Optimal initiation of Helicobacter pylori eradication in patients with peptic ulcer bleeding

AIM:To evaluate when Helicobacter pylori(***)eradication therapy(ET)should be started in patients with peptic ulcer bleeding(PUB).METHODS:Clinical data concerning adults hospitalizedwith PUB were retrospectively collected and ***,sex,type and stage of peptic ulcer,whether endoscopic therapy was performed or not,methods of *** detection,duration of hospitalization,and specialty of the attending physician were *** influencing the confirmation of *** infection prior to discharge were determined using multiple logistic regression *** *** eradication rates of patients who received ET during hospitalization and those who commenced ET as outpatients were ***:A total of 232 patients with PUB were evaluated for *** infection by histology and/or rapid urease *** these patients,53.7%(127/232)had confirmed results of *** infection prior to *** multivariate analysis,duration of hospitalization and ulcer stage were factors independently influencing whether *** infection was confirmed before or after *** the patients discharged before confirmation of *** infection,13.3%(14/105)were lost to *** the patients found to be ***-positive after discharge,41.4%(12/29)did not receive *** was no significant difference in the *** eradication rate between patients who received ET during hospitalization a n d t h o s e w h o c o m m e n c e d E T a s o u t p a t i e n t s[intention-to-treat:68.8%(53/77)vs 60%(12/20),P=0.594;per-protocol:82.8%(53/64)vs 80%(12/15),P=0.723].CONCLUSION:Because many patients with PUB who were discharged before *** infection status was confirmed lost an opportunity to receive ET,we should confirm *** infection and start ET prior to discharge.

Tribological behavior of TiN,AlTiN,and AlTiCrN coatings in atmospheric and vacuum environments

In this study,the tribological characteristics of TiN,AlTiN,and AlTiCrN coatings sliding against a SUS420J1 stainless steel pin were investigated in atmospheric and vacuum *** coatings were deposited on SUS440C substrates using the arc-physical vapor deposition *** friction and wear behavior of the coatings were evaluated based on the systematic analyses of the friction coefficient data as well as the physical and chemical state of the wear *** results revealed that the friction coefficients of the SUS440C specimen and AlTiCrN coatings increased,whereas those of the TiN and AlTiN coatings decreased when the environment was changed from atmospheric to *** was confirmed that the formation of an oxide layer and adsorption of oxides on the surface were dominant factors that influenced the tribological behavior in the atmospheric *** the other hand,the compatibility,oxidation inhibition,and droplets of the surface mainly affected the frictional characteristics in the vacuum *** results of this work are expected to aid in the selection of proper coating materials for tribological systems operating in a vacuum.

Energy harvesting model of moving water inside a tubular system and its application of a stick-type compact triboelectric nanogenerator

As the first invention to efficiently harvest electricity from ambient mechanical energy by using contact electrification, the triboelectric nanogenerator has elicited worldwide attention because of its cost-effectiveness and sustainability. This study exploits a superhydrophobic nanostructured aluminum tube to estimate electrical output for solid-water contact electrification inside a tubular system. The linearly proportional relationship of short-circuit current and open-circuit voltage to the detaching speed of water was determined by using a theoretical energy harvesting model and experimentation. A pioneering stick-type solid-water interacting triboelectric nanogenerator, called a SWING stick, was developed to harvest mechanical energy through solid-water contact electrification generated when the device is shaken by hand. The electrical output generated by various kinds of water from the environment was also measured to demonstrate the concept of the SWING stick as a compact triboelectric nanogenerator. Several SWING sticks were connected to show the feasibility of the device as a portable and compact source of direct power. The developed energy harvesting model and the SWING stick can provide a guideline for the design parameters to attain a desired electrical output; therefore, this study can significantly increase the applicability of a water-driven triboelectric nanogenerator.

As a new class of two-dimensional materials, two-dimensional (2D) heterostructures constructed from metal chalcogenides (MCs) have been gaining tremendous attention due to their unprecedented physical and chemical phenomena, mainly originated from their distinct structural features such as composition, architecture type, spatial arrangement of each component, crystal structure, exposed facet and interface, dimensionality in their heterostructures. Towards the realization of practical applications, synthetic approaches need a rational design with a variety of architecture types including laterally-combined, vertically-aligned, and conformally-coated 2D MC heterostructures. Among various synthetic routes, solution-based synthesis is thought of as an alternative to fabrication through high-cost setups since it can control those structural features in a cheap fashion. This review presents recent progress on solution-based synthesis to produce various 2D MC heterostructures with a focus on the synthetic fundamentals in terms of thermodynamic and kinetic aspects related to the growth mechanism. Four different synthetic approaches are reviewed: seeded growth, cation exchange reaction, colloidal atomic layer deposition, direct synthesis including one-step process and modified electrochemical method. We also provide some representative applications of 2D MC heterostructures and their hybrid composites in various fields including optoelectronics, thermoelectrics, catalysis, and battery. Finally, we offer an insight into challenges and future directions in a synthetic improvement of 2D MC heterostructures.

