Objective.:to retrospectively evaluate the feasibility and morbidity of total laparoscopic radical hysterectomy and lymphadenectomy. Methods.:We performed a retrospective review of all p.t.ents who underwent a total laparoscopic radical hysterectomy at our institution between May 2004 and August 2005. Data collected included age,body mass index,stage,histop.t.ologic subtype,tumor grade,estimated blood loss,perioperative blood transfusions,number and status of lymph nodes obtained,status of surgical margins,length of hosp.t.l stay,time to resumption of normal bladder function,intraoperative and postoperative complications,and disease-free interval. Results.:twenty p.t.ents underwent total laparoscopic radical hysterectomy during the study period. None of the surgeries required conversion to laparotomy. the median p.t.ent age was 41.5 years (range,25-76). Eighteen p.t.ents had cervix cancer (5 stage IA2 and 13 stage IB1),and 2 had endometrial cancer (1 stage IB and 1 stage IIIA). Among those with cervix cancer,12 had adenocarcinoma,4 squamous cell carcinoma,and 2 adenosquamous carcinoma. the median weight was 70 kg (range,49-112). the median number of resected pelvic lymph nodes was 13 (range,9-26). One p.t.ent had nodal disease. the surgical margins were free of disease in all cases. the median blood loss was 200 ml (range,25-700 ml). Only 1 p.t.ent required an intraoperative blood transfusion (1 U packed red blood cells). the median length of hosp.t.l stay was 1 day (range,1-5). there were 3 short-term complications-unintentional cystotomy,pulmonary embolus,and pneumomediastinum with subcutaneous emphysema. there were 2 long-term complications-vaginal eviscerations and a lymphocyst. the median time to resumption of normal bladder function was 16 days (range,13-29). the median follow-up.t.me was 8 months range (1-16). Objective. to determine if young, normal-weight women with endometrial cancer have unique risk factors and clinical characteristics when compared with young, overweight and obese women with endometrial cancer. Methods. Between 1989 and 2003, 1531 p.t.ents were treated for endometrial cancer. 188 (12%) of these women were premenopausal and under the age of 50 at time of diagnosis. the p.t.ents were divided into three groups based on body mass index (BMI): normal weight (BMI t (BMI 25-30 kg/m2) and obese (BMI ≥30 kg/m2). Clinical and p.t.ologic characteristics were compared. Results. Of the 188 young endometrial cancer p.t.ents, 47 (25%) were of normal weight, 31 (17%) overweight and 106 (56%) obese. 4%of normalweight, 12%of overweight and 0%of obese p.t.ents met criteria for Lynch syndrome/hereditary non-polyposis colorectal cancer (HNpCC). Irregular menstrual cycles were reported by 30%normal-weight, 32%of overweight and 61%of obese women (p = 0.001). 17%of normal-weight, 7%of overweight and 14%of obese women had a history of infertility (p = 0.723). Nulliparity was noted in 57%of normal-weight, 47%of overweight and 57%of obese pa-tients (p = 0.583). Diabetes was reported in 4%of normal-weight, 13%of overweight and 35%of obese p.t.ents (p t, 26%of overweight and 14%of obese p.t.ents had synchronous primary tumors of the endometrium and ovary. Conclusions. As expected, a significantly higher proportion of obese and overweight women had diabetes and irregular menstrual cycles. Among the normal-weight women, a high proportion were nulliparous, had a history of infertility, irregular menstrual cycles and synchronous tumors of the endometrium and ovary when compared with the general population. this suggests that hormonal factors, and possibly polycystic ovarian syndrome (pCOS), may contribute to the development of endometrial cancer in young normalweight women.

*** primary cancers of the endometrium and ovary a re found in 10%of women with ovarian cancer and 5%of women with endometrial cancer. the purpose of this study was to characterize p.t.ents diagnosed with synchronous p rimary cancers of the endometrium and ovary with an emphasis on risk factors. Me thods. Between 1989 and 2002, 84 p.t.ents with synchronous primary cancers of th e endometrium and ovary were identified. p.t.entswith uterine papillary serous c arcinoma were excluded. Clinical and p.t.ologic information was obtained from me dical records. parametric methods were used to compare clinical and p.t.ologic f eatures. Kaplan-Meier survival analyses were performed and compared using the l og rank test. Results. Median age at diagnosis was 50 years. Median body mass in dex (BMI) was 28 kg/m2. Fifty-one percent (43/84) of the women were premenopaus al and 33%(28/84)-were nulliparous. the most common presenting symptom was abn ormal vaginal bleeding; in those women with abnormal vaginal bleeding, 69%had s tage I ovarian cancer. Ovarian cancerwas an incidental finding in 48%of these p atients. Sixtyeight percent of p.t.ents (57/84) had endometrioid histology of bo th their endometrial and ovarian cancers. p.t.ents with early stage ovarian canc er tended to have a more favorable prognosis than those with advanced stage dise ase (median survival not reached in stage I and II versus 66 months in stage III and IV, p = 0.06). p.t.ents with concordant endometrioid histology had a favorable progno sis (median survival 119 versus 48 months in all other groups, p = 0.02). Conclu sions. In this large series of p.t.ents, women with synchronous primary cancers of the endometrium and ovary were young, obese, nulliparous, and premenopausal. p.t.ents with concordant endometrioid tumors of the endometrium and ovary had a favorable prognosis, with median survival approaching 10 years.

