the omega-3 group and 26 patients in the placebo *** omega-3 group received two capsules of Easyeye Dry?,total containing 600 mg of EPA and 1640 mg of DHA,while the placebo group received two capsules containing 3000 mg of olive *** patients take two pills once a *** examination of MGD scores,tear break-up time(TBUT),corneal staining test(NEI),strip meniscometry(SM tube),and ocular surface disease index(OSDI)scores were performed at baseline,after 4 and 8 ***:A total of 50 patients were *** were no differences in baseline characteristics between the two groups,such as age,sex,and other ocular examination *** TBUT,NEI,and OSDI scores significantly improved after 4 and 8 wk in both *** after 8 wk TBUT(6.00±1.62 s vs 5.08±1.28 s,P=0.034)and MGD score(7.2±1.8 vs 8.1±2.6,P=0.033)in the omega-3 group was more significantly improved than that of the placebo ***:Dry eye with the MGD patient,a high dose of DHA omega-3 dietary supplement can improve TBUT and MGD score after 8 wk,effective in stabilizing the tear film.
