Diagnostic Indicators for Blood Stasis Syndrome Patients with Gynaecological Diseases

Objective： To investigate the important diagnostic indicators for blood stasis syndrome （BSS） in patients of childbearing age with gynaecological diseases. Methods： A partial least squared-discriminant analysis （PLS-DA） were applied to BSS symptoms data of patients with gynaecological diseases, and the diagnostic indicators used by doctors of Korean medicine （DKMs） among BSS patients with gynaecological diseases were also investigated. Results： A total of 103 patients of childbearing age with gynaecological diseases and 40 healthy controls were enrolled. Among the 103 patients, 63 （61.7%） and 40 （38.8%） were diagnosed with BSS and non-BSS, respectively, and BSS patients exhibited a more severe extent of disease. A score plot of PLS-DA showed clearly different patterns among the 3 groups. Based on the variable importance on projection of PLS-DA model, menstrual pains, dark lumps in the menstrual blood, ileocoecal tenderness and resistance, sharp pains, and sublingual varicosities were selected as the top five most important indicators. Moreover, more than 75% of DKMs chose dark lumps in menstrual blood, menstrual pain, and dark menstrual blood as the diagnostic indicators of BSS in patients with gynaecological diseases, and more than 49% of them also considered sharp pains, dark red tongue, sublingual varicosities, and tendency to bruise easily as diagnostic indicators of BSS. Conclusion： DKMs focused on menstrual symptoms and certain gynaecological symptoms to diagnose BSS patients of childbearing age with female diseases.

Fabrication of Near Net Shape Metallic Foam via Plaster Mould

Fabrication characteristics are unstable in direct foaming method. Therefore, most of near net-shape metallic foams are produced, and investigated by powder metallurgy. Direct foaming method, however, has many benefits （i.e. reduce the unit cost of goods and fabrication process etc.） to fabrication of metallic foams. In this article, the fabrication characteristic of near net-shape metallic foams by direct foaming method was evaluated. Al and Plaster was used for base material and mould material respectively. Ca and TiH2 were added to molten Al as thickening and blowing agent for stable condition of bubbles. Thickening time was about 10 min with a stirring speed of 600 r/min. Foaming time was 30-120 s for evaluation of the optimum foaming condition. Amount of agent was selected by pre-experimental data. Porosity of near net-shape goods was measured by Archimedes method. On the other hand, it seems that increasing poring time and thickening agent make the poor porosity

Role of Jun amino-terminal kinase （JNK） in apoptosis of cavernosal tissue during acute phase after cavernosal nerve injury

The present study aimed to identify which mitogen-activated protein kinase （p38 or Jun amino-terminal kinase [JNK]） was involved in cavernosal apoptosis during the acute phase after cavernosal nerve crush injury （CNCI） in rats to ameliorate apoptosis of cavernosal tissue, such as smooth muscle （SM）. A total of twenty 10-week-old male Sprague-Dawley rats were divided equally into two groups： sham surgery （S） and CNCI （I）. The I group approximated the clinical situation of men undergoing radical prostatectomy using two 60-second compressions of both CNs with a microsurgical vascular clamp. At 2-week postinjury, erectile response was assessed using electrostimulation. Penile tissues were harvested for immunohistochemistry analysis of alpha-SM actin （（x-SMA）, western blot analysis, and double immunofluorescence analysis of （x-SMA and phosphorylated p38 or JNK, as well as double immunofluorescent of TUNEL and phosphorylated p38 or JNK. At 2-week postinjury, the I group had a significantly lower intracavernous pressure （ICP）/mean arterial pressure （MAP） and a lower area under the curve （AUC）/MAP than the S group. The I group also exhibited decreased immunohistochemical staining of α-SMA, an increase in the number of SM cells positive for phosphorylated JNK, an increased number of apoptotic cells positive for phosphorylated JNK, and increased JNK phosphorylation compared with the S group. However, there was no significant difference in p38 phosphorylation expression or the number of SM cells positive for phosphorylated p38 between the two groups. In conclusion, our data suggest that JNK, not p38, is involved in cavernosal apoptosis during the acute phase after partial CN damage.

