BACKGROUND Aprotinin has recently been associated with adverse outcomes in patients undergoing cardiac *** reviewed our experience with this agent in patients undergoing cardiac surgery at Duke University Medical Center. METHODS We retrieved data on 10,275 consecutive patients undergoing surgical coronary revascularization at Duke between January 1,1996,and December 31,*** fit data to a logistic-regression model predicting each patient’s likelihood of receiving aprotinin on the basis of preoperative characteristics and to models predicting long-term survival(up to 10 years) and decline in renal function,as measured by increases in serum creatinine levels. RESULTS A total of 1343 patients(13.2%) received aprotinin,6776 patients(66.8%) received aminocaproic acid,and 2029 patients(20.0%) received no antifibrinolytic therapy. All patients underwent coronary-artery bypass grafting,and 1181 patients(11.5%) underwent combined coronary-artery bypass grafting and valve *** the risk-adjusted model,survival was worse among patients treated with aprotinin,with a main-effects hazard ratio for death of 1.32(95%confidence interval[CI],1.12 to 1.55) for the comparison with patients receiving no antifibrinolytic therapy(P=0.003) and 1.27(95%CI,1.10 to 1.46) for the comparison with patients receiving aminocaproic acid(P=0.004).As compared with the use of aminocaproic acid or no antifibrinolytic agent,aprotinin use was also associated with a larger risk-adjusted increase in the serum creatinine level(P<0.001) but not with a greater risk-adjusted incidence of dialysis(P=0.56). CONCLUSIONS Patients who received aprotinin had a higher mortality rate and larger increases in serum creatinine levels than those who received aminocaproic acid or no antifibrinolytic agent.

背景门诊患者中，脉压（pulsepressure，PP）增大是公认的增加血管事件的危险因素，但对于冠状动脉搭桥（CABG）手术患者，脉压与急性围手术期血管事件的关系尚不明确。方法我们进行了一项前瞻性观察性研究，涉及5436例择期体外循环冠状动脉搭桥术患者，其中4801例符合标准并最终入选。综合分析数据包括病史、手术中、手术后生理和实验室检查、诊断性检查、临床事件。使用多因素logistic回归对手术前高血压病（收缩压、舒张压、PP）与缺血性心脏病、脑血管事件、死亡率进行分析，P〈0．05认为差异有显著性。结果917例（19．1％）发生致命性或非致命性血管并发症，包括146例（3．0％）脑血管意外和715例（14．9％）心血管并发症，院内死亡147例（3．1％）。手术前测量的所有血压变量中，脉压是增加手术后并发症风险的重要因素。脉压增加10mmHg（高于40mmHg阈值），脑血管并发症发生率就随之增加（校准比值比：1．12；95％可信区间『1．002～1．28]；P=0．026）。与PP≤80mmHg者比较，PP〉80mmHg的患者，脑血管事件和（或）神经源性死亡的发生率几乎翻倍（5．5％：2．8％；P=0．004）。同时发现，PP〉80mmHg与心脏并发症相关，可导致充血性心力衰竭的发生率升高52％，心源性死亡率升高近100％（P值分别为0．003和0．006）。结论PP增大是导致CABG术患者致命性和非致命性的心、脑血管并发症增加的一项重要、独立的因素。这些发现突显了手术前脉压与手术后急性心脑血管并发症之间的联系。

背景目前，有关围术期血压稳定性-9手术预后关系的研究很少，本研究旨在验证心脏手术患者中收缩压（systolic blood pressure，SBP）变异性与术后30天死亡率具有相关性这一假设。方法本研究从ECLIPSE试验中随机选取1512例围术期高血压患者，分析其SBP变异性。通过超出既定SBP可变范围的偏差×持续时间（曲线下面积）衡量SBP变异性。手术中既定的SBP可变范围为65—135mmHg，手术前和手术后为75—145mmHg，按10mmHg递增提高SBP低限，将既定的SBP可变汜44-围逐步缩窄到手术中105-135mmHg及手术前手术后115—145mmHg。应用多元逻辑回归分析评价收缩压变异性与从ECLIPSE试验结果中得到手术后30天死亡率的关系。结果超过既定范围的SBP变异性（手术中75—135mmHg及手术前手术后85。145mmHg）与手术后30天的死亡率显著相关。每增加60mmHg·min^-1·h^-1的收缩压偏差，手术后30天死亡风险的比值比为1．16（95％可信区间1．04—1．30）。当曲线下的面积从0增加到300mmHg·min^-1·h^-1对于低危组患者，30天死亡率的预测概率从0．2％增加到0．5％，而高危患者从42．4％增加到60．7％。结论心脏手术患者围术期血压变异性与30天死亡率相关，并与超出既定的SBP可变范围（手术中75—135mlnng及手术前手术后85—145mmHg）的偏差程度成正比。与低危组患者相比，高危组患者死亡率的预测值更高。

