To the Editor： Although rare, multiple adverse events associated with anti-tuberculosis （TB） medication can occur simultaneously in patients with underlying diseases. Here, we describe a 76-year-old man with several preexisting disorders in whom anti-TB medication caused multiple, simultaneous adverse events, including Stevens-Johnson syndrome （SJS）, hypothermia, and lactic acidosis.

Tip vortex cavitation noise of marine propeller became primary concems to reduce hazardous environmental impacts from commercial ship or to keep the underwater surveillance of naval ships. The investigations of the tip vortex and its induced noise are normally conducted through the model test in a water cavitation tunnel. However the Reynolds number of model-test is much smaller than that of the full-scale, which subsequently results in the difference of tip vortex cavitation inception. Hence, the scaling law between model- and full-scales needs to be identified prior to the prediction and assessment of propeller noise in full scale. From previous researches, it is generally known that the incipient caivtation number of tip vortex can be represented as a power of the Reynolds number. However, the power exponent for scaling, which is the main focus of this research, has not been clearly studied yet. This paper deals with the estimation of scaling exponent based on tip vortex cavitation inception test in both full- and model-scale ships. Acoustical measurements as well as several kind of signal processing technique for an inception criterion suggest the scaling exponent as 0.30. The scaling value proposed in this study shows slight difference to the one of most recent research. Besides, extrapolation of model-ship noise measurement using the proposed one predicts the full-scale noise measurement with an acceptable discrepancy.

Embryonic stem cells(ESCs) maintain their cellular identity through the systematic regulation of master transcription factors and chromatin remodeling complexes. Recent work has shown that the unusually large-scale enhancers—namely super-enhancers(SEs), on which BRD4, a member of the bromodomain and extraterminal domain(BET) family is highly enriched—could regulate pluripotency-related transcription factors. Moreover, inhibition of BRD4 binding on SEs has been shown to induce the differentiation of ESCs. However, the underlying mechanism of BRD4 inhibition-mediated stem cell differentiation remains elusive. Here we show that both mouse and human ESCs lose their capacity for self-renewal upon treatment with JQ1, a selective inhibitor of BET family including BRD4, with rapid suppression of pluripotency-associated genes. Notably, a high concentration of JQ1 could selectively eliminate ESCs via apoptosis, without affecting the functionality of differentiated somatic cells from ESCs, suggesting that inhibition of BET may have a beneficial effect on the development of pluripotent stem cell-based cell therapy.

Interstitial lung disease （ILD） is a comprehensive term referring to a group of lung diseases affecting the interstitium of the lung. Idiopathic pulmonary fibrosis （IPF） is the most common idiopathic ILD, and nonspecific interstitial pneumonia （NSIP） is the second most common. As the name suggests, NSIP is diagnosed atter many other diseases are excluded. The main pathological finding in NSIP is homogeneous interstitial inflammation with or without fibrosis. NSIP can be categorized by cellular type or fibrotic type, according to the grade of inflammation and fibrosis. The cellular type has mostly inflammatory lesions with good responses to steroid, but the fibrotic type has a large proportion of fibrosis mixed with inflammatory lesions and a relatively poor response to steroid treatment So far, the exact mechanism underlying idiopathic lED has not been clarified. Determining key regulators of these ILDs will be helpful in the diagnosis and development of novel drugs for ILD.

Parameter estimation methods can be classified into(1)manual(trial-and-error),(2)numerical optimization(optimization,sampling),(3)Bayesian inference(Bayes filter,Bayesian calibration),and(4)machine learning(generative model).Bayesian calibration has been widely used because it can capture stochastic nature of uncertain parameters.However,the results of Bayesian calibration could be biased by(1)the prior distribution assumed by the expert’s subjective judgment;(2)the likelihood function that cannot always describe the true likelihood;and(3)the posterior distribution approximation method,such as the Markov Chain Monte Carlo,which requires significant computation time.To overcome this,a new approach using a generator-regularized continuous conditional generative adversarial network(GRcGAN)is presented in this paper.Five target parameters of the DOE reference building model were selected.GRcGAN was trained to estimate uncertain parameters using simulated monthly electricity and gas use.GRcGAN can successfully estimate five uncertain parameters based on 1,000 training data points.The proposed approach presents a potential for stochastic parameter estimation.