Relationship between intraocular pressure and parameters of obesity in ocular hypertension

AIM:To evaluate the correlation between intraocular pressure(IOP)and various obesity-related health factors in patients with ocular hypertension in ***:A total of 40850 subjects underwent age,sex,body weight,and height assessments and automated multiphasic tests,including non-contact tonometry,automated perimetry,fundus photography,systolic/diastolic blood pressure measurement,and evaluation of obesity-related health parameters such as obesity index,body mass index(BMI),a body shape index(ABSI),and waist-to-height ratio(WtHR).Subjects were divided into ocular hypertension group and normal IOP group according to IOP after matching of age and ***:Of 40850 participants,1515(3.7%)had ocular hypertension,and 1515 with normal IOP were selected as controls using propensity score *** mean IOP of control group was 15.3±2.3 mm Hg,compared with 23.3±1.6 mm Hg in ocular hypertension ***,obesity index,BMI,and WtHR in the ocular hypertension group were significantly higher than in the normal IOP group(P<0.001,P<0.001,P=0.009,P=0.002).IOP of ocular hypertension was positively correlated with obesity index(P=0.027)and BMI(P=0.016),whereas IOP of control was positively correlated with blood pressure(P<0.001,P=0.002),obesity index(P<0.001),BMI(P<0.001),and WHtR(P=0.002).Systolic blood pressure(β=0.022,P<0.001)and body weight(β=0.016,P=0.02)were precursors of IOP in normal subjects,but sex(male;β=-0.231,P=0.008)and obesity index(β=-0.007,P=0.017)were precursors of ocular hypertension according to multiple regression ***:Among various obesity-related health parameters,obesity index is the best indicator for further increase in IOP in ocular hypertension group.

Enhanced intestinal lymphatic absorption of saquinavir through supersaturated self-microemulsifying drug delivery systems

The therapeutic potential of saquinavir, a specific inhibitor of human immunodeficiency virus(HIV)-1 and HIV-2 protease enzymes, has been largely limited because of a low solubility and consequnt low bioavailability. Thus, we aimed to design a supersaturated selfmicroemulsifying drug delivery system(S-SMEDDS) that can maintain a high concentration of saquinavir in gastro-intestinal fluid thorugh inhibiting the drug precipitation to enhance the lymphatic transport of saquinavir and to increase the bioavailability of saquinavir considerably. Solubilizing capacity of different oils, surfactants, and cosurfactants for saquinavir was evaluated to select optimal ingredients for preparation of *** the construction of pseudo-ternary phase diagram, SMEDDS formulations were established. A polymer as a precipitation inhibitor was selected based on its viscosity and drug precipitation inhibiting capacity. The S-SMEDDS and SMEDDS designed were administered at an equal dose to rats. At predetermined time points, levels of saquinavir in lymph collected from the rats were assessed. SMEDDS prepared presented a proper selfmicroemulsification efficiency and dispersion stability. The S-SMEDDS fabricated using the SMEDDS and hydroxypropyl methyl cellulose 2910 as a precipitation inhibitor exhibited a signficantly enhanced solubilizing capacity for saquinavir. The drug concentration in a simulated intestinal fluid evaluated with the S-SMEDDS was also maintained at higher levels for prolonged time than that examined with the SMEDDS. The S-SMEDDS showed a considerably enhanced lymphatic absoprtion of saquinavir in rats compared to the ***, the S-SMEDDS would be usefully exploited to enhance the lymphatic absorption of hydrophobic drugs that need to be targeted to the lymphatic system.

Acid suppressive drugs and gastric cancer: A meta-analysis of observational studies

AIM: To evaluate the association between acid suppressive drug use and the development of gastric cancer. METHODS: A systematic search of relevant studies that were published through June 2012 was conducted using the MEDLINE (PubMed), EMBASE, and Cochrane Library databases. The search included observational studies on the use of histamine 2-receptor antagonists (H 2 RAs) or proton pump inhibitors and the associated risk of gastric cancer, which was measured using the adjusted odds ratio (OR) or the relative risk and 95%CI. An independent extraction was performed by two of the authors, and a consensus was reached. RESULTS: Of 4595 screened articles, 11 observational studies (n = 94558) with 5980 gastric cancer patients were included in the final analyses. When all the studies were pooled, acid suppressive drug use was associated with an increased risk of gastric cancer risk (adjusted OR = 1.42; 95%CI: 1.29-1.56, I2 = 48.9%, P = 0.034). The overall risk of gastric cancer increased among H 2 RA users (adjusted OR = 1.40; 95%CI: 1.24-1.59, I2 = 59.5%, P = 0.008) and PPI users (adjusted OR = 1.39; 95%CI: 1.19-1.64, I2 = 0.0%, P = 0.377). CONCLUSION: Acid suppressive drugs are associated with an increased risk of gastric cancer. Further studies are needed to test the effect of acid suppressive drugs on gastric cancer.