Recovery of cerium from glass polishing slurry

Classifying extrahepatic bile duct metachronous carcinoma by de novo neoplasia site

Extrahepatic bile duct(EHBD)cancer may occur metachronously,and these cancers are resectable with a favorable *** aimed to identify the pattern of metachronous EHBD *** classified the cases of metachronous EHBD cancer reported in the literature thus far and investigated two new cases of metachronous EHBD cancer.A 70-year-old female underwent R0 bile duct resection for a type 1 Klatskin tumor(pT-1N0M0).A 70-year-old male patient underwent R0 bile duct resection for a middle bile duct cancer(pT2N1M0).Imaging studies of both patients taken at 14 and 24mo after first surgery respectively revealed a metachronous cholangiocarcinoma that required pancreaticoduodenectomy(PD).Histopathology of the both tumors after PD revealed cholangiocarcinoma invading the pancreas(pT3N0M0).Both patients have been free from recurrence for 6 years and 16 mo respectively after the second *** a review of the literature on these cases,we classified the pattern of metachronous EHBD cancer according to the site of de novo *** proximal remnant bile duct was most commonly *** EHBD cancer should be distinguished from an unresectable recurrent *** metachronous EHBD cancer may be helpful in identifying rare metachronous tumors.

Endoscopic papillary large balloon dilation for the removal of bile duct stones

Endoscopic papillary large balloon dilation(EPLBD)with endoscopic sphincterotomy(EST)has been widely used as the alternative to EST along with endoscopic mechanical lithotripsy(EML)for the removal of large or difficult bile duct ***,EPLBD without EST was recently introduced as its simplified alternative ***,we systematically searched PubMed,Medline,the Cochrane Library and EMBASE,and analyzed all gathered data of EPLBD with and without EST,respectively,by using a single standardized definition,reviewing relevant literatures,published between 2003and June 2013,where it was performed with largediameter balloons(12-20 mm).The outcomes,including the initial success rate,the rate of needs for EML,and the overall success rate,and adverse events were assessed in each and compared between both of two procedures:"EPLBD with EST"and"EPLBD without EST".A total of 2511 procedures from 30 published articles were included in EPLBD with EST,while a total of413 procedures from 3 published articles were included in EPLBD without *** the results of outcomes,the overall success rate was 96.5%in EPLBD with EST and97.2%in EPLBD without EST,showing no significant difference between both of *** initial success rate(84.0%vs 76.2%,P<0.001)and the success rate of EPLBD without EML(83.2%vs 76.7%,P=0.001)was significantly higher,while the rate of use of EML was significantly lower(14.1%vs 21.6%,P<0.001),in EPLBD with *** rate of overall adverse events,pancreatitis,bleeding,perforation,other adverse events,surgery for adverse events,and fatal adverse events were 8.3%,2.4%,3.6%,0.6%,1.7%,0.2%and 0.2%in EPLBD with EST and 7.0%,3.9%,1.9%,0.5%,0.7%,0%and 0%in EPLBD without EST,respectively,showing no significant difference between both of *** conclusion,recent accumulated results of EPLBD with or even without EST suggest that it is a safe and effective procedure for the removal of large or difficult bile duct stones without any additional risk of severe adv

Surface plasmon enhancement of photoluminescence in photo-chemically synthesized graphene quantum dot and Au nanosphere

Graphene quantum dots （GQDs） are promising candidates for potential applications such as novel optoelectronic devices and bio-imaging. However, insufficient light absorption to exhibit their intriguing characteristics. The strong confinement of light caused by the Au nanoparticles as an antenna can considerably boost the light absorption. With the assistance of ultraviolet irradiation, we prepared bluish-green luminescent nanospheres by the hybridiza- tion of GQD and Au nanoparticles （GQD/Au）. These nanospheres showed a photoluminescence quantum yield of up to 26.9%. The GQD/Au nanospheres were synthesized using a solution of GQDs and HAuC14 by a photochemical method with the reduction of GQDs and the formation of metallic Au. The GQDs and Au nanopartides self-assembled and aggregated into nanospheres via aurophilicity and hydrogen bonding interactions. The average size of the GQD/Au nanospheres was found to be in the range of 150-170 nm, which is much larger than that of the pristine GQDs （4-7 nm）. The GQD/Au nanospheres exhibited an absorption band at 541 nm, which indicates the presence of Au in the nanospheres. The typical absorbance features of GQDs were observed near 236 and 303 nrn. The photoluminescence characteristics were investigated using the excitation and emission spectra. The GQD/Au nanospheres exhibited two emission peaks at 468 and 529 nm in the visible range. The green fluorescent peak located at 529 nm was newly generated by the hybridization. The GQD/Au nanospheres showed an emission efficiency which was two times more than that of the intrinsic GQDs. The reason for this increase was the surface plasmon resonance from the Au particles, which improved the fluorescence property of the resulting nanospheres. These nanospheres can be perceived as outstanding candidates for applications such as displays, optoelectronic devices, and imaging of the biological samples with high emission intensity.

Comparative analysis on homogeneity of Pb and Cd in epoxy molding compounds

Reference materials for quantitative determination of regulated heavy metals, such as Pb and Cd in electronic components, were designed and investigated in terms of stability and homogeneity. Reference materials with two concentration levels of heavy metals were prepared by spiking Pb and Cd compounds to epoxy molding compounds made by mixing silica powders and epoxy resin. The concentration changes of the reference materials during stability test for 1 a were not observed. In the homogeneity assessment, the as-prepared reference materials were studied by using three different analytical tools, inductively coupled plasma atomic emission spectrometry (ICP-AES), X-ray fluoroescence spectrometry (XRF) and laser ablation ICP mass. The results show different homogeneities by the characteristics of analytical tools and the materials.