背景心外手术过程中的高血压常难以控制，并进而影响手术疗效。氯维地平是一种新型的2-氢吡啶L型钙通道阻滞剂，可有效降低动脉压，起效迅速，且半衰期极短。氯维地平治疗围手术期高血压的安全性事件评估研究（evaluation of clevidipine in the perioperative treatment of hypertension assessing safety events trial, ECLIPSE）旨在比较氯维地平（CLV）与硝酸甘油（NTG）、硝普钠（SNP）、尼卡地平（NIC）治疗心外手术患者急性高血压的疗效与安全性。方法我们分析了3项前瞻性、随机、开放性平行对照研究的资料，这些研究由61家医疗中心参与完成，比较了氯维地平与硝酸甘油、硝普钠、尼卡地平在心外手术患者的围手术期应用情况。共选择1964例患者，1512例按临床标准需紧急治疗高血压，符合随机化纳入标准。这些患者按1：1比例随机编入3个平行的对比治疗组。主要结局为30天内的死亡事件、心肌梗死、中风和肾功能不全，次要结局为血压控制的充分性与精确度的评估报告。结果氯维地平组与其他治疗组相比，心肌梗死、中风和肾功能不全的发生率均无显著差异。硝普钠治疗组与氯维地平组相比，死亡例数显著增加（P=0．04）。与硝酸甘油（P=0．0006）和硝普钠（P=0．003）相比，氯维地平能更有效地把血压控制在预定范围内。在控制血压于一个预定范围方面氯维地平与尼卡地平等效，但当预定血压控制范围较窄时，氯维地平较尼卡地平超出范围的幅度更小。结论氯维地平用于治疗心外手术患者的急性高血压安全有效。

美国心脏超声协会（American Society of Echocardiography，ASE）始成立于1975年，长期以来一直鼓励对超声心动图检查质量的评估。为此，已发表了一些超声心动图检查的指南，并不断进行修订。1995年，ASE发表了一系列专门用于超声心动图持续质量改进（Continuous Quality Improvement，CQI）的建议。超声心动图检查的临床广泛应用以及超声技术、检查方法和图像判读的复杂性是进行CQI项目的原因之一。本文旨在阐述美国心脏超声协会与美国心血管麻醉医师协会针对围手术期超声心动图检查CQI项目所发表的建议和指南。在ASE已发表的有关CQI项目文章的基础上，本文将涉及如下内容：1）提出围手术期开展CQI的理由，2）确定围手术期超声心动图检查的项目内容，3）提出与围手术期超声心动图检查有关的CQI原则，4）评估CQI项目在围手术期的有效性。本文所提出的建议和操作指南适用于任何围手术期间的超声心动图检查，包括手术前、手术中和手术后。任何具有Ⅲ级水平超声心动图检查的单位可独立实施CQI项目。

Survivorship characteristics and adaptive mechanisms of phytoplankton assemblages in ballast water

Phytoplankton diversity and abundance were determined in ballast water from 26 vessels in the Shanghai Yangshan Deep-Water Port from April 2015 to January 2016. In total, 84 species of phytoplankton were identified, belonging to 43 genera and 5 phyla. Bacillariophyta (75.0%, including 30 genera and 63 species) were the dominant algae in the ballast water. Their density ranged from (5.55±9.62) SD to (1.878±0.872)×10 3 cells/L, with a mean of 410.1 cells/L. Nine potentially harmful phytoplankton taxa were detected: Ceratium furca , Ce . marcroceros , Leptocylindrus danicus , Coscinodiscus radiatus , Co . granii , Prorocentrum micans , Melosira sulcata , Meuniera membranacea and Skeletonema costatum . Our survey and identification results showed that Microcystis aeruginosa , Ankistrodesmus falcatus and Scenedesmus survived in the high-salinity ballast water, even though they are freshwater species. We identified the common features of surviving phytoplankton and impacts on the phytoplankton assemblage of ballast water age and source. Our goal was to understand the adaptative mechanisms of phytoplankton in ballast water, providing statistical and theoretical support for future ballast water research and suggesting a scientific basis of ballast water management and inspection of vessels entering the port.

Previous studies have found differences between communities of benthic macroinvertebrates living in constructed tidal flats compared with natural *** analyzed the distributional characteristics of benthic macroinvertebrates in a blowing sand reclamation area(Area A),a dike-building silting area(Area B),an out-ofsiltation area(Area C),and a natural tidal flat(Area D)in order to characterize the community structure and diversity of benthic macroinvertebrates in a human-disturbed estuarine tidal-flat wetland.A total of 32 benthic macroinvertebrate species were identified(Area A=7;Area B=12;Area C=10;Area D=27).The diversity index was variable where:Area B0.05)in the abundance and biomass of Areas A,C,and *** abundance/biomass curve showed that the habitat in Areas A,B,and C could be categorized as moderately disturbed,whereas Area D was categorized as *** results of cluster analysis suggested distinct assemblages in the four sampling *** raised the altitude of tidal flats,weakening the tidal power and changing the physical and chemical parameters of the water,leading to a change in the community structure of benthic macroinvertebrates.