Effect of incision types for Artisan phakic intraocular lens implantation on ocular higher order aberrations

AIM： To evaluate the effect of incision types for Artisan phakic intraocular lens （PIOL） implantation on ocular higher-order aberrations （HOAs）.METHODS： A retrospective review was conducted of the patients who had undergone Artisan PIOL implantation for the correction of myopia and followed up for at least 6mo. Patients are classified into 2 groups considering the incision type： cornea group with patients undergone clear corneal incision; sclera group with patients undergone sclera tunnel incision. All patients with postoperative astigmatism of under 1 diopter （D） were included to minimize the effect of residual astigmatism on postoperative HOAs. Visual acuity, special equivalents, astigmatism, predictability （±1 D from target refraction）, HOAs （coma, trefoil, spherical aberration）, and corneal endothelial counts were analyzed preoperatively and 6mo ***： At the postoperative 6mo, all patients of both groups achieved uncorrected visual acuity of 16/20 or better, and significantly decreased the spherical equivalents compared with preoperative values. The predictability of refractive correction was 96% in the former, and 94% in the latter. Unlike the sclera group, preoperative astigmatism decreased significantly in cornea group at postoperative 6mo. The HOAs increased significantly at postoperative 6mo compared to the preoperative values in both groups, and the root mean square （RMS） total and trefoil wavefront aberration of cornea group were significantly higher than those of sclera ***： Although corneal incision significantly reduces preexisting astigmatism, the postoperative 6mo of total RMS and trefoil aberration change may deteriorate the visual quality after Artisan PIOL implantation.

Effect of drawing speed on microstructure distribution and drawability in twinning-induced plasticity steel during wire drawing

The effect of drawing speed on temperature rise and microstructure distribution in twinning-induced plasticity(TWIP)steel during wire drawing has been investigated to improve drawability for wire rod *** wire drawing process is performed at room temperature,heat is generated due to the plastic deformation and friction at the wire-die *** steel wires subjected to the low drawing speed(LD)of 0.5 m/min and the high drawing speed(HD)of 5.0 m/min were analyzed using the numerical simulation and electron backscatter diffraction ***,the specimens subjected to the HD had a higher drawability by about 18%compared to the LD,which is totally different from the general behavior of plain carbon pearlitic *** LD wire had uniform temperature distribution along the radial direction during wire *** contrast,the HD wire had a temperature gradient along the radial direction due to the higher frictional effect at surface:the minimum temperature of 58℃ at center area and the maximum temperature of 143 C at surface *** higher stacking fault energy of HD wire at the surface area due to the high temperature rise retarded twinning rate,resulting in the prevention of fast exhaustion in ductility in comparison with the LD wires since the earlier depletion of twins at surface area is known as the main reason for the fracture of TWIP steel during wire ***,HD process delayed the fracture strain of wire and increased the uniformity of microstructure and mechanical properties along the radial direction.

BACKGROUND Korean National Health Insurance(NHI)claims database provides ***,studies regarding accuracy of administrative database for pancreatic cancer(PC)have not been *** aimed to identify accuracy of NHI database regarding PC classified by international classification of disease(ICD)-10 *** To identify the accuracy and usefulness of administrative database in PC and the accurate ICD codes for PC with *** Study and control groups were collected from 2003 to 2016 at Seoul National University Bundang *** of PC were identified in NHI database by international classification of diseases,10th revision edition(ICD-10 codes)supported with V codes.V code is issued by medical doctors for covering 95%of medical cost by Korean *** to pathologic reports,definite or possible diagnoses were defined using medical records,images,and *** A total of 1846 cases with PC and controls were *** PC,only 410(22.2%)cases were identified as specific cancer sites including head in 234(12.7%)cases,tail in 104(5.6%)cases and body in 72(3.9%)*** PC,910(49.3%)cases were diagnosed by definite *** of these were adenocarcinoma(98.0%).The rates of definite diagnosis of PC were highest in head(70.1%)followed by body(47.2%)and tail(43.3%).False-positive cases were pancreatic cystic neoplasm and metastasis to the *** terms of the overall diagnosis of PC,sensitivity,specificity,positive predictive value,and negative predictive value were 99.95%,98.72%,98.70%,and 99.95%,*** accuracy was similar both in terms of diagnostic criteria and tumor *** Korean NHI claims database collected according to ICD-10 code with V code for PC showed good accuracy.

Cellular reprogramming technologies have been developed with different physicochemical factors to improve the reprogramming efficiencies of induced pluripotent stem cells(iPSCs).Ultrasound is a clinically applied noncontact biophysical factor known for regulating various cellular behaviors but remains uninvestigated for cellular ***,we present a new reprogramming strategy using low-intensity ultrasound(LIUS)to improve cellular reprogramming of iPSCs in vitro and in *** 3D microenvironment conditions,increased LIUS stimulation shows enhanced cellular reprogramming of the *** cellular reprogramming process facilitated by LIUS is accompanied by increased mesenchymal to epithelial transition and histone *** stimulation transiently modulates the cytoskeletal rearrangement,along with increased membrane fluidity and mobility to increase HA/CD44 ***,LIUS stimulation with HA hydrogel can be utilized in application of both human cells and in vivo environment,for enhanced reprogrammed cells into ***,LIUS stimulation with a combinatorial 3D microenvironment system can improve cellular reprogramming in vitro and in vivo environments,which can be applied in various biomedical fields.