AIM:To compare efficacy,patient compliance,acceptability,satisfaction,safety,and adenoma detection rate of sodium phosphate tablets(NaP,CLICOLONTM)to a standard 4 L polyethylene glycol(PEG)solution for bowel cleansing for adults undergoing colonoscopy.METHODS:In this multicenter,randomized,prospective,investigator-blind study,the relatively young(19-60years)healthy outpatients without comorbidity were randomly assigned to one of two arms.All colonoscopy were scheduled in the morning.The NaP group was asked to take 4 tablets,5 times the evening before and4 tablets,3 times early on the morning of the colonoscopy.The PEG group was asked to ingest 2 L of solution the evening before and 2 L early in the morning of the procedure.Adequacy of bowel preparation was scored using the Boston bowel preparation scale.RESULTS:No significant differences were observed between the NaP group(n=158)and PEG group(n=162)in bowel cleansing quality(adequate preparation93.0%vs 92.6%,P=0.877),patient compliance(P=0.228),overall adverse events(63.3%vs 69.1%,P=0.269),or adenoma detection rate(34.8%vs 35.2%,P=0.944).Patient acceptability,satisfaction,and patient rating of taste were higher in the NaP group than in the PEG group(P<0.001).CONCLUSION:NaP tablets,compared with PEG solution,produced equivalent colon cleansing,did not cause more side effects,and had better patient acceptability and satisfaction in the relatively young(age<60years)healthy individuals without comorbidity.An oral tablet formulation could make bowel preparation less burdensome,resulting in greater patient participation in screening programs.

BACKGROUND Gastric ‘indefinite for neoplasm/dysplasia'(IFND) is a borderline lesion that is difficult to diagnose as either regenerative or neoplastic. There is a need for guidance in the identification of a subset of patients, who have an IFND lesion with a higher risk of malignant potential, to enable risk stratification and optimal management.AIM To determine the clinical and pathologic factors for the accurate diagnosis of gastric IFND lesions.METHODS In total, 461 gastric lesions diagnosed via biopsy as IFND lesions were retrospectively evaluated. Endoscopic resection(n = 134), surgery(n = 22), and follow-up endoscopic biopsy(n = 305) were performed to confirm the diagnosis.The time interval from initial biopsy to cancer diagnosis was measured, and diagnostic delays were categorized as > 2 wk, > 2 mo, > 6 mo, and > 1 year. The IFND lesions presenting as regenerating atypia(60%) or atypical epithelia(40%)at initial biopsy were adenocarcinomas in 22.6%, adenomas in 8.9%, and gastritis in 68.5% of the cases.RESULTS Four clinical factors [age ≥ 60 years(2.445, 95%CI: 1.305-4.580, P = 0.005),endoscopic size ≥ 10 mm(3.519, 95%CI: 1.891-6.548, P < 0.001), single lesion(5.702, 95%CI: 2.212-14.696, P < 0.001), and spontaneous bleeding(4.056, 95%CI:1.792-9.180, P = 0.001)], and two pathologic factors [atypical epithelium(25.575,95%CI: 11.537-56.695, P < 0.001], and repeated IFND diagnosis [6.022, 95%CI:1.822-19.909, P = 0.003)] were independent risk factors for gastric cancer. With two or more clinical factors, the sensitivity and specificity for carcinoma were91.3% and 54.9%, respectively. Ten undifferentiated carcinomas were initially diagnosed as IFND. In the subgroup analysis, fold change(5.594, 95%CI: 1.458-21.462, P = 0.012) predicted undifferentiated or invasive carcinoma in the submucosal layers or deeper. Diagnostic delays shorter than 1 year were not associated with worse prognoses. Objective： To evaluate the clinical efficacy of electrical stimulation（ES） of auricular acupressure on reducing the ocular symptoms and signs before and after treatment for dry eye. Methods： The inclusion criteria were the tear film break-up time（TFBUT） below 5 s and a Schirmer test-Ⅰ below 5 mm in dry eyes with ocular symptoms for at least 6 months. Subjects were randomized into a treatment group（50 cases） with continuous low frequency ES under auricular acupressure at acupoints and a no ES under auricular acupressure（no-ES, control group, 50 cases） on the same acupoints. Auricular acupressure were stimulated with ES at 4 master points of both ears, which were performed twice a week for 4 weeks at each point for 30 s. The ocular symptoms, the TFBUT, and Schirmer test-Ⅰ were evaluated before and after this procedure. Results： There were significantly better scores in TFBUT（P=0.032）, the Schirmer test-Ⅰ（P=0.044） and ocular symptoms（P=0.029） at 3 months post-treatment in the treatment group than in the control group. The total effective rate in the treatment group was accomplished in 41（82%） of the 50 cases of dry eye. Conclusions： Auricular acupressure with ES at auricular acupoint improves ocular symptoms and signs of dry eye for a period of at least 3 months.
