Chronic diarrheal illnes. and malabs.rption are challenging diagnos.ic and clinical problems. The identification of the caus.tive pathogens.that are involved in gas.rointes.inal infections.is.often difficult. It took 85 years.after the firs. des.ription of a cas. of intes.inal lipodys.rophy by GeorgWhipple in 1907 until the caus.tive bacterium was.characterized by us.ng molecular genetics.techniques. We here report the complicated clinical cours. of a young patient with chronic diarrhea accompanied by s.vere, life- threate- ning malabs.rption with extens.ve weight los.. His.ology and glucos. hydrogen breath tes. were s.gges.ive of a bacterial overgrowth s.ndrome in the s.all bow el, but s.andard culture- bas.d techniques.and s.rology failed to identify the caus.tive bacteria. Thus. bacterial ribos.malDNA (16s.ribos.mal DNA) was.extract ed from duodenal biops. s.mples.and analyzed by community fingerprinting and s.e cies. s.ecific polymeras. chain reaction. s.enotrophomonas.mal- tophilia was.identified as.the caus. of chronic infectious.enteritis. Only s.ecific long- term antibiotic treatment with co- trimoxazole had a durable clinical effect and led to normalization of 16s.ribos.mal DNA profiles. This.cas. s.ows.the role of rare and uncommon bacteria in refractory and chronic human gas.rointes.inal infections. Genomic techniques. including 16s. bas.d s.ngle- s.rand conformation polymorphis. analys.s. will play an increas.ng role in the diagnos.s.of chronic infections.with facultatively pathogenic bacteria or in the clinical analys.s.of complex bacterial communities.s.ch as.the intes.inal bacterial microflora. Future enhancements.in detection techniques.will s.ow that chronic bacterial infections.are more frequent as.a caus. of gas.rointes.inal malfunction than com- monly thought.

In this.work, the mas. res.lting from s.lf energy is.obtained by utilizing the generalized relativity. The expres.ion for the mas. which res.lts.from the gravitational field is.finite. This.expres.ion is.found by cons.dering the mas. firs. as.s.all tiny s.ring and s.cond as.s.all s.here. A us.ful equation for the propagation of graviton waves.in s.ace indicates.that graviton propagates.as.travelling wave. By treating gravitation waves.as.wave packets.a plank quantum expres.ion for graviton energy dependent on the frequency is.als. found. The gravitational cons.ant (parameter) is.quantized als. in this.work.

Introduction: s.ver s.ps.s.and s.ptic s.ock contributes.to maternal morbidity and mortality. The etiology of s.ver s.ps.s.and s.ptic s.ock during pregnancy and pos.partum res.lt from obs.etric related or non-obs.etric related conditions. Objectives. It aimed to determine rate, characters. morbidity and mortality of s.ptic obs.etric cas.s.at Omdurman New Hos.ital. methods. It was.a des.riptive, pros.ective, analytic, cros.-s.ctional hos.ital bas.d total coverage s.udy;conducted at Omdurman New Hos.ital (ONH), Khartoum-s.dan. Res.lts. s.ver s.ps.s.and s.ptic s.ock rate 1.16 (13/1124 = 1.16%) of hos.ital pregnancy complication admis.ion. Hyperthermia, Tachycardia and hypotens.on are the main pres.nting clinical findings.and uterine infection is.the main focus.of s.ps.s. The mean average Intens.ve Care Unit (ICU) s.ay is.6.3-day. Organs.dys.unctions.are the main morbidity and mortality is.reported in five cas.s. Conclus.on: s.ver s.ps.s.and s.ptic s.ock contributes.in maternal morbidity and mortality. s.fe obs.etric care prevents.maternal s.ps.s.and improves.the outcome. management of s.ver s.ps.s.and s.ptic s.ock remains.a challenge in obs.etric medicine.

Comparative s.udy between Intraoperative Frozen s.ction and s.rape s.ear Cytology in the Diagnos.s.of Ovarian Neoplas.

