Systemic inflammation is a feature of chronic obstructive pulmo- nary disease （COPD）. Defects in T cell-mediated anti-inflamma- tory pathways such as cytotoxic T lymphocyte antigen-4 （CTLA- 4） may promote damaging inflammation. This study provides novel data implicating the impaired induction of an anti-inflam- matory molecule, CTLA-4 in the elevated inflammation observed in COPD patients. Low induction of CTLA-4 in COPD patients paralleled increased markers of systemic inflammation ex vivo and increased T-cell responses to a bacterial superantigen, staphylo- coccal enterotoxin-B （SEB） in vitro. This mechanism may explain the increased inflammation in COPD patients.

Selective fetal growth restriction (sFGR) is a severe condition that complicates 10% to 15% of all monochorionic diamniotic (MCDA) twin pregnancies. Pregnancies complicated with sFGR are at high risk of intrauterine demise or adverse perinatal outcome for the twins. Three clinical types have been described according to the umbilical artery (UA) Doppler pattern observed in the smaller twin: type I, when the UA Doppler is normal;type II, when there is persistent absent or reversed end-diastolic blood flow in the UA Doppler;and type III, when there is intermittent absent and/or reversed end-diastolic blood flow in the UA Doppler. Clinical evolution and management options mainly depend on the type of sFGR. Type I is usually associated with a good prognosis and is managed conservatively. There is no consensus on the management of types II and III, but in earlier and more severe presentations, fetal interventions such as selective laser photocoagulation of placental anastomoses or selective fetal cord occlusion of the smaller twin may be considered. This review aims to provide updated information about the diagnosis, evaluation, follow-up, and management of sFGR in MCDA twin pregnancies.

Phase-sensitive optical time-domain reflectometry(CDOTDR)is a well-established technique that provides spatiotemporal measurements of an environmental variable in real time.This unique capability is being leveraged in an everincreasing number of applications,from energy transportation or civil security to seismology.To date,a wide number of different approaches have been implemented,providing a plethora of options in terms of performance(resolution,acquisition bandwidth,sensitivity or range).However,to achieve high spatial resolutions,detection bandwidths in the GHz range are typically required,substantially increasing the system cost and complexity.Here,we present a novel OOTDR approach that allows a customized time expansion of the received optical traces.Hence,the presented technique reaches cm-scale spatial resolutions over t km while requiring a remarkably low detection bandwidth in the MHz regime.This approach relies on the use of dual-comb spectrometry to interrogate the fibre and sample the backscattered light.Random phase-spectral coding is applied to the employed combs to maximize the signal-to-noise ratio of the sensing scheme.A comparison of the proposed method with alternative approaches aimed at similar operation features is provided,along with a thorough analysis of the new trade-offs.Our results demonstrate a radically novel high-resolution OOTDR scheme,which could promote new applications in metrology,borehole monitoring or aerospace.

Retinal and optic nerve diseases are degenerative ocular pathologies which lead to irreversible visual loss. Since the advanced therapies availability, cell-based therapies offer a new all-encompassing approach. Advances in the knowledge of neuroprotection, immunomodulation and regenerative properties of mesenchymal stem cells(MSCs) have been obtained by several preclinical studies of various neurodegenerative diseases. It has provided the opportunity to perform the translation of this knowledge to prospective treatment approaches for clinical practice. Since 2008, several first steps projecting new treatment approaches, have been taken regarding the use of cell therapy in patients with neurodegenerative pathologies of optic nerve and retina. Most of the clinical trials using MSCs are in Ⅰ/Ⅱ phase, recruiting patients or ongoing, and they have as main objective the safety assessment of MSCs using various routes of administration. However, it is important to recognize that, there is still a long way to go to reach clinical trials phase Ⅲ-Ⅳ. Hence, it is necessary to continue preclinical and clinical studies to improve this new therapeutic tool. This paper reviews the latest progress of MSCs in human clinical trials for retinal and optic nerve diseases.

The aim of thisstudy was to conduct a randomized, double-blind and placebo-controlled study to investigate the effects of D-004, a lipid extract of the Roystonea regia fruit that prevents testosterone- and phenylepinephrine- induced prostate hyperplasia in rodents, on plasma oxidative markers in healthy men. We enrolled male volunteers （20-55 years） in good health and without lower urinary tract symptoms. Thirty-four eligible participants were randomized to placebo or D-004 （320 mg） capsules administered daily for 6 weeks. An interim check-up and a final visit were conducted after 3 and 6 weeks of therapy, respectively. Physical examinations were performed at each visit, and laboratory tests were performed at baseline and at treatment completion. Oxidative variables included plasma malondialdehyde （MDA）, total hydroxyperoxides （TOH）, sulphydryl （SH） groups and total antioxidant status （TAS）. We assessed treatment compliance and addressed adverse experiences （AEs） at weeks 3 and 6. At week 6, with D-004, the mean reductions of plasma MDA （26.7%）, TOH （18.8%） and SH groups （31.6%）, and the mean increase of TAS （35.3%） were significantly different from those of placebo （P 〈 0.001 for plasma TAS, P 〈 0.0001 for all other comparisons）. D-004 did not differ from the placebo in safety indicators. There were two withdrawals （both in the D-004 group）, with one due to dyspepsia （the only AE during the trial）. In conclusion, D-004 displayed antioxidant effects on plasma oxidative markers in healthy men, which was consistent with findings from laboratory experimental studies.