OBJECtIVE:Endometrial cancer is the most common gynecologic malignancy in the United States. the mean age at diagnosis is 61 years; however, 5- 30% of women are aged younger than 50 years at the time of diagnosis. the objective of this study was to conduct a clinical and p.t.ologic review of endometrial cancers diagnosed in premenopausal women aged younger than 50 years, to better identify the risk factors for this subgroup of women. MEtHODS: We conducted a retrospective cohort study of p.t.ents with histologically confirmed endometrial cancer treated at the University of texas, M. D. Anderson Cancer Center from 1989 to 2003. Clinical characteristics including age, body mass index (BMI), parity, diabetes, and personal or family history of cancer were obtained from the medical record. p.t.ologic information was obtained from p.t.ology reports. RESULtS: twelve percent (188/1531) of all p.t.ents with endometrial adenocarcinoma were aged younger than 50 years. the mean age at diagnosis was 41 years (range 21- 49 years). Mean BMI was 34 kg/m2 (range 18- 68); 58% of p.t.ents had a BMI of 30 or greater. Fifty- five percent were nulliparous and 39% reported irregular menstrual cycles. the incidence of both diabetes and hypertension was 23% . thirty- six p.t.ents (19% ) had synchronous ovarian cancers. CONCLUSION: We found that the majority of p.t.ents diagnosed with endometrial cancer at a young age were obese and nulliparous. In addition, we found a high incidence of synchronous primary ovarian cancers in this cohort of young, premenopausal women.

Coexisting tubular adenoma with a neuroendocrine carcinoma of colon allowing early surgical intervention and implicating a shared stem cell origin

High-grade colonic neuroendocrine carcinomas (NECs) are uncommon but extremely aggressive. their co-existence with tubular adenoma (tA) has rarely been reported. We present a 68-year-old man who was found on routine colonoscopy to have multiple colorectal tAs and an ulcerated lesion in the ascending colon. Microscopically, a poorly-differentiated invasive carcinoma juxtaposed with a tA was identified. Differential diagnosis included a poorly-differentiated adenocarcinoma, medullary carcinoma, high-grade NEC and lymphoma. the immunohistochemical profile showed positive staining for keratins, synaptophysin and chromogranin but negative for LCA, CDX2, CK7, CK20, ttF-1 and pSA, supporting the NEC diagnosis. Upon subsequent laparoscopic right hemicolectomy, the tumor was identified as a 3.0 cm umbilicated and ulcerated mass with an adjacent tA. Both tA and NEC showed positive staining for β-catenin indicating a shared colonic origin. the mitotic counts (77/10 high power fields) and a high proliferation rate (75% by Ki-67) corroborated a high-grade stratification. Mutational analysis indicated a wild-type BRAF and KRAS with mismatch repair proficiency. the AJCC (7p.t.p> edition) p.t.ologic stage is pt3, pN0, pMx. the p.t.ent received adjuvant chemotherapy with cisplatin/etoposides for three cycles and will be followed up for a year to detect recurrence. In conclusion, the co-existence of tA with high grade-NEC in our case allowed early identification and intervention of the otherwise asymptomatic but aggressive tumor. In addition, the finding of a high-grade NEC within a large tA in this case suggests a link between the two lesions and could represent a shared stem cell origin.

SMILE intraoperative complications: incidence and management

AIM: to report the intraoperative complications associated with small incision lenticule extraction(SMILE) and their management. MEtHODS: this was a retrospective consecutive interventional clinical study, carried out on p.t.ents with myopia and myopic astigmatism, who underwent SMILE procedure. type of intraoperative complications and their management were recorded. RESULtS: Our study comprised 282 eyes of 141 p.t.ents who were enrolled for SMILE surgeries. the intraoperative complications included lost vacuum(18 eyes, 6.38%), treatment decentration(6 eyes, 2.12%), wound bleeding(21 eyes, 7.45%), incomplete bubble separation(black islands)(3 eyes, 1.06%), the ep.t.elial defects(15 eyes, 5.32%). Incision tear(27 eyes, 9.57%), lenticule adherence to the cap(6 eyes, 2.12%), and cap perforation occurred in 2 eyes(0.7%). CONCLUSION: Although SMILE is a promising technique for the correction of myopia and myopic astigmatism with predictable, efficient, safe refractive and visual outcomes, complications can occur. However, most of them are related to inexperience and are included in the learning curve of the technique. More studies with a bigger number of eyes are required to efficiently evaluate the intraoperative complications and standardize their management strategies.

Evaluation of phosphorus bioavailability in El Mex Bay and Lake Mariut sediments

phosphorus(p) fractions and their bioavailability in the sediments from El Mex Bay and Lake Mariut in Egypt were investigated using different chemical extraction methods. Sodium bicarbonate(NaHC03)extractable p(Olsen-p) was the largest fraction(14.42%), followed by algal available p(AAp)(3.56%),water soluble p(WSp)(0.79%), and readily desorbable p(RDp)(0.06%) for El Mex Bay. While AAp contributed 9.94% to total p in sediments from Lake Mariut, Olsen-p 8.53%, WSp 4.11%, and RDp 0.92%.Summation of the bioavailable p fractions didn’t exceed the sediment quality guidelines, and, therefore, p doesn’t represent a danger to marine organisms. Correlation coefficients showed that no apparent relations between total p.t.) and iron(Fe), aluminum(Al), and calcium(Ca) in the sediments. Furthermore, Fe:p ratio was less than 15 indicting that there was not enough Fe in surface sediments to bind to p at most of the sampling sites. the positive correlation between tp and organic matter(OM) for Lake Mariut and El Mex Bay sediments indicated that the organic matter content of the sediment was a useful predictor of the total phosphorus content. Data from this study constitute a baseline of phosphorus bioavailability in sediments from El Mex Bay and Lake Mariut and could be used as a reference for future studies on the changes of bioavailable and residual phosphorus fractions over time.