Objective: The aim of this.work is.to compare between intraoperative frozen s.ction and s.rape s.ear cytology in the diagnos.s.of ovarian neoplas.. method: This.s.udy was.performed between march 2011 and march 2012, on 50 patients.pres.nted with ovarian mas.. Gros. examination of the tumor removed was.done by ins.ection and palpation. The s.ecimen was.then cut with a s.arp knife into two halves. The area was.s.raped with a s.arp s.alpel or the end of a glas. s.ide, depending upon the type of tis.ue. A s.mifluid drop thus.obtained was.s.read over a glas. s.ide. One to four s.ides.per cas. were taken from different repres.ntative areas. The s.ides.were labelled and immediately put into 95% ethyl alcohol and s.ained with hematoxylin-eos.n. The s.ecimens.were then fixed in formalin. Paraffin blocks.of the s.ctions.were proces.ed in the routine way and s.ctions.were s.ained with hematoxylin and eos.n (H and E). As.es.ment of the overall accuracy of the intraoperative diagnos.s.was.clas.ified as.concordant or dis.ordant. Res.lts. The s.ns.tivity, s.ecificity, pos.tive predictive value, negative predictive value and accuracy of s.raping technique in the diagnos.s.of benign ovarian mas.es.were 100%, 95.2%, 96.7%, 100% and 98% res.ectively. The s.ns.tivity, s.ecificity, pos.tive predictive value, negative predictive value and accuracy of s.raping technique in the diagnos.s.of border line ovarian mas.es.were 100%, 93.4%, 25%, 100% and 94% res.ectively. The s.ns.tivity, s.ecificity, pos.tive predictive value, negative predictive value and accuracy of s.raping technique in the diagnos.s.of malignant ovarian mas.es.were 80%, 100%, 100%, 88.2% and 92% res.ectively. The s.ns.tivity, s.ecificity, pos.tive predictive value, negative predictive value and accuracy of frozen s.ction in the diagnos.s.of benign ovarian mas.es.were 100%, 100%, 100%, 100% and 100% res.ectively. Impact of Age on s.rgical Outcomes.after Robot As.is.ed Laparos.opic Hys.erectomies

Objective: To es.imate the impact of patient’s.age on s.rgical outcomes.in patients.undergoing robotic hys.erectomy. methods. A retros.ective review of pros.ectively collected cohort data for a cons.cutive s.ries.of patients.undergoing gynecologic robotic s.rgery. Patient’s.age and perioperative variables.were collected from the databas., charts. and other hos.ital records.of all patients.undergoing robotic hys.erectomy. Res.lts. 399 patients.underwent robotic s.rgery for gynecologic dis.as.. 370 patients.who were under age 70 were compared with 29 patients.who were over age 70. When comparing all patients.under age 70 with patients.over age 70, the mean age was.48.4 and 77 (P?2 (P mean procedure time was.185 and 211 minutes.(min) (P?=?0.09), mean cons.le time?was.123 and 148 min (P?=?0.056), mean OR (Operating room) time?was.237 and 273, mean EBL (Es.imated blood los.) was.71 and 65 ml (P?=?0.74), Hb (Hemoglobin) drop?was.1.4 and 1.2 (P?=?0.45), uterine weight was.212 and 95 gm (P?= 0.98), and length of s.ay was.1.4 and 1.6 days.(P?=?0.33) (Table 1). The patients.over age 70, when procedures.were combined, had a s.atis.ically s.gnificant lower mean BmI, uterine weight and longer Operating room (OR) time. However, when s.ratified by the type of procedure performed, there was.no difference in s.rgery times.among thos. under 70 and over 70 years.of age. The elderly patients.were more likely to have cancer, which was.in almos. half the elderly patients. and thus.neces.itate s.aging. Thus.adding the performance of lymph node dis.ection likely res.lted in the increas.d length of the s.rgery time that was.noted in the combined group (Tables.1,2). There were no operative deaths. Conclus.ons. Advanced age does.not appear to be as.ociated with an increas.d ris. of morbidity, or advers. perioperative outcomes.in patients.undergoing robotic hys.erectomy.

Comparative s.udy on methods.for Preparation of Gold Nanoparticles

The gold nanoparticles.were prepared in many methods. differed in reducing and dis.ers.on agents.and s.ability. The firs. s.lution was.prepared by us.ng potas.ium bitartrate as.reducing agent and Polyethylene glycol as.a dis.ers.on factor. We got a s.lution of colloid gold with wine-red color that has.been meas.red the particle s.ze;it was.between 50 - 200 nanometers. The s.cond gold colloidal s.lution was.prepared by us.ng s.dium citrate as.a reducing agent and without adding a dis.ers.on agent, the color of the final s.lution is.dark grey, but this.s.lution did not remain s.able, depos.tion happened quickly and crys.als.s.parated from the s.lution. The third one was.prepared by us.ng s.dium citrate as.a reducing agent and the s.lution was.heated to a certain temperature, and the color of the s.lution became dark red after s.veral minutes. Either the fourth method has.been prepared gold colloid s.lution by us.ng potas.ium bitartrate as.a reductant for the gold ion. We have noted that no reaction under normal conditions. while the reaction by heating the s.lution to high temperature, and when added poly vinyl pyrrolidone will lead to a s.lution of colloild gold with dark purple-red color and s.able under normal conditions.for a long time. We meas.red the particles.s.ze in this.experiment;it was.les. than 100 nanometers. We conclude from the above that all the reducing agent needs.to appropriate conditions.to complete the proces. of reduction of gold ion, and to get the nano gold particles. therefore;thes. particles.in the colloid s.lution need to dis.ers.on factor to the s.rvival of the particles.wides.read s. that the s.lutions.with certain concentrations.of poly vinyl pyrrolidone can be proven in the proces. of s.ability of the gold colloid s.lution.
